The Physical Therapist's Role in the Management of the Multisystem Effects Associated With Chronic Kidney Disease: A Case Simulation Based on Clinical Practice Guidelines.

BACKGROUND AND PURPOSE: Chronic kidney disease (CKD) affects approximately 37 million Americans and is a leading cause of mortality. Chronic kidney disease is a systemic, progressive disease that impacts multiple bodily systems, including bone, skeletal muscle, heart, and vasculature, leading to conditions such as osteoporosis, sarcopenia, frailty, and heart failure. Despite these impairments, physical therapists (PTs) are not commonly integrated into the interdisciplinary care team. This simulated case study aims to provide PTs with plan of care recommendations for patients with CKD and to fa

Navigating TIPS in the era of ageing and metabolic comorbidity: a contemporary narrative review of risk, resilience, and outcomes.

The ageing of the cirrhotic population, together with the epidemiological shift toward metabolic dysfunction-associated steatohepatitis, is redefining the profile of patients referred for transjugular intrahepatic portosystemic shunt (TIPS). Although TIPS remains a cornerstone in the management of complications of portal hypertension, its abrupt hemodynamic and metabolic effects challenge the physiological reserve of multiple organ systems. Age-related cardiovascular remodeling, cirrhotic cardiomyopathy, impaired renal function and autoregulation, and reduced neurocognitive reserve interact wi

Predictive Value of Urine Tubular Biomarkers on Kidney Outcomes: Observations from EMPA-KIDNEY.

BACKGROUND: Urine tubular biomarkers have previously been associated with chronic kidney disease (CKD) progression independently of estimated glomerular filtration rate (eGFR) and albuminuria. We evaluated whether nine urine tubular biomarkers could improve prediction of kidney failure beyond serum creatinine and urine albumin-to-creatinine ratio among adults with CKD. METHODS: Among 5100 participants in EMPA-KIDNEY, a randomized trial which assessed the effects of empagliflozin 10 mg versus matching placebo among patients with CKD at risk of progression, nine urine creatinine-indexed tubular

Differential cardiovascular impacts of sodium salts: unveiling the distinct roles of sodium chloride and sodium bicarbonate-consequences for heart failure patients.

Misconceptions surrounding sodium compounds, particularly the interchangeable use of sodium and sodium chloride (table salt), persist within the medical community, influencing dietary recommendations and patient management especially in heart failure (HF) patients with chronic kidney disease (CKD). This narrative review aims to dissect these misconceptions and discusses the physiological impacts of sodium, chloride, and sodium bicarbonate on cardiovascular (CV) physiology. The conflation of sodium and sodium chloride in dietary recommendations has obscured critical differences in their physiol

Albuminuria and heart failure.

Albuminuria, typically measured by urinary albumin-to-creatinine ratio, is present in nearly half of all patients with heart failure. Even at low levels, albuminuria is a well-established, independent risk marker for incident heart failure and adverse outcomes in those with pre-existing heart failure. Currently, routine urinary albumin-to-creatinine ratio screening is not universally recommended in heart failure guidelines, unlike in guidelines for Type 2 diabetes, chronic kidney disease, and hypertension. Emerging data suggest that this may limit opportunities for early detection, risk strati

Hydrogen sulphide as a potential contributer to cardiovascular protection with sodium-glucose cotransporter 2 inhibition.

Sodium-glucose cotransporter 2 inhibitors (SGLT2i), developed for the treatment of diabetes mellitus, exert remarkable cardiovascular benefits beyond glycaemic control. The underlying mechanism of this pluripotent protection is heterogeneous and involves interactions with a number of haemodynamic, metabolic and cellular signalling pathways. Emerging findings demonstrate that a natural gaseous product of arterial wall, H2S, participates in a number of physiological reactions, and its deficiency is associated with cardiovascular pathologies, such as arterial hypertension, heart failure and chron

The multifaceted role of kallistatin in human diseases: mechanistic insights and translational potential.

Kallistatin, a multifunctional serine protease inhibitor, is widely distributed with tissue-specific effects. It may serve as a new diagnostic biomarker and therapeutic target for human diseases. Through binding to its two structural elements and specific receptors, it regulates differential signaling cascades, and thus has a wide spectrum of biological functions. In cardiovascular diseases like hypertension, atherosclerosis, and heart failure, it exerts protective effects by improving endothelial function, anti-inflammation, and regulating lipid metabolism. In liver diseases, high hepatic exp

Expert perspectives on incorporating glucagon-like peptide-1 receptor agonist in diabetes and chronic kidney disease: challenges and opportunities.

Chronic kidney disease (CKD) is a complex and progressive condition ultimately leading to premature death. Diabetes is the leading cause of end-stage kidney disease worldwide. Up till 2024, international clinical guidelines have established three therapeutic pillars to delay CKD progression in people with type 2 diabetes (T2D): renin-angiotensin system inhibitors, sodium-glucose cotransporter 2 inhibitors, and the non-steroidal mineralocorticoid receptor antagonist finerenone. With the recent results from the Evaluate Renal Function with Semaglutide Once Weekly study, the glucagon-like peptide

Reshaping the cardiovascular continuum in the management of arterial and venous cardiovascular disease: a narrative review.

The prevalence of cardiovascular disease (CVaD) is expected to double in the next 25 years, fueled by increasing prevalence of diabetes mellitus, obesity and hypertension. Cardiovascular-kidney-metabolic syndrome is a clinical entity requiring a holistic approach to prevention and management. Another aspect of this syndrome is chronic venous disease (CVeD), which is common in patients with CVaD. This review describes presentations at a symposium by the European Association of Preventive Cardiology (Milan, Italy; April 2025), discussing the interconnectedness of conditions on the CVaD continuum

Ethnic disparities in metabolic dysfunction-associated steatotic liver disease and clinical outcomes.

BACKGROUND: While metabolic dysfunction-associated steatotic liver disease (MASLD) has become increasingly prevalent worldwide, multi-ethnic differences within a shared geography and lifestyle, have not been fully examined. What remains poorly characterized are the clinical outcomes for ethnic minorities in the U.S., particularly Asians and Native Hawaiian and Pacific Islanders (NHPI), compared with White people/persons/person. METHODS: Adults (aged ≥18 years) diagnosed with MASLD between January 2008 and December 2018 were identified in the TriNetX national database using the ICD-10 codes an

Renin-angiotensin-aldosterone system (RAAS) antagonists in the perioperative setting: An updated review.

Renin-angiotensin-aldosterone system (RAAS) antagonists are essential in managing heart failure, hypertension, chronic kidney disease, and atherosclerotic cardiovascular disease. However, their perioperative use remains controversial due to concerns about intraoperative hypotension and vasoplegia. This review offers an updated synthesis of guideline recommendations, mechanistic insights, and clinical trial evidence, including POISE-3, STOP-OR-NOT, and SPACE, along with a practical decision-making framework for internists. We examine the physiological basis for holding or continuing RAAS antago
