The Intersection of Heart Failure with Preserved Ejection Fraction and Chronic Kidney Disease.

Heart failure and chronic kidney disease (CKD) often co-exist. Heart failure with preserved ejection fraction (HFpEF) contributes to significant morbidity and mortality among patients with CKD. Among patients with CKD, along with traditional risk factors for heart failure, numerous CKD-specific pathophysiological pathways contribute to the heightened risk of HFpEF development. However, the heterogeneity of HFpEF presents numerous challenges for diagnosis, treatment, and prevention, which has led to various phenotyping approaches. Phenotyping HFpEF may allow for better HFpEF characterization fo

Hypoxia-inducible factor-1α in cardiovascular disease, mechanistic insights, pathophysiological roles, and therapeutic targeting strategies.

Hypoxia-inducible factor-1α (HIF-1α) is a critical mediator of hypoxic response at the transcriptional level and an emerging player in cardiovascular disease (CVD). Under normoxic conditions, HIF-1α undergoes prolyl hydroxylase (PHD)-mediated hydroxylation, which promotes its recognition by the von Hippel-Lindau (VHL) E3 ubiquitin ligase and subsequent proteasomal degradation. In contrast, hypoxic conditions inhibit this degradation pathway, leading to stabilization of HIF-1α, its nuclear translocation, heterodimerization with HIF-1β, and activation of transcriptional programs that regulate an

Epigenetic Regulation and Molecular Mechanisms in Cardiovascular Diseases: A Review of Recent Advances and Therapeutic Implications.

Cardiovascular diseases (CVDs) remain the leading cause of death worldwide, with growing evidence indicating that epigenetic mechanisms play a central role in their onset and progression. This review provides a comprehensive overview of current knowledge on the epigenetic regulation and molecular mechanisms involved in CVDs, as well as their potential therapeutic implications. The findings demonstrate that DNA methylation, histone modifications, and non-coding RNAs are key regulators of gene expression associated with cardiac hypertrophy, atherosclerosis, myocardial infarction, and heart failu

Neutrophil Extracellular Traps: Potential Therapeutic Targets of Traditional Chinese Medicine and Natural Products for Cardiovascular Diseases.

Cardiovascular disease (CVD) remains a leading cause of global morbidity and mortality, and its initiation and progression are closely associated with multiple molecular mechanisms. Neutrophil extracellular traps (NETs) are mesh-like structures composed of DNA, histones, and antimicrobial proteins that are released by neutrophils during inflammation or infection. They play a crucial role in innate immune defense. However, when the dynamic balance of NETs is disrupted by excessive formation, persistent accumulation, or impaired clearance, NETs are no longer merely bystanders. Instead, they acti

The effect of bedtime vs. morning antihypertensive treatment on cardiovascular events: A meta-analysis.

BACKGROUND: Antihypertensive medications are essential for preventing cardiovascular events and have traditionally been taken in the morning. However, recent studies have suggested that taking the medication at bedtime may be more effective in reducing cardiovascular risk. This study aimed to examine the association between dosing time and cardiovascular outcomes. METHODS: Randomized controlled trials were retrieved through a systematic literature review using PubMed and Embase. The primary outcome was major adverse cardiovascular events (MACE), defined as a composite of all-cause (or cardiov

Metabolic syndrome and a broken heart: trust your gut or risk your heart.

Gut dysbiosis is increasingly recognized as a contributor to heart failure; however, its specific role in the development of metabolic syndrome-induced heart failure (MetS-HF) remains poorly defined. A defining feature of MetS-HF is cardiac steatosis, which drives lipotoxicity, maladaptive remodeling, and progressive cardiac dysfunction. This review integrates mechanistic and translational evidence showing how gut microbiota dysbiosis initiates and exacerbates MetS-HF by disrupting lipid homeostasis, leading to myocardial steatosis, metabolic remodeling, cardiomyocyte death, adverse structural

Costs and consequences of cardiovascular toxicity due to Bruton's tyrosine kinase inhibitors in chronic lymphocytic leukemia.

INTRODUCTION AND OBJECTIVES: BTK inhibitors (BTKis) have revolutionized the treatment landscape of chronic lymphocytic leukemia (CLL). However, a higher risk of cardiovascular toxicity is reported among CLL patients treated with BTKis. This study aims to estimate the costs and consequences of cardiovascular toxicity associated with the different BTKis used for CLL treatment in Portugal. METHODS: The incidence rates and health-related outcomes consequences among CLL patients, including bleeding events, de novo atrial fibrillation (AF) and hypertension were retrieved from the literature for the

Galectin-1 Is a Marker but Not a Mediator of Heart Failure With Preserved Ejection Fraction.

BACKGROUND: The immune system is emerging as a key player in driving cardiac remodeling in heart failure with preserved ejection fraction (HFpEF). Galectin-1 (Lgals1) is a carbohydrate-binding protein that we previously identified as being upregulated in cardiac myeloid cells in a preclinical model of HFpEF. Our objective was to determine the role of galectin-1 in HFpEF in both preclinical models and clinical cohort studies. METHODS: Galectin-1 was measured using the Olink proximity extension assay in human cohorts. HFpEF was induced in mice with myeloid-specific and global deletion of galect

Myocardial injury in patients with acute ischemic stroke: Prevalence and types of triggers of myocardial demand ischemia.

INTRODUCTION: Acute myocardial injury occurs in about every fourth patient in the early phase after ischemic stroke. It may be caused by an imbalance between myocardial oxygen supply and demand, potentially leading to type 2 myocardial infarction (MI). However, little is known about the prevalence of potential triggers of such demand ischemia in acute stroke. PATIENTS AND METHODS: Consecutive patients with and without post-stroke acute myocardial injury (elevated high-sensitivity cardiac troponin T [hs-cTnT] levels with a rise/fall >20%) were matched for age and sex and retrospectively screen

Screening for undiagnosed atrial fibrillation in community pharmacies using mobile electrocardiogram technology: a quasi-experimental cross-sectional study (PREVENIM ICTUS).

BACKGROUND: Atrial fibrillation (AF) is a common, often silent, arrhythmia that markedly increases stroke risk yet remains undiagnosed in many high-risk adults. Mobile electrocardiogram technology in community pharmacies has detected 1-5% new AF internationally, but real-world pharmacist-led data in Southern Europe are scarce. Our study screened adults ≥ 55 years with cardiovascular risk factors in Spanish pharmacies to determine the frequency of undiagnosed AF and facilitate early stroke prevention. METHODS: A quasi-experimental multicenter cross-sectional study was conducted in Community Ph

The fate of the Fontan circulation.

Since the first operation performed in 1968, the worldwide population of patients living with Fontan circulation is dramatically growing, with 40% reaching adulthood in the current era. Despite this remarkable improvement in survival, some patients struggle with Fontan-related complications including heart failure, arrhythmias, end-organ dysfunction, and premature death. This review aims at describing common complications of the Fontan circulation, proposing a clinical and mortality risk score to better stratify this complex population as well as exploring the current state of art in catheter

A Three-Year Observational Study of the Prevalence of Left Ventricular Diastolic Dysfunction in Asymptomatic Individuals With Cardiovascular Risk Factors.

BACKGROUND: Left ventricular diastolic dysfunction (LVDD) precedes heart failure and may remain undetected in asymptomatic individuals. Early identification is particularly relevant in populations with cardiometabolic risk factors, where subclinical LVDD could refine risk stratification and guide preventive strategies. OBJECTIVE: To determine the prevalence of clinically significant LVDD (grade II-III) among asymptomatic adults aged 51-70 years and to compare its burden across major cardiovascular risk groups. METHODS: This retrospective cross‑sectional study analyzed 10,434 transthoracic ec

Overweight and obesity association with mortality in patients with heart failure and reduced or preserved ejection fraction-a cohort study.

BACKGROUND: Obesity is a risk factor for incident heart failure, but patients with excess weight and heart failure have lower mortality. This "obesity paradox" may be explained either by a favourable effect of the adipose tissue or by confounding. We aimed to assess if body mass index (BMI) is associated with lower mortality after extensive adjustment for prognostic factors in patients with reduced (HFrEF) or preserved (HFpEF) ejection fraction. METHODS: Prospective, observational study including consecutive patients hospitalized for acute heart failure. Two years hazard of mortality was asse

Effect of Sodium Restriction on Cardiovascular Outcomes in Patients With Hypertension and Heart Failure: A Systematic Review and Meta-Analysis.

Dietary sodium restriction is a cornerstone of cardiovascular disease management guideline recommendations. However, sodium restriction's impact on hard clinical endpoints in patients with heart failure remains controversial, showing conflicting results compared to the established benefits in essential hypertension. A systematic review and meta-analysis of randomized controlled trials (RCTs) and observational cohort studies evaluating the effect of sodium restriction on all-cause mortality and cardiovascular hospitalization was conducted. Databases were searched through December 2025. Random-e

Temporal changes in first-phase ejection fraction during evolution of heart failure with preserved ejection fraction and afterload-induced heart failure in mice.

The interplay between systolic and diastolic dysfunction in heart failure with preserved ejection fraction (HFpEF) progression is unclear. First-phase ejection fraction (EF1), a sensitive marker of early systolic function, aids in assessing systolic-diastolic relationships in human hypertension and aortic stenosis. This study examines temporal changes in these relationships in mouse models of HFpEF and elevated afterload. Mouse models of abdominal aortic banding (AAB) and HFpEF (induced by hypertension and high fat feeding) underwent comprehensive serial echocardiography. In AAB, EF1 significa

Cardiovascular Disease and Cancer: Two Sides of the Same Coin.

BACKGROUND: Growing evidence shows that the relationship between cancer and cardiovascular disease is not unidirectional. The recognized impact of cardiovascular risk factors and cardiovascular disease on cancer risk has led to the coining of the term "reverse cardio-oncology." SUMMARY: Cardiovascular risk factors such as smoking, hypertension, diabetes, dyslipidemia, and obesity are involved in several pathophysiologic mechanisms that also underlie cancer development and progression. These mechanisms include inflammation, oxidative stress, and DNA damage. In addition, cardiovascular diseases
