BACKGROUND: Panvalvular infective endocarditis (IE) involving all 4 cardiac valves is extremely rare, with a high mortality. Management is complex when surgical intervention is precluded by severe comorbidities.
CASE SUMMARY: An 81-year-old woman with end-stage renal disease and metastatic bronchial carcinoma presented with Enterococcus faecalis bacteremia. Initial transthoracic echocardiography was unremarkable, but transesophageal echocardiography (TEE) revealed vegetations on all 4 valves with severe aortic and tricuspid regurgitation. Surgery was precluded by prohibitive risk. Despite tar

The gut microbiome has emerged as an important modulator of cardiovascular diseases, yet data on aortic valve disorders, particularly aortic stenosis (AS), remain limited. This study aimed to characterize gut microbiome differences in patients with bicuspid (BS) and tricuspid aortic stenosis (TS) using aortic regurgitation (AR) patients as a clinically comparable control group with similar age distribution, comorbidities, and metabolic profiles. A total of 122 patients were included in this prospective cross-sectional study: 33 AR, 22 BS, and 67 TS patients. Microbiome profiling was conducted

Truncus arteriosus (TA) is a rare conotruncal malformation in which a single arterial trunk supplies the systemic and pulmonary circulations. Severe left ventricular hypoplasia in association with TA is exceptional and creates a complex neonatal circulatory disorder. In a forensic setting, such malformations may be encountered as unexpected neonatal deaths, particularly when antenatal diagnosis is absent. We report the forensic autopsy findings of a term male neonate with a birth weight of 4260 g, born to first-cousin parents after a pregnancy without regular antenatal follow-up. Respiratory d

BACKGROUND: The treatment of primary mitral and secondary tricuspid regurgitation (MR, TR) in the setting of lung agenesis poses major technical challenges, and data on the feasibility of endoscopic tricuspid valve repair in adult congenital anatomy remain limited.
CASE SUMMARY: We report the case of a 76-year-old male with severe primary MR due to Barlow's disease, torrential secondary TR, biatrial dilatation, persistent atrial fibrillation, and post-capillary pulmonary hypertension. Anatomical anomalies included dextrocardia, hypoplastic right lung, and scimitar vein syndrome. The patient h

BACKGROUND: Most of the current evidence on valvular heart disease (VHD) comes from high-income countries. Access to treatment and the impact of VHD on quality of life may vary substantially across nations, largely due to socioeconomic differences.
OBJECTIVE: This study aimed to estimate the burden of VHD and its association with mortality in a middle-income country.
METHODS: We included participants from the ELSA-Brasil cohort who underwent transthoracic echocardiography at baseline (3,267 participants; 1,539 males; mean age 60.6 ± 8.8 years) across six Brazilian states. Participants were c

Valvular cardiomyopathy has emerged as a key concept linking valvular heart disease (VHD) with myocardial remodeling and the development of heart failure (HF). This state-of-the-art review provides a comprehensive overview of pathophysiological mechanisms, multimodality imaging, and implication for valve interventions across the spectrum of VHD. We systematically address HF in the context of aortic valve disease, including aortic stenosis and aortic regurgitation, mitral valve disease with a particular focus on mitral regurgitation, and tricuspid valve regurgitation. For each valvular lesion,

BACKGROUND: Bicuspid aortic valve (BAV) is a significant risk factor for aortic stenosis (AS). Yet, the outcomes of transcatheter aortic valve implantation (TAVI) remain unclear, as most randomized trials excluded such patients. Furthermore, most published evidence on TAVI in BAV comes from high-income countries, which may not reflect realities of underserved populations.
OBJECTIVES: Evaluate temporal trends, procedural, and in-hospital outcomes between BAV and tricuspid aortic valve (TAV) patients.
METHODS: Retrospective observational study based on a Brazilian multicenter registry of TAVI

BACKGROUND Neurofibromatosis type 1 (NF1) and Marfan syndrome (MFS) are genetically determined systemic disorders. Their simultaneous occurrence is exceptionally rare, with only a few cases reported in the literature. CASE REPORT A 13-year-old boy was admitted to a cardiology clinic due to mitral and tricuspid valve prolapse. He was tall, with severe scoliosis and distinctive dysmorphic features typical of MFS. More than 20 cafe-au-lait spots, characteristic of NF1, were present on his skin. The family history included NF1 in his twin brother, mother, maternal uncle, and maternal grandmother.

Aortic aneurysms have traditionally been studied as a single pathological entity, with emphasis on valve morphology. While histopathological differences between bicuspid (BAV) and tricuspid (TAV) aortopathy are well documented, less is known about how these mechanisms interact with aneurysm location within the ascending aorta. This gap complicates risk assessment and surgical planning beyond diameter-based criteria. We aimed to evaluate histopathological features of ascending aortic aneurysms across three topographical subgroups-root, tubular, and combined involvement-and assess modification b

Triple valve surgery is performed in only a small minority of valvular procedures and carries substantial perioperative risk and a demanding postoperative course. Pericardial complications after cardiac surgery are well recognized, but progression to constrictive physiology following a minimally invasive triple valve operation is exceptionally uncommon and poorly characterized. We describe an elderly woman with longstanding hypertension, dyslipidemia, and paroxysmal atrial fibrillation who underwent minimally invasive replacement of the aortic, mitral, and tricuspid valves for combined multiva

Background/Objectives: Pleural effusions are frequently encountered on pre-procedural computed tomography (CT) scans in patients undergoing transcatheter aortic valve implantation (TAVI). While often regarded as a marker of congestion and advanced cardiovascular disease, their clinical and prognostic significance in contemporary TAVI populations remains poorly understood. This study investigated the relationship between AI-derived pleural effusion volume, cardiovascular dysfunction, and long-term mortality after TAVI. Methods: Consecutive patients undergoing transfemoral TAVI between 2016 and

BACKGROUND: Bioprosthetic tricuspid valves are prone to structural deterioration over time. In patients with multiple prior sternotomies and functioning left-sided prostheses, repeat surgery may carry prohibitive risk, prompting consideration of transcatheter alternatives.
CASE SUMMARY: A 42-year-old patient with rheumatic heart disease and a history of 3 prior sternotomies for mitral, aortic, and tricuspid valve replacement presented with recurrent admissions for diuretic-resistant right heart failure complicated by congestive hepatopathy (Child-Pugh B cirrhosis). Imaging demonstrated severe

Valvular heart disease (VHD) contributes substantially to cardiovascular mortality. We evaluated long-term VHD mortality trends among older adults to inform healthcare planning. CDC WONDER mortality data (1999 to 2023) were analyzed for adults aged ≥65 years to calculate age-adjusted mortality rates (AAMRs). Joinpoint regression identified temporal trends, alongside a subtype analysis stratifying AAMRs by nonrheumatic aortic (ICD-10 I35), mitral (ICD-10 I34), tricuspid (ICD-10 I36), and chronic rheumatic (ICD-10 I05 to I09) valve disorders. Age-period-cohort modeling assessed generational effe

Aortic valve repair (AVr) has evolved into a reproducible surgical strategy for selected patients with aortic regurgitation (AR), offering preservation of native tissue, avoidance of prosthesis-related complications, and favorable long-term outcomes when performed at expert centers. This narrative review summarizes contemporary concepts, indications, preoperative imaging work-up, and procedure selection for AVr in tricuspid (TAV) and bicuspid (BAV) aortic valves, with an emphasis on functional mechanisms and standardized annular and root stabilization. Recent international guidelines and key s

Carcinoid heart disease (CHD) is a serious cardiac manifestation of carcinoid syndrome (CS), characterized by progressive fibrotic degeneration of cardiac valves and right-sided heart failure. We report the case of a 72-year-old man with a history of metastatic small-intestinal neuroendocrine tumor and CS diagnosed four years earlier, who presented with worsening dyspnea and peripheral edema. Transthoracic echocardiography revealed characteristic findings of advanced CHD, including severely thickened, retracted, and immobile tricuspid and pulmonary valve leaflets, resulting in severe tricuspid

Background. Transcatheter aortic valve implantation (TAVI) is most extensively documented for the treatment of aortic stenosis in patients with tricuspid valves. To compare the 30-day and 1-year outcomes after TAVI in patients with tricuspid, bicuspid and bioprosthetic valves who participated in the COMPARE-TAVI 1 trial. Methods. In COMPARE-TAVI 1, all-comer patients treated with transfemoral TAVI were randomized to treatment with one of two balloon-expandable valves. In this study, these patients were divided into three groups based on valve anatomy. 30-day and 1-year outcomes among patients

BACKGROUND: Artificial intelligence (AI) diagnostic models are typically developed in hospital-based populations enriched for disease prevalence and severity, where distinctions between health and disease are pronounced. When deployed in community-dwelling populations with lower prevalence and milder phenotypes, these distinctions may become less well-defined, raising questions about transportability and the impact of disease spectrum on model performance.
OBJECTIVES: The purpose of this study was to evaluate how differences in disease spectrum influence the transportability of AI-based diagn
