Targeting the aldosterone-mineralocorticoid receptor pathway in cardiovascular-kidney-metabolic syndrome.

The concept of cardiovascular-kidney-metabolic (CKM) syndrome integrates cardiovascular, kidney and metabolic disorders into a single pathophysiological continuum characterized by chronic inflammation, oxidative stress, fibrosis and progressive organ dysfunction. A central driver of CKM syndrome progression is inappropriate activation of the aldosterone-mineralocorticoid receptor (MR) pathway, which promotes adverse remodelling of the heart, kidney, vasculature, adipose tissue, liver and pancreas. Steroidal MR antagonists (MRAs) and non-steroidal MRAs mitigate maladaptive MR signalling and imp

Pharmacological treatment strategies to prevent heart failure across the cardiovascular-kidney-metabolic spectrum: an EJHF Expert Consensus Statement.

Heart failure (HF) remains a major cause of morbidity, mortality, and increased healthcare costs worldwide, and is recognized as the result of progressive cardiovascular-kidney-metabolic (CKM) syndrome. Obesity, type 2 diabetes, chronic kidney disease, hypertension, and atherosclerotic cardiovascular disease frequently coexist and interact through shared pathophysiological mechanisms that promote HF onset, especially with preserved ejection fraction (HFpEF). This European Journal of Heart Failure (EJHF) Expert Consensus Statement synthesizes contemporary evidence supporting pharmacological str

Early and Stage-Dependent Cardiopulmonary Fitness Impairment in Pediatric CKD.

KEY POINTS: Children with CKD exhibit markedly lower cardiopulmonary fitness with an absolute maximal oxygen uptake deficit of 10 ml/kg per minute compared with reference values. More than half of the patients show impaired fitness as early as CKD stage 3, highlighting early functional vulnerability. Maximal oxygen uptake is independently associated with modifiable parameters commonly targeted by rehabilitation, supporting tailored intervention strategies. BACKGROUND: Children with CKD are at high cardiovascular risk, yet cardiopulmonary fitness is rarely assessed in routine care. Maximal oxy

The Physical Therapist's Role in the Management of the Multisystem Effects Associated With Chronic Kidney Disease: A Case Simulation Based on Clinical Practice Guidelines.

BACKGROUND AND PURPOSE: Chronic kidney disease (CKD) affects approximately 37 million Americans and is a leading cause of mortality. Chronic kidney disease is a systemic, progressive disease that impacts multiple bodily systems, including bone, skeletal muscle, heart, and vasculature, leading to conditions such as osteoporosis, sarcopenia, frailty, and heart failure. Despite these impairments, physical therapists (PTs) are not commonly integrated into the interdisciplinary care team. This simulated case study aims to provide PTs with plan of care recommendations for patients with CKD and to fa

Navigating TIPS in the era of ageing and metabolic comorbidity: a contemporary narrative review of risk, resilience, and outcomes.

The ageing of the cirrhotic population, together with the epidemiological shift toward metabolic dysfunction-associated steatohepatitis, is redefining the profile of patients referred for transjugular intrahepatic portosystemic shunt (TIPS). Although TIPS remains a cornerstone in the management of complications of portal hypertension, its abrupt hemodynamic and metabolic effects challenge the physiological reserve of multiple organ systems. Age-related cardiovascular remodeling, cirrhotic cardiomyopathy, impaired renal function and autoregulation, and reduced neurocognitive reserve interact wi

Predictive Value of Urine Tubular Biomarkers on Kidney Outcomes: Observations from EMPA-KIDNEY.

BACKGROUND: Urine tubular biomarkers have previously been associated with chronic kidney disease (CKD) progression independently of estimated glomerular filtration rate (eGFR) and albuminuria. We evaluated whether nine urine tubular biomarkers could improve prediction of kidney failure beyond serum creatinine and urine albumin-to-creatinine ratio among adults with CKD. METHODS: Among 5100 participants in EMPA-KIDNEY, a randomized trial which assessed the effects of empagliflozin 10 mg versus matching placebo among patients with CKD at risk of progression, nine urine creatinine-indexed tubular

Differential cardiovascular impacts of sodium salts: unveiling the distinct roles of sodium chloride and sodium bicarbonate-consequences for heart failure patients.

Misconceptions surrounding sodium compounds, particularly the interchangeable use of sodium and sodium chloride (table salt), persist within the medical community, influencing dietary recommendations and patient management especially in heart failure (HF) patients with chronic kidney disease (CKD). This narrative review aims to dissect these misconceptions and discusses the physiological impacts of sodium, chloride, and sodium bicarbonate on cardiovascular (CV) physiology. The conflation of sodium and sodium chloride in dietary recommendations has obscured critical differences in their physiol

Albuminuria and heart failure.

Albuminuria, typically measured by urinary albumin-to-creatinine ratio, is present in nearly half of all patients with heart failure. Even at low levels, albuminuria is a well-established, independent risk marker for incident heart failure and adverse outcomes in those with pre-existing heart failure. Currently, routine urinary albumin-to-creatinine ratio screening is not universally recommended in heart failure guidelines, unlike in guidelines for Type 2 diabetes, chronic kidney disease, and hypertension. Emerging data suggest that this may limit opportunities for early detection, risk strati

Heterogeneous trajectories of health-related quality of life and their association with prognosis in patients with coronary heart disease and chronic heart failure.

AIMS: Currently, there is limited knowledge regarding the long-term health-related quality-of-life (HRQoL) trajectories of patients with coronary heart disease (CHD) and chronic heart failure (CHF) after discharge. In this study, we identified the HRQoL trajectories of these people through group-based trajectory modelling (GBTM) and explored their prognosis. METHODS AND RESULTS: A total of 658 patients with CHD and CHF were recruited in this multi-centre, prospective cohort study from June 2017. Health-related quality of life was assessed using the Kansas City Cardiomyopathy Questionnaire-12
