Barriers and facilitators to STI and HIV testing among African and Caribbean heritage communities in high-income countries mapped to the behaviour change wheel: a scoping review.

BACKGROUND: African and Caribbean communities in high-income countries face disproportionate sexually transmitted infection (STI) risks. In the US, the gonorrhoea rate among non-Hispanic Blacks is 7.7 times greater than that among non-Hispanic Whites, and the chlamydia rate is 5.6 times greater. In the UK, black Caribbeans have the highest gonorrhoea and chlamydia rates among all ethnic minority groups. Identifying barriers to and facilitators of HIV/STI testing is crucial for developing effective interventions. This scoping review maps current evidence on multilevel factors influencing STI te

Worldwide prevalence of Chlamydia trachomatis in infertile women: A systematic review and meta-analysis.

INTRODUCTION: Chlamydia trachomatis (CT) infection is a prevalent sexually transmitted disease (STD). It seems that CT infections ascending to the uterus can result in serious reproductive health complications, including infertility. Therefore, this systematic review and meta-analysis was conducted to assess the worldwide prevalence of CT among infertile women. METHODS: A systematic search was conducted across three major databases, Medline (via PubMed), Web of Science, and Scopus, to identify eligible studies between 2010 and July 2025. Keywords such as "Chlamydia trachomatis," "infertility,

Global, Regional, and National Burden of Infertility Attributable to Sexually Transmitted Infections From 1990 to 2021, With Projections to 2050.

OBJECTIVE: To explore the specific impact of sexually transmitted infections (STIs) on infertility including differences in disease burden among different regions and populations and temporal trends. METHODS: Data on STI-attributable infertility were extracted from the Global Burden of Diseases Study 2021. The study focused on two types of infertility (primary and secondary) and three STIs (chlamydia, gonococcal, and other STIs). The disease burden was measured using the age-standardized prevalence rate (ASPR) and the age-standardized years lived with disability (YLD) rate (ASYR). The estimat

An organotypic ectocervix mucosa model to study host-pathogen interaction of sexually transmitted infections.

Organotypic models closely recapitulate in vivo human tissues and organs and thus overcome limitations of animal and conventional cell cultures in studying human specific infections. Infections of the female reproductive tract by sexually transmitted pathogens typically start in the lower female reproductive tract, including the vagina and ectocervix, before ascending to the upper reproductive tract. These lower reproductive tract infections are often asymptomatic, facilitating silent transmission between partners and result in severe complications due to delayed diagnosis and treatment. Howev

Reduction in overtreatment of gonorrhoea and chlamydia through point-of-care testing compared with syndromic management for vaginal discharge: a modelling study for Zimbabwe.

BACKGROUND: Zimbabwe's national guidelines for sexually transmitted infection (STI) management recommend that high-risk women presenting with vaginal discharge syndrome (VDS) are prescribed antibiotics for gonorrhoea (Neisseria gonorrhoeae (NG)), chlamydia (Chlamydia trachomatis (CT)), trichomoniasis (Trichomonas vaginalis (TV)) and bacterial vaginosis (BV). The performance of this approach depends on its clinical interpretation and implementation. Here, we investigate the potential relative impact of an NG/CT/TV point-of-care (POC) test on undertreatment, overtreatment and disease burden in t

Association between partner treatment and repeat sexually transmitted infections positivity in pregnant women in East London, South Africa.

OBJECTIVES: Sexually transmitted infections (STIs) are common in pregnant women. Effective partner treatment of women with an STI is essential to prevent reinfection. We evaluated the impact of partner notification and treatment based on the occurrence of repeat STIs in pregnant women in South Africa. METHODS: We used data from one of the intervention arms in a randomised clinical trial of STI diagnostic screening strategies in pregnancy. In this cohort, women were tested at their first antenatal care visit (<27 weeks gestational age) using onsite Xpert test assays (Cepheid, Sunnyvale, Califo

Sexually transmitted infections in pregnancy: Prevalence, coinfections, and pregnancy complications.

INTRODUCTION: Sexually transmitted infections (STIs) during pregnancy can lead to relevant maternal-fetal complications. The role of other microorganisms such as Mycoplasma genitalium in gestational screening remains a matter of debate. In this context, we analyzed the prevalence of STIs and microorganisms considered saprophytes, as well as their association with pregnancy complications. METHODS: We conducted a longitudinal study including 742 pregnant women during the first trimester (2021-2023). Vaginal samples were tested by PCR for Chlamydia trachomatis, Neisseria gonorrhoeae, Mycoplasma

Epidemiological analysis of sexually transmitted infections pathogens and related microorganisms in South Korea: coinfection patterns and pathogen correlations.

BACKGROUND: Sexually transmitted infections (STIs) affect hundreds of millions of people worldwide each year and remain a major public health concern. The adoption of multiplex PCR, which allows simultaneous detection of multiple pathogens, has led to the accumulation of large-scale data on STI coinfections. In this study, we analyzed multiplex PCR results for 12 microorganisms included in a commercial STI testing panel to characterize epidemiological patterns in South Korea and to quantitatively evaluate pathogen–pathogen coinfection relationships using lift index and phi coefficient. METHODS

Detection of undiagnosed STIs in patients with suspected UTI using pooled urine PCR screening.

INTRODUCTION: Sexually transmitted infections (STIs) remain a major global public health concern, with a high proportion of asymptomatic cases. Undiagnosed infections may lead to significant clinical, economic, and epidemiological consequences. Patients with sterile pyuria and suspected urinary tract infection (UTI) represent a relevant target population for STI screening. METHODS: We evaluated the real-world performance of a targeted STI screening strategy based on urine sample pooling in patients with suspected UTI, genitourinary symptoms, leukocyturia, and negative urine culture. RESULTS:

From syndromic management to diagnostic-led management: a report from an expert meeting on introducing sexually transmitted infection testing in low-resource settings.

INTRODUCTION: Syndromic management remains the primary approach for identifying and treating sexually transmitted infections (STIs) in low-resource settings. While faster and inexpensive, it leads to overtreatment, undertreatment and missed opportunities for surveillance. Recent advances in point-of-care (POC) diagnostic technologies for Chlamydia trachomatis, Neisseria gonorrhoeae, Trichomonas vaginalis and other STIs present new opportunities to improve STI management. METHODS: In September 2024, the Gates Foundation convened a 3-day expert meeting in Nairobi, Kenya, bringing together 44 gl

Leveraging current capacity to address the high prevalence of Chlamydia trachomatis, Neisseria gonorrhoeae, and Trichomonas vaginalis in South Africa: Modelling potential costs and benefits of near point-of-care GeneXpert testing for STIs.

South Africa has high sexually transmitted infections (STIs) prevalence and currently implements syndromic management, which has limitations including untreated asymptomatic infections and excess antibiotic use. Diagnostic tools like GeneXpert may offer potential improvements. We evaluated potential costs and benefits of GeneXpert for STIs testing. We developed a static analytical model using existing data over a one-year time horizon (January-December 2024) from a provider perspective. We estimated costs (2024 USD), outcomes, cost-effectiveness, and budget impact of syndromic management and m

Sexually transmitted infection prevalence following azithromycin mass drug administration for trachoma in Nauru: a before-and-after cross-sectional survey.

BACKGROUND: Sexually transmitted infections (STIs) are a major public health problem globally, particularly in low and middle-income countries. In the Western Pacific region, including Nauru, STIs are highly prevalent. Syndromic management misses a high proportion of infections and screen-and-treat strategies are not currently feasible at scale in these settings. Mass drug administration (MDA), widely used for control of neglected tropical diseases, may provide an alternative approach to the population control of STIs. METHODS: We used a before-and-after study design to evaluate the impact of
