Scrub typhus is a potentially fatal and neglected tropical infectious disease caused by the bacterium Orientia tsutsugamushi; limited information is available on scrub typhus in infants. In the present retrospective study, the clinical presentation, laboratory findings, and outcomes of infants with scrub typhus admitted to a tertiary care hospital in India were investigated over a decade. A retrospective review of 49 infants (<1 year) with laboratory-confirmed scrub typhus admitted between 2014 and 2023 was conducted. Clinical, laboratory, and outcome data were analyzed to identify predictors

Chikungunya fever is an arboviral disease caused by an alphavirus belonging to the family Togaviridae. It commonly presents with fever, severe arthralgia, rash, and myalgia, while severe pulmonary involvement causing acute respiratory distress syndrome (ARDS) is rare. We report a 62-year-old man with obesity, diabetes mellitus, hypertension, obstructive sleep apnoea, coronary artery disease, and interstitial lung disease, who presented with high-grade fever, severe arthralgia, facial puffiness, and rapidly progressive dyspnoea, following recent travel to Lucknow, India. On admission, he had se

BACKGROUND: Scrub typhus has emerged as a significant cause of acute febrile illness in India, with increased morbidity when diagnosis is delayed. Its nonspecific clinical presentation and limited availability of advanced diagnostic facilities pose challenges in timely identification. Although the indirect immunofluorescence assay is the reference standard, it is not widely accessible in resource-limited settings, necessitating the use of rapid and reliable diagnostic tests.
OBJECTIVE: The objective of this study was to evaluate the diagnostic performance of a rapid immunochromatographic test

BACKGROUND/AIM: The role of tumor-associated neutrophils (TANs) in the tumor microenvironment (TME) remains unclear in lung cancer. Various aspects of TANs, such as their maturation stage, activation state, and enzyme release, are controversial. This study focused on neutrophil elastase (NE) from TANs, aiming to evaluate its role in lung cancer and determine whether NE could be a potential therapeutic target.
MATERIALS AND METHODS: Lung cancer and promyelocytic leukemia cell lines were used to evaluate whether NE is associated with neutrophil recruitment using a chemotaxis assay. The associat

RATIONALE: Murine typhus, caused by Rickettsia typhi, is a globally distributed flea-borne rickettsiosis. Although rarely recognized, it can trigger hemophagocytic lymphohistiocytosis (HLH), a life-threatening hyperinflammatory syndrome. Nonspecific febrile illness and atypical petechial eruptions frequently lead to delayed or missed diagnoses. This report aims to illustrate the diagnostic process and clinical implications of murine typhus-associated HLH in a pediatric patient, and to evaluate the utility of metagenomic next-generation sequencing (mNGS) as an unbiased detection tool for occult

Rickettsioses are emerging zoonotic diseases in Southeast Asia caused by vector-borne, intracellular Rickettsia bacteria that threaten public health, animal welfare, and food security. Despite their importance to rural livelihoods, the epidemiology of rickettsial exposure in livestock remains poorly understood. This study used abattoir-based serology to evaluate livestock as sentinels of environmental exposure to Rickettsia in Lao PDR and to identify spatial hotspots and risk factors for seropositivity. Abattoir-based serological data were generated from cattle, pigs, and water buffalo samples

BACKGROUND: Murine typhus, a flea-borne infection caused by Rickettsia typhi, often presents with nonspecific symptoms that delay diagnosis. While usually self-limiting, it can rarely progress to multiple organ dysfunction syndrome (MODS). We report the first case of murine typhus complicated by symmetrical peripheral gangrene (SPG), in which metagenomic next-generation sequencing (mNGS) enabled rapid diagnosis and guided timely doxycycline therapy.
CASE PRESENTATION: A 69-year-old female from South China was hospitalized with persistent abdominal pain and low-grade fever. She was a farmer an

Flea-borne typhus (FBT) is an infection caused by the bacteria Rickettsia typhi. It is usually an acute undifferentiated febrile illness; however, approximately one-quarter of patients suffer from organ-specific complications. In the present report, the case of a 51-year-old woman who originally presented to the clinic with a febrile illness is described. After being given supportive care with intravenous fluids and antiemetics for presumed viral infection, she returned to the emergency department the next day in a hypotensive state and experiencing respiratory distress, requiring emergent int

BACKGROUND: Murine typhus is a flea-borne disease caused by Rickettsia typhi that typically presents as an acute febrile illness. The diagnosis is often missed, leading to delays in appropriate treatment. A qPCR targeting a single gene (ompB) for R. typhi is widely used for diagnosis; however, it has low sensitivity for detecting bacterial DNA in patients' blood. We aimed to increase sensitivity of detection of R. typhi using qPCR by targeting a gene containing repetitive sequences, sca2 (surface cell antigen 2).
METHODOLOGY: We compared diagnostic accuracy with the standard assay targeting s

Murine typhus (also called flea-borne or endemic typhus) is an undifferentiated febrile illness caused by the bacterium Rickettsia typhi. The disease, transmitted by rat and cat fleas, is endemic to seaboard regions worldwide. Recently, murine typhus has reemerged as an increasingly recognized cause of febrile illness in the United States, especially in Texas and Southern California. In addition to fever, manifestations often include headache, malaise, myalgias, and a maculopapular rash in approximately half of cases. Although usually considered a mild illness, when untreated, symptoms can las

BACKGROUND: Scrub typhus and murine typhus, caused by Orientia tsutsugamushi and Rickettsia typhi, respectively, are important causes of febrile illness in Laos. Although several studies have assessed rickettsial infection in selected provinces, the nationwide distribution remains unclear. This study aimed to estimate exposure to scrub typhus group (STG) and typhus group (TG) across twelve provinces of Laos and identify potential hotspots.
METHODOLOGY: We screened 1,200 serum samples from blood donors (100 per province) for STG and TG IgG antibodies using enzyme-linked immunosorbent assays (E

Murine typhus, caused by Rickettsia typhi, is a globally distributed flea-borne disease. Antimicrobial susceptibility data are largely based on reference strains, with little evaluation of variability across clinical isolates. The plaque assay, the gold standard for antibiotic susceptibility testing, requires up to 14 days to complete. Quantitative real-time PCR (qPCR) offers a rapid alternative for determining minimum inhibitory concentrations (MICs). Although qPCR-based susceptibility testing has been described, formal statistical assessment of agreement between methods remains limited. We e

Murine typhus, caused by Rickettsia typhi, is a flea-borne zoonosis that typically presents with fever, headache, myalgias, and rash. Central nervous system involvement is uncommon, and neuroimaging findings that mimic posterior reversible encephalopathy syndrome (PRES) are rarely reported. This case highlights a rare neuroinfectious presentation of murine typhus with PRES-like imaging findings in a young woman. A previously healthy woman in her 30s presented with persistent retro-orbital headaches, intermittent fevers, tremors, dysarthria, photophobia, myalgias, progressive weakness, and recu

Murine typhus is a flea-borne disease caused by the bacteria Rickettsia typhi that occurs worldwide, but predominantly in endemic areas. The clinical presentation of typhus is often nonspecific, leading to delays in diagnosis and treatment. The severity of the disease varies widely. Most cases are relatively mild, but some patients develop a very severe form and may die. We report a case of severe murine typhus complicated by septic shock, alveolar hemorrhage, and acute respiratory distress syndrome in a teenager who was born in Somalia and had immigrated from Kenya to Switzerland. Rickettsia typhi-induced murine typhus is underrecognized in southern U.S. regions such as Texas. While typical symptoms include fever, rash, and mild pulmonary involvement, severe presentations such as pneumonia or acute respiratory distress syndrome (ARDS) can occur. The radiographic "crazy-paving" pattern is nonspecific and can be seen in a variety of infectious and inflammatory pulmonary conditions, creating diagnostic uncertainty. Because pulmonary alveolar proteinosis (PAP) typically prompts consideration of invasive diagnostic evaluation, recognizing infectious mimics has important clin

During World War II, Polish physicians Eugeniusz Łazowski and Stanisław Matulewicz fabricated a typhus epidemic, which they reported in detail only after the war. They injected inactivated Proteus bacteria to patients suffering from mild, flu-like ailments in order to trigger a positive Weil-Felix reaction in their serum, the standard diagnostic tool for typhus at that time. These falsely labelled typhus patients would then be protected from seizure by German occupiers, who were much concerned about transmission of this highly deadly disease. According to Łazowski and Matulewicz, this action s

Epidemic louse-borne typhus caused by Rickettsia prowazekii and transmitted by the human body louse, remains a major public health threat in many developing regions. Historical records indicate that outbreaks have resulted in up to million cases annually, with mortality ranging from thousands to millions. Early and accurate diagnosis is critical for improving the efficacy of antibiotic therapies. However, existing serological diagnostic methods often suffer from limited sensitivity, specificity, and reliability. In this study, we applied integrated in silico immunoinformatics and structural bi

Scrub typhus, caused by Orientia tsutsugamushi, is hyperendemic across the "tsutsugamushi triangle" and is increasingly recognized as a cause of diverse neurological complications. Although meningoencephalitis and seizures are the most frequently reported central nervous system manifestations, vascular, central autoimmune, and peripheral nervous system complications are increasingly being documented. Three mechanistically distinct neurological syndromes, cerebrovascular, central immune-mediated, and peripheral immune-mediated, can complicate scrub typhus. Each remains individually uncommon, an

Tsutsugamushi disease is an infectious disease caused by Orientia tsutsugamushi and transmitted by chiggers. The ancient classical form (summer onset) has almost disappeared, but the new form (spring and autumn onset) occurs in 300-500 cases annually. Symptoms include fever, rash, and bites. If the disease worsens, it can lead to meningoencephalitis, disseminated intravascular coagulation, and become fatal. Diagnosis involves confirming an increase in specific serum antibodies or detecting the gene in crusts. Prompt tetracycline treatment is important.
