Role of Telemedicine in Congenital Heart Disease Care Models in Low- and Middle-Income Countries: A Scientific Statement From the American Heart Association.

Telemedicine is increasingly recognized as a transformative strategy to address critical challenges in low- and middle-income countries, which comprise 63% of all nations, 84% of the global population, and 90% of children <15 years of age. These regions experience multiple barriers, including geographic remoteness, economic constraints, limited healthcare infrastructure, and a shortage of trained healthcare professionals. Congenital heart disease, a highly specialized field often centralized in urban centers, is particularly affected, leaving many children without timely or equitable access to

Rheumatic Heart disease in the Young: Treatment needs, Hospitalization burden, and Measuring economic impact: The RHYTHM Study.

INTRODUCTION: Acute rheumatic fever (ARF) and its sequela, rheumatic heart disease (RHD), continue to be major contributors to morbidity and mortality among acquired pediatric cardiac diseases in underprivileged populations. The resulting health expenditures lead to catastrophic financial strain on affected families. MATERIALS AND METHODS: This study analyzed 48 children admitted with ARF or RHD. Data collection focused on total healthcare costs, out-of-pocket expenditures (OOPE) by families, and associated financial impact. RESULTS: A total of 70 hospital admissions were evaluated. The mean

Implementation of a national rheumatic fever and rheumatic heart disease prevention and control program in Egypt: rationale and objectives.

BACKGROUND: Rheumatic heart disease (RHD) remains a major public health challenge in Egypt, with a school-age prevalence of 0.9%-2.5%. Despite national initiatives since 2006, control has been constrained by low public and provider awareness, suboptimal secondary prophylaxis, misdiagnosis, and fragmented surveillance. This report outlines the rationale and structure of Egypt's national RF/RHD prevention and control program, which integrates screening, digital surveillance, capacity building, and research to support scalable RHD control in endemic settings. METHODS: The program comprises (1) m

CAR T cells for autoimmune diseases: 2026 update of the EBMT-ISCT-JACIE practice harmonisation and consensus recommendations.

Autoimmune diseases are a heterogeneous group of chronic diseases characterised by loss of immune tolerance with substantial morbidity and mortality. Since 2021, chimeric antigen receptor (CAR) T cells have recently emerged as a promising treatment option for patients with severe, refractory autoimmune diseases. In 2024, an international expert panel from the European Society for Blood and Marrow Transplantation (EBMT), the International Society for Cell and Gene Therapy (ISCT), and the Joint Accreditation Committee of ISCT and EBMT (JACIE) released the first best-practice recommendations on C

The association between systemic inflammation as measured by calprotectin and autoantibody load in rheumatoid arthritis.

BACKGROUND: Plasma calprotectin is a marker of neutrophil activity. Its relation to the clinical phenotype and autoantibodies in rheumatoid arthritis (RA) is only partly understood. We therefore aimed to investigate the association between calprotectin and clinical parameters, CRP, ESR, anti-CCP2 and other anti-modified protein autoantibodies (AMPAs) in patients with RA. METHODS: Rheumatoid factor (RF), anti-cyclic citrullinated peptide 2 (CCP2) isotypes and plasma calprotectin were centrally measured in a cross-sectional cohort of 4521 Scandinavian RA patients using fluoroenzyme immunoassays

Contemporary approaches to Sydenham's chorea - where evidence meets reality.

INTRODUCTION: Sydenham's chorea (SC) is the most common acquired chorea of childhood and a major neurological manifestation of acute rheumatic fever. Although often considered self-limiting, it can cause substantial motor, psychiatric, cognitive, educational, and social morbidity, with relapses in a relevant proportion of patients. AREAS COVERED: In this critical perspective, the authors critically examined current approaches to SC management, focusing on three complementary pillars: eradication of Group A β-hemolytic streptococcal infection and long-term secondary antibiotic prophylaxis; sym

Trends in circulatory system disease hospitalizations in England and Wales: A 21-year secular-trend analysis.

Cardiovascular diseases (CVDs) are the primary reason for mortality worldwide. Patients with CVD were at higher risk of drug-related adverse events and functional impairment and were more likely to be admitted to the hospital. This study aims to study trends for CVD hospital admission (HA) in England and Wales during the past 21 years. A secular-trend analysis study design was utilized to explore the trend of CVD HAs. Poisson regression was used to examine the trend in HAs. To determine trends in admission rates over time, we implemented Joinpoint regression analysis. The permutation test was

Penicillin, an elixir to safeguard myocardial function in patients with rheumatic heart disease?

OBJECTIVE: Valvular heart diseases occur following different evitable and inevitable etiologies. Rheumatic valvular heart disease is among preventable etiologies and is almost disappeared in developed nations. In recent literature there are few low-quality evidences available about therapeutic efficacy of penicillin in maintaining and enhancing myocardial function in patients with rheumatic heart disease. In present study we aimed to investigate importance of penicillin in maintaining myocardial function indices within normal range. METHODS: In this retrospective descriptive-analytic study we

Critical Rheumatic Mitral Stenosis Unmasked During Pregnancy Complicated by Preeclampsia With Severe Features and Managed With Urgent Balloon Mitral Valvuloplasty.

Pregnancy imposes significant hemodynamic changes that can unmask previously undiagnosed cardiovascular disease and complicate management when concurrent obstetric conditions are present. We present the case of a 32-year-old immigrant woman at 21 weeks' gestation with a limited medical history due to a significant language barrier, who presented with hypertensive crisis and was diagnosed with preeclampsia with severe features. During her evaluation, she developed worsening dyspnea and acute respiratory decompensation. Further cardiac assessment revealed critical rheumatic mitral stenosis, acco

From detection to retention: redefining success in screening for rheumatic heart disease in Sub-Saharan Africa.

Rheumatic heart disease remains a leading cause of preventable cardiovascular morbidity and premature mortality among children and young adults in Sub-Saharan Africa. Advances in portable echocardiography, standardised World Heart Federation diagnostic criteria, and task-sharing strategies have substantially expanded the detection of latent rheumatic heart disease. However, increased diagnostic capacity has not consistently translated into improved long-term clinical outcomes. This viewpoint argues that the principal challenge in rheumatic heart disease control has shifted from underdiagnosis

Acute Rheumatic Fever: Atrial Flutter Associated With Carditis.

BACKGROUND: Acute rheumatic fever (ARF) is an immune-mediated sequela of group A streptococcal infection that can progress to rheumatic heart disease (RHD), defined by permanent rheumatic valvular damage. Although uncommon in the United States, delayed recognition may lead to advanced valve disease and arrhythmias. CASE SUMMARY: A 10-year-old African-American male presented after syncope following 4 months of unexplained bilateral ankle arthritis. He was found to be in atrial flutter. Echocardiography showed a flail mitral valve with severe regurgitation, moderate tricuspid regurgitation, and

Allelic Frequency of Single-Nucleotide Polymorphisms in CYP2C9, VKORC1, and CYP4F2 Genetic Variants in Patients With Mechanical Heart Valves on Acenocoumarol Drug Therapy.

Background Patients with mechanical heart valves require lifelong anticoagulation with vitamin K antagonists (VKAs), such as acenocoumarol. However, acenocoumarol has a narrow therapeutic index, and genetic variants in CYP2C9, VKORC1, and CYP4F2 significantly influence individual dose requirements and the risk of adverse events. Limited data exist on the prevalence of these clinically important variants in Central Indian populations. Objectives To determine the frequency of clinically significant genetic variants, alleles, and metabolizer phenotypes of the CYP2C9, VKORC1, and CYP4F2 genes, and

Long-Term Changes and Disparities in the Global Burden of Rheumatic Heart Disease between 1990 and 2021: An Analysis Based on the Global Burden of Disease (GBD) Study.

HIGHLIGHTS: Global age-standardized mortality and DALY rates of rheumatic heart disease (RHD) declined substantially between 1990 and 2021 (56% and 53%, respectively), reflecting major progress in disease control. Years of life lost (YLL) decreased by 59%, whereas years lived with disability (YLD) increased by 11%, indicating a shift from fatal outcomes toward chronic nonfatal burden. Marked sociodemographic disparities persist, with low- and low-middle-SDI regions-particularly South Asia-bearing a disproportionate share of global RHD deaths and DALYs. This comprehensive GBD 2021 analysis prov

OBJECTIVES: Systemic sclerosis (SSc) is characterised by generalised vasculopathy and multi-organ fibrosis. Cognitive impairment may develop among SSc patients, affecting attention, memory and solving complicated tasks. Previous studies have identified microvascular abnormalities in the brain; however, no complex investigation has clarified these cognitive symptoms. This study examined the associations between cognitive functions and detailed clinical parameters regarding SSc. METHODS: One hundred and sixty patients with SSc and 62 age- and sex-matched healthy controls were studied and follow
