Sex-specific long-term outcomes after combined valve and coronary artery surgery.

BACKGROUND: The purpose of this study is to compare sex-specific, long-term outcomes after combined valve and coronary artery bypass graft surgery (CABG). METHODS: Between 1990 and 2000, 1,567 patients underwent combined valve and CABG surgery at our institution. Our surgical database was linked to a governmental administrative hospital discharge database and a registry of deaths to obtain long-term follow-up. All patients underwent CABG plus aortic (62%), mitral (31%), or multiple valve surgery (7%). RESULTS: Women had more preoperative risk factors than men (namely, hypertension, diabetes

Predictive risk factors in double-valve replacement (AVR and MVR) compared to isolated aortic valve replacement.

BACKGROUND: The operative risk of combined aortic and mitral surgery is still between 5 and 13 %, whereas isolated AVR normally causes complications in less than 4 % of all patients. Thus, it was the aim of the study to compare both procedures and to evaluate risk stratification in our patient cohort. PATIENTS AND METHODS: The inhospital mortality and complication rates were analyzed in both groups over a period of 4 years. There were 396 patients with isolated AVR, and 98 patients with AVR and MVR. For both groups, we investigated 16 possible risk factors for perioperative death or severe co

Surgical intervention for advanced valvular heart disease in 227 cases.

BACKGROUND: Although the results of surgical treatment in cardiac valve disease continue to improve, the postoperative mortality rate and the rate of complications in patients with advanced valvular heart disease (AVHD) are still very high. We did this retrospective study to summarize the surgical experience of heart valve replacement for patients with AVHD and discuss effective ways to improve the surgical outcome. METHODS: From January 1994 to October 2003, surgical procedures of heart valve replacement were performed on 227 (136 men and 91 women) patients with AVHD in our Department of Car

Validation of noncompaction criteria in dilated cardiomyopathy, and valvular and hypertensive heart disease.

BACKGROUND: Echocardiographic characteristics typical of isolated left ventricular noncompaction (IVNC) have been well defined. The aim of this study was to validate diagnostic criteria of IVNC in valvular or hypertensive heart disease (HHD) or dilated cardiomyopathy. METHODS: We conducted a retrospective analysis of records and blind review of videotapes of all 19 patients with IVNC seen within 7 years in comparison with randomly selected patients from the same study period with dilated cardiomyopathy (31 patients), HHD (22 patients), and chronic severe valvular heart disease: mitral regurgi

Impact of the maze operation combined with left-sided valve surgery on the change in tricuspid regurgitation over time.

BACKGROUND: Atrial fibrillation (AF) has been reported to be a predisposing factor for the progression of TR in patients with previous mitral or combined mitral/aortic valve surgery. We hypothesized that the maze operation (MAZE) can prevent the progression of tricuspid regurgitation (TR) in these patients. METHODS AND RESULTS: We analyzed 170 patients (age, 45.5+/-10.9 years) who had undergone mitral or combined mitral/aortic valve surgery. On the basis of preoperative rhythm, patients were divided into 3 groups; GrI was composed of 44 patients with sinus rhythm, GrII of 48 who had undergone

[Tricuspid replacement and short-term follow-up: summary of 42 cases].

OBJECTIVE: To summarize the experience in tricuspid valve replacement (TVR). METHODS: From March 1997 to June 2004, 42 patients underwent isolated or combined TVR. Of the cases, 20 cases had prior cardiac operation (tricuspid valve had been repaired in 8). Indication of TVR: (1) irreparable and/or progressive tricuspid lesions; (2) intolerable tricuspid dysfunction after tricuspid repair. Instead of tricuspid repair, TVR was preferred when one of the following co-existed: moderate to severe increase of pulmonary vascular resistance; residual left heart dysfunction; previously repaired tricusp

Increased morbidity and mortality in patients with antiphospholipid syndrome undergoing valve replacement surgery.

OBJECTIVE: Heart valve abnormalities are commonly found in patients with antiphospholipid syndrome but experience with valve replacement in such patients is limited. We analyzed the results of valve replacement in patients with this condition at our institution. METHODS: Between 1989 and 2002, 10 patients with antiphospholipid syndrome (8 women, 2 men; aged 38-73 years, mean 49 years) with severe mitral valve disease (n = 7), aortic valve disease (n = 2), or combined mitral-aortic disease (n = 1) underwent valve replacement. We reviewed retrospectively their clinical data, operative and posto

The influence of coronary artery disease on quality of life after mechanical valve replacement.

BACKGROUND AND AIM OF THE STUDY: Coronary artery disease (CAD) is known to impact negatively on long-term survival following valve replacement (VR). However, its influence on quality of life (QOL) remains undefined in patients with mechanical VR. METHODS: A total of 318 consecutive patients undergoing VR with the St. Jude Medical (SJM) mechanical valve were matched for age and gender with 318 patients who had VR (SJM valve) and coronary artery bypass grafting (VR+CABG). The VR group comprised 197 men and 121 women; the VR+CABG group also comprised 197 men and 121 women. The mean age of all pa

Noncompacted myocardium in Ebstein's anomaly: initial description in three patients.

Exercise intolerance in Ebstein's anomaly is usually attributed to desaturation secondary to right-to-left shunting as a result of a small or distorted left ventricle (LV), significant tricuspid valve regurgitation, right ventricular dysfunction, or a combination of these. We observed one boy (age 15 years) and two women (ages 20 and 29 years) with severe Ebstein's anomaly and strikingly abnormal LV myocardium resembling the features described for LV noncompaction. LV size and systolic function were normal in the two women; the boy had a dilated LV with severely diminished ejection fraction. T

[Surgical treatment of mitral valve malformations--25 years of experience].

During a period of 25 years (1979-2004), 24060 operations with extracorporeal circulation were performed. Of them 9536 (39.6%) were valvular procedures and 763 were combined with coronary bypass grafting. 4315 pts were operated (45.3% of valvular operations) because of mitral valve disease 5559 mitral valves were corrected, including multivalvular and combined procedures. Initially, mitral surgeries were predominant, but currently aortic valves are operated two times more frequently. In 85.5% of cases mitral valve replacement was necessary (with prosthetic valve), but 803 pts underwent reconst

Surgical treatment of late tricuspid regurgitation after left cardiac valve replacement.

BACKGROUND: The development of late tricuspid regurgitation (TR) following left cardiac valve replacement is an important complication, as it is associated with a severe impairment of exercise capacity and a poor symptomatic outcome. The pathogenesis of this condition remains poorly defined. It is still a challenge in terms of its prevention, treatment and indications for surgical correction. AIMS: To investigate the possible pathogenesis and report the surgical results of the late TR after left cardiac valve replacement. METHODS: There were 56 patients with moderate to severe TR after left

Cardiac autotransplantation for the treatment of permanent atrial fibrillation combined with mitral valve disease.

OBJECTIVE: The results of current surgical options for the treatment of permanent atrial fibrillation associated with mitral valve surgery are widely different, particularly for extremely dilated left atria. The aim of this study is to assess the efficacy of cardiac autotransplantation in restoring a normal sinus rhythm via a consistent reduction in the left atrium volume associated with a complete isolation of the pulmonary veins. METHODS: From April 2000 to April 2002, 28 patients (men/women, 5/23) underwent cardiac autotransplantation for the treatment of mitral disease and concomitant per
