Aetiology of chronic constrictive pericarditis.

In a consecutive series of 32 cases of chronic constrictive pericarditis treated by pericardiectomy during the past 25 years, four were attributable to rheumatoid disease, two to trauma, one to sarcoidosis, and four, at a maximum, to tuberculosis. In the remaining 21 cases of undetermined aetiology there was no evidence of tuberculosis. It appears, therefore, that tuberculosis was not a common cause of chronic constrictive pericarditis during the period under review, which included the 1950s and early 1960s when tuberculosis was widespread.

[Visceral manifestations of rheumatoid polyarthritis].

The commonest extra-articular symptoms of rheumatoid arthritis include nodules, vasculitis, neuro-muscular involvement and keratoconjunctivitis sicca with dry mouth, characteristic of Sjögren's syndrome. However, laboratory tests demonstrate that there is a high percentage of pericarditis and involvement of the lungs, liver and pancreas, which are clinically silent. Personal statistics on extra-articular symptoms in 110 cases of classic rheumatoid arthritis are presented.

[Pericarditis in adults in Abidjan. Study of one hundred observations (author's transl)].

The goal of this work is to precise the respective aetiologic prevalence and the bases of diagnosis of pericarditis in Ivory Coast. The epidemiologic study has been done about 197 cases diagnosed between march 1972 and april 1978. These results are comparable to those collected previously in West Africa: the pericarditis is present in 1% of the patients admitted in the hospital and in 5.6% of the cardiovascular diseases of the unit of internal medicine in the Treichville Hospital (Abidjan). The semeiologic and evolutive study has been done with the 100 best documented records, with 90 acute an

Effects of prednisolone and anti-macrophage serum on the development of carditis and arthritis due to group A hemolytic streptococci in mice.

The effects of prednisolone and anti-macrophage serum (AMS) on the development of cardiac and articular lesions were compared in experimentally infected mice with group A hemolytic streptococci. The predonine-treated mice had a higher mortality rate and greater abscess formation than in the AMS-treated mice. The predonine-treated group had a higher incidence of cardiac lesions due to chronic pericarditis and chronic myocarditis, and the former was particularly frequent. Pathologic findings seen in the AMS-treated mice were those of acute pancarditis. The AMS-treated animals also had lesions of

Primary cardiac amyloidosis: A case presentation.

A young man with primary cardiac amyloidosis presented clinically as a case of 'restrictive' cardiomyopathy with an initial diagnosis of constrictive pericarditis, probably of tuberculous origin. Cardiac catheterization showed features compatible with restrictive cardiomyopathy or constrictive pericarditis but pericardial biopsy was negative. Percutaneous right ventricular endomyocardial biopsies established amyloid infiltration of the myocardium, but rectal, tongue and liver biopsies were all negative for amyloidosis. Bence-Jones protein was absent from the urine. Serum electrophoresis, immun

[Adult Still's disease with constrictive pericarditis, Sjögren's syndrome and varied leucocytic abnormalities (author's transl)].

Adult Still's disease is a newly described disorder that is of interest not only for the rheumatologist but also for the internist since fever or extra-articular symptoms can reveal this disorder, which appears to be identical to the Wissler-Fanconi's syndrome. We report a new case characterized by the occurrence of a constrictive pericarditis with clinical features of protracted fever, by the late appearance of a Sjögren's syndrome and by peculiar blood abnormalities: hyperleucocytosis up to 80 000 after splenectomy, transient Felty's syndrome, blood monocytosis and bone marrow mastocytosis.

Cardiovascular disease in Papua New Guinea.

The chest X-ray films of 251 patients with cardiovascular lesions, collected in the major hospitals of Papua New Guinea, were reviewed. Cor pulmonale (18%) and rheumatic heart disease (16%) were the predominant cardiac disorders, and cor pulmonale was the most common cause of cardiac failure (58%). Cardiomegaly or cardiac failure of unknown cause, possibly due to cardiomyopathy or myocarditis, made up 9% of the group and may be more important than has been thought previously. Aortic-arch calcification typical of atheroma was present in 21% of the patients, and is thus quite common, even though

[Pleuropericarditis: early systemic manifestation of an active chronic hepatitis (author's transl)].

A case of relapsing pleuropericarditis is reported in which no etiological factors were found initially, especially in relation to any possible collagen disease. A retrospective review of the clinical, biological, and histological findings, coupled with the nature of the progression of the disease, suggested to the authors that this was the first expression of a systemic disease which later developed signs of Gougerot-Sjögren's syndrome, Hashimoto's thyroiditis, and, three and a half years later, an active chronic hepatitis.

[Analysis of the causes of death in patients with rheumatoid arthritis].

The post-mortem examinations performed from 1943 to 1977 in Tartu were examined for the frequency of rheumatoid arthritis as cause of death. Here was the result that since 1968 in increasing number this clinical picture was observed and that during the last five years it appeared in 1.4% of the cases. As cause of death in patients whose basic disease was rheumatoid arthritis most frequently and uraemia was found either on the basis of an amyloidosis of the kidneys or of a chronic glomerulonephritis. Also the number of fatal complications among a long-lasting glucocorticosteroid therapy is not

[Chronic constrictive pericarditis complicating an apparently stable rheumatoid arthritis. Report of a new case (author's transl)].

The authors report a case of constrictive pericarditis complicating an apparently stable sero-negative rhumatoïd arthritis, a rarely described pathological association. The patient had a serious polyserositis due to an extremely marked and inflammatory pericardial constriction. The rhumatoïd origin was confirmed by pericardial effusion, histological and immunofluorescent studies. The inflammatory pericardial process rendered pericardiectomy virtually impossible and resulted in a fatal outcome.

Rheumatoid arthritis with systemic necrotizing arteritis.

An autopsy case of rheumatoid arthritis with active polyarthritis, systemic necrotizing arteritis, pleuritis, pericarditis, rheumatoid nodules in a few organs and a healing gastric ulcer was reported. Histologically, systemic necrotizing arteritis was characterized by vascular changes of the following three types: Granulomatous arteritis with a characteristic arrangement of mesenchymal cells forming a palisade around coagulation necrosis of media and some of them formed a rheumatoid nodule-like lesion in the wall (RA type); Fibrinoid arteritis very similar to the Kussmaul-Maier type periarteri

Clinicopathologic analysis of cardiac dysfunction in 52 patients with sickle cell anemia.

Clinical studies have long suggested the presence of a specific cardiomyopathy in sickle cell anemia secondary to intracoronary thrombosis and subsequent infarction. Fifty-two autopsy patients were studied (48 with SS hemoglobin, 4 with S-C or S-Thal hemoglobin) to ascertain the range of cardiac pathologic abnormalities associated with this disease. The average age was 17 years (range 1 month to 48 years). Renal failure and infection were the most common causes of death; the former was a more common cause in adults than in children. Right and left ventricular hypertrophy and dilatation were th

Arthritis and heart lesions. A study of 25 cases with pericarditis or valvular lesions associated to inflammatory joint disease.

Clinical and laboratory data of 25 patients with inflammatory joint disease and pericarditis or valvular heart disease are reviewed. The patients were divided in two groups: 11 presented pericarditis and 14 valvular heart disease. The patients studied presented a spectrum of diseases ranging from classical sero-positive rheumatoid arthritis to ankylosing spondylitis. Acute rheumatic fever was in no case the actual joint disease. In contrast to the patients with valvular heart disease nearly all patients with pericarditis were rheumatoid arthritis factor positive and signs of a generalized syst

Corticosteroid treatment of juvenile chronic polyarthritis over 22 years.

Between the years 1952 and 1974, 1293 children aged from 1-14 years with the adult type of rheumatoid arthritis and Still's disease were admitted, of whom 793 or 61.2% received oral corticosteroids, mostly in the form of continuous therapy. In all children with Still's syndrome (476 cases, of whom 84.4% received corticosteroid therapy), as well as in 75% of the children with rheumatoid arthritis (817 cases, 47.1% under corticosteroid treatment) the therapy had already been initiated by the referring institutions. The mortality and morbidity of corticosteroid treated children is compared with t

[Extra-articular manifestations of chronic polyarthritis].

Although the chronic destructive joint processes dominate the clinical picture of chronic rheumatoid arthritis, it must not been forgotten that the disease is a systemic one, whereby certain organs or organ systems are affected either clinically or subclinically. The various possible pathogenetic mechanisms are discussed and the particular type of patient with rheumatoid arthritis who appears to be prone to extra-articular manifestations is described. Subsequently, the individual organic manifestations and the clinical picture are discussed, as well as the incidence and significance of these f
