Bicuspid aortic valve stenosis is characterized by increased angiogenesis, inflammation, and a higher valvular-to-systemic calcification ratio than tricuspid aortic valve stenosis.

Calcific aortic valve stenosis is the most common valvular heart disease requiring treatment. Although both tricuspid (AVS) and bicuspid (bAVS) aortic valve stenoses become calcified and functionally impaired in advanced stages, the pathophysiology of these conditions remains unclear. We investigated this using a multitechnology approach on explanted AVS, bAVS, and aortic regurgitation (AR) control valves. Because of technical limitations in processing heavily calcified aortic valve tissue, we established Kawamoto's film method for human aortic valve tissue, enabling the production of well-pre

Applied Anatomy of Perimembranous Ventricular Septal Defect for Transcatheter Device Closure.

Perimembranous ventricular septal defect is a common congenital heart disease effectively treated by transcatheter device closure with high success rates and minimal complications. This review categorizes perimembranous ventricular septal defects into 7 morphologic variants based on anatomical extension and correlates these with echocardiographic classifications: type 1 is central perimembranous; type 2 is superior-anterior extension; type 3 is superior-posterior extension; type 4 is inferior-anterior extension; type 5 is inferior-posterior extension; type 6 is confluent extensions with more t

Preoperative Main Pulmonary Artery Diameter Indexed to Body Surface Area Independently Predicts Mortality After Transcatheter Aortic Valve Implantation in a Chinese Population.

BACKGROUND: Pulmonary hypertension (PH) commonly accompanies severe aortic stenosis and adversely affects outcomes after transcatheter aortic valve implantation (TAVI). However, the prognostic value of computed tomography-derived pulmonary artery remodeling, particularly the main pulmonary artery diameter (mPAD) indexed to body surface area (mPAD/BSA), remains uncertain. Therefore, this study aimed to investigate the prognostic significance of mPAD/BSA in patients with severe aortic stenosis and concomitant PH undergoing TAVI. METHODS: We retrospectively analyzed 122 consecutive patients unde

Optimal approach to valve-sparing root replacement and aortic valve repair in aortic insufficiency.

Despite the technically challenging nature of valve-sparing root replacement, restoration of aortic valve competence can be achieved through a complete understanding of the aortic root anatomy and pathophysiology. In this review, we discuss the physiology of normal aortic root function as well as pathophysiologic mechanisms contributing to aortic valve incompetence. Preoperative planning for valve-sparing root replacement is discussed, including the use of imaging to guide surgical decision-making. Pitfalls and pearls of the critical technical steps for both valve-sparing root replacement and

Postoperative Right Ventriculo-Pulmonary Artery Coupling Assessed by TAPSE/PASP as a Predictor of Long-Term Outcomes After TAVI.

BACKGROUND: Right ventriculo-pulmonary artery (RV-PA) coupling, commonly assessed by the ratio of tricuspid annular plane systolic excursion to pulmonary artery systolic pressure (TAPSE/PASP), has emerged as an important prognostic marker in various cardiovascular diseases. However, its predictive value after transcatheter aortic valve implantation (TAVI) remains insufficiently defined. This study aimed to evaluate whether postoperative TAPSE/PASP is independently associated with long-term all-cause mortality in patients undergoing transfemoral TAVI. METHODS: We retrospectively analyzed 786 c

Transcatheter aortic valve intervention in mixed Sievers type 1 bicuspid aortic valve disease.

INTRODUCTION AND OBJECTIVES: In patients with tricuspid aortic valve stenosis, the coexistence of aortic regurgitation (AR) at baseline has been associated with improved outcomes following transcatheter aortic valve implantation (TAVI). However, its prognostic impact in patients with bicuspid aortic valve (BAV) stenosis remains unknown. METHODS: We enrolled consecutive patients with severe Sievers type 1 BAV stenosis undergoing TAVI at 24 international centers. Patients were stratified into 2 groups according to baseline AR severity: pure BAV stenosis (ie, no or mild AR) and mixed BAV disease

Outcomes of Heart Transplantation in Single-Ventricle Physiology: A Retrospective Single-Center Experience with Emphasis on Surgical Complexity.

Background: Patients with single-ventricle physiology represent a high-risk group for heart transplantation. Due to complex anatomical and physiological challenges, including multiple prior sternotomies, pulmonary artery abnormalities, and systemic consequences of altered circulation, they represent both a surgical and a clinical challenge. We aimed to analyze perioperative challenges, as well as early and long-term complications, in this specific group of patients. Methods: We performed a retrospective data analysis of a high-volume heart transplant center, focusing on patients with single-ve

Thirty-day outcomes of a novel biomimetic balloon-expandable transcatheter heart valve in patients with small aortic annuli.

BACKGROUND: Transcatheter aortic valve implantation (TAVI) in patients with small aortic annuli (SAA) is associated with an increased risk of prosthesis-patient mismatch (PPM). AIMS: This study assesses the 30-day performance of the novel balloon-expandable DurAVR transcatheter heart valve (THV), which features a unique single-piece biomimetic leaflet design, in patients with SAA. METHODS: This pooled analysis derived from first-in-human and early feasibility studies includes all patients with SAA (defined as an aortic annular area from 346 mm2 to 452 mm2) treated with the small-sized DurAVR

Outcomes of Transcatheter Aortic Valve Replacement in Mixed Aortic Valve Disease: A Comparison Between Bicuspid and Tricuspid Valves.

BACKGROUND: The impact of aortic valve morphology on outcomes after transcatheter aortic valve replacement in patients with mixed aortic valve disease, characterized by the coexistence of aortic stenosis and regurgitation, remains poorly defined. This study aimed to compare outcomes of transcatheter aortic valve replacement between bicuspid and tricuspid aortic valve morphologies in patients with mixed aortic valve disease. METHODS: We retrospectively analyzed 282 patients with mixed aortic valve disease who underwent transcatheter aortic valve replacement between January 2019 and June 2024,

A novel method of proximal isovelocity surface area correction for quantitative assessment of tricuspid regurgitation severity.

AIMS: The primary quantitative measure of tricuspid regurgitation (TR) severity uses the proximal isovelocity surface area (PISA) method to calculate effective regurgitant orifice area (EROA) and regurgitant volume (RegVol). However, EROAPISA has been demonstrated to underestimate TR severity due to reliance on several geometric assumptions. The aim of this study was to evaluate whether correcting for these parameters would improve this underestimation. METHODS AND RESULTS: Patients with at least mild TR identified on transthoracic echocardiography were included in the study. EROAPISA and ERO

Great debate: surgical aortic valve replacement is first choice for aortic stenosis in patients with a life expectancy beyond 5 years.

Transcatheter aortic valve implantation (TAVI) is increasingly used for the treatment of aortic stenosis. Recently published guidelines favour now TAVI in all patients 70 years or older who have tricuspid valves and suitable anatomy. However, while a number of randomized controlled trials confirm that TAVI and surgical aortic valve replacement (SAVR) provide equivalent outcomes up to 5 years, data beyond 5 years are still scarce and real-world registry data report indeed conflicting results. Higher rates of complications after TAVI such as pacemaker requirement and paravalvular regurgitation,

Real-world Objects Scaffold Visual Working Memory for Features: Increased Neural Engagement When Colors Are Remembered as Part of Meaningful Objects.

Visual working memory is a core cognitive function that allows active storage of task-relevant visual information. Contrary to the common assumption that the capacity of this system is fixed with respect to a single feature dimension, recent research has shown that working memory performance for a simple visual feature-color-is improved when this feature is encoded as part of a real-world object relative to an unrecognizable scrambled object. Using EEG (n = 24), we here demonstrate that this performance benefit is supported by increased neural engagement during the retention period, as indexed

Planimetry of Aortic Valve Area Using CTA: Cutoff Derivation for Stenotic Bicuspid and Tricuspid Valves.

BACKGROUND: Computed tomography based planimetric assessment of the anatomic aortic valve area (aAVACTA) in aortic stenosis is routinely performed. Unlike transthoracic echocardiography (TTE)-based effective AVA by TTE, it lacks clearly defined severity cutoff values, limiting clinical utility. METHODS: In this retrospective single-center analysis with computed tomography angiography data from 2013 to 2025, cutoffs were determined from 1294 TTE-based conclusive severe or nonsevere patients by congruence of maximum velocity, mean pressure gradient, and effective AVA by TTE. 3D transesophageal echocardiographic assessment of cusp prolapse mechanisms in tricuspid aortic root aneurysms and correlation with valve-sparing surgical outcomes.

BACKGROUND: Valve-sparing root replacement (VSRR) prevents prosthesis-related complications in aortic root aneurysms but lacks objective feasibility criteria. Cusp prolapse frequently coexists with aortic root aneurysms, but its effect on VSRR outcomes remains unclear. We characterized prolapse mechanisms using three-dimensional (3D) transesophageal echocardiography (TEE) and examined the correlation between imaging features and surgical success as well as midterm outcomes. METHODS: This retrospective cohort study analyzed the data of 203 consecutive patients considered for VSRR. Cusp prolaps

Regional mechanical and microstructural variations in ascending thoracic aortic aneurysms influenced by tricuspid and bicuspid aortic valves.

Ascending thoracic aortic aneurysms (ATAAs) exhibit regional biomechanical heterogeneity linked to microstructural changes and comorbidities. This study investigated microstructure and mechanical property variations across six regions of the ATAA wall from 10 patients after repair surgery. Planar biaxial tensile tests and two-photon microscopy were performed to determine material parameters for a microstructure-motivated constitutive model. These parameters describe the behavior of the aortic wall across a wide range of stretch values and ratios. Our results revealed significant inter- and int

The importance of sex differences in valvular heart disease from an imaging point of view.

Sex differences in valvular heart disease (VHD) represent an emerging focus in cardiovascular imaging, with implications spanning aetiology, pathophysiology, chamber remodelling, and prognosis. This review aims to illustrate how multimodality imaging can be applied to address sex-specific differences in VHD, with the goal of improving disease grading, staging of extra-valvular cardiac damage, and risk stratification across the whole VHD spectrum.

Randomized controlled trials in valvular heart disease: the evolving role of multimodality imaging.

Valvular heart disease represents a significant global health burden, with an estimated prevalence of 2.5% in high-income countries and projected increases due to population ageing. Randomized controlled trials in valvular heart disease have undergone substantial evolution, shifting from mortality-focused endpoints toward comprehensive assessments integrating imaging parameters and patient-centered outcomes. Cardiovascular imaging modalities, including echocardiography, cardiac computed tomography, and cardiac magnetic resonance, have become pivotal in trial design, patient selection, and endp
