Combined multiple-valve procedures: a five-year experience with 125 patients.

One hundred twenty-five patients underwent 128 operations for combined multiple-valve procedures, with an overall early mortality of 16%. Highest mortality was associated with mitral and tricuspid valve disease (28.5%), followed by triple-valve disease (18.2%) and aortic and mitral valve disease (14%). Left ventricular end-diastolic pressure, cardiac index, mean pulmonary artery pressure, pulmonary artery wedge pressure, and arteriovenous oxygen difference were all significantly different in patients with regard to early mortality. Late follow-up of 94% has been achieved in 105 early survivors

Does preservation of the posterior chordae tendineae enhance survival during mitral valve replacement?

During a 30-month period, 51 patients underwent mitral valve replacement. There were 3 hospital deaths (5.9%), 2 of which were due to ventricular rupture. The 3 patients who died were among 13 patients in whom mitral valve replacement was combined with tricuspid or aortic valve operation or both. Postmortem findings in the 2 patients who died of ventricular rupture showed that the ventricular tears were located between the atrioventricular groove and the unresected papillary muscle stumps, in an area of ventricle formerly tethered by the posterior chordae tendineae. In the last 14 patients in

Management of tricuspid valve insufficiency. Replacement with Hancock bioprosthesis or suture annuloplasty.

Forty-seven patients who underwent tricuspid valve replacement (TVR) or tricuspid annuloplasty (TA) combined with aortic (AVR) or mitral valve replacement (MVR) at Henry Ford Hospital, from 1971 to 1977 are reviewed. Thirty-three patients underwent TVR with a Hancock valve combined with MVR or AVR; hospital mortality was 30% (10 to 33) and was higher at the beginning of the series when smaller valves were used. No valve dysfunction has been seen with mean follow-up time of 36.7 months, ranging from 4 to 60 months. Good functional improvement was accomplished in all patients. Fourteen patients

Clinical evaluation of the Björk-Shiley tilting disc valve in the tricuspid position. Early and late results in 10 isolated and 51 combined cases.

Tricuspid valve replacement with the Björk-Shiley tilting disc valve was performed in 61 consecutive patients with either organic disease causing valve malfunction or functional regurgitation of severe degree. The early mortality rate was 21% (13/61) for the entire series. It was 10% (1/10) for tricuspid valve replacement alone, 27% (9/33) for mitral and tricuspid valve replacement, and 18% (3/17) for triple valve replacement. Age over 60 years, functional capacity group IV (N.Y.H.A.) and heart volume over 900 ml/m2 BSA were factors associated with a high operative mortality. There were 3 late

Thirty normal subjects who did not have valvular disease showed no evidence of tricuspid regurgitation or premature ventricular contractions during injection except in one case. On the other hand, 24 patients with combined aortic and mitral valve disease showed mild to moderate tricuspid regurgitation in 11 patients and severe tricuspid regurgitation in eight patients. Only two patients developed frequent run of premature ventricular contraction during injection. Se

[Results of implanting aortic valve xenografts into the tricuspid position].

From 1968 to 1976 transplantation of the tricuspid valve was performed on 47 patients with combined form of rheumatic heart disease. Hospital mortality was 36%. The follow-up period in 30 cases ranged from one to 7 1/2 years. Ten patients died in the late postoperative periods, only in one of them the cause of death was associated with incompetence of the transplanted valve. According to the findings of dynamic clinical follow-up, the function of the transplant in the other patients was quite satisfactory; at the same time, peculiarities of the phonocardiographic picture of valves treated by d

[Late results of closed methods of the surgical treatment of multiple valvular rheumatic heart defects].

The late results of surgical treatment of 178 patients with follow-up periods of 6 months to 8 years are analysed. The late results were much worse in stage IV disease, marked calcinosis of the valves, concomitant mitral or tricuspid regurgitation, intraatrial thrombosis, as well as in cases with a combination of these complications. Analysis of the actuarial curve showed that unfavourable dynamics of the remote outcomes were usually associated with inadequate aortic or tricuspid commissurotomy. It is underscored that a change in the surgical tactics in the direction of aortic valve prosthetic

Pathology of chronic rheumatic heart disease with particular reference to tricuspid value involvement.

This study is based on a retrospective analysis of 144 autopsied cases of rheumatic heart disease. Majority of cases (79%) were chronic and 21% were acute in nature. The mortality was high and was maximum below the age of 30 years. The males predominated over females. A high incidence of organic tricuspid valve involvement was observed (45.8%). Tricuspid stenosis was observed in 3 cases. Latter was associated with aortic and mitral valve involvement. Mitral stenosis was severe in all the three cases. Criteria, both gross and microscopic for the involvement of the tricuspid valve have been desc

Rheumatic aortic stenosis in young patients presenting with combined aortic and mitral stenosis.

This report describes 30 patients under the age of 30 years with rheumatic aortic stenosis, presenting with combined aortic and mitral stenosis. Three patients had additional tricuspid stenosis. Twenty-eight patients gave a history of rheumatic polyarthritis. The diagnosis was confirmed by right and left heart catheterisation in all. The murmur of aortic stenosis was not initially present in 8 out of 10 patients in congestive heart failure. Aortic valve calcification was not seen. Cineangiography showed a tricuspid aortic valve in all, unlike congenital aortic stenosis. A unique feature of thi

Straddling right atrioventricular valve in criss-cross atrioventricular relationship.

Two cases of complex congenital heart disease are described in which systemic and pulmonary bloodstreams crossed at the atrioventricular level. Both patients were examined clinically, echocardiographically, and by cardiac catheterization, including angiography, and both underwent cardiac surgery and had intraoperative mapping of their conduction systems. Both patients were found to have levocardia and situs solitus of viscera and atria, large ventricular septal defect, and straddling right atrioventricular valve. One patient had atrioventricular discordance but with the left ventricle anterior

Serratia marcescens endocarditis: a regional illness associated with intravenous drug abuse.

From 1969 to 1974, 19 cases of Serratia marcescens endocarditis were observed in the San Francisco Bay Area. Seventeen patients were intravenous drug users, and Serratia caused 14% of all addict-associated endocarditis in San Francisco. Serratia strains were nonpigmented and had typical antibiotic sensitivities, except that 9 of the isolates exhibited colonial variation, with each variant having different antibiotic sensitivities. Aortic or mitral valves were involved in 13 patients, and heart failure developed in 9 of these. Twelve patients had embolic episodes to brain, iliofemoral arteries,

Mitral valve replacement with ball and tilting disc valve prosthesis. A clinical and haemodynamic study.

A survey is presented on the results of 100 consecutive patients who underwent mitral valve (78 patients) or mitral and aortic and/or tricuspid valve replacement (22 patients) with ball or disc valve prosthesis. The patients were followed-up from 6 to 54 months postoperatively. The mortality for mitral valve replacement was 13 (17%) early deaths (up to one month from operation) and 5 (6%) late deaths. The corresponding figures for multiple valve replacements were 9 (41%) and 3 (14%). A direct correlation was found between early mortality and preoperative functional class IV of the New York Hea

Local deep hypothermia for combined valvular and coronary heart disease.

Local deep hypothermia was utilized as the sole method of myocardial protection in 113 consecutive aortic valve replacements. Thirty-six patients had simultaneous revascularization, mitral valve replacement, tricuspid valve replacement, or a combination of these. In follow-up for as long as 36 months there have been 3 hospital deaths (2.6%), all occurring in the group having isolated aortic valve replacement. No hospital deaths occurred among patients undergoing a combined procedure.

Pregnancy in patients after valve replacement.

This report is based on information obtained from a questionnaire sent to major cardiac centres in the United Kingdom. This produced details of 39 pregnancies in 34 patients after valve replacement. The 39 pregnancies gave rise to 30 healthy babies. The small size of the series probably reflects both the increasing rarity of young women with rheumatic heart disease in this country and the cautious attitude of their cardiologists. This makes it likely that these women represented the best end of the spectrum of cardiac function after valve replacement. Twenty-four pregnancies in 20 women who we

First heart sound and ejection sounds. Echocardiographic and phonocardiographic correlation with valvular events.

To provide additional information on the relation of valvular events to the principal components of the first heart sound (s1), combined echocardiograms and phonocardiograms were recorded in 49 subjects, chosen because of audible splitting of S1 or a combination of S1 and an ejection sound. The subjects included 14 normal persons, 16 patients with a variety of predominantly right-sided heart conditions, 7 with mitral stenosis, 3 with pulmonary stenosis and 9 with aortic valve disease or systemic hypertension. A precise relation was found between completion of closure of the atrioventricular (A

Cardiac pathology after valve replacement by disc prosthesis. A study of 61 necropsy patients.

Clinical and necropsy observations are described in 61 patients who had one or more cardiac valves replaced with a discoid prosthesis of the Hufnagel type. The most common (31 percent) cause of death among the 45 patients who died early (less than 65 days after operation) appeared to be prosthetic disproportion; that is, the prothesis was too big for the aorta or ventricular cavity into which it was inserted so that inadequate space was present between the margins of the disc and the endocardium of ventricle or intima of aorta. Prosthetic thrombosis occurred in only 3 of the 45 patients who di
