Comparison of Diagnostic Performance of Commercially Available Serological and Molecular Tests for Detection of Orientia tsutsugamushi in South Korea: A Single-Center Prospective Study.

Background: Scrub typhus is commonly misdiagnosed because of nonspecific clinical features and limited data on the performance of diagnostic tests. This study aimed to evaluate the accuracy of commercially available serological and molecular assays for diagnosing scrub typhus. Methods: Adult patients with suspected scrub typhus who visited a tertiary-care hospital in South Korea from July 2022 to December 2024 were prospectively enrolled. Scrub typhus was confirmed by either a positive real-time polymerase chain reaction (PCR) result for Orientia tsutsugamushi or a ≥ four-fold increase in the

Analysis of the Epidemiological Characteristics and Risk Factors for Severe Progression of Scrub Typhus in the Dali Region of China.

BACKGROUND We analyzed the epidemiological traits and risk factors of severe scrub typhus cases in Dali, China, to form a theoretical basis for local prevention and control. MATERIAL AND METHODS We analyzed the epidemiological characteristics of patients with scrub typhus admitted to our hospital from January 1, 2015, to December 31, 2024. Based on the severity of scrub typhus, the patients were divided into a severe scrub typhus group (n=89) and mild scrub typhus group (n=370). Logistic regression analysis was used to identify the risk factors for severe scrub typhus. RESULTS This study inclu

Orientia tsutsugamushi a Likely Contributor to Stillbirths and Neonatal Deaths in Bangladesh, 2017-2024.

Scrub typhus, caused by Orientia tsutsugamushi, is a mite-borne infection endemic to Asia, the Pacific Islands, and northern Australia. This study reports the first detection of O. tsutsugamushi DNA in postmortem specimens from stillbirths and neonatal deaths collected through the CHAMPS surveillance project from 2017 to 2024 in Bangladesh. O. tsutsugamushi DNA was detected in 11 cases including 1.7% of stillbirths (n = 461) and 0.7% of neonates (n = 443). Ninety percent of these deaths occurred between August and January; among stillbirths, detection was highest in cerebrospinal fluid samples

Leptotrombidium imphalum Chiggers as Vector for Scrub Typhus in Human Settlements, India, 2022-2023.

Scrub typhus is a common bacterial infection in many parts of Asia. The causative agent, Orientia tsutsugamushi, is transmitted by trombiculid mite (chigger) larvae that require small mammals as maintaining hosts. We studied the prevalence of O. tsutsugamushi infection in mites and small mammals in villages and land surrounding them in South India to determine high-risk settings. We identified 12,431 mite larvae on 883 small mammals, 32% of which were bandicoot rats, 31% black rats, and 31% Asian house shrews. Leptotrombidium imphalum was the most common mite species and the only species assoc

Epidemiology and risk factors in a scrub typhus cluster in Odisha, India, 2023: Investigating with a One Health approach.

INTRODUCTION: Scrub Typhus accounts for approximately one-fourth of all acute undifferentiated febrile illness cases in India. In September 2023, Odisha, an eastern Indian state, reported several scrub typhus deaths. We investigated a scrub typhus cluster in Block-A, Odisha, to describe the epidemiology and assess risk factors. METHODOLOGY: We conducted an unmatched (1:1) case-control investigation. Line-list of scrub typhus IgM-ELISA tested individuals was collected from the district laboratory. Cases were defined as scrub typhus IgM-ELISA positive residents of Block-A between July1 and Octob

Outcomes of noninvasive versus invasive respiratory support in pediatric scrub typhus-associated acute respiratory failure: a retrospective cohort study from South India.

Scrub typhus is an important cause of acute febrile illness in children that may progress to acute respiratory failure (ARF). However, evidence for noninvasive respiratory support (NIRS) in this setting is limited. The main objective of this study is to compare outcomes of NIRS versus invasive mechanical ventilation (IMV) in pediatric scrub typhus-related ARF and identify predictors of NIV failure. This study included children aged 28 days-15 years with confirmed scrub typhus and ARF who were admitted to the pediatric critical care unit of a tertiary center in South India. Patients were classi

Nasopharyngeal Swabs for Orientia tsutsugamushi Detection in Doxycycline Treatment: A Prospective Cohort Study in Hainan, China.

Scrub typhus, caused by Orientia tsutsugamushi, remains a neglected cause of acute febrile illness. Molecular testing of blood supports early diagnosis, yet once doxycycline is started, blood qPCR positivity can drop rapidly, complicating short-term follow-up and relapse surveillance. We compared detection across multiple clinical specimens and evaluated nasopharyngeal swabs (NPSs) as noninvasive supplementary specimens during treatment initiation. In a prospective single-center cohort from Hainan, China, we enrolled 20 patients with scrub typhus. Blood, NPS, urine, and stool were collected be

Establishing an external quality assurance scheme for the detection of Orientia tsutsugamushi IgM in clinical samples: Strengthening quality control of scrub typhus diagnosis in Indian laboratories.

Scrub typhus, caused by Orientia tsutsugamushi, is a re-emerging zoonotic disease that is associated with significant morbidity and mortality in many parts of India. Early and accurate diagnosis, primarily through the detection of IgM antibodies, plays a pivotal role in patient management and disease surveillance. However, there is a paucity of diagnostic methods in our country, and only in recent years have more diagnostic assays been rolled out. To address the heterogeneity in the laboratory diagnosis of scrub typhus, an external quality assurance scheme (EQA Scheme) for the detection of O t

Rapidly Progressive Cytophagic Histiocytic Panniculitis in a Child Triggered by Trauma and Scrub Typhus: A Dramatic Response to Cyclosporine.

Cytophagic histiocytic panniculitis (CHP) is a rare and potentially life-threatening inflammatory panniculitis characterized by lobular lymphohistiocytic infiltration and cytophagic "bean-bag" histiocytes. Because its early features overlap with infectious panniculitis and hemophagocytic lymphohistiocytosis (HLH), diagnosis is often delayed, contributing to significant morbidity. We describe a rapidly progressive pediatric case of CHP triggered by antecedent trauma and scrub typhus infection. A 10-year-old girl presented with seven days of high-grade fever and painful erythematous nodules over

How to differentiate pediatric enteroviral meningoencephalitis from scrub typhus meningoencephalitis: A single-center retrospective study from Yunnan Province, China.

BACKGROUND: During the summer-autumn epidemic season, pediatric scrub typhus meningoencephalitis (ST) and enteroviral meningoencephalitis (EV) often present with overlapping clinical manifestations and routine cerebrospinal fluid (CSF) findings, making early differential diagnosis challenging and delaying appropriate treatment. Practical diagnostic strategies based on readily available parameters are lacking. OBJECTIVE: To evaluate whether common clinical and laboratory indicators can differentiate pediatric ST meningoencephalitis from EV meningoencephalitis. METHODS: This single-center retr

Concurrent splenic and renal infarction in scrub typhus: expanding the vasculitic spectrum.

BACKGROUND: Scrub typhus is a mite-borne zoonotic infection caused by Orientia tsutsugamushi, characterized by systemic vasculitis and multiorgan involvement. While hepatic, pulmonary, and renal dysfunction are well recognized, ischemic infarction of solid organs is distinctly uncommon. CASE PRESENTATION: A 35-year-old previously healthy woman presented with acute febrile illness, respiratory symptoms, and left upper-quadrant abdominal pain. Examination revealed tachypnea, hypoxia, and focal abdominal tenderness without rash or eschar. Laboratory evaluation showed neutrophilic leukocytosis, th

A Study on Prevalence of Malarial Retinopathy among Malaria Cases and to Determine Its Presence as a Marker of Severe Malaria: Observations from Eastern India.

BACKGROUND: Malarial retinopathy refers to a constellation of changes seen in severe or complicated malaria cases. These include: retinal whitening, vessel changes-whitening, tramlining, retinal hemorrhages, and papilledema. There are very few Indian studies on this entity. Since retina can be easily visualized by direct ophthalmoscopy, this study was done to determine prevalence of malarial retinopathy among malaria cases and to determine relationship between malarial retinopathy and severity of the disease. MATERIALS AND METHODS: The study was done at Indoor and Outdoor Departments of Tropi

Evaluation of Oral Doxycycline, Azithromycin, or Sequential Doxycycline-Azithromycin Treatment for Scrub Typhus.

BACKGROUND: Scrub typhus is a leading cause of febrile illness across the Asia-Pacific region. Doxycycline is the first-line therapy, with azithromycin as an alternative; sequential treatment (doxycycline followed by azithromycin) is used for nonresponders. However, comparative real-world effectiveness for sequential therapy remains uncertain. METHODS: We conducted a single-center, non-interventional target-trial emulation at the 970th Hospital of the People's Liberation Army (January 2023 - June 2025). Consecutive patients ≥12 years receiving oral doxycycline, azithromycin, or sequential dox

From obstacles to opportunities: Innovations and evolution of strategies in scrub typhus vaccine research.

Scrub typhus, caused by the obligate intracellular bacterium Orientia tsutsugamushi, remains a major public health burden in the Asia-Pacific region and is increasingly reported beyond its traditional endemic range. Despite its clinical significance, progress toward vaccine has been limited, largely due to extensive antigenic diversity and the difficulty of inducing durable cross-protective immunity. This review attempts to retrace the historical path of scrub typhus vaccine development from whole-cell killed preparations to subunit proteins, recombinant constructs, and epitope-driven strategi

Understanding the spatiality in neglected tropical diseases: An ethnographic study of scrub typhus in Puducherry, India.

Individuals from marginalised backgrounds are often "placed" in spatial planes marked by disadvantages and poor resource availability, impacting their health outcomes. This study explored the role of spatiality in shaping health outcomes, with scrub typhus-a neglected tropical disease (NTD) of zoonotic origin-as an illustrative case. Employing a spatial justice framework, the study investigated the intersection of geospatial and socio-spatial factors influencing health and illness experience in Puducherry, India. An interdisciplinary mixed-methods approach was employed, involving geospatial ho

DNA barcoding reveals cryptic diversity in chiggers (Trombiculidae) parasitizing small mammals across Peninsular Malaysia.

Chiggers (Acariformes: Trombiculidae) are ectoparasites of small mammals and important vectors of scrub typhus, yet their taxonomy and molecular diversity in Southeast Asia remain poorly resolved. This study represents the first DNA barcoding and phylogenetic assessment of chiggers from small mammals across Peninsular Malaysia, integrating mitochondrial (COI) and nuclear (28 S rDNA) genes. A total of 192 chiggers representing 11 morphospecies from six genera were sequenced, with tree topologies inferred using maximum likelihood and supported by multiple species delimitation methods. The COI ge
