Tissue distribution of Orientia tsutsugamushi in rodents: a molecular and immune diagnostic approach.

BACKGROUND: Scrub typhus is caused by a bacterium Orientia tsutsugamushi, which is transmitted to humans by infected chigger mites found in scrub vegetation. This study examined the tissue distribution of O. tsutsugamushi in rodents collected from different districts in Kerala, India. Tissue tropism helps to understand disease spread to cells and tissues, leading to inflammation and multi-organ failure. METHODS: O. tsutsugamushi infection was screened using molecular and immunological diagnostic tools in 137 rodents. Of these, 78 were positive for O. tsutsugamushi in blood samples by both mol

Acute Cerebellitis: A Rare Complication of Scrub Typhus.

Scrub typhus, caused by Orientia tsutsugamushi , is an emerging zoonotic infection that typically presents as an acute febrile illness. While neurological manifestations are increasingly recognized, cerebellar involvement remains exceedingly rare. We report the case of a 42-year-old man from Uttar Pradesh who presented with high-grade fever, followed by slurred speech and gait ataxia. Clinical examination revealed cerebellar signs, including dysmetria and dysdiadochokinesia. Laboratory testing confirmed scrub typhus via a positive immunoglobulin M enzyme-linked immunosorbent assay, and other i

High levels of Interleukin - 13 in patients with scrub typhus correlate with disease severity.

Scrub typhus is a zoonotic infectious disease caused by Orientia tsutsugamushi infection. It has a high fatality rate if not treated in time. Currently, there are still limited reliable biomarkers for predicting the disease progression of scrub typhus. A Retrospective Analysis of the Clinical Features of Scrub Typhus and the Exploration of Early Indicators for Progression to Moderate-Severe Disease. Using the Luminex method, we quantified ten cytokines in the serum of patients with moderate-severe scrub typhus (n = 51), mild scrub typhus (n = 32), and healthy controls (n = 62). We compared ser

Antibiotic therapy failure and clinical outcomes in scrub typhus patients from Guangzhou city, southern China.

Antibiotic therapy failure in scrub typhus (ST) has raised concerns about first-line antibiotic efficacy, necessitating reevaluation of recommended antibiotic therapies. A multicenter retrospective analysis included 2029 non-severe ST patients hospitalized in Guangzhou from 2012 to 2018. Antibiotic therapy failure was defined as needing to switch initial antibiotics due to persistent fever, clinical deterioration, or clinician-assessed need. The clinical outcomes of three antibiotic therapies were compared. Antibiotic therapy failure led to significantly greater medical needs, as evidenced by

Orientia tsutsugamushi binds to multiple C-type lectin receptors.

Orientia tsutsugamushi, the agent of scrub typhus, is an obligate intracellular bacterium whose atypical cell wall lacks many classical pathogen-associated molecular patterns but is enriched in neutral glycans. Its recognition by phagocytes is driven by a heat-stable ligand that triggers innate cytokine responses, yet the nature of this ligand and the receptors sensing it remain incompletely understood. While activation of innate immunity via toll-like (TLR) and nucleotide-binding and oligomerization domain-like receptors has been described, recognition via C-type lectin receptors (CLRs) has r

A humanized IFN-γ mouse model reveals skin eschar formation, enhanced susceptibility and scrub typhus pathogenesis.

Scrub typhus, caused by Orientia tsutsugamushi (Ot) bacteria, is a serious acute febrile illness associated with significant mortality. No effective vaccine is currently available, largely due to the complex Ot strain diversity and an incomplete understanding of protective immune mechanisms. To overcome these challenges, there is a critical need for a suitable animal model that mimics human disease through the natural route of infection. Here, we report for the first time that a genetically engineered humanized mouse strain (with triple knockout/knock-in of IFN-γ and its receptors, abbreviated

Orientia tsutsugamushi a Likely Contributor to Stillbirths and Neonatal Deaths in Bangladesh, 2017-2024.

Scrub typhus, caused by Orientia tsutsugamushi, is a mite-borne infection endemic to Asia, the Pacific Islands, and northern Australia. This study reports the first detection of O. tsutsugamushi DNA in postmortem specimens from stillbirths and neonatal deaths collected through the CHAMPS surveillance project from 2017 to 2024 in Bangladesh. O. tsutsugamushi DNA was detected in 11 cases including 1.7% of stillbirths (n = 461) and 0.7% of neonates (n = 443). Ninety percent of these deaths occurred between August and January; among stillbirths, detection was highest in cerebrospinal fluid samples

Leptotrombidium imphalum Chiggers as Vector for Scrub Typhus in Human Settlements, India, 2022-2023.

Scrub typhus is a common bacterial infection in many parts of Asia. The causative agent, Orientia tsutsugamushi, is transmitted by trombiculid mite (chigger) larvae that require small mammals as maintaining hosts. We studied the prevalence of O. tsutsugamushi infection in mites and small mammals in villages and land surrounding them in South India to determine high-risk settings. We identified 12,431 mite larvae on 883 small mammals, 32% of which were bandicoot rats, 31% black rats, and 31% Asian house shrews. Leptotrombidium imphalum was the most common mite species and the only species assoc

Outcomes of noninvasive versus invasive respiratory support in pediatric scrub typhus-associated acute respiratory failure: a retrospective cohort study from South India.

Scrub typhus is an important cause of acute febrile illness in children that may progress to acute respiratory failure (ARF). However, evidence for noninvasive respiratory support (NIRS) in this setting is limited. The main objective of this study is to compare outcomes of NIRS versus invasive mechanical ventilation (IMV) in pediatric scrub typhus-related ARF and identify predictors of NIV failure. This study included children aged 28 days-15 years with confirmed scrub typhus and ARF who were admitted to the pediatric critical care unit of a tertiary center in South India. Patients were classi

Nasopharyngeal Swabs for Orientia tsutsugamushi Detection in Doxycycline Treatment: A Prospective Cohort Study in Hainan, China.

Scrub typhus, caused by Orientia tsutsugamushi, remains a neglected cause of acute febrile illness. Molecular testing of blood supports early diagnosis, yet once doxycycline is started, blood qPCR positivity can drop rapidly, complicating short-term follow-up and relapse surveillance. We compared detection across multiple clinical specimens and evaluated nasopharyngeal swabs (NPSs) as noninvasive supplementary specimens during treatment initiation. In a prospective single-center cohort from Hainan, China, we enrolled 20 patients with scrub typhus. Blood, NPS, urine, and stool were collected be
