HeatRisk and air quality risk recommendations: A narrative review and action algorithm for the care of people at risk for and living with cardiovascular disease.

BACKGROUND: Cardiovascular disease remains the leading cause of mortality worldwide and is influenced by climate change-related exposures. Non-optimal temperature and poor air quality exposure worsens cardiovascular outcomes, particularly for individuals with preexisting cardiovascular disease. While the Centers for Disease Control and Prevention and Environmental Protection Agency recommend tools like HeatRisk and Air Quality Index, there is limited guidance for implementation in those with preexisting cardiovascular disease. This narrative review addresses this gap. METHODS: A literature re

β3-Adrenergic Receptor Agonism Reduces Right Ventricular Systolic Pressure in a Preclinical Model of Heart Failure With Preserved Ejection Fraction-Associated Pulmonary Hypertension.

PurposePulmonary hypertension (PH) commonly complicates heart failure with preserved ejection fraction (HFpEF), driving morbidity and mortality, yet no therapies exist for HFpEF-PH. β3-adrenergic receptor (ADRB3) agonists may provide cardiometabolic and anti-inflammatory benefits. We evaluated pharmacological ADRB3 modulation in a preclinical HFpEF-PH model.MethodsHFpEF-PH was induced in male and female AKR/J mice via high-fat diet feeding for 16-20 weeks. At 12-16 weeks of feeding, mice were randomized to low-dose mirabegron (LDM, 3.6 mg/kg/day), high-dose mirabegron (HDM, 14.4 mg/kg/day), ne

Network Analysis of Chronic Disease Diagnosis Co-documentation in Insurance Claims Among Older Arkansans.

BACKGROUND: Multimorbidity among older adults presents growing clinical and economic challenges, particularly in medically underserved states such as Arkansas. However, it remains unclear whether chronic conditions documented in routine healthcare encounters exhibit structured patterns of diagnosis co-documentation or primarily reflect incidental co-documentation. Understanding these patterns may inform future studies of multimorbidity and healthcare utilization. OBJECTIVE: To characterize statewide, sex-stratified patterns of claim-level diagnosis co-documentation among Arkansas adults aged

Plasma proteomic analysis identified potential therapeutic targets for pulmonary arterial hypertension.

Pulmonary arterial hypertension (PAH) remains a progressive disease with high mortality. By integrating the UK Biobank Pharma Proteomics Project (49,272 participants, 337 incident cases) with drug-target Mendelian randomization, we identified 398 plasma proteins associated with incident PAH and 19 with potential causal relevance, 15 of which are druggable. Then we prioritized 5 candidates for expression validation in the Sugen5416 and hypoxia rat model and confirmed significant upregulation of TNFRSF10A and downregulation of UMOD in lung tissue, consistent with human plasma proteomic trends. Chronic kidney disease as a systemic disorder: filtration phenotypes, inflammation and cardiovascular risk in the cardio-kidney metabolic era.

Chronic kidney disease (CKD) is increasingly recognized as a systemic disorder driven by metabolic dysfunction, vascular injury, inflammation, and emerging filtration phenotypes. In the cardio-kidney-metabolic (CKM) era, renal biomarkers such as estimated glomerular filtration rate (eGFR) and albuminuria provide essential insight into early metabolic-renal stress and cardiovascular (CV) vulnerability. This narrative review synthesizes contemporary evidence linking CKD to cardiovascular disease (CVD), emphasizing the multidimensional interactions among filtration impairment, glomerular barrier

Large-scale plasma proteomic profiling enhances prediction of heart failure among hypertensive individuals: prospective insights from the UK Biobank.

BACKGROUND: Hypertension is a major risk factor for heart failure (HF), but current clinical models have limited ability to identify high-risk individuals before overt disease. Plasma proteomics may capture early molecular changes and improve risk prediction. METHODS: We analysed 16,128 hypertensive UK Biobank Pharma Proteomics Project participants without baseline HF. Plasma levels of 2,911 proteins were measured using the Olink Explore platform. Participants were randomly split 1:1 into training and validation sets. HF-associated proteins were identified using Cox models in the training set

Diagnosis and Treatment of Cardiovascular Conditions in Medicare Advantage In-Home Health Risk Assessments.

BACKGROUND: Medicare Advantage insurers conduct in-home health risk assessments (HRAs) to identify high-risk patients and facilitate appropriate care. Critics argue this practice increases risk-adjusted payments without improving quality. Comprehensive data on use of in-home HRAs and downstream treatment are lacking. OBJECTIVE: To examine trends in cardiovascular conditions diagnosed during in-home HRAs, characteristics associated with HRA-based diagnoses, and treatment rates after diagnoses. DESIGN: Cohort study. SETTING: Population-based. PARTICIPANTS: Medicare Advantage beneficiaries.

Heart Transplantation in Adult Fontan Patients: New Insights from Fontan-Associated Liver Disease Beyond Traditional Predictors.

BACKGROUND: Fontan patients are considered high risk for heart transplantation (HT) owing to their complex anatomy, multiple prior chest entries, and universal Fontan-associated liver disease (FALD). The impact of FALD and other preoperative factors remains, however, uncertain. OBJECTIVES: We sought to identify factors associated with 90-day mortality after isolated HT in adults with Fontan circulation, with particular focus on the relationship between liver-related parameters and post-transplant outcomes. METHODS: A retrospective cohort study of all adult Fontan patients undergoing isolated

Life-Course Blood Pressure Trajectories and Focal Myocardial Fibrosis in Older Age in MyoFit46.

BACKGROUND: The impact of life-course blood pressure (BP) trajectories on later-life focal left ventricular (LV) fibrosis by late gadolinium enhancement (LGE) remains elusive. METHODS: MyoFit46 prospectively recruited ≈500 participants from a 1946 birth cohort for a cardiovascular magnetic resonance scan at 77 years, assessing LV LGE calculated using the 3-SD method or 5-SD method. Systolic BP (SBP) and diastolic BP were recorded at 36, 43, 53, 63, 69, and 77 years. For each participant, annual rates of BP change (steepness) and area under the BP trajectory (cumulative burden) for each age in

Calcium-sensing receptor-NLRP3 inflammasome signaling in cardiovascular diseases: Preclinical evidence and translational perspectives.

The chronic low-grade inflammation driven by NLR family pyrin domain containing 3 (NLRP3) inflammasome is increasingly acknowledged as an underlying mechanism of myocardial infarction, heart failure, atrial fibrillation, hypertension, and atherosclerosis. Extracellular Ca2+, amino acids, and other ligands link to intracellular Ca2+ overload, mitochondrial dysfunction, reactive oxygen species generation, thioredoxin-interacting protein activation, and NF-κB-dependent inflammasome priming via the calcium-sensing receptor (CaSR). Experimental studies indicate that CaSR may activate the NLRP3 infl

Echocardiographic Assessment and Clinical Applications of the Left Atrial Stiffness Index in Cardiovascular Disease.

The left atrial stiffness index is an emerging echocardiographic marker that integrates pressure-related burden and atrial reservoir deformation to provide a composite measure of left atrial pressure-deformation coupling. This review summarizes the pathophysiological basis of left atrial stiffness, the methodological principles underlying echocardiographic left atrial stiffness index assessment, and the current evidence supporting its clinical applications across cardiovascular diseases. The strongest evidence is available in heart failure and atrial fibrillation, in which increased left atria

Free Mitochondrial DNA and Mitophagy Dysfunction in Diabetic Kidney Disease: A Central Mitochondrial-Inflammatory Axis Driving Progressive Renal Injury.

Diabetic kidney disease (DKD) remains the leading cause of chronic kidney disease and end-stage renal failure worldwide despite significant advances in glucose-lowering and renoprotective therapies. Emerging evidence identifies mitochondrial dysfunction as a central pathogenic mechanism linking metabolic stress to chronic inflammation and progressive renal injury. In the diabetic kidney, hyperglycemia, lipotoxicity, and oxidative stress impair mitochondrial quality control pathways, particularly mitophagy, a targeted mitochondrial degradation pathway, leading to the accumulation of dysfunction

It Is Possible to Save the Kidney With Renal Artery Occlusion Without Autotransplantation: A Case Report.

Renal artery stenosis is a rare but important cause of renovascular hypertension in children that may lead to malignant hypertension and renal failure. A 13-year-old girl presented with malignant hypertension and acute kidney injury, and imaging demonstrated occlusion of both renal arteries. Endovascular recanalization was unsuccessful because of complete, heavily fibrotic occlusions involving a long segment of the right renal artery extending to the hilum, leaving no suitable distal artery for reconstruction, and a 7.3-mm segment of the left renal artery at its aortic origin. To preserve rena
