Primer on Renovascular Hypertension in Children: Focus on Endovascular Intervention.

Pediatric renovascular hypertension (RVHTN) results from flow-limiting disease of the renal arterial vasculature and is a potentially treatable cause of pediatric hypertension. Causes of pediatric RVHTN include idiopathic causes, fibromuscular dysplasia, neurofibromatosis, vasculitis, traumatic vascular injury, aneurysms, and aberrant renal arterial supply. Diagnostic imaging allows screening for and confirmation of pediatric RVHTN. Renal sonography with Doppler evaluation is the initial test of choice, followed by CT angiography and MR angiography. However, these modalities may not demonstrat

Collecting duct-derived (pro)renin receptor contributes to 2-kidney, 1-clip-induced ischemic nephropathy and renovascular hypertension in mice.

The 2-kidney, 1-clip (2K1C) Goldblatt model features overactivation of the systemic renin-angiotensin system (RAS) due to increased renin release from juxtaglomerular cells. However, no previous study has functionally assessed the potential involvement of the intrarenal RAS in this model. Within the kidney, the (pro)renin receptor (PRR) is predominantly expressed in the collecting duct (CD), where it plays a key role in regulating the intrarenal RAS under physiopathological conditions. In the present study, we used a mouse model of CD-specific deletion of PRR (CD PRR KO) to examine the role of

Approach to pediatric renovascular hypertension.

Renovascular hypertension (RVH) is an important cause of hypertension in children, but it can be cured if the cause is removed in some cases. Therefore, early diagnosis and appropriate treatment are the goals of pediatricians' care. The cause of renal artery stenosis differs in prevalence by country. Recently, the importance of diagnosing RVH as a genetic syndrome increased. This paper reviews the current literature on the diagnosis and treatment of RVH in children.

Kidney autotransplantation for renovascular hypertension due to hilar renal artery aneurysm: India's first in a child.

A boy in middle childhood with recurrent haematuria and refractory hypertension for 3.5 years was referred to our hospital with CT findings of a large right renal artery aneurysm. Following an unsuccessful endovascular intervention, a multidisciplinary team approach was set up, as well as a plan for right nephrectomy, complete excision of the aneurysm, reconstruction of the cut end of the branches of the renal artery using internal iliac artery (IIA) 'Y' graft followed by autotransplantation. The IIA 'Y' graft was then anastomosed with the external iliac artery, and the renal vein was anastomo

Combined percutaneous renal artery denervation and angioplasty: still too early to consider them complementary?

In case of renovascular hypertension, percutaneous renal artery transluminal angioplasty alone is still far from demonstrating predictable clinical results. Assuming a potential synergistic effect, we reported the first case of combined percutaneous renal denervation and angioplasty as intention-to-treat strategy in a very high cardiovascular risk patient.

Unmasking Renal Artery Pseudoaneurysm: A Rare and Elusive Cause of Hypertension.

A 49-year-old man with hypertension for 2 years, was incidentally found to have right intrarenal aneurysms during a routine abdominal ultrasound examination. He reported fluctuating blood pressure readings over the past 18 months. Laboratory parameters were within normal limits, with a serum creatinine level of 1.05 mg/dL. An abdominal CT angiogram revealed a large lobulated intrarenal aneurysm in the lower pole, which communicated with another smaller cortical aneurysm. Additional features suggested the presence of an arteriovenous (AV) fistula. Upon further questioning, the patient disclosed

Pickering syndrome facilitated by seronegative immune mediated necrotizing myopathy: a case report.

BACKGROUND: Immune-mediated necrotizing myopathies (IMNM) are rare types of idiopathic inflammatory myopathies characterized by severe proximal muscle weakness and muscle fibre necrosis with a paucity of inflammatory cells. Serologically, IMNM are further classified into three subtypes based on the presence or absence of two specific autoantibodies: the anti-3-hydroxy-3-methylglutaryl coenzyme A reductase and the anti-signal recognition particle. Especially the seronegative form exhibits a higher risk of various extra skeletal muscle involvement, including cardio-myositis. Nevertheless, severe

Unveiling the molecular mechanism of diosmin to resolve cardiovascular dysfunction and remodeling in hypertension.

This study aimed to examine the potential effects and mechanisms of diosmin, a flavone glycoside, on blood pressure and cardiovascular abnormalities in a rat model of hypertension caused by renal artery stenosis. Male Sprague-Dawley rats were operated on two-kidney, one-clip (2K-1C) to induce renovascular hypertension. Following three weeks of surgery, 2K-1C rats were subjected to daily oral administration of diosmin (25 or 50 mg/kg) or telmisartan (5 mg/kg) for four weeks. Diosmin significantly reduced blood pressure and enhanced left ventricular (LV) performance and vascular endothelial func

A case of malignant hypertension due to sarcoidosis.

Sarcoidosis is a systemic inflammatory disease that is difficult to diagnose. This report describes a 54-year-old man who presented with an incidental finding of abnormal hilar shadows on imaging, indicating sarcoidosis. During follow-up, he experienced a sudden blood pressure increase (210/140 mmHg) accompanied by headaches, nausea, hypertensive retinopathy, and renal dysfunction, diagnosed as malignant hypertension. Contrast-enhanced computed tomography (CT) revealed wedge-shaped areas of poor enhancement in both kidneys, suggesting impaired perfusion due to interlobar artery compression. Re

Renal Artery Involvement in Pediatric-Onset Takayasu Arteritis: Renal Characteristics and Mid-Term Follow-up Outcomes.

Renal artery involvement (RAI) can cause renovascular hypertension and/or organ dysfunction in Takayasu arteritis (TA). The literature includes few data regarding renal involvement in pediatric-onset TA patients. We aimed to describe the renal characteristics, management, and outcomes in TA patients with RAI.We retrospectively reviewed the clinical, laboratory, and radiological findings, treatments, and outcomes in TA patients with RAI, diagnosed between 2008 and 2022.Nine of 20 pediatric-onset TA patients had RAI; their median age was 14.7 (13.1-15.5) years. Five patients had bilateral RAI. S

A retrospective cohort study of 203 patients with Takayasu arteritis: experience from a Brazilian tertiary center.

BACKGROUND: Takayasu arteritis (TAK) is a rare form of systemic vasculitis that primarily affects the aorta and its major branches. Despite several epidemiological studies on TAK, only a few descriptive studies have been conducted in our country, which motivated us to conduct this study. METHODS: This single-center retrospective cohort study included Brazilian patients with TAK who underwent follow-up at a tertiary outpatient clinic between January 2000 and June 2024. Patient data were extracted from electronic medical records using standardized and parameterized information. RESULTS: A total

A Case of Drug-Resistant Renovascular Hypertension due to Renal Artery Stenosis Successfully Treated by Nephrectomy of the Affected Kidney.

INTRODUCTION: Renal artery stenosis (RAS) reduces renal blood flow and activates the renin-angiotensin-aldosterone (RAA) system, resulting in renovascular hypertension (RVH). CASE PRESENTATION: We report a case of a 49-year-old woman with RVH due to bilateral renal artery stenosis, predominantly on the right. Despite pharmacological treatment, blood pressure remained poorly controlled; leading to severe heart failure that required dialysis. Percutaneous renal angioplasty was considered but deemed technically difficult. The patient underwent a successful laparoscopic right nephrectomy, leading

Intrarenal Generation of Aldosterone Contributes to Ischemia-Induced Hypertension and Nephropathy in Mice.

BACKGROUND: The past few years have witnessed a significant advancement in aldosterone (Aldo)-targeted therapies for the management of treatment-resistant hypertension and chronic kidney disease, which often exist in tandem. While Aldo is believed to predominantly originate from the adrenal glands, this study provides evidence to support the involvement of intrarenal Aldo biosynthesis in the pathogenesis of ischemic nephropathy and hypertension in a two-kidney, one-clip (2K1C) model. METHODS: We generated inducible renal tubule-specific deletion of C11B2 (RT C11B2 KO) and characterized the ph

Increased reactive oxygen species in the spinal cord drive renal sympathetic vasomotor activity in Goldblatt hypertensive rats.

Increased production of reactive oxygen species (ROS) in brain regions contributes to the sympathetic vasomotor overactivity in Goldblatt hypertension (2K1C). Previously, studies reported that overexpression of spinal angiotensin II (Ang II) type I (AT1) contributes to sympathetic vasomotor overactivation in 2K1C rats. Considering that Ang II leads to an imbalance of oxidative stress, the present study evaluated the role of spinal ROS in regulating the activity of sympathetic preganglionic neurons in 2K1C rats. Hypertension was induced by clipping the left renal artery. Six weeks after clippin
