Neurofibromatosis Type 1: An Imaging Review of Multisystem Manifestations in Children.

Neurofibromatosis type 1 (NF1) is a common inherited tumor predisposition syndrome with multisystem involvement of the central and peripheral nervous systems, vasculature, skeleton, skin, and internal organs. Many clinically relevant manifestations are internal, age-dependent, progressive, or initially asymptomatic, making imaging central to diagnosis, surveillance, and treatment. This review provides a radiology-centered overview of the major imaging manifestations of NF1 and emphasizes pattern recognition, modality selection, and imaging features that should prompt closer follow-up, function

Clinicopathological Review of Renovascular Hypertension and Hypertensive Emergency-related Nephropathy Based on Case Series.

Renal complications resulting from persistent, uncontrolled hypertension include renovascular hypertension (RVH) and hypertensive emergency (crisis)-related nephropathy (HEN), both of which are characterized by hyperreninemia and are involved in the progression of nephropathy. In RVH, renal artery stenosis leads to increased renin secretion from the affected kidney, which induces focal segmental glomerulosclerosis (FSGS) lesions in the contralateral kidney, resulting in proteinuria. In HEN, severe hypertension (>200/120 mmHg) induces endothelial cell proliferation in renal arterioles, resultin

Influence of Albumin Treatment Duration on Outcome of Terlipressin Therapy in Patients With HRS-AKI.

BACKGROUND AND AIMS: Volume expansion with albumin is recommended to establish a diagnosis of hepatorenal syndrome-acute kidney injury (HRS-AKI). The optimal duration of albumin administration is debated and requires balancing the risk of overtreatment with the need for timely diagnosis and treatment. Therefore, this study aimed to compare the impact of different albumin treatment durations on terlipressin response and the prognosis of patients with HRS-AKI treated with terlipressin and albumin. METHODS: This multicenter, retrospective secondary analysis included patients with suspected HRS-A

Endocrine Hypertension in Children and Adults: Beyond Primary Aldosteronism.

Endocrine hypertension refers to elevated blood pressure resulting from an identifiable hormonal cause that is potentially treatable. Endocrine hypertension occurs in ∼30% of young adults with hypertension (18-40 years), in 30-50% of patients with resistant hypertension, most commonly due to primary aldosteronism, and can occur in a small yet clinically significant subset of children with hypertension. Other causes of endocrine hypertension include excess mineralocorticoid production, cortisol excess or impaired cortisol metabolism, disorders of the mineralocorticoid receptor or epithelial sod

Awareness and Acceptability of Whole Breast Reconstruction After Mastectomy: A Cross-Sectional Survey Study.

BACKGROUND: Parenteral nutrition associated cholestasis (PNAC) is a frequently observed pathology in extremely low birth weight (ELBW) infants. Its pathogenesis is determined by the composition and duration of parenteral nutrition (PN) as well as the tolerance of enteral feeds (EF). "Aggressive" nutrition is increasingly used in ELBW infants to improve postnatal growth. Little is known about the effect of "aggressive" nutrition on the incidence of PNAC. We analyzed the influence of implementing an "aggressive" nutritional regimen on the incidence of PNAC and growth in a cohort of ELBW infants.

Community Health Worker Support for Hispanic and Latino Individuals Receiving Hemodialysis: The Navigate-Kidney Randomized Clinical Trial.

IMPORTANCE: Hispanic and Latino individuals with hemodialysis-dependent kidney failure experience social barriers that may increase their risk for volume-related complications, including fluid accumulation between dialysis sessions or interdialytic weight gain (IDWG). Higher IDWG contributes to cardiovascular complications, hospitalizations, and poor quality of life. Minimizing IDWG is prioritized by patients, clinicians, and payers; yet, high IDWG remains common despite dietary education. OBJECTIVE: To determine if community health worker (CHW) support reduces IDWG among Hispanic and Latino

Soluble MAdCAM-1 as a biomarker in metastatic renal cell carcinoma.

Patients with metastatic renal cell carcinoma treated with immune checkpoint inhibitors or antiangiogenic tyrosine kinase inhibitors may develop resistance driven by gut dysbiosis, which disrupts the MAdCAM-1-α4β7 axis and promotes the recruitment of immunosuppressive IL-17-producing T regulatory (Tr17) cells into tumors. We evaluated soluble MAdCAM-1 (sMAdCAM-1) as a prognostic biomarker in 1,051 patients from three cohorts: JAVELIN Renal 101 (avelumab plus axitinib versus sunitinib), SURF (sunitinib) and NIVOREN (nivolumab after tyrosine kinase inhibitors). In the JAVELIN cohort, baseline sM

The Lifestyle Intervention in memory clinics of General and academic Hospitals Trial (LIGHT): Rationale and study design of a randomized controlled trial to reduce modifiable dementia risk.

INTRODUCTION: Dementia risk reduction through lifestyle modification has much potential but is not yet implemented in routine clinical care. Currently, there are no preventive interventions available for memory clinic patients. Therefore, the aim of The Lifestyle Intervention in the memory clinics of General and academic Hospitals Trial (LIGHT) is to examine the (cost)effectiveness of a multidomain intervention combining lifestyle coaching with risk self-management for patients with subjective cognitive decline (SCD) and mild cognitive impairment (MCI). METHODS: LIGHT is a 1-year multi-center

Incidental Finding of Secondary Focal Segmental Glomerulosclerosis in Renal Allograft due to Renal Artery Stenosis.

BACKGROUND: Focal segmental glomerulosclerosis (FSGS), which can be caused by many etiologies, is one of the major causes of nephrotic syndrome and end-stage kidney disease. Multiple etiologies, including glomerular hyperperfusion, can cause secondary FSGS. Unilateral renal artery stenosis can lead to FSGS of the contralateral native kidney, but it is not well demonstrated whether transplant renal artery stenosis can cause a similar change in a single transplanted kidney. CASE PRESENTATION: A female underwent kidney transplantation at the age of 17 on January 14, 2020. Her renal function rema

Post-transplant pelvic arteriovenous malformation in a living kidney transplant recipient.

Arteriovenous malformations are rare vascular anomalies that may be congenital or occur secondary to trauma or surgery. Here, we report a rare case of a pelvic arteriovenous malformation that gradually enlarged after living-donor kidney transplantation. The recipient, a man with end-stage kidney disease from IgA nephropathy, underwent ABO-compatible living kidney transplantation from his father at 6 years prior. A non-contrast computed tomography scan at 3 years post-kidney transplant incidentally revealed a pelvic vascular lesion that progressively enlarged upon serial imaging. Subsequent con

Association between early proteinuria reduction and kidney outcomes in Japanese patients with IgA nephropathy: The Japan Chronic Kidney Disease Database.

BACKGROUND: Proteinuria reduction is an established surrogate endpoint for IgA nephropathy (IgAN) progression in international trials, but evidence in Japanese populations is scarce. We aimed to assess the association between early changes in proteinuria and long-term kidney outcomes in a large, real-world cohort of Japanese patients with IgAN. METHODS: This nationwide, multicenter cohort study utilized the Japan Chronic Kidney Disease Database (J-CKD-DB) data. We analyzed 1,039 IgAN patients (mean age 48.8 years; 46.0% women), categorized by dipstick proteinuria change at 9 ± 1 months from b

Comparative effects of salt substitutes on blood pressure, cardiovascular events and mortality: a systematic review and network meta-analysis.

BACKGROUND: Hypertension is a major risk factor for cardiovascular disease. Salt substitutes may reduce sodium intake while maintaining palatability, but comparative effects across formulations remain uncertain. METHODS: We conducted a systematic review and frequentist random-effects network meta-analysis of randomised controlled trials in adults comparing salt substitutes with regular salt, other substitutes or no intervention. Databases (PubMed, Embase, CENTRAL, CNKI, Wanfang), WHO-ICTRP and ClinicalTrials.gov were searched from inception to Oct 3, 2025 (PROSPERO CRD42023451859). We assesse

Cardiovascular-kidney-metabolic syndrome in Indonesia: a commentary on the need for integrated management and policy action.

Cardiovascular-kidney-metabolic (CKM) syndrome is a term introduced to reflect the multidirectional relationship between metabolic disorders, kidney disease, and cardiovascular disease (CVD). The prevalence of CKM syndrome, accompanied by its increasing morbidity and mortality, has led researchers, clinicians, and policymakers to undertake synergistic and collaborative efforts in various countries. CKM Syndrome can be categorized into four stages: stage 0 (no risk factors), stage 1 (excessive or dysfunctional adiposity), stage 2 (metabolic risk factors or moderate to high-risk chronic kidney d

TRPC6 inhibition for the treatment of focal segmental glomerulosclerosis: a randomised, placebo-controlled, phase 2 trial of BI 764198.

BACKGROUND: In focal segmental glomerulosclerosis (FSGS), transient receptor potential cation channel, subfamily C, member 6 (TRPC6) overactivity might cause podocyte loss and progressive kidney function decline. This exploratory study assessed the safety and efficacy of a novel once-daily oral selective TRPC6 inhibitor, BI 764198. METHODS: This multicentre phase 2, double-blind, placebo-controlled, randomised controlled trial assessed BI 764198 (20 mg, 40 mg, or 80 mg once daily) versus placebo over 12 weeks in participants aged 18-75 years with biopsy-confirmed primary FSGS (based on the ab

Effectiveness of Multicomponent Interventions in Slowing Progression of CKD Stages G3-G4: A Systematic Review and Meta-Analysis.

KEY POINTS: Multicomponent interventions with two or more intervention strategies are increasingly used to manage patients with CKD G3-G4. Existing multicomponent interventions had a modest effect on slowing CKD progression, although the certainty of evidence was low for eGFR and moderate for eGFR slope. Among multicomponent interventions, physician-led team-based care seemed superior to non-physician-led team-based care, albeit the clinical relevance of the benefit was modest. BACKGROUND: Various multicomponent interventions (MCIs) have been increasingly used for managing patients with CKD G
