Reduced Risk of MASLD and Cardiometabolic Disease Following Renal Denervation Therapy in Patients With Resistant Hypertension.

INTRODUCTION: Autonomic dysfunction increases the risk of cardio-kidney-metabolic (CKM) syndrome including metabolic dysfunction-associated steatotic liver disease (MASLD), type 2 diabetes (T2D), cardiovascular disease (CVD) and chronic kidney disease (CKD). Renal denervation reduces renal sympathetic nerve activity and lowers blood pressure independent of long-term pharmacotherapy, with potential downstream effects on metabolic and hepatic pathways. We aimed to evaluate the impact of renal denervation, compared with intensive antihypertensive therapy, on cardiometabolic disease risk. METHODS

The Chronic Renal Insufficiency Cohort (CRIC) study measured hepcidin, cardiovascular, kidney and metabolic outcomes in chronic kidney disease.

INTRODUCTION: Abnormal iron homeostasis plays a critical role in extra-hematopoietic disorders. The relationship of hepcidin, the master iron regulator, to clinical outcomes, has not been characterized in a large cohort of chronic kidney disease (CKD). METHODS: To determine if plasma hepcidin level predicts cardiovascular (CV), kidney, and metabolic outcomes in patients with CKD, we studied 3826 patients enrolled in the Chronic Renal Insufficiency Cohort (CRIC) study. Hazard ratios for hepcidin tertiles were estimated from Cox Proportional Hazards regression for primary outcomes of incident h

MEDA-PKD: a dynamic, real-time analytics platform for precision care in autosomal dominant polycystic kidney disease.

The use of medical real-world data is constrained by fragmented data infrastructures and the absence of tools integrating longitudinal clinical with molecular data at scale. Autosomal dominant polycystic kidney disease (ADPKD), the most common monogenic cause of kidney failure, exemplifies this gap given its phenotypic complexity and the need for individualized treatment decisions. We present MEDA-PKD (MEdical DAta translation service of Polycystic Kidney Disease), an interactive, near-real-time analytics platform that integrates clinical routine with research data from the German AD(H)PKD Tol

Artificial intelligence-enabled deep clinical phenotyping in heart failure with preserved ejection fraction: from patient subtypes to precision care.

Heart failure with preserved ejection fraction (HFpEF) is a clinical syndrome defined by shared manifestations but produced by diverse combinations of cardiometabolic, vascular, inflammatory, renal, pulmonary, atrial, skeletal-muscle, and aging-related mechanisms. This heterogeneity limits the value of classifications based only on left ventricular ejection fraction and helps explain why many mechanistically plausible interventions have produced neutral average results. Deep phenotyping can move the field from descriptive heterogeneity toward biologically coherent and clinically actionable sub

KDOQI Commentary on the 2025 AHA/ACC Guideline for the Prevention, Detection, Evaluation, and Management of High Blood Pressure in Adults.

Hypertension prevention, diagnosis, and management are central to modifying the progression of chronic kidney disease (CKD). Although the nephrology community has long recognized CKD as an independent risk factor for cardiovascular disease, CKD has not historically been incorporated in cardiovascular risk prediction models. The new 2025 American Heart Association and American College of Cardiology (AHA/ACC) Guideline for the Prevention, Detection, Evaluation, and Management of High Blood Pressure in Adults addresses this gap by formally adopting the PREVENT calculator, which combines cardiovas

Acute kidney disease after acute kidney injury: incidence, risk factors, and association with de novo chronic kidney disease.

Acute Kidney Disease (AKD) is defined as acute kidney injury (AKI) persisting for 7 to 90 days and represents a window for interventions to improve renal outcomes. However, data on AKD remain underexplored. This retrospective cohort study at Tongji Hospital, China, included 23,868 adult patients who survived at least one AKI episode between January 2015 and April 2024. Using electronic medical record data, we aimed to quantify the incidence of AKD among AKI survivors, identify independent risk factors, and evaluate the association between AKD and de novo chronic kidney disease (CKD). Among the

Beyond Hemodynamics: Uremic Toxins, Post-Translational Modifications, and Inflammatory-Metabolic Remodeling in Uremic Cardiomyopathy.

Uremic cardiomyopathy has traditionally been attributed to pressure overload, volume overload, hypertension, and neurohormonal activation in chronic kidney disease. However, growing experimental and clinical evidence indicates that nonhemodynamic mechanisms also contribute to myocardial remodeling in uremia. These mechanisms include retention of uremic toxins and protein-bound solutes, mediators of mineral metabolism, gut-derived metabolites, inflammatory signaling, mitochondrial dysfunction, and post-translational protein modifications. In this review, we discuss uremic cardiomyopathy as a sy

Systemic Sclerosis-Specific Determinants of Major Adverse Cardiovascular Events: Development of an Exploratory Cardiovascular Risk Model Beyond Traditional Risk Factors.

Background: Patients with systemic sclerosis (SSc) have an increased burden of cardiovascular disease, resulting from the combined effects of traditional cardiovascular risk factors, chronic inflammation, endothelial dysfunction, and disease-specific vascular involvement. Current cardiovascular risk prediction tools may not adequately capture the complex pathophysiological mechanisms contributing to cardiovascular complications in SSc. Objectives: This study aimed to evaluate the association between traditional cardiovascular risk factors and SSc-specific clinical characteristics with major ad

Sex-specific clinical profiles in patients with acute ischemic stroke and chronic kidney disease.

BACKGROUND: Chronic kidney disease (CKD) is common among patients with acute ischemic stroke (AIS) and is associated with worse outcomes. Although sex-related differences in stroke are well recognized, it remains unclear whether clinical characteristics associated with sex differ according to CKD status. We characterized sex-associated clinical profiles among patients with AIS with and without documented CKD. METHODS: We conducted a retrospective analysis of the PRISMA Health Stroke Registry. Baseline characteristics were compared between patients with and without documented CKD in the overal

β3-Adrenergic Receptor Agonism Reduces Right Ventricular Systolic Pressure in a Preclinical Model of Heart Failure With Preserved Ejection Fraction-Associated Pulmonary Hypertension.

PurposePulmonary hypertension (PH) commonly complicates heart failure with preserved ejection fraction (HFpEF), driving morbidity and mortality, yet no therapies exist for HFpEF-PH. β3-adrenergic receptor (ADRB3) agonists may provide cardiometabolic and anti-inflammatory benefits. We evaluated pharmacological ADRB3 modulation in a preclinical HFpEF-PH model.MethodsHFpEF-PH was induced in male and female AKR/J mice via high-fat diet feeding for 16-20 weeks. At 12-16 weeks of feeding, mice were randomized to low-dose mirabegron (LDM, 3.6 mg/kg/day), high-dose mirabegron (HDM, 14.4 mg/kg/day), ne

Network Analysis of Chronic Disease Diagnosis Co-documentation in Insurance Claims Among Older Arkansans.

BACKGROUND: Multimorbidity among older adults presents growing clinical and economic challenges, particularly in medically underserved states such as Arkansas. However, it remains unclear whether chronic conditions documented in routine healthcare encounters exhibit structured patterns of diagnosis co-documentation or primarily reflect incidental co-documentation. Understanding these patterns may inform future studies of multimorbidity and healthcare utilization. OBJECTIVE: To characterize statewide, sex-stratified patterns of claim-level diagnosis co-documentation among Arkansas adults aged

Chronic kidney disease as a systemic disorder: filtration phenotypes, inflammation and cardiovascular risk in the cardio-kidney metabolic era.

Chronic kidney disease (CKD) is increasingly recognized as a systemic disorder driven by metabolic dysfunction, vascular injury, inflammation, and emerging filtration phenotypes. In the cardio-kidney-metabolic (CKM) era, renal biomarkers such as estimated glomerular filtration rate (eGFR) and albuminuria provide essential insight into early metabolic-renal stress and cardiovascular (CV) vulnerability. This narrative review synthesizes contemporary evidence linking CKD to cardiovascular disease (CVD), emphasizing the multidimensional interactions among filtration impairment, glomerular barrier

Free Mitochondrial DNA and Mitophagy Dysfunction in Diabetic Kidney Disease: A Central Mitochondrial-Inflammatory Axis Driving Progressive Renal Injury.

Diabetic kidney disease (DKD) remains the leading cause of chronic kidney disease and end-stage renal failure worldwide despite significant advances in glucose-lowering and renoprotective therapies. Emerging evidence identifies mitochondrial dysfunction as a central pathogenic mechanism linking metabolic stress to chronic inflammation and progressive renal injury. In the diabetic kidney, hyperglycemia, lipotoxicity, and oxidative stress impair mitochondrial quality control pathways, particularly mitophagy, a targeted mitochondrial degradation pathway, leading to the accumulation of dysfunction

Hypertension in Advanced Chronic Kidney Disease: Saving the Kidneys or the Heart?

As one of the known modifiable risk factors for cardiovascular disease, hypertension control is an attractive strategy for mitigating the risk of chronic kidney disease progression and cardiovascular disease. However, trial data have not always been consistent in terms of the cardiovascular and kidney benefits of intensive blood pressure lowering in patients with advanced kidney dysfunction. Here, we provide an overview of the trial-grade evidence for intensive blood pressure lowering and the use of different antihypertensive agents to achieve blood pressure control, focusing on cardiorenal ou

Kidney-brain neural interaction in cardiorenal syndrome: insights from volume-overloaded spontaneously hypertensive rats with aorto-caval fistula.

OBJECTIVE: We hypothesized, first, that impairment of autoregulation of renal blood flow (RBF), glomerular filtration rate (GFR), and of the pressure-natriuresis in spontaneously hypertensive rats (SHR) is present in early phase of heart failure and precedes its decompensation. Second, that both the total renal denervation (T-RDN) and/or selective renal afferent denervation (A-RDN) would improve renal autoregulation and pressure-natriuresis in SHR with heart failure. METHODS: Heart failure was elicited by volume overload achieved using aorto-caval fistula (ACF). T-RDN and A-RDN were performed
