Comprehensive genetic analysis based on multi - omics reveals novel therapeutic targets for mitral valve prolapse and drug molecular dynamics simulation.

OBJECTIVE: Mitral valve prolapse (MVP), the most prevalent primary valvular disease, serves as a direct risk factor for multiple cardiovascular disorders and exhibits a high prevalence in the general population. As no specific pharmacological therapies currently exist for MVP, the identification of precise therapeutic targets is imperative. METHOD: We conducted comprehensive causal genetic inference by integrating genetic data from expression quantitative trait loci (eQTL) and genome-wide association studies (GWAS). Analytical approaches included Mendelian Randomization (MR), colocalization a

From Symptoms to Solution: A Diagnostic Challenge of Whipple's Disease.

Whipple's disease is caused by the bacterium Tropheryma whipplei. The classic presentation of this disease consists of arthralgias, weight loss, diarrhea, and abdominal pain. However, Whipple's disease can also have other manifestations across multiple organ systems, ranging from lymphadenopathy to pleuropulmonary disease to endocarditis to skin hyperpigmentation and even CNS involvement. We present the case of a 39-year-old male with multiple organ system manifestations of Whipple's disease. He presented with a 50-pound unintentional weight loss, early satiety, arthralgias, chronic cough, ple

Proteomic Analysis of Valsartan for Attenuating Disease Evolution in Early Sarcomeric Hypertrophic Cardiomyopathy (VANISH) Clinical Trial.

BACKGROUND: In hypertrophic cardiomyopathy (HCM), the mechanisms through which pathogenic sarcomere variants (G+) lead to left ventricular hypertrophy (LVH) are not understood. METHODS: VANISH (Valsartan for Attenuating Disease Evolution in Early Sarcomeric Hypertrophic Cardiomyopathy) was a multicenter, double-blind, placebo-controlled, randomized trial testing valsartan's ability to attenuate phenotypic progression in early sarcomeric (G+LVH+) and subclinical HCM (G+LVH‒). The outcome was a composite z-score reflecting change in cardiac remodeling from baseline to year 2 (end of study). Bas

The role of echocardiography in the diagnosis of heart failure with preserved ejection fraction.

Heart failure (HF) with preserved ejection fraction (HFpEF) is the most common form of HF in older adults. While manifest as distinct clinical phenotypes, almost all patients with HFpEF will present with exercise intolerance or exertional dyspnea. Distinguishing HFpEF from other clinical conditions remains challenging, as the accurate diagnosis of HFpEF involves integrating a diverse array of cardiovascular (CV) structural and physiologic inputs. Owing to its intrinsic ability to characterize the structure and function of the myocardium, cardiac valves, pericardium, and vasculature, echocardio

Children are Less Likely Than Adults to Develop Complete Heart Block Following TAVR.

TAVR is an alternative to surgical aortic valve [AoV] replacement. In adults, rates of atrioventricular block [AVB] requiring permanent pacemaker [PPM] placement in the modern era are 4-24%. Post-TAVR conduction abnormality incidence and risk factors are unknown in children and young adults. Describe post-TAVR conduction abnormalities in children and young adults. Retrospective single-center review of patients undergoing TAVR (9/2014 to 6/2021). Patients with pre-existing complete AVB or PPM were excluded (N = 1). The relationship between described adult risk factors for AVB and primary outcom

Tricuspid valve leaflet remodeling in sheep with biventricular heart failure: A comparison between leaflets.

Tricuspid valve leaflets are dynamic tissues that can respond to altered biomechanical and hemodynamic loads. Each leaflet has unique structural and mechanical properties, leading to different in vivo strains. We hypothesized that these intrinsic differences drive heterogeneous, disease-induced remodeling between the leaflets. Although we previously reported significant remodeling changes in the anterior leaflet, the responses among the other two leaflets have not been reported. Using a sheep model of biventricular heart failure, we compared the remodeling responses between all tricuspid leafl

Transaxillary TAVR Complicated by Subclavian Pseudoaneurysm and Vertebral Artery Sacrifice.

OBJECTIVES: We describe the endovascular management of multiple vessel-related complications in a case of transcatheter aortic valve replacement (TAVR) through surgical right transaxillary access. KEY STEPS: We present the case of a TAVR performed through the right transaxillary approach and detail the treatment of an early subclavian dissection and a subacute subclavian pseudoaneurysm, successfully managed through endovascular interventions. POTENTIAL PITFALLS: Significant subclavian disease can raise the questions whether to prepare the vessel before the procedure and whether to proceed sh

Aortic stenosis intricacies beyond the valve: A bidirectional Mendelian randomization study.

AIMS: Numerous observational studies have reported an association between calcific aortic valve stenosis (CAVS) and an increased risk of other cardiovascular diseases (CVDs). However, the potential causal association is uncertain. METHODS AND RESULTS: A bidirectional Mendelian randomization (MR) analysis was performed to confirm the causal association between CAVS and the risk of seven common CVDs. The meta-analysis was used to estimate the combined causal effect with multiple MR results. The MR analysis indicated that genetically predicted CAVS is associated with the increased risk of heart

Aortic Valve Replacement in Women: A Pooled Analysis of the RHEIA and PARTNER 3 Trials.

BACKGROUND: In women with severe aortic stenosis, there are limited data regarding outcome differences following transcatheter (TAVR) vs surgical aortic valve replacement (SAVR). OBJECTIVES: The authors sought to examine outcomes of TAVR vs SAVR in a patient-level pooled analysis of women in the RHEIA and PARTNER 3 trials. METHODS: Patients in both trials were randomly allocated to a balloon-expandable SAPIEN 3/Ultra valve or to surgical bioprostheses. Individual patient data of female participants in the 2 trials were pooled. The primary endpoint was all-cause mortality, all stroke, or reho

Transcatheter Treatment of Patient With Mitral Valve Endocarditis and Severe Aortic Stenosis and Insufficiency.

BACKGROUND: Patients in cardiogenic shock secondary to endocarditis and multivalvular heart disease have a high surgical risk. Transcatheter valve aspiration followed by transcatheter valve intervention might be a strategy to salvage carefully selected patients. CASE SUMMARY: A 63-year-old woman with subaortic membrane and severe aortic stenosis/regurgitation was scheduled for membrane resection, aortic root enlargement, and aortic valve replacement. Two weeks before operation, she developed mitral valve endocarditis. Because of cardiogenic shock, she was placed on venoarterial extracorporeal

Limited-Access Zero-Contrast BASILICA Transcatheter Aortic Valve Replacement.

OBJECTIVE: Severe anaphylactic reactions to intra-arterial contrast agents, access site limitations, and high-risk coronary artery occlusion may all possibly prohibit the success of a transcatheter aortic valve replacement (TAVR). We describe a case including all 3 of these components and demonstrate the importance of multimodality imaging, including computed tomography (CT), fluoroscopy, and trans-esophageal echocardiography (TEE). KEY STEPS: 1) Obtain orthogonal BASILICA angle from CT. 2) Set up a single femoral access for BASILICA and subsequent TAVR. 3) Place a coronary wire with the radi

From Dental Procedures to Endocarditis: Cardiobacterium hominis Infection of a Bioprosthetic Mitral Valve.

Prosthetic valve endocarditis (PVE) is an extremely rare but serious complication in patients with bioprosthetic heart valves following procedures such as dental work, especially when prophylactic antibiotics are not administered. We present the case of a 67-year-old male with a bioprosthetic mitral heart valve who developed subacute endocarditis caused by Cardiobacterium hominis after undergoing a dental procedure without antibiotic prophylaxis. The patient presented to the emergency department with a six-month history of worsening fatigue, weakness, intermittent fevers, and lower extremity e

Biomarkers of postoperative cardiac surgery-associated acute kidney injury: Narrowing the field.

BACKGROUND: Cardiac surgery-associated acute kidney injury (CSA-AKI) is commonly observed after cardiac surgery and has been shown to be associated with increased morbidity and mortality. This study was conducted using the Kidney Disease Improving Global Outcomes (KDIGO) criteria to analyze potential perioperative biomarkers of CSA-AKI. METHODS: Blood was collected from patients intraoperatively on entry into the pericardium and at 4, 12, 24, and 48 hours postoperatively. Repeated-measures, mixed-model analysis was conducted to determine which cytokines and/or chemokines were associated with

Early Stent Frame Fracture of SAPIEN 3 Valve in the Pulmonary Position.

A 15-year-old post-REV patient underwent transcatheter pulmonary valve replacement with a 23 mm SAPIEN 3 for right ventricular outflow tract (RVOT) stenosis and severe regurgitation. At 9 months, recurrent stenosis revealed multiple frame fractures. Balloon redilation up to 24 mm was limited by aortic root compression and recoil, prompting surgery. This first reported case of early SAPIEN 3 fracture in the pulmonary position highlights structural risks in complex RVOT anatomies.

Anesthesia Experience With Aortic Valve Replacement Guided by Blood Viscoelasticity Testing in a Late Post-Fontan Patient: A Case Report.

Coagulation management poses a challenge for post-Fontan patients requiring anesthesia for aortic valve replacement (AVR). This is because post-Fontan patients have a history of multiple sternotomies and resultant strong adhesions, in addition to being potentially deficient in coagulation factors, as observed in several post-Fontan patients. Moreover, coagulation factors are further diluted and consumed during cardiopulmonary bypass (CPB). In this report, we present the anesthetic management during AVR in a post-Fontan patient. A 32-year-old post-Fontan female patient underwent valve replaceme

Flow Hemodynamics in Children with Tetralogy of Fallot: A Comparison Study of Cardiac Catheterization and Phase-Contrast MRI.

Pulmonary hemodynamics are necessary to measure for lifelong surveillance of cardiovascular function in children with tetralogy of Fallot (TOF). To measure cardiac index, invasive cardiac catheterization using the Fick principle or phase-contrast MRI (PC-MRI) can be used. However, estimates of oxygen consumption and other assumptions used in the Fick method may lead to significant inaccuracy, which are further exacerbated by disease characteristics such as pulmonary regurgitation. By comparison, PC-MRI can directly measure blood flow and regurgitant fraction. To investigate the potential compl

Cardiovascular Aging.

Aging is a slow, progressive, and inevitable process that affects multiple organs and tissues, including the cardiovascular system. The most frequent cardiac and vascular alterations that are observed in older adults (especially patients aged ≥80 years) are diastolic and systolic dysfunction, progressive stiffening of the vascular wall and endothelial impairment usually driven by an excess of extracellular matrix (ECM) and profibrotic substances, reduced levels of matrix metalloproteinases (MMPs), or by amyloid and calcium deposits in myocardium and valves (especially in aortic valves). A Rare Case of Pediatric Infective Endocarditis Initially Presenting as Meningitis in a Previously Healthy Child.

Infective endocarditis (IE) is a rare but life-threatening condition in children, particularly in those without heart disease. Delayed diagnosis can lead to serious systemic complications. We report the case of an eight-year-old previously healthy girl, initially misdiagnosed with bacterial meningitis, who developed multiple septic embolic events (cerebral infarcts, digital gangrene, and splenic and renal infarction) despite being on antibiotic therapy. Repeat echocardiograms revealed mitral valve vegetation and an aortic root abscess. Blood cultures (aerobic and anaerobic) grew methicillin-se

Hodgkin Lymphoma: An Unusual Presentation With Hemophagocytic Lymphohistiocytosis.

Hodgkin lymphoma (HL) can present with non-specific systemic symptoms, often mimicking infection, autoimmune disease, or post-surgical complications. When compounded by recent major surgery and ongoing inflammatory features, diagnosis can be significantly delayed. This case highlights the diagnostic complexity of HL in a postoperative setting, complicated by recurrent sepsis-like presentations, persistently raised inflammatory markers, and overlapping differentials, including graft infection and hemophagocytic lymphohistiocytosis (HLH). A 71-year-old male underwent elective abdominal aortic an
