Severe Renal Phenotype Across A Multigenerational Tuberous Sclerosis Complex (TSC) Family.

BACKGROUND: Tuberous sclerosis complex (TSC) is an autosomal dominant genetic disorder characterized by the formation of hamartomas in the brain, kidney, and heart, along with other complex clinical manifestations, including TSC-associated neuropsychiatric disorder (TAND). Development of genotype-phenotype correlations within TSC can aid clinicians in providing prognostic data and improve clinical management. We present here a multigenerational family who has a pathogenic variant in TSC2 displaying a severe renal phenotype. METHODS: A 23-year-old Caucasian male (Patient 1) was determined to h

Sex differences in cardiovascular-kidney-metabolic syndrome and new onset cardiovascular outcomes.

BACKGROUND: Cardiovascular-Kidney-Metabolic (CKM) syndrome integrates metabolic risk factors, chronic kidney disease (CKD), and cardiovascular dysfunction, and is closely linked to cardiovascular disease (CVD) risk. We aimed to evaluate sex-specific differences in prevalence, components, and outcomes of CKM syndrome to inform strategies for personalized cardiovascular prevention. METHOD: We included 6563 participants without established CVD from the Multi-Ethnic Study of Atherosclerosis (MESA). CKM stages were defined according to American Heart Association criteria. We assessed sex-specific

Bradycardia, Renal Dysfunction, Atrioventricular Node Blockade, Shock, and Hyperkalemia (BRASH) Syndrome: Clinical Features, Outcomes, and Therapeutic Implications.

INTRODUCTION: BRASH syndrome, characterized by the combination of bradycardia, renal dysfunction, atrioventricular node-blocking agent use, shock, and hyperkalemia, is an increasingly recognized but underdiagnosed clinical pattern, particularly in elderly patients with multiple comorbidities. Data regarding its clinical presentation, management, and short-term outcomes remain limited. METHODS: We conducted a single-center retrospective cohort study including patients admitted to the emergency department between 2019 and 2022. Patients met clinical criteria including bradycardia, hyperkalemia

Ascending aorta to iliac artery bypass in coral reef aorta with multidrug resistant hypertension and worsening renal function.

Coral reef aorta is a rare condition characterized by heavily calcified atherosclerotic disease that involves the juxtarenal aorta, leading to potentially life-threatening complications (congestive heart failure, refractory hypertension, and visceral or limb ischemia). Here, we report the case of an elderly woman with resistant hypertension and worsening kidney function with a solitary right kidney, who underwent successful open surgical repair using the ascending aorta as an inflow source to a bypass graft to the renal, mesenteric and iliac arteries. This approach provides an effective and du

A randomised open-label pilot trial comparing mycophenolate mofetil with no immunosuppression in limited cutaneous systemic sclerosis (MINIMISE-Pilot).

OBJECTIVES: Mycophenolate mofetil (MMF) is routinely used in early diffuse cutaneous systemic sclerosis (dcSSc) but not in limited cutaneous (lc)SSc. This may miss an opportunity to slow disease progression. MINIMISE-Pilot tested the feasibility of an open-label event-driven randomised trial of MMF vs no immunosuppression in lcSSc. METHODS: We tested the feasibility of a trial evaluating the impact of MMF on a novel event-driven composite endpoint. The MINIMISE endpoint measures time to worsening of lcSSc determined by progressive lung fibrosis, pulmonary hypertension, scleroderma renal crisi

Short-Term Exposure to Air Pollution Increases Mortality From Hypertension and its Multiorgan Complications: A Case Crossover Study of 2.1 Million Deaths in China.

BACKGROUND: Short-term air pollution exposure is a known trigger for cardiovascular events, yet how this risk varies across different progression stages of hypertensive patients remains unclear. OBJECTIVES: This study sought to quantify the mortality risk and burden due to hypertensive diseases of different progression stages in association with air pollution. METHODS: We conducted a nationwide, individual-level, time-stratified, case-crossover study including >2.1 million hypertension-related deaths across mainland China (2013-2019). Daily concentrations of fine particulate matter (PM2.5),

Delineating the trajectory of adult chronic diseases and healthcare use for 22q11.2 microdeletion in a general population context.

BACKGROUND: Children with complex genetic diseases increasingly survive to adulthood, but adult health is poorly understood. Using a genetics-first approach we investigated the incidence and accrual of cardiovascular and other outcomes in people with molecularly confirmed 22q11.2 microdeletion (22q-cases) compared with general population controls (population-comparators). METHODS: Using a retrospective matched cohort study design, we linked 365 adult 22q-cases (median age 32 years; 51% female) to health administrative data for ∼15 million individuals with universal healthcare, identifying 3,6

Designing Next-Generation Cardiometabolic Outcome Trials for Obesity Medicines.

The prevalence of obesity has reached pandemic proportions and is having enormous public health effects. Obesity increases the risk of type 2 diabetes, hypertension, chronic kidney disease, and cardiovascular (CV) disease including coronary artery disease, heart failure, atrial fibrillation, and stroke. Although initial randomized clinical trials of weight-loss strategies through diet and exercise or early medical therapies did not lead to improved CV outcomes, more recent trials using glucagon-like peptide-1 (GLP-1) receptor agonists in individuals with obesity have demonstrated CV benefits.

Mineralocorticoid Receptor Antagonists: An Overview of History and Evolution.

Mineralocorticoid receptor antagonists (MRAs) have significantly evolved since the introduction of the first steroidal MRA, spironolactone, in the 1950s. Initially discovered for treating hypertension and heart failure (HF), the clinical applications of MRAs have been expanded to chronic kidney disease (CKD) and diabetic nephropathy. Steroidal MRAs, such as spironolactone and eplerenone, effectively suppress mineralocorticoid receptor activation but are associated with side effects like hyperkalemia and endocrine abnormalities. Current research aims to optimize MRAs further for broader therape

Associations of Left Atrial Dimension and Ventricular Mass With Long-Term Health Outcomes: Framingham Heart Study.

BACKGROUND: Higher left atrial end-systolic dimension (LASD) and left ventricular mass (LVM) are associated with higher risk of cardiovascular disease (CVD) over short-term follow-up (<10 years). However, data on long-term associations are limited. OBJECTIVES: The objective of the study was to evaluate associations of echocardiography-derived LASD and LVM with risk of mortality, CVD, dementia, hypertension, diabetes, and chronic kidney disease over long-term follow-up among Framingham participants. METHODS: Clinical covariates and up to 3 serial echocardiographic observations were collected

Comorbidity-Driven Inflammation in HFpEF: Immune Profiling and Therapeutic Targets.

PURPOSE OF REVIEW: Chronic low-grade inflammation underlies the heterogenous pathophysiology of heart failure with preserved ejection fraction (HFpEF) influenced by patient comorbidities. This review summarizes circulating inflammatory mediators in HFpEF, explores how comorbidities shape distinct immune signatures, and discusses current and emerging treatment strategies. RECENT FINDINGS: Comorbidities, including hypertension, obesity, type 2 diabetes, and chronic kidney disease, drive distinct immune profiles in HFpEF through dysregulated cytokine signaling. Circulating mediators, including I

The Association Between Self-Care and Health Literacy in Patients With Chronic Diseases: A Systematic Review and Meta-Analysis.

BACKGROUND: Chronic diseases are a major global health burden, contributing to morbidity, mortality and healthcare costs. Self-care is essential for effective disease management, with health literacy (HL) and digital health literacy (eHL) playing a role in enabling individuals to engage in health-promoting behaviours. However, the relationship between HL and self-care remains inconclusive, necessitating further investigation to clarify its impact. OBJECTIVE: To synthesise evidence on the association between HL and self-care in chronic diseases and identify mediating and moderating factors inf

The Cardiovascular Burden of Diabetes: Risk Factors, Clinical Phenotypes, and Personalized Cardiometabolic Management.

Type 2 diabetes (T2D) exhibits substantial phenotypic heterogeneity, resulting in diverse cardiovascular (CV) outcomes driven by multiple pathophysiological mechanisms beyond hyperglycemia alone. T2D should be recognized as a systemic cardiometabolic condition in which insulin resistance, chronic inflammation, oxidative stress, and endothelial and microvascular dysfunction promote a broad spectrum of cardiovascular diseases. The traditional "one-size-fits-all" approach to cardiovascular risk management has been proven insufficient, as individuals with T2D display marked variability in clinical

Current Trends in the Epidemiology of Obesity and the Association Between Obesity and Metabolic Liver Disease (MASLD/MASH).

Excess adiposity and obesity are increasingly prevalent, with incidence rising in many parts of the world; both trigger a state of metabo-inflammation in adipose tissue, creating a systemic milieu characterised by insulin resistance and pro-inflammatory and fibrogenic states. Along with systemic inflammatory cues, perturbations in systemic and individual organ metabolic homeostasis result in metabolically driven cell stress, injury and death; this activates innate immune signals promoting fibrogenesis and organ dysfunction. The phenotype of end-organ diseases is determined by the balance betwe

Daily steps offset risks of sedentary behavior in the All of Us research program.

Sedentary behavior is associated with increased mortality and chronic diseases, yet it remains unclear whether higher daily step counts can mitigate these risks. In this study, we analyzed longitudinal sedentary and step data from Fitbit devices in the All of Us Research Program to examine incident diagnoses of chronic conditions. We show that greater sedentary time was associated with higher risk of obesity, diabetes mellitus, hypertension, coronary artery disease, heart failure, chronic kidney disease, metabolic dysfunction-associated steatotic liver disease, chronic obstructive pulmonary di

Advances in Hyperkalemia Management and the Emerging Role of Sodium Zirconium Cyclosilicate.

Hyperkalemia is a common and potentially life-threatening complication in patients with chronic kidney disease and heart failure. It also represents a major barrier to the optimal use of renin-angiotensin-aldosterone system inhibitors (RAASi), which are central to improving cardiorenal outcomes. Contemporary guidance, including the 2025 Korean Society of Nephrology Practical Recommendations for the Management of Hypertensive Kidney Disease, emphasizes the maintenance of normokalemia as a key strategy for enabling continuation and optimization of RAASi therapy. Sodium zirconium cyclosilicate (S

Role of Senescence in the Pathophysiology of Preeclampsia and Future Health.

Preeclampsia is a pregnancy-specific hypertensive disorder affecting up to 5% of pregnancies worldwide and is one of the leading causes of maternal and fetal morbidity and mortality globally. It is increasingly recognized that preeclampsia, from both clinical and pathophysiological standpoints, is a heterogeneous disease and that several mechanisms may lead to a clinical syndrome of hypertension, systemic disease, and proteinuria. Beyond its acute obstetric consequences, preeclampsia confers a significantly increased risk of future hypertension, cardiovascular disease, chronic kidney disease,
