Orientia tsutsugamushi Alters Mitochondrial Function and Selectively Associates with VDAC.

Orientia tsutsugamushi is an obligate intracellular alphaproteobacterium and the causative agent of the potentially fatal rickettsiosis, scrub typhus. During infection, O. tsutsugamushi replicates exclusively in the eukaryotic cytosol near mitochondria and alters host metabolic pathways governed by mitochondria. We report that O. tsutsugamushi induces mitochondrial enzymatic impairment and structural abnormalities without altering mitochondrial abundance or the levels of proteins that maintain mitochondrial homeostasis. Confocal and structured illumination microscopy revealed a selective spati

Scrub Typhus-Associated Inflammatory Oligoarthritis Mimicking Tubercular Arthritis in an Endemic Setting.

Scrub typhus is a common cause of acute febrile illness in endemic regions; however, musculoskeletal involvement is usually limited to myalgia, and true inflammatory arthritis is rarely reported. We describe a 19-year-old previously healthy male military recruit who presented with a two-month history of progressive bilateral knee swelling and left ankle pain, accompanied by fever, weight loss, and lymphadenopathy. Clinical evaluation revealed inflammatory oligoarthritis with culture-negative synovial fluid. Given the chronicity of symptoms and systemic features, tubercular and septic arthritis

Case Report: An imported severe case of paediatric scrub typhus with Karp B subgenotype in non-endemic Northern China, Beijing.

Scrub typhus, a zoonosis caused by Orientia tsutsugamushi (O. tsutsugamushi), remains a significant public health threat in the Asia-Pacific region. This disease is transmitted through the bite of infected trombiculid mite larvae (chiggers) and typically manifests as acute undifferentiated fever during the early stage. Despite the availability of targeted antibiotic therapies, delayed diagnosis frequently leads to severe complications and fatal outcomes. Here, we report a severe imported paediatric case in Beijing, a city in China's temperate zone, involving a 12-year-old girl with a recent tr

Acarological investigation and detection of Orientia tsutsugamushi (Rickettsiales: Rickettsiaceae) in field-collected chiggers (Acari: Trombiculidae) and small mammals from scrub typhus positive localities in Pahang, West Malaysia.

Scrub typhus (ST) is an acute febrile illness caused by Orientia tsutsugamushi (Hayashi, 1920) that is transmitted to humans through the bite of infected chiggers. In recent years, ST has re-emerged as a public health concern in Malaysia. In 2024, more than 50 laboratory-confirmed human cases of ST were reported in Pahang. A collaborative investigation was undertaken on a subset of 26 cases to identify potential vector sources and animal hosts associated with O. tsutsugamushi transmission. Chigger-based sampling was conducted by trapping small mammals in suspected infection hotspots, including

Integrative taxonomy of Leptotrombidium species collected from murine hosts in Thailand with emphasis on L. deliense.

Leptotrombidium chiggers (larval trombiculid mites) are medically important vectors of scrub typhus caused by Orientia tsutsugamushi. We performed comparative genetic and morphometric analyses of four Leptotrombidium taxa (L. deliense, L. imphalum, L. turdicola, and Leptotrombidium cf. guzhangense), isolated from murine rodents from various localities across Thailand. Phylogenetic analysis was performed using mitochondrial and nuclear gene sequences using specimens of L. deliense from different geographical localities. Quantitative morphological variation was examined using linear measurements

Epidemiological trends and regional risk factors for scrub typhus in Busan, Republic of Korea: 2019-2023.

BACKGROUND: Scrub typhus, an acute disease caused by Orientia tsutsugamushi and transmitted by larval chigger mites, is a representative autumn febrile illness in the Republic of Korea. We examined epidemiological characteristics underlying spatial heterogeneity in Busan Metropolitan City to inform region-specific prevention and control policies. METHODS: We analyzed 1,645 confirmed and suspected scrub typhus cases reported in Busan from 2019 to 2023. Crude incidence rates and age-standardized incidence rates (ASIRs) per 100,000 population were calculated using the 2021 mid-year population. T

Differential spleen immune signatures and germinal center responses during acute infection with Orientia tsutsugamushi Karp versus Gilliam strains.

INTRODUCTION: Scrub typhus is an emerging and neglected tropical disease caused by Orientia tsutsugamushi (Ot). Immunity in scrub typhus patients is known to be short-lived; however, its underlying mechanisms remain unclear. No reports have examined humoral immune signatures to clinically prevalent Ot strains. METHODS: We compared two clinically relevant Ot strains, Karp and Gilliam, in a C57BL/6 murine model. Using equivalent infectious doses, we assessed splenic B cell and germinal center (GC) responses during acute infection through flow cytometry, immunohistology, serological analyses, an

Eschars and estate fever: A case series from the Johorean oil palm heartland highlighting the changing face of scrub typhus in Malaysia.

Scrub typhus, a zoonotic bacterial infection caused by the arthropod-borne, Gram-negative, obligate intracellular bacillus Orientia tsutsugamushi and transmitted by larval chiggers, is an underdiagnosed cause of acute undifferentiated febrile illness. Its clinical spectrum ranges from a self-limited denguelike illness to rapidly progressive multiorgan dysfunction driven by smallvessel vasculitis, including pneumonitis, acute kidney injury, hepatitis, encephalitis and circulatory shock. Early recognition is frequently missed because the characteristic eschar may be absent or easily overlooked i

Scrub Typhus Presenting With Meningoencephalitis and Multiorgan Dysfunction Syndrome.

Scrub typhus is an acute febrile illness endemic in India and remains an important yet often underdiagnosed cause of severe systemic infection. It is caused by Orientia tsutsugamushi and is characterized by endothelial injury leading to widespread vasculitis. We report the case of a 62-year-old male with a history of chronic alcohol use and no known comorbidities, who presented with a short history of fever, myalgia, arthralgia, mild confusion, and gait difficulty. The illness rapidly progressed to scrub typhus septicemia complicated by pulmonary edema, pleural effusion, meningoencephalitis, a

Differentiating Spotted Fever From Scrub Typhus Based on Clinical and Laboratory Features: A Retrospective Cohort Study From South India.

BACKGROUND: Scrub typhus and spotted fever are the two most common rickettsial infections in India. Both conditions differ, however, in terms of epidemiological risk, clinical presentation, and prognosis. We aimed to compare the clinical and laboratory characteristics of patients diagnosed with scrub typhus or spotted fever to identify disease indicators that are useful in low-resource endemic settings. METHODS: We conducted a retrospective cohort study among adults admitted with undifferentiated fever lasting 5-15 days at a tertiary care hospital in coastal Karnataka, India, between January

Surveillance of vector-borne infections: Dogs as sentinel animals for Orientia and Rickettsia exposure in five regions in Chile.

Domestic dogs have been proposed as sentinels for various infectious diseases, including rickettsial infections. In Chile, the only endemic human rickettsiosis is scrub typhus caused by Ca ndidatus Orientia chiloensis, an emerging infectious disease, discovered in recent years in southern Chile. The geographic distribution of scrub typhus and other rickettsial infections across Chile is incompletely understood. This study using domestic dogs as sentinel investigated the potential exposure to members of the three main groups of rickettsiae, scrub typhus orientiae (STGO), spotted fever group ric

Plasma and Intracellular Concentrations of Doxycycline and Azithromycin in Patients with Severe Scrub Typhus.

BACKGROUND/OBJECTIVES: Scrub typhus, a life-threatening infection caused by Orientia tsutsugamushi, is treated with doxycycline or azithromycin. In severe disease, combination therapy with azithromycin and doxycycline had better clinical outcomes than either drug alone. However, it is not clear what causes the improved efficacy. To understand the same, we examined the plasma concentrations, intracellular concentrations, and efficacy of doxycycline, azithromycin, and both drugs in combination in 51 patients with severe scrub typhus. METHODS: A randomly selected subset of adult (>18 years) part
