BACKGROUND: Early diagnosis of scrub typhus and Japanese spotted fever remains challenging because initial manifestations are often nonspecific. In endemic areas, nearly 30% of patients were not correctly diagnosed at the first visit to a participating site. Previous studies have mainly treated rash and eschar as present-or-absent findings. We examined how rash and eschar came to clinical attention in patients correctly diagnosed at the first visit to a participating site and in those diagnosed later.
METHODOLOGY/PRINCIPAL FINDINGS: We analyzed 217 patients with scrub typhus or Japanese spott

BACKGROUND: Rickettsial diseases, including scrub typhus and murine typhus, are major yet persistently under-recognised causes of acute febrile illness in Southeast Asia. Limited diagnostic capacity, ecological complexity, and non-specific clinical presentation have historically contributed to the underestimation of their burden.
METHODS: We synthesised 25 years (2001-2025) of integrated epidemiological, clinical, diagnostic, molecular, ecological, and treatment research conducted across Southeast Asia. Evidence from prospective surveillance, hospital-based cohorts, seroepidemiology, molecula

BACKGROUND: Nucleic acid testing (NAT) is essential for pathogen identification, disease diagnosis, and biomedical research. Traditional amplification-based methods, while widely used, often face challenges such as long processing times and susceptibility to false positives. Additionally, single-molecule detection approaches are limited by weak signal intensities and rapid photobleaching. To address these limitations, we present an amplification-free biosensing platform for the highly sensitive and specific single-molecule detection of Orientia tsutsugamushi DNA.
RESULTS: This platform employ

Murine typhus is a reemerging infectious disease with the potential for severe manifestations. We conducted a retrospective study to examine severe illness in those hospitalized with murine typhus in Galveston, Texas. We identified 149 cases, of which 119 (79.8%) were hospitalized and 33 (28%) required admission to the intensive care unit (ICU). ICU patients were older than non-ICU patients (54.9 vs. 47.2 years; p<0.02). Thrombocytopenia was more severe in the ICU group (101 × 103 µ/L) compared to the non-ICU group (137 × 103 µ/L) (p<0.01). The median time from healthcare contact in the emerge

Orientia tsutsugamushi (Ot), the causative agent of scrub typhus, is an obligate intracellular bacterium naturally maintained in Leptotrombidium mites, yet its interactions within arthropod hosts remain poorly understood. Here, we employed two tick cell lines, Ixodes scapularis ISE6 and Rhipicephalus microplus BME/CTVM23, as arthropod models to investigate the intracellular lifecycle of Ot strains TA686 and Karp. Both strains efficiently infected tick cells and replicated within them, with ISE6 supporting more robust growth. Electron microscopy images revealed that Ot maintains its characteris

BACKGROUND: Scrub typhus is a major yet often unrecognised cause of acute undifferentiated febrile illness (AUFI) in India. Delayed diagnosis contributes substantially to organ dysfunction and mortality, but accurate estimates of disease burden are limited by heterogeneous diagnostic practices. Understanding the proportion of clinically suspected cases attributable to scrub typhus is essential for improving early recognition and treatment.
METHODS: We performed a systematic review and meta-analysis of studies reporting IgM enzyme-linked immunosorbent assay (ELISA) confirmed scrub typhus among

Background and objectives Scrub typhus is endemic in India and neighbouring countries. India's north-eastern region (NER) is geographically and ecologically distinct from the rest of the country, with variations in climate, ecology, land use patterns, and population behaviours, heightening the risk of scrub typhus among the residents. Scrub typhus remains an under-explored zoonotic disease in the region. Methods A systematic review was conducted to estimate the burden of scrub typhus in the NER of India. Six databases were searched for studies on the prevalence, seroprevalence, and case fatali

Scrub typhus is a notable zoonosis, yet natural host-associated pathogen prevalence and genotypic diversity remain underexplored. A total of 261 small mammals comprising rodents and shrews captured in Uttar Pradesh, India, were screened for Orientia tsutsugamushi (OT) using nested PCR. The sample included Rattus rattus (n = 28), Rattus norvegicus (n = 42), Rattus tanezumi (n = 42), Bandicota bengalensis (n = 10), Mus musculus (n = 15), and Suncus murinus (n = 124). The overall molecular prevalence was 11.9% (95% CI: 8.2-16.4) with detection rates of 13.1% in rodents and 12.1% in shrews. Preval

Typhus (FBT), caused by Rickettsia typhi, rarely causes neurological disease. Herein, we describe neurological involvement in two cases of FBT. In the first case, an adult presented with persistent fever which deteriorated into status epilepticus. He was successfully treated with doxycycline and made a complete recovery. In the second case, a patient suffered an ischemic stroke and had a protracted clinical course but ultimately made a near complete recovery. In addition to these cases, we conducted a comprehensive narrative review of 43 cases of neurologic involvement in FBT reported from 198

Orientia tsutsugamushi (OT), an obligate intracellular Gram-negative bacterium of the Rickettsiaceae family, causes scrub typhus (ST), a re-emerging vector-borne infectious disease with significant morbidity and mortality in South and Southeast Asia. Despite its widespread prevalence and mortality rates reaching up to 30% in untreated individuals, no effective vaccines or targeted therapeutics are currently available.Compounding these challenges are limitations in existing diagnostic methods, increasing antibiotic resistance, and complex interactions among host, pathogen, and vector. BACKGROUND: Scrub typhus is a mite-borne zoonosis endemic to the Asia-Pacific region. Although ocular complications are occasionally reported during later stages of infection, direct mechanical ocular injury related to mite exposure has rarely been reported. Recognition of early ocular manifestations may facilitate timely diagnosis and treatment of this potentially serious infection.
CASE PRESENTATION: We describe a rare case of a 9-year-old Taiwanese boy who developed acute ocular and systemic symptoms after exposure to mites during a visit to Alishan Forest, Taiwan. Live mites were identifi

Scrub typhus is a mite-borne rickettsial infection that typically presents with fever, rash, and eschar; however, atypical manifestations may lead to delayed or missed diagnosis. The present report involves a case of a 65-year-old woman who presented with persistent xerostomia (dry mouth) as the sole ongoing symptom at the time of evaluation, without rash, eschar, or ongoing fever. She had a history of outdoor exposure in an endemic area, and laboratory testing revealed transient liver dysfunction and hyponatremia associated with polydipsia. Although empiric antimicrobial therapy was not initi

BACKGROUND: Scrub typhus can rapidly progress to severe disease with multi-organ dysfunction. Secondary hemophagocytic lymphohistiocytosis (HLH) is an increasingly recognized hyperinflammatory complication associated with high mortality, but adult data and simple admission-time risk tools remain limited.
METHODS: We conducted a single-center retrospective cohort study of hospitalized adults with scrub typhus at the First Affiliated Hospital of Guangzhou Medical University (May 2013-July 2025). Severe scrub typhus was defined by major organ involvement, shock, or in-hospital death. Variables a

UNLABELLED: Orientia tsutsugamushi is a mite-transmitted obligate intracellular bacterium that causes the potentially deadly zoonosis, scrub typhus. The absence of genetic tools for Orientia has limited studies of the microbe-host interactions that underlie scrub typhus. To address this gap, we developed a protocol for transforming and achieving allelic exchange in O. tsutsugamushi str. Ikeda. From evaluating multiple cell lines and antibiotics, we found that contact-inhibited EA.hy926 human endothelial-like cells best supported Orientia replication, and that chloramphenicol was an effective s

Scrub typhus is a mite-borne rickettsial infection caused by Orientia tsutsugamushi and is transmitted through the bite of larval trombiculid mites. It is an important cause of acute febrile illness in endemic regions. Although the disease typically presents with fever, rash, lymphadenopathy, and eschar, its clinical manifestations are highly variable and may include severe systemic complications. We report a case of scrub typhus presenting with symptomatic hypocalcaemia and hypophosphataemia probably the result of renal tubular electrolyte loss, highlighting a potentially under-recognized met
