Autologous pericardial soft-ring annuloplasty may offer improved long-term control of rheumatic functional tricuspid regurgitation: a propensity-matched cohort study.

BACKGROUND: To compare the long-term efficacy of autologous pericardial soft-ring annuloplasty (P-TVP) versus prosthetic ring annuloplasty (A-TVP) for functional tricuspid regurgitation (FTR) secondary to rheumatic heart disease (RHD). METHODS: A total of 251 consecutive RHD patients undergoing tricuspid annuloplasty from December 2013 to December 2023 were retrospectively analyzed. Among them, 139 underwent P-TVP and 112 received A-TVP. Propensity score matching (1:1) yielded 64 comparable pairs. Follow-up at 6,24,48 and 96 months assessed TR recurrence and right-heart remodeling. Multiple i

Premature mortality projection for cardiovascular diseases to 2030: a sub-national evaluation towards the SDG 3.4 goals in China.

BACKGROUND: Sustainable Development Goal (SDG) 3.4 aims to reduce premature mortality from non-communicable diseases (NCDs) by one-third by 2030 compared to 2015 levels. To assess whether achieving the goal in China by 2030 is feasible, we investigated the subnational progress in reducing premature mortality rates from cardiovascular diseases (CVD) in China by 2030. METHODS: We used national and provincial results of China from the Global Burden of Disease (GBD) 2023 as empirical data for projections. For each sex, province, and year, the probability of death due to CVD was calculated from th

Bringing severe non-communicable disease care to district-level hospitals in Nepal: PEN-Plus experience and regional policy implications.

Severe non-communicable diseases (NCDs) such as type 1 diabetes mellitus (T1DM), sickle cell disease (SCD), thalassaemia and congenital heart conditions are prevalent in low-resource settings but remain largely unaddressed in the primary care system. Nepal's Package of Essential NCD Interventions-Plus (PEN-Plus) initiative demonstrates how peripheral provincial hospitals (former district hospitals) can effectively deliver decentralised, equitable care for childhood-originated NCDs such as T1DM, SCD, thalassaemia, rheumatic and congenital heart diseases, epilepsy and childhood asthma. Led by th

Infections of Selected Nonvalvular Cardiovascular Devices: A Science Advisory From the American Heart Association.

Nonvalvular cardiovascular device infections, although rare in most cases, represent substantial clinical challenges due to their life-threatening potential and diagnostic complexity. Although infections associated with prosthetic valves, cardiovascular implantable electronic devices, vascular grafts, and mechanical circulatory support devices have garnered considerable attention and specific guidance, infections of other nonvalvular cardiovascular devices have not. Therefore, this Science Advisory focuses on a contemporary profile of other nonvalvular device infections and includes an algorit

Revisiting an immunoneuropsychiatry classic: Sydenham's chorea.

INTRODUCTION: Sydenham chorea (SC) is the major neurologic manifestation of rheumatic fever. It is defined by characteristic involuntary movements and frequently associated with neuropsychiatric symptoms that may develop weeks or months following a Group A beta hemolytic streptococcal (GAS) infection. AREAS COVERED: The authors searched PubMed database for recent original studies and systematic reviews on SC pathogenesis, clinical spectrum and therapeutic approaches. EXPERT OPINION: Compelling evidence supports that autoantibodies targeting neuronal antigens underly the pathogenesis of SC. I

A non-conjugated peptide-adjuvant system of synthetic peptide-vaccine elicited potent antibody response and opsonic protection against clinical GAS strains.

Group A Streptococcus (GAS) is a causative agent of life-threatening infections, including pharyngitis, rheumatic fever, and rheumatic heart disease. Currently, no commercial vaccines are licenced to prevent GAS infections. Synthetic peptide-based subunit vaccines composed of minimal antigenic fragments derived from the pathogenic M-protein, represent an attractive approach for mitigating GAS infection transmission. However, such small antigens are poorly immunogenic, necessitating their co-administration with strong immunostimulatory agents (adjuvants). Herein, we proposed the use of self-ass

Metabolomic Atlas of Cardiovascular Diseases: Mapping Shared and Specific Signatures.

BACKGROUND: Cardiovascular disease (CVD) remains the leading global cause of death. While metabolic dysregulation is central to CVD pathogenesis, the extent to which distinct clinical subtypes exhibit unique or shared metabolic signatures remains unclear. OBJECTIVES: The purpose of this study was to systematically characterize metabolomic patterns across a broad spectrum of CVD subtypes and to delineate both shared and subtype-specific metabolic features. METHODS: We analyzed nuclear magnetic resonance-based metabolomic data (325 metabolites) from 244,567 UK Biobank participants, including 2

Cardiovascular Involvement in Autoimmune Rheumatic Diseases: Evaluation Using Cardiovascular MR Imaging.

One-third of the mortality in autoimmune rheumatic diseases (ARDs) is due to cardiovascular disease (CVD). CVD in ARDs includes: (1) macro- or microvascular coronary artery disease (CAD), (2) inflammatory cardiomyopathy, due to myocardial microvasculopathy, myopericarditis, (3) heart failure with preserved ejection fraction or reduced ejection fraction, (4) valvular heart disease, and (5) non-coronary vascular disease. Cardiovascular magnetic resonance (CMR), a noninvasive imaging modality, provides functional and tissue characterization information and is the ideal tool for diagnosis/follow-u

Prosthetic Valve Thrombosis: Current Management, Surgical Treatment, and the Evolution of Thrombolysis.

Prosthetic valve thrombosis (PVT) remains one of the most serious and potentially life-threatening complications of mechanical heart valves and continues to pose a major clinical challenge worldwide. Its incidence varies widely depending on anticoagulation quality, valve type, and access to specialized care, with a higher burden in regions where rheumatic heart disease remains prevalent. Historically, urgent surgery was considered the standard therapy because it provides immediate restoration of valve function; however, operative mortality is significant, particularly in critically ill patient

Transseptal Mitral Valve-in-Valve Implantation Using an Arteriovenous Loop.

BACKGROUND: Transseptal mitral valve-in-valve (ViV) implantation is an established therapy for failed surgical bioprostheses. However, massive biatrial enlargement can preclude delivery, leading to procedural failure. CASE SUMMARY: A 75-year-old woman presented with heart failure after surgical mitral bioprosthesis implantation. Echocardiography revealed severe mitral bioprosthetic failure and massive biatrial enlargement. Computed tomography angiography showed a septal-annular angle of 106°. During the procedure, the stiff wire and the delivery system repeatedly prolapsed into the giant atri

Early Implementation and Clinical Outcomes of an Integrated Rheumatic Heart Disease Screening and Treatment Program in Northern Uganda.

BACKGROUND: The World Health Organization recommends echocardiographic screening and secondary prophylaxis for rheumatic heart disease (RHD), but evidence on how to implement these interventions within healthcare systems is limited. We report early implementation and clinical outcomes of an RHD program in northern Uganda. METHODS: From May 2023, the District Health Office in Kitgum trained primary healthcare workers in RHD screening and implemented registry-based secondary antibiotic prophylaxis in public facilities. We analyzed program and registry data through December 31, 2024. Data were o

RNA methylation in cardiovascular remodeling: Molecular mechanisms, biomarkers, and therapeutic strategies.

N6-methyladenosine (m6A) and 5-methylcytosine (m5C) are dynamic and reversible RNA modifications that play important roles in cardiovascular diseases (CVDs). By regulating RNA stability, splicing, transport, translation, and degradation, m6A and m5C shape key pathological processes including endothelial dysfunction, inflammation, apoptosis, fibrosis, impaired contractility, and metabolic remodeling. Core regulators, including METTL3, METTL14, fat mass and obesity-associated protein (FTO), AlkB homolog 5 (ALKBH5), and YTH family proteins for m6A, as well as NSUN2, DNMT2, TET2, and ALYREF YBX1 f

Interleukin-1β Drives Disease Progression in Arrhythmogenic Cardiomyopathy.

Arrhythmogenic cardiomyopathy (ACM) is a genetic form of heart failure that affects 1 in 5,000 people globally and is caused by mutations in cardiac desmosomal genes including PKP2, DSP, and DSG2. Individuals with ACM suffer from ventricular arrhythmias, sudden cardiac death, and heart failure. There are few effective treatments and heart transplantation remains the best option for many affected individuals. Here we performed single nucleus RNA sequencing and spatial transcriptomics on myocardial samples from patients with ACM and control donors. We identified disease-associated spatial niches

Rheumatic heart disease in children and adolescents, part 1: epidemiology, pathogenesis and diagnosis.

Rheumatic heart disease (RHD) remains a leading cause of acquired cardiovascular disease in children and adolescents globally, despite dramatic declines in high-income settings. This review synthesises contemporary evidence on the epidemiology, pathogenesis, natural history and diagnosis of rheumatic fever (RF) and RHD in young populations, with a focus on regions with high disease burden.Over recent decades, improvements in socio-economic conditions and access to healthcare have led to marked reductions in RF and RHD across much of Europe, North America and parts of Asia. In contrast, the dis

Prevalence and determinants of sacroiliac joint bone marrow oedema in the general population in Germany: a population-based cross-sectional study.

BACKGROUND: MRI-detected bone marrow oedema in sacroiliac joints is central to diagnosing axial spondyloarthritis, influencing treatment decisions including anti-inflammatory therapy initiation. However, the prevalence of bone marrow oedema in the general population remains unknown, restricting interpretation of MRI findings and potentially leading to overdiagnosis when imaging findings are considered without clinical context. We aimed to establish the prevalence and determinants of sacroiliac bone marrow oedema in the general adult population. METHODS: In this national, population-based, cro

Inflammatory biomarkers of atherosclerosis in SLE that may improve SLE-specific cardiovascular risk assessment tools.

Cardiovascular (CV) events (CVE) are a leading cause of morbidity and mortality in systemic lupus erythematosus (SLE), and affected patients display a two- to three-fold higher risk than the general population. This increased CV risk remains underestimated by traditional CV risk algorithms, which do not account for SLE-specific drivers of accelerated atherosclerosis. The aim of this review is to examine key biomarkers and immune-mediated pathways involved in immune dysregulation and vascular injury. By comparing their contribution to SLE pathogenesis and atherosclerotic plaque development, we
