Evaluation of large-scale plasma proteomics for prediction of heart failure in individuals with a full range of glucose metabolism profiles.

AIMS: Individuals with abnormal glucose metabolism are at a significantly higher risk of developing heart failure (HF). However, strategies for early identification of HF in this high-risk population remain inadequate. This study aimed to identify plasma protein biomarkers associated with HF development and construct predictive models to identify at-risk individuals. METHODS AND RESULTS: We analyzed HF development in abnormal glucose metabolism population using data from 6517 participants in discovery cohort and 2783 in validation cohort, all from the UK Biobank, with no prior history of HF.

Identification of Risk Factors and Prediction Models for Acute Kidney Injury in Hospitalized Elderly Patients with Heart Failure.

BACKGROUND: Elderly heart failure (HF) patients have an increased risk of acute kidney injury (AKI) during hospitalization. Identifying risk factors and developing a prediction model are clinically important. STUDY METHOD: We retrospectively enrolled 7076 elderly HF patients hospitalized from January 2019 to December 2023. All predictors were restricted to data available within 24 hours of admission and prior to AKI occurrence. Candidate predictors were first screened by univariate analysis (P<0.05) and then further refined by LASSO regression, and the selected variables were ultimately enter

PDSNet: Patient-Disease Dual Spatial Similarity Neural Networks for Predicting Heart Failure Risk Using Short Electronic Health Records.

Heart failure (HF) is a complex and heterogeneous syndrome caused by diverse factors, such as atrial fibrillation, diabetes, and pulmonary hypertension. The intricate pathophysiology of HF, coupled with variability in patient demographics and presentations, poses significant challenges to the effectiveness of existing deep learning models in HF risk prediction. In this paper, we propose a novel deep neural network, PDSNet, which leverages a new dual patient-disease spatial similarity strategy to improve HF risk prediction using short electronic health records. First, we develop ontology graphs

Population attributable risk of traditional, under-recognised and female-specific factors for heart failure in women.

BACKGROUND: Heart failure (HF) imposes a major health burden on women. While traditional (well-established) risk factors are well studied, less attention has been given to psychosocial, environmental and female-specific reproductive risk factors. This study quantifies the contribution of these risk domains to incident HF in women. METHODS: We included 233 125 women from the UK Biobank with a median follow-up of 13.7 years. 22 risk factors were grouped as well-established (eg, hypertension), under-recognised (eg, depression, socioeconomic deprivation) or female-specific (eg, early age at menop

Cardiovascular disease in Sickle cell: mechanisms, diagnostics and therapeutic advances.

Cardiovascular complications are increasingly recognised as a major driver of morbidity and early mortality in patients with sickle cell disease (SCD), yet they remain underdiagnosed and underappreciated. This contemporary review synthesises current knowledge across a spectrum of cardiovascular manifestations-including myocardial dysfunction, pulmonary hypertension, cardiac iron overload, arrhythmias, myocardial infarction, stroke and sudden death-with emphasis on their unique pathophysiological mechanisms in SCD. We highlight emerging diagnostic tools such as cardiac magnetic resonance with T

Central Pressure Variability After Low-Level Tragus Stimulation in Acute Decompensated Heart Failure.

BACKGROUND: Increased variability in central aortic systolic pressure (CASP) and pulse pressure (PP) are predictors of poor cardiovascular disease outcomes. Low-level tragus stimulation (LLTS) is a noninvasive method to reduce sympathetic tone via vagal afferent fibers. It is unknown if LLTS has a favorable effect on elevated variability in CASP and PP in patients with acute decompensated heart failure (HF) (ADHF). METHODS: Patients hospitalized for ADHF after initial stabilization (median 80 years, males 60%) were randomly assigned to active or sham group, and LLTS (20 Hz, 1 mA) was delivere

The Narrative Review: Advancements in Heart Failure Diagnosis and Management using Artificial Intelligence: A New Era of Patient Care.

Heart Failure (HF) is a prevalent medical illness worldwide that affects millions and is a substantial economic burden. Its epidemiological impact is on the rise due to factors such as the ageing of the population, increasing rates of diabetes and hypertension, and better survival post-myocardial infarction. Some limitations in HF management include diagnostic challenges, sudden progression of the disease, and rising rates of readmission. Continuous monitoring and limited therapeutic interventions add further complexity to care. Artificial Intelligence(AI) is essential in health care and has p

Blood pressure variability is associated with heart failure risk, elevated NT-proBNP, and elevated high-sensitivity troponin: the Multi-Ethnic Study of Atherosclerosis.

BACKGROUND: It is not well established whether blood pressure variability (BPV) is associated with risk of incident heart failure (HF) as well as with subclinical markers of HF and myocardial injury. We investigated these relationships in the Multi-Ethnic Study of Atherosclerosis (MESA). METHODS: We examined the association between visit-to-visit BPV (estimated by variability independent of the mean-VIM) and HF in MESA (2000-2012), a community-based cohort study of 6814 individuals free of clinical cardiovascular disease (including HF) at baseline, using Cox models and joint longitudinal-surv

Comparing diagnostic tools for heart failure with preserved ejection fraction across community and clinical cohorts.

BACKGROUND: Diagnosing heart failure with preserved ejection fraction (HFpEF) remains challenging, particularly in older adults. While the Heart Failure Association (HFA)-PEFF and H2FPEF Scores offer structured diagnostic approaches, their clinical utility is still debated. This study aims to compare the diagnostic accuracy of HFpEF Scores versus inclusion criteria used in sodium-glucose cotransporter-2 inhibitors (SGLT2i) trials, age-adjusted N-terminal pro B-type natriuretic peptide (NT-proBNP) thresholds and the universal definition of heart failure (HF). METHODS: Diagnostic tools were ass

The role of the right ventricle in pulmonary arterial hypertension and imaging methods for non-invasive evaluation.

INTRODUCTION: Pulmonary arterial hypertension (PAH) is a progressive disease characterized by the remodeling of pulmonary circulation, leading to increased pulmonary vascular resistance (PVR) and right ventricular (RV) overload. While the right heart may initially compensate, many patients ultimately face right heart failure and higher risk of mortality. Significant progress has been made in RV management, including advancements in molecular understanding, development of monitoring tools, and the exploration of new therapeutic pathways. Left ventricular hypertrophy and myocardial fibrosis in heart failure with preserved ejection fraction: mechanisms and treatment.

Heart failure with preserved ejection fraction (HFpEF) accounts for nearly half of all heart failure cases and is characterized by phenotypical heterogeneity with a high prevalence of multiple, often overlapping cardiometabolic disorders. Comorbidities such as hypertension, obesity, or diabetes are present in many HFpEF patients and are hypothesized to contribute to adverse cardiac remodelling and myocardial fibrosis through a variety of haemodynamic and metabolic impairments, with nearly half of all HFpEF patients exhibiting left ventricular (LV) hypertrophy or concentric remodelling. Myocard

Association between plasma phosphate/pyrophosphate ratio and computed tomography-derived aortic valve calcification score in an unselected cohort of cardiovascular patients.

AIMS: Inorganic pyrophosphate (PPi) is an endogenous inhibitor of soft tissue calcification. A disturbed equilibrium between pro- and anti-mineralization agents, like extracellular phosphate (Pi) and PPi, has been implicated in the mechanism of aortic valve calcification (AVC). We aimed to investigate the association of the plasma PPi concentration and Pi/PPi ratio with the degree AVC in cardiovascular patients. METHODS AND RESULTS: One hundred and fifty-four patients referred for cardiac computed tomography (CT), including 43 individuals with severe aortic stenosis, were prospectively enroll

The complement-dendritic cell-endothelial cell crosstalk in vascular inflammation.

Inflammation is a highly co-ordinated process involving multiple immune components, including the complement system, dendritic cells (DCs), and endothelial cells (ECs). All of them play pivotal roles in immunity and host defence and have a significant impact on cardiovascular health and disease. While individual functions of the complement system, DCs and ECs are well-characterized, their multifaceted interplay in modulating inflammation and disease is insufficiently elucidated. The complement system exerts predominantly pro-inflammatory effects, influencing the immune, cardiovascular, and ner
