Roles of the Altitude-Adapted Immune Microenvironment in Pulmonary Vascular Remodeling in High-Altitude Pulmonary Hypertension: A Review.

High-altitude pulmonary hypertension (HAPH) is a debilitating condition caused by chronic hypobaric hypoxia at high altitudes, leading to progressive pulmonary vascular remodeling and right heart failure. The altitude-adapted immune microenvironment plays a pivotal but underappreciated role in HAPH progression: innate immune cells mediate early endothelial damage and proinflammatory signaling, while adaptive immune dysregulation sustains chronic inflammation. This review focuses on hypoxic-induced altitude-adapted immune microenvironment alterations - including immune cell phenotypic reprogram

Intensive blood pressure control to prevent major cardiovascular events in individuals with high-normal blood pressure (prehypertension): PRINT-TAHA9 randomized clinical trial.

BACKGROUND: The optimal treatment for individuals with high-normal blood pressure (BP, systolic BP 130-139 mmHg and diastolic BP < 90 mmHg) is debated. This study evaluates whether pharmacologically reducing systolic BP to below 130 mmHg could prevent major adverse cardiovascular events (MACE) in high-normal BP cases with no comorbidities and 10-year atherosclerotic cardiovascular disease (ASCVD) risk ≥ 7.5%. METHODS: In this randomized, controlled, parallel, unicentric trial, participants were assigned to either an intervention group (pharmacotherapy plus diet control) or a control group (di

Regulating the Impact of Hypertension on Stroke: Therapeutic Potential of a Peptide Mimetic of Tyrosine Phosphatase STEP.

BACKGROUND: In spite of the advances in understanding the pathophysiology of stroke, successful treatment remains a major challenge. The lack of consideration of preclinical studies with associated comorbidities is a primary factor for the translational failure, as ~94% of patients with stroke have ≥1 preexisting comorbidities. Because hypertension is the most common comorbid condition in patients with stroke and predisposition to hypertension is known to worsen stroke outcome, we evaluated the efficacy of a novel neuroprotective peptide derived from the brain-enriched and neuron-specific tyro

Natural products modulate mitochondria-associated membranes to prevent cardiovascular diseases.

BACKGROUND: Cardiovascular disease (CVD) represents a leading global cause of morbidity and mortality, and its prevalence and associated mortality are projected to rise significantly alongside the aging of the population. Aging, as a primary and non-modifiable risk factor, contributes to the progressive decline in cardiovascular structure and function, thereby predisposing individuals to a spectrum of CVDs, including coronary artery disease (CAD), heart failure (HF), and hypertension. A growing body of experimental evidence indicates that mitochondrial and endoplasmic reticulum (ER) dysfunctio

AI-assisted multimodal assessment for right ventricular function from echocardiography predicts mortality in patients with pulmonary hypertension and right heart failure.

Right ventricular (RV) strain is an important factor in evaluating RV function. This study aimed to predict mortality in patients with pulmonary hypertension (PH) and right heart failure (RHF) using an artificial intelligence (AI)-assisted multimodal echocardiography approach. A total of 586 patients with PH and RHF were enrolled. Transthoracic echocardiography (TTE) was performed either before hospital admission in the outpatient department or after admission in the inpatient unit. RV structure and function were assessed using two-dimensional measurements and speckle tracking strain analysis

Small left ventricle identifies a high-risk phenotype in patients hospitalized for heart failure with preserved or mildly reduced ejection fraction.

In heart failure with mildly reduced (HFmrEF) or preserved ejection fraction (HFpEF), a smaller left ventricular end-diastolic diameter (LVEDD) has been associated with lower stroke volume and functional capacity. However, its association with clinical outcomes remains unexplored. We evaluated the prognostic impact of small LV in patients hospitalized with acute HFpEF. We performed a single-center retrospective analysis of a prospectively collected cohort of patients admitted for acute heart failure (AHF) with LVEF >40%. Small LV was defined according to ASE guidelines. The primary outcome was

The role and therapeutic potential of purinergic signaling pathway in metabolic syndrome-related atherosclerosis.

Metabolic syndrome (MetS) is a cluster of interrelated and co-occurring metabolic disorders, including abdominal obesity, hyperglycemia, dyslipidemia, the reduced high-density lipoprotein (HDL) cholesterol, and/or hypertension. Cardiovascular diseases are the leading causes of death and disability in metabolic syndrome. Each component of MetS is an independent risk factor for cardiovascular diseases, and the combination of these factors can increase the incidence and severity of cardiovascular diseases, such as atherosclerosis, myocardial infarction, and heart failure, with atherosclerosis bei

SIRT1: The first key to unlocking the mystery of cardiovascular diseases.

Cardiovascular disease, (CVD) the leading cause of death worldwide, encompasses various heart and vascular disorders that significantly elevate morbidity and mortality rates. Sirtuin 1 (SIRT1), a NAD+ dependent deacetylase, plays a key role in cardiovascular health and pathology by regulating fundamental processes such as endothelial function, oxidative stress, inflammation, senescence, metabolism, cardiac hypertrophy, and heart failure. Through these mechanisms, SIRT1 emerges as critical factor in the pathophysiology of CVDs, including heart failure, atherosclerosis, hypertension, and myocard

Sex-Specific Differences in PVAT Pathobiology.

Perivascular adipose tissue (PVAT) has been widely studied over the past 2 decades for its anticontractile function, morphology, and adipokine secretion. However, most data were obtained from male sex, with few studies on female PVAT. Indeed, the PVAT secretome, adipocyte phenotype, progenitor cells, and responses to cardiovascular risk factors exhibit sex differences and may be modulated by sex hormones. Here, we compiled data evaluating PVAT based on sex differences or focused on the female sex. Sex comparisons in PVAT function and morphology under physiological conditions and in pathologica

Integrative transcriptomic profiling links telomere dysfunction to cGAS-STING activation in heart failure signatures in mice and humans.

AIMS: Cardiomyocyte telomere shortening is evident during heart failure pathogenesis. Conversely, mice with engineered telomerase deficiency develop myocardial dysfunction accompanied by p53 activation and mitochondrial repression. We aimed to address fundamental gaps in knowledge: whether cardiac dysfunction in mice lacking telomerase components arises from myocardial-intrinsic effects or systemic consequences of telomere shortening, which broader transcriptional programs follow, and what implications these have for clinical heart failure. METHODS AND RESULTS: We generated telomerase-deficie
