Late Progressive Multivalvular Regurgitation After Heart Transplantation With Preserved Graft Function.

BACKGROUND: Late clinical decline after heart transplantation is commonly attributed to rejection, cardiac allograft vasculopathy, or ventricular dysfunction. Progressive structural valvular disease is less frequently emphasized when graft systolic function is preserved. CASE SUMMARY: A 22-year-old woman underwent orthotopic heart transplantation at age 13 for familial restrictive cardiomyopathy. Nearly 10 years later, she developed progressive exertional fatigue, reduced exercise tolerance, and dizziness despite preserved graft systolic function and no significant cardiac allograft vasculopa

Transcatheter Aortic Valve Implantation in Complex Anatomy: An Anatomy-Driven Framework for Bicuspid Stenosis, Pure Aortic Regurgitation, and the Small Annulus in the Era of Lifetime Management.

Background: Transcatheter aortic valve implantation is established for severe calcific tricuspid aortic stenosis, but its application in bicuspid aortic valve (BAV) stenosis, pure native aortic regurgitation (AR), and small aortic annulus anatomy remains limited by heterogeneous morphology, device-specific constraints, and uncertain long-term outcomes. These settings require decisions that extend beyond immediate procedural success to encompass coronary access, reintervention, valve durability, and associated aortopathy. This review synthesizes current evidence and proposes an anatomy-driven f

Simulation-guided outlet valve repair: early integration of computationally-derived principles of free-edge length in bicuspidization repair.

BACKGROUND: Symmetric bicuspidization repair for outlet valve pathologies relies on the surgeon's subjective assessment. Recent computational and ex-vivo work suggests sizing free-edge length to 1.57 times aortic diameter is optimal. This principle was applied clinically at our institution, and this study compares post-implementation outcomes with historical results. METHODS: All patients with attempted bicuspidization from 2011 to 2025 were queried. Primary outcome was freedom from valve-related reoperation or valve replacement. Secondary outcomes were cross-clamp time, number of bypass runs

Interobserver Agreement in Transthoracic Echocardiography between Cardiology Residents and Attending Cardiologists: A Cross-Sectional Study.

BACKGROUND: Transthoracic echocardiography (TTE) is essential for making clinical decisions regarding cardiovascular disorders. Cardiology trainees frequently conduct and interpret TTE examinations; however, discrepancies between their interpretations and those of attending cardiologists can impact patient management. The objective of this study was to assess the concordance between cardiology trainees and attending cardiologists in the interpretation of TTE findings. METHODS: This study included 374 inpatients and outpatients referred to for TTE examinations. TTE studies were independently p

Lifetime management of valvular heart disease in younger and low risk patients.

Valvular heart disease in younger and low-risk patients presents unique therapeutic challenges because treatment decisions must account not only for immediate procedural success but also for the durability of the index intervention, the feasibility of future reinterventions, and the preservation of subsequent treatment options. Consequently, lifetime management has emerged as a longitudinal treatment paradigm in which every intervention influences the sequence, complexity, and outcomes of future procedures. Over the past decade, transcatheter valve therapies have expanded rapidly from a treatm

Improved Visualization of Heart Valves With 4D-CT Using a Novel Motion Correction Algorithm.

OBJECTIVE: Motion artifacts remain a limitation of cardiac CT when assessing both native and prosthetic heart valves, particularly during phases of rapid motion. This may lead to reduced image quality, potentially impairing the evaluation of valvular movement and pathology. The purpose of this study was to evaluate whether a novel reconstruction-based motion correction algorithm improves image quality and reduces motion artifacts of native and prosthetic heart valves compared with standard reconstructions in photon-counting detector CT (PCD-CT). MATERIALS AND METHODS: In this retrospective si

Complex surgical management of right-sided infective endocarditis following ventricular septal defect closure with residual shunt and ascending aortic pseudoaneurysm: a case report.

INTRODUCTION AND IMPORTANCE: Right-sided infective endocarditis is an uncommon condition, especially in patients with surgically repaired congenital heart disease. A residual shunt after ventricular septal defect closure can act as a substrate for late-onset infection, but such cases are rarely reported. High-velocity jet flow from a residual shunt may contribute to endothelial injury and subsequent infection. Furthermore, differentiating a late pseudoaneurysm at a prior cannulation site from an infectious aortic aneurysm is essential for guiding the appropriate treatment strategy. CASE PRESE

Assisted Reproductive Technology and Risk of Congenital Heart Defects in the Offspring: A Systematic Review and Meta-Analysis.

BACKGROUND: To date, >10 million children have been conceived via assisted reproductive technology (ART) globally. However, there are some concerns about potential adverse outcomes associated with ART-conceived pregnancies, including the risk of congenital heart defects (CHDs). This meta-analysis aimed to evaluate the association between ART and the risk of major CHDs as well as their specific subtypes. METHODS: Major electronic databases were screened for studies comparing the incidence of CHD following ART-conceived pregnancies versus spontaneous conception. The primary end point was study-

[Aortic valve disease - Interventional aortic valve replacement in asymptomatic patients and the use of artificial intelligence].

Aortic valve diseases are among the most common structural heart diseases in adults. Aortic stenosis (AS), in particular, poses a growing challenge due to demographic trends, as it continues to be underdiagnosed. The current 2025 ESC/EACTS guidelines mark a paradigm shift: for the first time, early intervention is recommended for selected asymptomatic patients. At the same time, various AI tools support early patient screening. Also new, with a Class IIb recommendation, is that TAVI may be considered for patients with a bicuspid aortic valve and increased surgical risk, provided the anatomy is s

Multi-valvular burden score and one-year mortality in ICU patients undergoing echocardiography: an exploratory cohort study.

BACKGROUND: Multiple regurgitant lesions coexist in critically ill patients, yet cohort-level analyses typically represent each valve separately. We evaluated whether a Multi-Valvular Burden (MVB) score could summarize regurgitant involvement and whether its prognostic information differed from models retaining individual valve grades. METHODS: This retrospective cohort used MIMIC-IV v3.1 linked with MIMIC-IV-ECHO. Adults with an ICU admission and complete ordinal grading of tricuspid, mitral, and aortic regurgitation on an index echocardiogram within 72 h of ICU admission were included. MVB

Cyanotic Heart Disease.

Cyanotic congenital heart defects are malformations of the heart that affect its structure and function. The etiology of most of these defects is multifactorial, but some common causes include maternal conditions, genetic defects, and genetic mutations. Six of the most common cyanotic congenital heart defects include Tetralogy of Fallot, transposition of the great arteries, persistent truncus arteriosus, total anomalous pulmonary venous return, tricuspid atresia, and Ebstein's anomaly. These defects share several common features, however, each is distinguished by key cardiac abnormalities and

Acute Rheumatic Fever: Atrial Flutter Associated With Carditis.

BACKGROUND: Acute rheumatic fever (ARF) is an immune-mediated sequela of group A streptococcal infection that can progress to rheumatic heart disease (RHD), defined by permanent rheumatic valvular damage. Although uncommon in the United States, delayed recognition may lead to advanced valve disease and arrhythmias. CASE SUMMARY: A 10-year-old African-American male presented after syncope following 4 months of unexplained bilateral ankle arthritis. He was found to be in atrial flutter. Echocardiography showed a flail mitral valve with severe regurgitation, moderate tricuspid regurgitation, and

Pleiotropic effects of GDF-15 to regulate nutritional status: perspectives from body composition to nutrition-related disorders.

Growth differentiation factor 15 (GDF-15), a stressful cytokine of the transforming growth factor-β (TGF-β) superfamily, plays pivotal roles in diverse physiological and pathological processes. Most recently, its pleiotropic effects regarding energy metabolism and nutritional modulation are of intense scrutiny. Its physiological functions involve signaling pathways such as GDF-15/glial cell-derived neurotrophic factor family receptor α-like protein (GFRAL)/rearranged during transfection (RET), phosphatidylinositol 3-kinase (PI3K)/Protein kinase B (Akt)/mammalian target of rapamycin (mTOR), nuc

The adaptive learning taxonomy for responsible large language model integration in higher education.

UNLABELLED: Large Language Models (LLMs) are being rapidly deployed in higher education, yet institutions lack integrated, theoretically grounded frameworks to guide responsible adoption. While meta-analytic evidence reports large positive effects on academic performance (g = 0.867; Wang and Fan 2025), emerging research reveals a "cognitive paradox": performance gains may coincide with diminished metacognitive accuracy and self-regulation-a gap no existing framework comprehensively addresses across adaptation design, data governance, pedagogical targeting, and learner agency simultaneously. Strengthening non-communicable disease health systems through sustainable health partnerships in the Horn of Africa using lessons from the Wolverhampton-Somaliland Global Health Partnership.

BACKGROUND: The purpose of our study was to assess the effect of hypolipidemiant drugs on serum markers of vascular inflammation (E-Selectin, VCAM-1 and MCP-1) in dyslipidemic men without cardiovascular disease. MATERIAL/METHODS: 84 dyslipidemic men were consecutively recruited from the Lipid Unit of a tertiary hospital. The patients were placed on statins (n=44) or fibrates (n=22), depending on the lipid profile, for 4 months. In the control group (n=18), a hypolipidemiant diet alone was indicated. RESULTS: Baseline levels of VCAM-1 and MCP-1 were not correlated with the lipid profile. By c

Correlations between early ultrasound markers and the outcomes of staged lateral guided bone regeneration.

PURPOSE: Guided bone regeneration (GBR) is a widely used strategy for augmenting lost jawbone. The extent of bone gain varies among patients and typically remains unknown until after a healing period of 4-6 months. This study aims to demonstrate the use of novel ultrasonography techniques to provide predictive values for bone gain. METHODS: This study included 16 subjects with 20 edentulous ridges undergoing GBR for lateral bone deficiencies of 1-3 mm in the posterior mandible. Ultrasound (US) images were captured at the surgical site 10 days postoperatively. Four US-derived parameters were a

High diagnostic yield of family echocardiographic screening of children with bicuspid aortic valve.

AIMS: Bicuspid aortic valve (BAV) is present in 0.8% of newborns, with associated aortopathy in 33%. While the prevalence of BAV and associated aortopathy in first-degree relatives of symptomatic BAV patients ranges from 5 to 10% and 5 to 32%, data from unselected probands are limited. Uniform family screening guidelines are lacking. We aimed to determine the prevalence of BAV and aortopathy in first-degree relatives of children diagnosed neonatally with BAV. METHODS AND RESULTS: In this case-control family study based on the Copenhagen Baby Heart Study (n > 25 000 newborns enrolled 2016-18),
