Hypertension in autoimmune rheumatic diseases - a position paper from the European Society of Hypertension working group on small arteries. Part B: Recommendations for the clinical management of hypertension.

Clinical management of hypertension in patients with autoimmune rheumatic diseases (ARDs) is challenging. Firstly, the impact on blood pressure (BP) of several antirheumatic drugs varies and the available evidence points towards a wide range of effects. Secondly, management of hypertension in terms of diagnosis, BP thresholds and therapeutic targets currently follows the general recommendations, even though patients with ARDs present a markedly adverse cardiovascular profile. Thirdly, lifestyle interventions in terms of non-pharmaceutical management follow the general recommendations. Notably,

Recurrent Candida parapsilosis Complex Endocarditis in a Patient With a Mechanical Mitral Prosthetic Valve Managed Conservatively in a Resource-Limited Setting: A Case Report.

Infective endocarditis is an infection of the endocardium and cardiac valves. Fungal etiologies are rare, accounting for 1-3% of cases, and affect prosthetic valves in 0.1% of cases. We present a case of recurrent fungal endocarditis, caused by Candida parapsilosis in a patient with a mechanical mitral prosthetic valve successfully managed with medical therapy alone in a resource-limited setting. This report highlights the diagnostic and therapeutic challenges of fungal endocarditis and demonstrates that under appropriate clinical conditions, such as the absence of congestive heart failure, he

SGLT2 Inhibition as a Novel Therapeutic Strategy in Rheumatic Heart Disease.

Rheumatic heart disease (RHD) remains a major cause of cardiovascular morbidity and premature mortality among children and young adults, particularly in low- and middle-income countries. Current management relies on antibiotic prophylaxis and late-stage valve interventions, with no approved pharmacologic therapies targeting the immune-inflammatory and fibrotic processes driving disease progression. Sodium-glucose cotransporter 2 (SGLT2) inhibitors, originally developed for glycemic control, have demonstrated cardiovascular and renal benefits beyond metabolic effects. Their pleiotropic properti

Management of Arrhythmias During Pregnancy in Africa.

Cardiac arrhythmias represent a significant clinical concern that have been always addressed. Moreover, Pregnancy introduces profound hormonal, autonomic, and hemodynamic changes that add more risk to cardiac patients and increase arrhythmia susceptibility. In our continent, Africa, there is an added risk because of the high prevalence of rheumatic heart disease, peripartum cardiomyopathy, and limited access to healthcare, contributing to adverse maternal and fetal outcomes. This article addresses the epidemics, mechanisms, and management of arrhythmias during pregnancy in African settings, hi

Rheumatoid arthritis-associated interstitial lung disease: screening, diagnosis, and treatment-an expert group consensus statement.

Interstitial lung disease (ILD) is a common and morbid complication of rheumatoid arthritis. Despite its prevalence and impact on patients, there is no agreement on how to screen for rheumatoid arthritis-associated ILD, when to initiate treatment, and the optimal treatment regimen. Due to a scarcity of high-quality studies to guide strict evidence-based guidelines, a multidisciplinary group of rheumatoid arthritis-associated ILD experts was convened to discuss the current body of evidence, synthesise the evidence qualitatively, and provide clinically useful consensus statements pertaining to t

Induction of Murine Models of Autoimmune Myocarditis, Valvulitis, and Cardiomyopathy.

Myocarditis and valvulitis are inflammatory diseases affecting myocardium and heart valves. Myocarditis, a viral-induced disease of myocardium, may lead to dilated cardiomyopathy and loss of heart function, and valvulitis leads to deformed heart valves and altered blood flow in rheumatic heart disease. Animal models recapitulating these diseases are important for understanding the human conditions. Cardiac myosin is a major autoantigen in heart tissue, and antibodies and T cells to cardiac myosin are evident in inflammatory heart diseases. This article is a practical guide to the induction and

Case Report: Sapho syndrome complicated by chest pain successfully treated with relocation.

Synovitis, acne, pustulosis, hyperostosis, and osteitis (SAPHO) syndrome is a rare chronic autoinflammatory disease characterized primarily by cutaneous manifestations and osteoarticular involvement, and remains challenging to treat. Its pathogenesis is not fully understood and may involve genetic, immunological and environmental factors. Environmental factors play a significant role in autoimmune diseases such as asthma, urticaria, and rheumatic heart disease. Avoiding repeated exposure to sensitizing environmental trigger (e.g., microorganism or allergens) shows considerable potential in the

Caseous Mitral Annular Calcification Causing Left Ventricular Outflow Tract Obstruction and Embolic Stroke in Hypertrophic Cardiomyopathy.

BACKGROUND: Caseous mitral annular calcification is a variant of annular calcification that may alter mitral valve geometry and represent a potential embolic source. CASE SUMMARY: A 50-year-old woman with exertional dyspnea was diagnosed with hypertrophic cardiomyopathy with left ventricular outflow tract obstruction. Transesophageal echocardiography revealed caseous calcifications of the posterior mitral annulus, causing leaflet displacement, promoting systolic anterior motion. Despite optimized medical therapy, she developed ischemic stroke, with imaging suggesting liquefactive degeneration

Prediagnosis frailty and survival after cancer in middle-aged and older adults: a population-based cohort study.

BACKGROUND: Frailty is a multidimensional syndrome reflecting diminished physiological reserve and has been associated with adverse outcomes in middle-aged and older adults with cancer. However, its long-term prognostic significance when assessed prior to cancer diagnosis remains insufficiently characterized. METHODS: We conducted a population-based prospective cohort study using data from the 2006 wave of the Health and Retirement Study (HRS), with follow-up through December 31, 2020. Participants were middle-aged and older adults aged 52 years or older, were cancer-free at baseline, and sub

Myocardial Injury in Rheumatic Diseases: Immune and Microcirculatory and Molecular Mechanisms of Cardiomyopathies.

Immune-mediated myocardial injury is an important yet underrecognized manifestation of systemic rheumatic diseases and represents a biologically heterogeneous process extending beyond traditional cardiovascular complications such as pericardial disease or accelerated atherosclerosis. This review aimed to summarize current evidence regarding the molecular mechanisms underlying autoimmune myocardial injury and propose an integrated pathogenic framework. A structured narrative review of the literature was performed, focusing on molecular and cellular mechanisms, disease-specific pathogenic pathwa

A case report: camptodactyly-arthropathy-coxa vara-pericarditis syndrome: confirmation of a previously undescribed PRG4 splicing variant using synovial tissue RNA studies.

OBJECTIVES: Camptodactyly-arthropathy-coxa vara-pericarditis (CACP) syndrome is an inherited childhood noninflammatory arthropathy that can mimic juvenile idiopathic arthritis (JIA). It consists of congenital or early-onset camptodactyly with arthropathy, but not all patients develop hip or heart involvement. We present a compelling case report that underscores the pivotal role of synovial RNA sequencing in diagnosing and understanding hereditary arthropathies. METHODS: We report an 18-year-old man with presumed recalcitrant JIA who had a previously undescribed mutation, p.Glu940*, resulting

The role of macrophage-myofibroblast transition in the pathogenesis of multi-organ fibrosis.

Fibrotic repair is an innate protective mechanism activated following tissue injuries to promote structural stability and adaptability. However, when this process persists beyond appropriate temporal and spatial limits, it becomes pathological, culminating in irreversible cellular damage and organ dysfunction-similar to a sponge losing its flexibility due to excessive adhesions. These pathological alterations fundamentally rely on myofibroblast-mediated intercellular transition and the accumulation of extracellular matrix (ECM). Increasing evidence indicates that myofibroblasts can originate f
