CERAS: covered endovascular reconstruction of the aorta and side branches. Description of the technique of revascularizing more than two visceral branches in patients with a coral reef aorta.

Coral reef aorta represents a distinct vascular pathology characterized by large calcified exophytic plaques that grow into the aortic lumen typically at the level of the visceral and renal arteries. In most cases, these arteries are also heavily calcified and stenosed or occluded. Symptoms vary depending on the affected artery and differ from renovascular hypertension, and renal insufficiency to cardiac decompensation and chronic limb threatening ischemia. This report describes two patients with a coral reef aorta and stenosed aortic side branches. Both patients had experienced episodes of pu

Computational assessment of fibromuscular dysplasia-related renal artery stenosis.

BACKGROUND: Fibromuscular dysplasia (FMD) is a potentially curable cause of renovascular hypertension, primarily affecting middle-aged women. Percutaneous angioplasty is the recommended treatment for patients with hemodynamically significant renal artery stenosis (RAS). However, no established non-invasive method currently exists to estimate the pressure drop across stenosis and assess its hemodynamic impact. This study aimed to assess the clinical applicability of an image-based computational fluid dynamic (CFD) pipeline, functioning as a digital twin of the renal vasculature, for non-invasiv

Pediatric Renovascular Hypertension: Diagnosis and Management.

Pediatric renovascular hypertension (RVH) is a significant cause of secondary hypertension in children, often resulting from renal artery stenosis (RAS) due to nonatherosclerotic etiologies such as fibromuscular dysplasia (FMD), mid aortic syndrome (MAS), and syndromic conditions like neurofibromatosis type 1 (NF-1). Early diagnosis is critical to prevent long-term complications, including renal dysfunction and cardiovascular disease. Diagnostic evaluation begins with ultrasound, which, despite its limitations, remains the first-line modality. Advanced imaging techniques such as CT angiography

Unilateral page kidney in end-stage renal disease patient presenting as recurrent flash pulmonary edema: A rare association.

Page kidney refers to the extrinsic compression of renal parenchyma causing renovascular hypertension. We present the case of a 58-year-old female with refractory hypertension and recurrent episodes of flash pulmonary edema following trivial trauma. Despite initial management with percutaneous drainage of the perinephric hematoma, she eventually required a nephrectomy due to persistent symptoms. The symptoms resolved post nephrectomy. The case highlights the importance of early diagnosis and timely intervention in Page kidney to prevent long-term complications.

Refractory Renovascular Hypertension from Paraganglioma: A Case Report.

BACKGROUND Accurate diagnosis and effective management of secondary hypertension can be challenging. Renovascular hypertension is a common etiology of secondary hypertension. Pheochromocytomas and paragangliomas are neuroendocrine tumors that secrete catecholamines, which are rarer causes of secondary hypertension. The combination of these 2 conditions is even rarer, but should not be overlooked. CASE REPORT Here, we present the case of a young woman with hyper-reninemic hypertension caused by unilateral renal artery stenosis, which recurred despite percutaneous transluminal renal angioplasty.

A Literature Review of Renal Artery Stenosis: Presentation, Diagnosis, and Management Options in Secondary Hypertension.

Renal artery stenosis (RAS) remains a critical and often underdiagnosed cause of secondary hypertension due to its insidious progression. This traditional review examines the epidemiology, various presentations of RAS, pathophysiology, and diagnostic modalities, with a focus on treatment options based on selected clinical trials. The prevalence of RAS is notably higher in individuals with systemic atherosclerotic conditions, making atherosclerosis the most common cause of RAS. Less frequently, fibromuscular dysplasia may be involved. Although RAS presents as hypertension in most cases, some pe

Gastrin releasing peptide receptor antagonism in the paraventricular nucleus exerts antihypertensive effects in 2K1C rats.

Hypertension is a leading global risk factor for cardiovascular diseases marked by sympathetic overactivation. The role of centrally expressed bombesin and its receptors in elevating sympathetic activity and blood pressure has been reported. However, whether gastrin releasing peptide receptor (GRPR), one of bombesin-like peptide receptors, in the paraventricular nucleus (PVN) mediates renovascular hypertension and its potential mechanism remain unclear. In this study, we explored the following: (1) the role of GRPR in the PVN in the development of hypertension in two-kidney one-clip (2K1C) hyp

Malignant Hypertension Complicated by Posterior Reversible Encephalopathy Syndrome (PRES).

Clinical signs and symptoms of posterior reversible encephalopathy syndrome (PRES) include headache, altered mental state, vision loss, and seizures. Neuroradiological results exhibiting white matter abnormalities, primarily in the parieto-occipital areas of the brain, are linked to PRES. Hypertensive encephalopathy, eclampsia, renal failure, and immunosuppressive or anticancer treatment are the most common conditions associated with PRES. In the context of increasing renovascular hypertension resulting from bilateral atherosclerotic renal artery stenosis, we present a case with PRES coupled w

Kidneys in Children with Tuberous Sclerosis Complex-An Up-to-Date Review.

Background: Tuberous sclerosis complex (TSC) is a rare genetic disorder characterized by the growth of benign tumors in various organ systems, with particularly significant effects on the kidneys. Renal manifestations of TSC include angiomyolipomas (AMLs), renal cysts, and a higher risk of renal cell carcinoma (RCC). Nephrological monitoring is crucial for the early detection of kidney changes, the management of hypertension, and the assessment of the risk of developing chronic kidney disease. Ultrasound is typically the initial imaging choice for diagnosis and monitoring, with magnetic resona

Imaging in Renovascular Hypertension: State of the Art.

Renovascular hypertension (RVH) results from activation of the renin-angiotensin system by renal arterial stenosis (RAS). RAS can result from either intrinsic narrowing or extrinsic compression. Intrinsic narrowing may be caused by atherosclerosis, fibromuscular dysplasia, vasculitis, dissection, thrombus, aneurysm, arteriovenous fistula, neurofibromatosis type 1, or midaortic syndrome. Extrinsic compression can be from retroperitoneal fibrosis, neoplasm, or subcapsular hematoma or fluid. Diagnostic evaluation for RVH is triggered by either a high index of suspicion in a patient with hypertens

How to do it - Endovascular solutions for the renovisceral segment in coral reef aortas.

Background: Coral reef atherosclerosis of the renovisceral aorta leads to end-organ and extremity malperfusion due to ischaemia and/or embolic events. Those mostly female patients typically present with renovascular hypertension, chronic mesenteric ischaemia, and symmetric peripheral artery disease of all stages, including critical limb ischaemia. Mortality and morbidity are high in open surgery. Materials and methods: Here we describe step by step the individually designed endovascular solutions with multiple arterial accesses using stentgrafts and open stents in 8 consecutive patients. Renal Artery Stenting for Traumatic Dissection.

Renal injuries are frequently seen in the context of trauma, with currently well-established guidelines for classification and management, such as those prescribed by the American Association for the Surgery of Trauma. However, traumatic dissection of the renal arteries specifically is uncommon; clarity regarding optimal management is currently lacking, despite the possible long-term sequelae, including loss of nephron function and renovascular hypertension. While medical management alone had sometimes been suggested, endovascular stenting is potentially a viable minimally invasive interventio

Atherosclerotic renal artery stenosis in the post-CORAL Trial Era. A narrative review.

Atherosclerotic renal artery stenosis (ARAS) represents a common manifestation of systemic atherosclerosis and remains an underrecognized cause of secondary hypertension, chronic kidney disease, and cardiovascular morbidity. Although often clinically silent, progressive narrowing of the renal artery may result in renovascular hypertension, ischemic nephropathy, or cardiac destabilization syndromes such as recurrent pulmonary edema. The pathophysiology of ARAS extends beyond simple flow limitation, involving renin-angiotensin-aldosterone system activation, oxidative stress, microvascular rarefa

2-Kidney-1-clip hypertension is not attenuated in mice lacking the transient receptor potential vanilloid type 1 (TRPV1) channel.

INTRODUCTION: Chemical ablation of renal sensory nerves using agonists for transient receptor potential vanilloid-1 (TRPV1) lowers arterial blood pressure (ABP) in multiple experimental models of hypertension. Interestingly, both afferent renal nerve activity and arterial blood pressure were significantly attenuated in male Trpv1-/- rats after 2-kidney-1-clip (2K1C) renovascular hypertension. However, TRPV1 expression in sensory neurons differs across species and is lower in mice versus rats or humans. Therefore, the current study assessed the proportion of TRPV1 in mouse renal sensory neurons

Follow-up Outcomes from the first National Fibromuscular Dysplasia Clinic in the United Kingdom.

INTRODUCTION: Fibromuscular dysplasia (FMD) is a rare disease with diverse clinical presentations. The need for a bespoke clinic providing individualized care was deemed a patient priority at the United Kingdom's (UK) first patient information day on FMD. In response, a multidisciplinary clinic was established at Salford Royal Hospital in November 2019, integrating renal, neurology, interventional radiology, and neuro-radiology expertise. It has since evolved into the UK's first national FMD clinic. This study aimed to describe baseline patient characteristics and identify contributing factors

Polymorphic Ventricular Tachycardia as a Manifestation of Unilateral Renal Artery Stenosis.

Renovascular hypertension (RVH) is an under-recognized but potentially reversible cause of secondary hypertension with significant cardiovascular consequences. Excess aldosterone resulting from renin-angiotensin-aldosterone system activation leads to potassium loss, QT-interval prolongation, and increased susceptibility to malignant ventricular arrhythmias. Hypokalemia combined with hypertension-mediated left-ventricular hypertrophy further increases myocardial electrical instability, predisposing to polymorphic ventricular tachycardia (PVT). We report a woman in her fifties who presented with

Sonographic Anatomy and Normal Measurements of the Human Kidneys: A Comprehensive Review.

Ultrasound is the primary, non-invasive imaging modality for evaluating renal anatomy and function in both acute and chronic settings. Familiarity with normal kidney morphology, cortical and parenchymal thickness, echogenicity, and Doppler parameters is essential for differentiating normal findings from early manifestations of disease. This review summarizes established reference ranges and anatomical variants from the 1950s to 2025, highlighting differences related to age, sex, body habitus, and ethnicity. Practical emphasis is placed on the interpretation of renal size, cortical thickness, e

Flash Pulmonary Oedema in a Patient With Unilateral Renal Artery Stenosis and Preserved Contralateral Function: A Medically Managed Case Report.

Flash pulmonary oedema (Pickering phenotype) is a high-risk, under-recognised manifestation of renovascular disease. A man in his 50s presented with abrupt dyspnoea, hypertensive crisis, type 1 respiratory failure, and pulmonary oedema. His response to intravenous diuretics was limited, and he was stabilised in the ICU with glyceryl trinitrate and continuous positive airway pressure. Ultrasound showed a small scarred right kidney of 7.9 cm. Echocardiography found a left ventricular ejection fraction (LVEF) of 25% at presentation. Three months later, cardiac MRI showed LVEF of 40% with diffuse

A narrative review of the role of renal artery intervention in renovascular hypertension.

Renovascular hypertension is a form of secondary hypertension caused by renal artery stenosis and often shows a limited response to medical treatment. Over recent years, renal artery interventions, primarily angioplasty and stenting, have been increasingly used as treatment options in selected patients. This narrative review summarizes current techniques, clinical outcomes, and evidence related to renal artery interventions in the management of renovascular disease. It also highlights existing knowledge, challenges, emerging technologies, and future directions for improving patient selection,
