Renal function trajectories in 'De Novo' heart failure with reduced ejection fraction: impact on outcomes.

AIMS: Heart failure (HF) often coexists with chronic kidney disease (CKD), impacting prognosis. This study aims to evaluate renal function trajectories and their impact on major clinical outcomes in a cohort of patients hospitalised for 'de novo' HF with reduced ejection fraction (HFrEF). METHODS: This is a prospective cohort study that included patients hospitalised for 'de novo' HFrEF at two university hospitals. Renal function was assessed using the CKD-Epidemiology Collaboration formula. Total mortality and the combined total mortality and HF readmissions were evaluated during follow-up.

Targeting GLP1 receptor for reducing global cardiovascular risk.

BACKGROUND: Overweight and obesity are increasingly recognized as major contributors to cardiovascular (CV) and renal dysfunctions, even in the absence of traditional metabolic comorbidities such as dyslipidemia, hypertension, or type 2 diabetes mellitus (T2DM). Recent evidence demonstrates that individuals classified as metabolically healthy obese (body mass index ≥30 kg/m2) remain at elevated risk for atherosclerotic CV disease, and heart failure (HF). This has led to an emerging concept of the cardio-kidney-metabolic (CKM) syndrome, a pathophysiological continuum that underscores the interc

Tryptophan metabolites clearance in hemodialyzed patients.

Hemodialysis (HD) and hemodiafiltration (HDF) procedures have limited efficacy regarding removal of selected toxins. Tryptophan (Trp) metabolites formed through the kynurenine (KYN) pathway are known to contribute to kidney failure (KF) complications, some of which may affect the survival of neurons. The aim of this study was to investigate the removal of Trp and its metabolites, KYN, kynurenic acid (KYNA) and 3-hydroxykynurenine (3-OHKYN) in patients treated by HD or HDF and the impact of patients' comorbidities on toxins removal. The study was conducted in 50 patients with KF. Serum level of

Causal demonstration of adiposity-induced adipose-specific signaling derangements in the pathogenesis of the clinical features of the cardiovascular-kidney metabolic syndrome.

The American Heart Association has recognized that excess dysfunctional fat and its secretion of adipokines are upstream causal mechanisms that drive the clinical and pathophysiological features of the cardiovascular-kidney-metabolic (CKM) syndrome. The identity of these signaling molecules can be discerned by experimental adipose tissue-specific silencing studies showing that biological events induced by adiposity and occurring selectively in adipose tissue can promote each element of the CKM syndrome. Suppression of the renin-angiotensin system specifically and selectively only in adipose ti

Epicardial adipose tissue volume outperforms density in association with cardiorenal complications in hypertensive patients.

BACKGROUND: Epicardial adipose tissue (EAT) dysfunction is closely related to a variety of cardiovascular diseases. However, controversy persists regarding the association between EAT characteristics and hypertension (HTN). The present study aims to clarify the changes of EAT volume and density in HTN patients, and explore their relationships with cardiorenal complications. METHODS: A total of 257 individuals were enrolled in this study for analysis, including 156 HTN patients and 101 non-HTN participants. EAT volume and density were measured using coronary computerized tomography angiography

Prospective Associations of Obesity and Obesity Severity With 9 Cardiovascular Outcomes: The Cross-Cohort Collaboration.

BACKGROUND: Obesity is an established risk factor for cardiovascular disease (CVD); however, the overall and sex-specific relationships across the full spectrum of body mass index (BMI), particularly severe obesity defined as class 2 (BMI 35 to <40.0 kg/m2) and class 3 (BMI ≥40 kg/m2), and long-term CVD outcomes remain incompletely described. METHODS: We included 289 875 participants (mean age, 60.3 years; 79.2% women) from 21 cohorts of the Cross-Cohort Collaboration enrolled between 1948 and 2015 with harmonized BMI data, a BMI ≥18.5 kg/m2, and at least one of nine adjudicated outcomes: tim

Combined Burden of Heart Failure and Arterial Hypertension as Predictors of Adverse Outcomes in Hospitalized COVID-19 Patients.

Background: Cardiovascular comorbidities are major determinants of poor outcomes among patients admitted with COVID-19. However, the prognostic role of arterial hypertension alone remains uncertain. Little is known about the cumulative impact of concomitant hypertension and heart failure. This study assessed whether the combined burden of arterial hypertension and pre-existing heart failure identifies a high-risk phenotype for adverse in-hospital outcomes among COVID-19 patients. Methods: In this retrospective, real-world cohort study, 395 consecutive adults hospitalized with confirmed COVID-1

Prevalence and risk factors of pulmonary hypertension in maintenance hemodialysis patients based on echocardiography.

This study aims to use echocardiography to explore the prevalence of pulmonary hypertension (PH) in maintenance hemodialysis patients with different vascular accesses and analyze the risk factors for combined PH. A cross-sectional analysis was conducted on 757 end-stage renal disease patients receiving maintenance hemodialysis. Patients were categorized by the type of access used during echocardiographic examination: central vein catheter (CVC) and autogenous arteriovenous fistula (AVF)/arteriovenous graft (AVG). They were further grouped by tricuspid regurgitation velocity to determine PH pre

Modifiable Uncontrolled Frailty (mFI-5) in Lumbar Fusion Outcomes.

STUDY DESIGN: Retrospective Cohort. OBJECTIVE: To investigate impact of controlled/uncontrolled sub-stratification of modified frailty index (MFI-5) on postoperative outcomes after lumbar fusion for degenerative disease. BACKGROUND: mFI-5 outperforms age as a predictor of postoperative risk. Sub-stratification of mFI-5 by controlled versus uncontrolled frailty has allowed for better risk prognostication in spinal deformity cases. METHODS: Adult patients who underwent elective, single-level lumbar fusion (2017-2021) were identified. Fusions for infection/trauma/tumor/revision were excluded.

Indole-3 Acetate Limits Dysbiosis-Driven Diastolic Failure via Hcrt Neurons.

BACKGROUND: The nervous, gastrointestinal, renal, and cardiovascular systems orchestrate ion-fluid homeostasis and impose reciprocal adaptations to hypertensive challenges. Mechanistic insight into the interorgan crosstalk is fundamental for tackling pathogenesis of hypertensive heart disease. METHODS: We integrated gut microbiome profiling and targeted metabolomics in a zebrafish model of ion dyshomeostasis-induced diastolic dysfunction to identify microbial metabolites linked to hypertensive cardiac remodeling. To dissect the gut-brain-heart axis, we depleted microbiota, supplemented specif

Ascending aorta to iliac artery bypass in coral reef aorta with multidrug resistant hypertension and worsening renal function.

Coral reef aorta is a rare condition characterized by heavily calcified atherosclerotic disease that involves the juxtarenal aorta, leading to potentially life-threatening complications (congestive heart failure, refractory hypertension, and visceral or limb ischemia). Here, we report the case of an elderly woman with resistant hypertension and worsening kidney function with a solitary right kidney, who underwent successful open surgical repair using the ascending aorta as an inflow source to a bypass graft to the renal, mesenteric and iliac arteries. This approach provides an effective and du
