Mechanisms underlying rheumatic heart disease immune-mediated pathogenesis: T-cell responses, cytokine networks, and immune dysregulation as drivers of valvular injury.

Acute rheumatic fever is an immune-mediated sequela of Streptococcus pyogenes (group A Streptococcus) infection, presenting with fever, arthritis, chorea, rash, and, crucially, carditis. Without effective secondary prophylaxis and care, acute rheumatic fever can progress to chronic rheumatic heart disease, a leading yet under-recognised cause of cardiovascular morbidity and death that disproportionately affects children and young adults aged 5-24 years in low-resource settings. Robust evidence implicates the role of antigenic mimicry-particularly between streptococcal M protein and cardiac or

Iliofemoral Venous Stenosis Complicating Large-Bore Access for Balloon Mitral Valvotomy, Unmasking Incidental May-Thurner Anatomy.

BACKGROUND: Large-bore transvenous access for balloon mitral valvotomy (BMV) can be complicated by unrecognized iliofemoral venous disease, occasionally unmasking a congenital variant. CASE SUMMARY: A 51-year-old woman with rheumatic mitral stenosis and NYHA functional class III-IV dyspnea, on acenocoumarol for atrial fibrillation, was scheduled for BMV. After right femoral venous access and transseptal puncture, hardware >8-F could not be advanced; venography showed a tight, recoiling right external iliac vein stenosis, not responding to intravenous nitroglycerine, treated with angioplasty a

Use of an AI metabolomics score to predict cardiovascular risk among patients with Psoriasis in the MGB and UK Biobanks.

BACKGROUND: Systemic inflammation in psoriasis accelerates atherosclerosis and confers excess cardiovascular risk that is not fully captured by traditional clinical predictors. Metabolomic profiling may identify disease-specific metabolic signatures associated with cardiovascular events and enhance risk prediction beyond conventional factors. OBJECTIVES: To determine whether plasma metabolomic profiling improves cardiovascular risk prediction in patients with psoriasis and to develop and externally validate an artificial intelligence-driven metabolomic risk score (MetaPsoAI) for incident card

GMMA as adjuvant and carrier for mucosal delivery of group A streptococcus glycoconjugate vaccine.

Group A streptococcus (GAS) infection is usually initiated by colonization of mucosal sites such as the pharynx, followed by invasion and systemic dissemination. Development of a vaccine that can prevent colonization is enticing as it would prevent subsequent infections and all associated autoimmune sequalae such as rheumatic heart disease (RHD). In this study we investigated the mucosal adjuvanticity of GMMA when used as a carrier protein for GAS carbohydrate (GAC) conjugate vaccine (GAC-GMMA). Mice were immunized sublingually or intranasally with GAC-GMMA or GAC-CRM197, in the presence or ab

Integrated Clinical, Biological, and Echocardiographic Factors Independently Associated With In-Hospital Mortality in Infective Endocarditis: A Single-Centre Cohort Study From Morocco.

Background Infective endocarditis (IE) remains associated with substantial in-hospital mortality worldwide, with a disproportionate burden in low- and middle-income countries (LMICs). Identifying clinical, biological, microbiological, and echocardiographic factors associated with adverse outcomes may help characterize patients at increased risk of death. Objectives To identify clinical, biological, microbiological, and echocardiographic factors independently associated with in-hospital mortality among patients hospitalized with IE in a North African tertiary referral centre. Methods We conduct

Beyond Atrial Fibrillation: Clinical and Echocardiographic Predictors of Ischaemic Stroke and Systemic Thromboembolism in Rheumatic Heart Disease: A Systematic Review and Meta-Analysis.

PURPOSE: Atrial fibrillation (AF) is the best-established thromboembolic risk factor in rheumatic heart disease (RHD), but stroke and thromboembolism also occur without documented AF. We systematically synthesised associations between non-AF clinical/echocardiographic predictors and thromboembolism in RHD, comparing AF-adjusted with unadjusted estimates. METHODS: PubMed/MEDLINE, Embase, Scopus, and Web of Science were searched, supplemented by Google Scholar and citation screening. Eligible studies evaluated RHD patients with ≥1 non-AF predictor of stroke or thromboembolism. Two reviewers per

Advanced Rheumatic Heart Disease Complicated by Pregnancy, Mitral Valve Disease, Pulmonary Hypertension, and Left Atrial Thrombus: A Multidisciplinary Case Report from Resource-Limited Settings.

INTRODUCTION: Rheumatic heart disease (RHD) remains a leading cause of cardiovascular morbidity and mortality among pregnant women in resource-limited settings. Pregnancy induces significant hemodynamic changes that can decompensate pre-existing valvular heart disease, leading to life-threatening complications for both mother and fetus. CASE PRESENTATION: A 27-year-old pregnant woman (gravida 6, para 5) at approximately 34-38 weeks presented with progressive shortness of breath, palpitations. Mortality involving non-rheumatic valvular heart disease and atrial fibrillation in the United States: Long-term trends and implications.

Non-Rheumatic valvular heart disease (NRVHD) and atrial fibrillation (AF) are major contributors to cardiovascular mortality in the U.S. Their coexistence increases the risk of stroke, heart failure, and death. This study examines national trends in deaths involving both NRVHD and AF in the United States from 1999 to 2023. Centers for disease control and prevention - Wide-Ranging Online Data for Epidemiologic Research data (1999-2023) were analyzed for adults with AF (I48) and NRVHD (I34-I36) as contributing causes of death. Age-adjusted mortality rates (AAMRs) were calculated per 100,000 popul

Patient-Specific Analysis of Ventricular Vortex Formation and Flow Energetics in Rheumatic and Calcific Mitral Stenosis.

PURPOSE: Mitral stenosis (MS) accounts for 12% of cardiac patients with single valve disease. There are two main causes: rheumatic disease of the mitral valve (MV) and mitral annular calcification (MAC). While both result in increased transmitral pressure gradients, each has unique effects on valve morphology and pathophysiology. We aimed to study differences in transmitral vortex formation and stroke energetics between the two valve types. We created an in silico experimental system using an immersed finite element-finite difference fluid-structure interaction (FSI) scheme and performed three

Worsening heart failure is not a single endpoint: a severity-gradient perspective from digoxin in rheumatic heart disease.

The composite of cardiovascular or all-cause death and worsening heart failure (WHF) has become the principal efficacy endpoint of heart failure trials. Yet WHF is not one event: it spans outpatient oral-diuretic escalation, urgent intravenous-diuretic visits, hospitalisation, and HF-related death-tiers that differ markedly in frequency, ascertainment, and prognostic weight. A composite WHF benefit therefore cannot be properly interpreted without knowing which tier carries the signal, and we argue that this tier is both phenotype- and setting-dependent. Two trials published on the same day in

Rheumatic Heart Disease Burden and Sex Differences in China and ASEAN Countries, 1990-2030: A Comparative Analysis from the Global Burden of Disease Study 2023.

BACKGROUND: Rheumatic heart disease (RHD) remains a major public health concern in low‑ and middle‑income countries. Systematic comparisons between China and the ten ASEAN member states examining sex disparities are scarce. METHODS: Using Global Burden of Disease Study 2023 data, we analyzed RHD burden in China and ten ASEAN countries from 1990 to 2023. Key measures included age‑standardized prevalence (ASPR), mortality (ASMR), and disability‑adjusted life‑years rates (ASDR). Temporal trends were assessed via estimated annual percentage changes (EAPCs) and joinpoint regression. Decomposition

Towards a regional and national cardiothoracic biobank in India: translating a Dutch model to address a poorly characterised disease burden.

Cardiothoracic disease in India presents earlier and against a different biological background than in the high-income populations from which most evidence and guidelines derive. Coronary disease occurs about a decade earlier in South Asians; rheumatic heart disease remains endemic, with India carrying a quarter to a half of the global burden; and acute type A aortic dissection presents in the mid-to-late forties, with a higher reported proportion of connective-tissue disease than Western registries. The biological substrate of these conditions in Indian patients is largely uncharacterised, an

A Case of Eosinophilic Granulomatosis With Polyangiitis Presenting With Conjunctival Lesions During Treatment With IL-5 Inhibitors.

Eosinophilic granulomatosis with polyangiitis (EGPA) is a systemic rheumatic disease characterized by necrotizing vasculitis in the small vessels with eosinophilic inflammation. EGPA involves multiple organs, such as the heart, lungs, kidneys, gastrointestinal tract, nerves, and ophthalmic organs. Ophthalmic involvement includes conjunctival nodules, orbital myositis, orbital inflammatory syndrome, dacryoadenitis, and cranial nerve palsy. Notably, conjunctival nodules in patients with EGPA are very rare lesions but may present with atypical eyelid swelling. Herein, we present a case of EGPA wi
