Association of Pre-Transplant Left Ventricular Longitudinal Strain With Long-Term Cardiovascular Outcomes Following Kidney Transplantation.

PURPOSE: Cardiovascular disease (CVD) remains the leading cause of death among kidney transplant recipients (KTRs). Current guidelines recommend using left ventricular global longitudinal strain (GLS) for the early identification of cardiac dysfunction, but its value in KTRs has not been well studied. METHODS: We performed a single-center retrospective cohort study of adult KTRs who underwent KT between January 2015 and January 2024 and had an adequate-quality pre-transplant echocardiogram. Patients without follow-up at our institution or with atrial fibrillation were excluded. GLS was measur

Incidence and risk factors of short-term cardiovascular events in esophageal cancer patients undergoing radiotherapy.

Advances in cancer treatment have improved survival rates, but radiation induced cardiovascular events remain a concern, especially in esophageal cancer patients undergoing radiotherapy. This study investigates the incidence and risk factors for cardiovascular events in this population. From 2009 to 2016, 5302 esophageal cancer patients were analyzed in the retrospective case control study. Patients were categorized based on the occurrence of cardiovascular events, and analyses of the association between radiotherapy and cardiovascular events. Demographic data, comorbidities, and treatment det

Clinical characteristics and five-year outcomes in a prospective Russian real-world heart failure cohort.

BACKGROUND: Despite advances in therapy, data on long-term survival and temporal mortality patterns in real-world heart failure (HF) populations, particularly during the critical early period after diagnosis or clinical destabilization, remain scarce. This study aimed to analyze long-term survival and identify factors associated with mortality in a prospective Russian real-world HF cohort that is relatively underrepresented in the international registry literature. METHODS: A prospective 5-years registry study consecutively enrolled 150 patients with HF in February-May 2018. Participants unde

The health effects of extreme heat.

Extreme heat is a significant direct health threat from climate change, with rising temperatures and frequent heatwaves increasingly stressing communities and health services across Africa. High baseline temperatures, widespread outdoor labour, limited cooling access and structural vulnerabilities heighten population exposure. The physiological impacts are severe: from extreme heat overwhelming thermoregulation, leading to dehydration, cardiovascular strain, direct cellular injury and potentially rapid progression to heat exhaustion, to the most severe and dangerous form, heat stroke, which is

Sex-dependent differences in mitochondrial protein acetylation in metabolic condition, oxidative stress, vascular dysfunction, hypertension, and cardiovascular disease.

One half of adults have hypertension, which is a major risk factor for stroke, myocardial infarction, heart failure, and vascular dementia. There is an urgent need for new therapies, particularly for women with hypertension. Hypertension affects women in all phases of life; however, the hypertension rate increases in women much more steeply, and hypertensive vascular and kidney damage is significantly higher in women. Despite great burden, only 1 in 4 patients have their blood pressure under control. Hypertension accounts for 1 in 5 deaths among American women, posing a greater burden for wome

Cardiovascular-Kidney-Metabolic Syndrome: Development of an ICD-10-CM Coding Framework.

BACKGROUND: Cardiovascular-kidney-metabolic (CKM) syndrome is a multisystem construct describing the intertwined progression of cardiometabolic risk factors, chronic kidney disease (CKD), and cardiovascular disease. The American Heart Association (AHA) proposed CKM stages (0-4) for risk stratification and prevention. However, CKM lacks a single ICD-10-CM (International Classification of Diseases, Tenth Revision, Clinical Modification) code, which hinders standardized stage identification in electronic health records and claims data. OBJECTIVE: This study aimed to develop an AHA-aligned ICD-10

Burden of incident cardiovascular events in giant cell arteritis using data from a nationwide French cohort.

Giant cell arteritis (GCA) is one of the most common adult vasculitis. As large-scale studies on cardiovascular events in GCA are missing in France, our study examine their incidence using the French national medico-administrative health database. We included patients hospitalized for GCA between January 2013 and December 2022 who received an initial prescription of corticosteroids ≥ 1000 mg over 40 days within one year of the hospitalization. Each patient was matched with three controls by age, sex, and residence. Cardiovascular events and mortality were compared using a multivariate Cox mode

Aquaporin-1 (AQP1) mediates cardiac oedema and mitochondrial dysfunction in cardiometabolic HFpEF: New insights into the mechanisms of SGLT2 inhibitors.

Myocardial oedema contributes significantly to cardiomyocyte dysfunction and death. A 3.5 % increase in water content can result in a 30-50 % reduction in cardiac output. Myocardial oedema is an early complication of prediabetes and is associated with ventricular remodelling and diastolic dysfunction; however, its regulatory mechanisms remain unclear. In this study, we aimed to investigate the role of Aquaporin-1 (AQP1) in myocardial oedema and mitochondrial dysfunction during prediabetes and explore whether the sodium-glucose cotransporter 2 (SGLT2) inhibitor empagliflozin (EMPA) exerts cardi

Cardiometabolic regulation by adipocyte-derived leptin and the brain melanocortin system.

Tonic activation of central nervous system (CNS) leptin receptors (LepRs) and melanocortin 4 receptors (MC4Rs) is critical for maintaining normal cardiometabolic function. Deficiency of CNS LepR or MC4R signaling causes severe obesity and is accompanied by multiple metabolic abnormalities including insulin resistance, glucose intolerance, and hyperlipidemia that are only partially explained by obesity. Defective LepR and MC4R signaling also causes dysfunction of the sympathetic nervous system (SNS) and blood pressure (BP) regulation. Hyperleptinemia and activation of the CNS melanocortin syste

Update on Obesity and Its Relationship to Atherosclerotic Cardiovascular Disease and Associated Risk Factors.

Background/Objectives: Obesity is a multifactorial chronic condition characterized by the accumulation of excess adiposity and complex interplay between intrinsic and extrinsic factors. It is an increasingly common condition, closely implicated with the incidence and progression of cardiovascular disease and its risk factors. This narrative review synthesizes and summarizes recent evidence on obesity, with a focus on the diagnosis of obesity, an exploration of both visceral and subcutaneous adipose tissue, available interventions for obesity ranging from dietary modifications to novel anti-obe

Coronary Flow Reserve in Adults: Pathophysiology, Assessment Modalities, Clinical Applications, and Prognostic Significance.

Coronary flow reserve (CFR) is a fundamental physiological index defined as the ratio of maximal coronary blood flow during hyperemia to resting flow. It provides an integrated assessment of the entire coronary circulation, from epicardial arteries to the microvasculature. Non-invasive assessment, particularly with transthoracic Doppler echocardiography (TTDE), is valuable in clinical practice for evaluating the functional impact of moderate obstructive lesions and determining the status of coronary microcirculation. Impairment of coronary microcirculation, detected by reduced CFR, is present

Real-world outcomes of Finerenone in patients with diabetic kidney disease in Saudi Arabia.

BACKGROUND: Diabetic kidney disease (DKD) represents a significant microvascular complication associated with type 2 diabetes mellitus (T2DM), markedly elevating the risk of kidney failure, cardiovascular events, and premature mortality. Despite advancements in therapeutic management, such as renin-angiotensin-aldosterone system (RAAS) blockade and sodium-glucose cotransporter 2 (SGLT2) inhibitors, residual risk remains significant. Finerenone, a novel nonsteroidal and selective mineralocorticoid receptor antagonist (MRA), has demonstrated substantial cardiorenal benefits in clinical trials; h
