Rheumatoid arthritis (RA) is a prevalent autoimmune disease affecting approximately 1% of the global population, which significantly increases the risks of cardiovascular disease (CVD). RA-associated cardiovascular risks, such as pericarditis, myocarditis, heart failure, myocardial infarction, arrhythmias, atherosclerosis, and endothelial dysfunction, are major contributors to mortality in RA patients. These risks are driven by a complex interplay of chronic immune-inflammatory activation, metabolic dysregulation, genetic factors, and the adverse effects of long-term drug therapy. Despite adva

Juvenile idiopathic arthritis (JIA) represents the most common cause of chronic arthritis in childhood; however, not all early-onset arthropathies are inflammatory in origin. We report the case of a 4-year-old girl initially diagnosed with oligoarticular JIA and treated with methotrexate followed by a tumor necrosis factor inhibitor, without significant clinical improvement and despite persistently normal inflammatory markers. Clinical reassessment raised suspicion of a non-inflammatory arthropathy, supported by characteristic radiographic findings including metaphyseal flaring of the distal f

The European Society of Cardiology (ESC) released new guidelines in 2025 for the management of myocarditis and pericarditis, which include systemic autoimmune rheumatic diseases as one of the potential aetiologies. Myopericardial involvement in systemic autoimmune rheumatic diseases is common and associated with poor outcomes; however, the unique pathophysiology of myopericardial involvement in systemic autoimmune rheumatic diseases poses specific challenges and limits the applicability of the generic diagnostic pathways proposed in the 2025 ESC guidelines to the systemic autoimmune rheumatic

INTRODUCTION: Pericardial mesothelioma is an exceedingly rare pericardial neoplasm. It has atypical clinical symptoms and imaging characteristics that often lead to an inconclusive diagnosis. The diagnosis of a rare tumor such as pericardial mesothelioma, which can present with a variety of manifestations, requires a multidisciplinary approach.
CASE PRESENTATION: A 36-year-old Caucasian female patient without significant past medical history was admitted to the hospital with chest pain and a high fever and was diagnosed with acute pericarditis. The following month, the patient was treated for

Pericarditis is often an asymptomatic complication of rheumatoid arthritis. It rarely progresses towards tamponade or chronicity. There is little data on the therapeutic management of this serious complication. Recently, some rare data evaluating the place of biotherapy in rheumatoid pericarditis have been published. We report the observation of a patient with immunopositive rheumatoid arthritis escaping several lines of treatment who developed tamponade which recurred after surgical drainage. Only combotherapy with rituximab and anakinra allowed the induction of rheumatological remission and

Rheumatoid arthritis (RA) is a multifactorial autoimmune disease characterized with symmetrical progression of joint deformity that is often diagnosed at a chronic condition with other associated pathological conditions such as pericarditis, keratitis, pulmonary granuloma. Despite the understanding of RA pathophysiology in disease progression, current clinical treatment options such as disease-modifying anti-rheumatic drugs (DMARDs), biologics, steroids, and non-steroidal anti-inflammatory drugs (NSAIDs) provide only palliative therapy while causing adverse side effects such as off-target mult

BACKGROUND: Elderly-onset rheumatoid arthritis (EORA) is a distinct clinical entity of Rheumatoid Arthritis (RA), which begins after the age of 60 years, and which is distinguished by its clinical features from young-onset rheumatoid arthritis (YORA).
OBJECTIVE: This case report aims to highlight the importance of increased vigilance, because in many cases EORA can begin with pulmonary, cardiological, dermatological, but also locomotor lesions.
CASE PRESENTATION: We present an excellent case of a 68-year-old male patient, a heavy and long-term tobacco user, with arterial hypertension, dyslip

UNLABELLED: Chronic arthritis in children is widely associated with juvenile idiopathic arthritis (JIA); however, several rare monogenic disorders may mimic its clinical presentation. Misdiagnosis can result in unnecessary immunosuppressive treatment and delay appropriate care. This study aimed to share our experience with monogenic disorders presenting as chronic arthritis and highlight their distinguishing clinical and imaging features. This retrospective cohort study included patients initially suspected of having JIA who were later diagnosed with a monogenic disorder. Clinical, laboratory,

BACKGROUND Type II cryoglobulinemic vasculitis is a systemic syndrome that usually develops in the background of chronic HCV infection. Cases of non-hepatitis C (HCV)-related type II cryoglobulinemic vasculitis pose diagnostic and therapeutic challenges in patients with pre-existing hematologic and rheumatic conditions. CASE REPORT We present the case of an 82-year-old woman with a history of skin-limited hypocomplementemic urticarial vasculitis (HUV), monoclonal gammopathy of unknown significance (MGUS), and monoclonal B-cell lymphocytosis (MBL), with new-onset pericarditis, pleuritis, worsen

BACKGROUND: Systemic lupus erythematosus (SLE) is a chronic autoimmune disease characterized by multisystem inflammation and is a common cause of pericarditis and pericardial effusion, but significant pericardial effusion and cardiac tamponade are rare and even rarer as the first manifestation.
CASE SUMMARY: We report the case of a young male who presented with fever, recurrent pericarditis, and polyserositis with pericardial and bilateral pleural effusion. On examination, he was haemodynamically unstable and the pericardial effusion had considerable dimensions and an urgent pericardiocentesi

In the early 19th century, Auguste François Chomel gathered the knowledge on rheumatism, systematised it, and published it in his majestic work Pathologie Générale. In his treatise, for the first time, rheumatic heart was discussed. Taking into account the opinions of the ancient Greek physicians, he had described the disease as an acute or chronic manifestation of the inflammation of the heart due to rheumatism. His publication rendered rheumatic heart an issue in vogue for his era, a disease being mentioned in all textbooks of the internal pathology.

OBJECTIVES: This study evaluated the characteristics of serosal involvement in adult-onset Still's disease (AOSD).
METHODS: Patients meeting the Yamaguchi classification criteria were classified into AOSD with and without serosal involvement according to their manifestations and sonography/radiography. Clinical data was retrospectively reviewed from 102 patients with AOSD in two centres.
RESULTS: Forty-two patients (41.2%) had serosal involvement. The frequencies of pulmonary infiltrate and impaired liver function were significantly higher in patients with serosal involvement (p = .002 and p

BACKGROUND: Camptodactyly-arthropathy-coxa vara-pericarditis (CACP) syndrome, caused by biallelic pathogenic mutations in the proteoglycan 4 (PRG4) gene, is characterized by early-onset camptodactyly, noninflammatory arthropathy, coxa vara deformity, and rarely, pericardial effusion. This syndrome can mimic juvenile idiopathic arthritis. CACP syndrome is caused by mutations in the proteoglycan 4 (PRG4) gene. To date, only 36 pathogenic mutations have been reported in this gene, but none have been reported from Azerbaijan.

Psoriatic arthritis (PsA) is a chronic, immune-mediated inflammatory condition, and the proinflammatory cytokine tumor necrosis factor-α (TNF-α) plays a major pathogenic role in the development and progression of PsA. Anti-TNF-α therapies, such as the monoclonal antibody infliximab, are used to treat patients whose PsA has not responded favorably to conventional anti-rheumatic drugs. However, exposure to anti-TNF-α therapeutics can lead to drug-induced lupus erythematosus (DILE), which may rarely be accompanied by cardiac manifestations. Here, we describe a rare case of drug-induced lupus eryt

BACKGROUND: Constrictive pericarditis represents a chronic condition and systemic inflammatory diseases are a known, yet uncommon, cause. Pericardial involvement is seldom reported in primary Sjögren's syndrome, usually occurring in association with pericardial effusion or pericarditis. We report a case of constrictive pericarditis with an insidious course and unusual evolution associated with primary Sjögren's syndrome. Due to the challenging nature of the diagnosis, clinical suspicion and multimodality imaging are essential for early identification and prompt initiation of treatment. Long-te

Systemic lupus erythematosus (SLE) is a chronic rheumatic disorder. Endothelin-1, a vasoconstrictor, belongs to the endothelin family and is associated with vascular-related damages. To date, association between ET-1 and pathogenesis of SLE remains unclear. This case-control study was carried out by 314 SLE, 252 non-SLE diseases patients and 500 healthy controls. Serum ET-1, CCN3, IL-28B levels were detected by ELISA, and ET-1 gene polymorphisms (rs5369, rs5370, rs1476046, rs2070699, rs2071942, rs2071943, rs3087459, rs4145451, rs6458155, rs9369217) were genotyped with Kompetitive Allele-Specif

Systemic lupus erythematosus (SLE) is a chronic inflammatory condition with various cardiovascular sequelae. Pericarditis is the most common cardiac manifestation, yet patients also have a markedly elevated risk of premature atherosclerosis and acute coronary syndrome (ACS). This makes the diagnosis of ischaemic chest pain both challenging and crucial in these patients. Here, we examine the case of a 39-year-old male who presented with acute myopericarditis and pericardial tamponade in the setting of newly diagnosed SLE. Several days later, the patient experienced an infero-septal non-ST-eleva
