Right-ventricular outflow tract dysfunction is a common late sequela of congenital heart disease, particularly after tetralogy of Fallot repair. Balloon-expandable valves such as Melody and Sapien have transformed care for conduits and bioprostheses, but until recently most patients with large or irregular outflow tracts were not candidates for percutaneous therapy. This review appraises the Venus P-Valve, the first self-expanding transcatheter pulmonary valve for large right-ventricular outflow tracts, summarizing evidence on efficacy, safety, and regulatory status. We performed a narrative r

Cavalier King Charles Spaniels (CKCS) are predisposed to a hereditable form of myxomatous mitral valve disease (MMVD) with a high incidence and a high risk of disease progression. Gender is recognized as a risk factor for the development of the disease in the breed. However, patient's morphological traits associated with disease progression have not been identified. The aim of this study was to analyze the association between preclinical MMVD progression and morphometric features in CKCSs. This was a retrospective, time-to-event, longitudinal study. Medical records of 302 CKCSs were reviewed f

BACKGROUND: For patients undergoing surgical valve procedures with concomitant coronary artery disease, current guidelines recommend that coronary artery bypass grafting (CABG) should be anatomically guided on the basis of stenosis severity, as assessed by coronary angiography. We aimed to test whether a physiologically guided strategy using angiography-derived fractional flow reserve (FFR) could improve clinical outcomes in this population.
METHODS: FAVOR IV-QVAS is an investigator-initiated, multicentre, randomised, triple-blind trial done at 12 tertiary hospitals in China. Eligible patient

Stroke is a major cause of death worldwide, with most cases being ischemic in presentation. Cardioembolic phenomenon contributes to a significant number of ischemic strokes. Cardiac myxomas, although generally benign, are the most common tumors arising in the left atrium. Left atrial tumors tend to grow into the atrial lumen and can cause symptoms similar to heart failure with mitral valve disease. Cardiac myxomas have typical pedunculated, gelatinous features and are at risk of microthrombi formation. However, when these typical features are not present, sessile cardiac myxomas can be mistake

Mitral annular calcification (MAC) is a common, degenerative calcific process of the mitral valve annulus readily identified on echocardiography and classifiable by extent as mild, moderate, or severe. Epidemiologic and registry data link MAC to older age, metabolic risk factors, female sex, and markers of cardiac remodeling. MAC is associated with impaired coronary microvascular function, suggesting endothelial dysfunction as a mechanistic bridge to coronary artery disease (CAD). Multiple imaging- and angiography-based studies report a consistent association between MAC and prevalent, severe,

A 60-year-old woman presented with polyarthritis in the aftermath of an upper respiratory tract infection. Further investigation revealed infective mitral valve endocarditis necessitating acute surgical valve replacement due to Streptococcus dysgalactiae, as evidenced by positive blood and intraoperative cultures, with disseminated infection and septic emboli in kidneys, multiple joints, and brain. Subsequent colonoscopy revealed a hitherto undetected rectal adenocarcinoma. The co-occurrence of S. dysgalactiae endocarditis and colorectal malignancy in this patient, who was within the age range

Paper-based microfluid devices have great potential to perform complex assays like nucleic acid amplification tests (NAATs) in point-of-care (POC) and low resource settings, but have not often succeeded because there is frequently need for affordable and simple solutions to address complex fluidics and sample delivery without risk of contamination. These issues are exacerbated when multiple sample processing procedures must be automated by valve integration in the absence of advanced supportive instrumentation or trained personnel. For NAAT application in remote or home environments, valving w

Neisseria gonorrhoeae (NG), a commonly encountered sexually transmitted pathogen, rarely presents with cardiac involvement and therefore may be underrecognized in this context. Gonococcal infective endocarditis (IE) is associated with an aggressive clinical course characterized by rapid valvular destruction and systemic embolization. Diagnosis is often challenging, particularly when standard blood cultures fail to isolate the causative organism. A 53-year-old man was admitted following a ventricular fibrillation arrest associated with ST-segment elevation and an acute headache. Magnetic resona

Multimodal imaging is a key part of infective endocarditis (IE) diagnostic guidelines. However, we present a case of confirmed IE where evidence of disease was absent on multiple imaging modalities. A patient was admitted following multi-territory ischemic strokes. He had had an aortic root graft with tissue aortic valve replacement and a previous ischemic stroke. During admission, the patient developed pyrexia of unknown origin. Four blood cultures were positive for Staphylococcus haemolyticus. BACKGROUND: Severe aortic stenosis (AS) and mitral regurgitation (MR) are frequently undertreated and characterized by persistent sex, racial and ethnic, socioeconomic, and geographic disparities despite effective valve therapies. Whether automated electronic clinician notification (ECN) alerts improve the evaluation and treatment of AS and MR across health systems is unknown.
OBJECTIVES: The purpose of this study was to evaluate whether ECN alerts improve guideline-directed evaluation and treatment of significant AS and MR across multiple health systems.
METHODS: ALERT is a multisystem, clu

Minimally invasive, long-term, and precisely controlled drug delivery is essential for treating bladder diseases such as interstitial cystitis and bladder cancer. However, conventional approaches, including injection-based delivery and indwelling catheters, offer limited controllability, cause patient discomfort, and increase the risk of infection and tissue irritation. Existing intravesical devices further lack active control over drug release, are restricted to single therapeutic agents, and may induce bladder overactivity due to continuous mechanical stimulation. Here, we present a strategy

OBJECTIVES: A higher frequency of valvular heart disease is seen among Systemic Sclerosis (SSc) patients. Advanced aortic valve stenosis leads to significant morbidity in these patients with multiple other comorbidities. In this study, we aim to define the short- and long-term outcomes of transcatheter aortic valve replacement (TAVR) procedures in SSc patients.
METHODS: We undertook a retrospective chart review of all patients with SSc who underwent a TAVR procedure at our institution over a defined 11-year period. Demographics as well as short- and long-term outcomes were identified.

Primary congenital glaucoma is a rare but vision-threatening disease. Transscleral micropulse cyclophotocoagulation (MPCPC) delivers diode laser energy in short pulses with intervening off periods, allowing tissue cooling and potentially reducing adjacent thermal damage. However, evidence on MPCPC for childhood glaucoma among Japanese patients remains limited. In this series, we describe our experience with MPCPC in refractory cases of primary congenital glaucoma. All procedures were performed under general anesthesia. All four patients had primary congenital glaucoma that had remained uncontr

Gastroesophageal reflux disease (GERD) has been associated with transient lower esophageal sphincter relaxations (TLESRs), which are considered a predominant mechanism of reflux and are modulated in part by the crural diaphragm. Long-term proton pump inhibitor (PPI) therapy remains the standard of care but is associated with drug dependency and systemic adverse effects. Strengthening the crural diaphragm through resistive respiratory training represents a mechanistically plausible non-pharmacological approach. We report a case in which a 14-day isometric diaphragmatic resistance training proto

INTRODUCTION: Aortic stenosis (AS) and hypertension (HTN) frequently co-exist in older adults, worsening disease progression and increasing mortality risk compared to either condition alone.
AIM: To examine mortality trends and demographic-geographic disparities from comorbid AS and HTN in the U.S. from 1999-2020.
METHODS: Data were extracted from the CDC WONDER Multiple Cause of Death database. Crude and age-adjusted mortality rates (CMRs, AAMRs) per 100,000 were calculated. Joinpoint regression estimated annual (APC) and average annual percent change (AAPC). Trends were projected to 2030 u

Background: Segmental arterial mediolysis (SAM) is a non-inflammatory vasculopathy that primarily affects the abdominal visceral arteries leading to hemorrhage, ischemia, or pseudoaneurysms. Its presentation can be mimicked by other vasculopathies including vasculitis involving the medium-sized blood vessels making it difficult to diagnose. Case Presentation: A 55-year-old woman presented with a two-hour history of sudden-onset, severe epigastric pain radiating to the chest. She was noted to be hypotensive with low hemoglobin 8.8 g/dL suspicious for a hemorrhagic cause. Her case was complicate

Background: Hypoplastic left heart syndrome (HLHS) is defined as "a spectrum of congenital cardiovascular malformations with normally aligned great arteries without a common atrioventricular junction, characterized by underdevelopment of the left heart with significant hypoplasia of the left ventricle including atresia, stenosis, or hypoplasia of the aortic or mitral valve, or both valves, and hypoplasia of the ascending aorta and aortic arch". Without treatment, HLHS is usually lethal in the neonate. Many hypotheses have been advanced to explain the etiology of HLHS; however, no single theory
