Current Trends and Future Perspectives of Bradycardia, Renal Failure, Atrioventricular Nodal Blockade, Shock, and Hyperkalemia (BRASH) Syndrome: A Narrative Review.

BRASH syndrome is defined as a clinical condition in which bradycardia, renal failure, atrioventricular (AV) nodal blockade, shock, and hyperkalemia interact to form a self-perpetuating negative spiral. Geriatric practitioners are increasingly likely to encounter elderly patients with this syndrome who are taking AV nodal blocking agents, such as calcium channel blockers (CCBs) or β-blockers. However, it remains unclear how the heart failure (HF) pandemic and coronavirus disease 2019 (COVID-19) have influenced the incidence, triggers, management, and clinical course of BRASH syndrome. Therefor

Heart failure associated with renal artery stenosis and Pickering syndrome: the critical role of endovascular revascularization: a case series and meta-analysis.

BACKGROUND: Heart failure associated with renal artery stenosis (RAS) and Pickering syndrome, characterized by recurrent flash pulmonary oedema (FPE) in the setting of bilateral RAS or unilateral stenosis in a solitary kidney, represent 2 severe forms of renovascular disease. Despite their clinical significance, the role of endovascular treatment is still debated due to inconsistent findings from randomized trials. METHODS: This study combines a case series of patients with Pickering syndrome undergoing percutaneous transluminal renal angioplasty (PTRA) with a meta-analysis evaluating the eff

Intensive blood pressure control in heart failure with preserved ejection fraction: a symptom-modifying strategy.

Patients with heart failure with preserved ejection fraction (HFpEF) account for over half of heart failure (HF) cases globally and the presence of HFpEF is closely linked with systemic hypertension, which contributes to the structural, hemodynamic, and symptomatic burden of the disease. HFpEF is a heterogeneous syndrome in which concentric remodeling/hypertrophy and diastolic dysfunction commonly arise from chronic pressure overload (often hypertension) and/or age-related arterial stiffening, metabolic comorbidity, and other myocardial substrates. This review critically examines the rationale

Mineralocorticoid receptor antagonists: Efficacy and safety in chronic kidney disease.

The mineralocorticoid receptor (MR) has a multifaceted role in normal physiologic functions. However, research has uncovered the deleterious consequences of overactivation of the MR. It has been implicated in chronic kidney disease (CKD) via its strong association with glomerulosclerosis, interstitial fibrosis, proteinuria, and decline in glomerular filtration rate, as well as in cardiovascular diseases through mechanisms such as excessive extracellular matrix accumulation, oxidative stress, haemodynamic changes, and sustained inflammation. As such, efforts have poured into developing therapeu

Progression of exaggerated blood pressure variability in annual 24-h ambulatory blood pressure monitoring associated with autonomic dysfunction: a pathological case report.

This study describes the case of an 83-year-old man with long-standing hypertension, urinary dysfunction, and constipation. Annual 24-h ambulatory blood pressure monitoring revealed increased blood pressure variability and reduced nocturnal dipping. One year earlier, exaggerated blood pressure fluctuations and postprandial hypotension had emerged, which persisted despite the reduced use of antihypertensive medication. The tests confirmed orthostatic and postprandial hypotension and low heart rate variability, indicating autonomic dysfunction. Cognitive function was preserved and the patient's

Renal denervation in 2026: trial evidence, guideline recommendations and implementation strategies for clinical practice.

Renal nerves are critical modulators of kidney function and blood pressure (BP) control. Their influence on sodium and fluid retention, renin release and vasoconstriction rendered them a therapeutic target more than a century ago when surgical splanchnicectomy emerged as one of the first effective antihypertensive therapies. Revisiting the concept using catheter-based approaches to ablate renal afferent and efferent nerves more selectively with various energy sources demonstrated the efficacy of renal denervation in reducing sympathetic nerve activity. A series of sham-controlled randomised cl

Beyond Numbers: CKD-EPI Versus MDRD in Primary Care-Differences in Chronic Kidney Disease Stage Classification in 117,055 Patients.

Background/Objectives: Chronic kidney disease (CKD) is a global public health concern, posing significant diagnostic and management challenges in primary care. Estimated glomerular filtration rate (eGFR) is central to CKD staging, yet different estimating equations may yield substantially different stage classifications when applied to the same population. This study aims to compare the eGFR-based CKD stage classification and stage distribution obtained using the Chronic Kidney Disease: Epidemiology Collaboration (CKD-EPI) and Modification of Diet in Renal Disease (MDRD) equations in a large p

Selenoprotein P Deficiency Is Associated with Early Signs of Kidney Disease and Hospitalization Risk in Heart Failure.

UNLABELLED: Selenium (Se), an essential trace element, is linked to poor prognosis in heart failure (HF) and kidney disease. Se deficiency (serum Se < 70 μg/L) has been associated with increased cardiovascular mortality. Selenoprotein P (SELENOP), the main Se transporter, reflects bioavailable Se. Selective glomerular hypofiltration syndrome (SGHS), defined by a cystatin C-based eGFR < 0.7 of creatinine-based eGFR, signals early kidney dysfunction and worsens HF outcomes. The prognostic role of SELENOP for SGHS and kidney-related hospitalization in HF remains unclear. PURPOSE: To assess wheth

Gout flare burden as a marker of chronic kidney disease risk.

BACKGROUND: Gout is a common inflammatory arthritis with substantial cardiometabolic and renal comorbidity. Although hyperuricemia and baseline kidney function are established predictors of chronic kidney disease (CKD), the prognostic value of long-term gout flare frequency is unclear. This study aimed to examine the association between annual gout flare frequency and the risk of incident CKD. METHODS: We conducted a large-scale retrospective cohort study using de-identified electronic health record data from the TriNetX Global Collaborative Research Network. Adults with at least one treated g

ESKD, Major Cardiovascular Events, and Death Associated With Systemic Inflammation.

INTRODUCTION: Systemic inflammation may play a role in the progression of chronic kidney disease (CKD) and in the development of major cardiovascular events (MACE). Whether this effect is independent of traditional cardiovascular risks is unclear. METHODS: In this study, we compared the incidence of end-stage kidney disease (ESKD) (i.e., chronic dialysis or renal transplantation), MACE, and death in patients with CKD with high sensitivity C-reactive protein (hsCRP) ≥ 2 mg/l versus hsCRP < 2 mg/l, using the TRINETX platform. RESULTS: Overall, 163,854 subjects with CKD (defined as estimated gl
