Kidney transplant outcomes in patients with complement dysregulation.

BACKGROUND: Complement dysregulation is a key mechanism underlying atypical hemolytic uremic syndrome and complement-mediated thrombotic microangiopathy in kidney transplantation, and is associated with post-transplant recurrence, graft dysfunction, and graft loss. Although Western cohort data are well established, integrated transplant outcome data combining genetic analysis, anti-factor H (AFH) antibody profiling, and individualized therapy from Indian and other South Asian populations remain limited, despite a distinct biologic profile in this region. AIM: To evaluate clinical characterist

Association of hematologic cancer with characteristics, causes, and outcomes of patients with cardiovascular admissions.

OBJECTIVE: Data on the characteristics and outcomes of patients with hematologic cancers undergoing cardiovascular (CV) admissions in real-world settings are limited. METHODS: This study used data from the National Inpatient Sample (2016-2020), including all CV admissions. Patients were stratified based on the presence of hematologic cancers and their subtypes. The primary outcome was the cause of CV admission, and the secondary outcome was all-cause mortality. Descriptive statistics and multivariable logistic regression (adjusted odds ratios [aOR]) were used to compare outcomes across groups

Large-Scale Disease-Wide Association Study Identified Predisposition Links Between Influenza and Cardiovascular Diseases.

BACKGROUND: Influenza remains a major global health threat and, akin to "long COVID," has been linked to prolonged multi-organ comorbidities ("long flu"). Cardiovascular diseases (CVDs) are increasingly recognized in the pathogenesis of influenza, yet most prior work emphasizes acute or short-term outcomes and rarely contrasts multiple endpoints over extended horizons. METHODS: This study conducted a disease-wide association study (DWAS), assembling individuals with clinically coded influenza without pneumonia (n = 5136) and population comparators (n = 314,673). Using age- and sex-adjusted Co

Plasma proteomics linking primary and secondary diseases: Insights into molecular mediation from UK Biobank data.

BACKGROUND: Diabetes, hypertension, and dyslipidemia are major risk factors for cardiovascular, neurological, renal, and pulmonary diseases, yet clinically accessible molecular mediators linking these cardiometabolic conditions to downstream complications remain unclear. METHODS: We analyzed plasma proteomic data from 53,030 UK Biobank participants with longitudinal follow-up. Mediation analysis evaluated circulating proteins linking three primary cardiometabolic diseases to 18 secondary outcomes. Mendelian randomization assessed potential causal relationships, and machine learning evaluated

Systemic versus local regulation of vitamin D.

PURPOSE OF REVIEW: New clinical data and technical advances challenge the long-held belief that the kidney is the predominant organ for vitamin D synthesis and degradation. This review highlights recent discoveries in systemic and local vitamin D regulation and their potential to influence future development of novel treatment concepts. RECENT FINDINGS: New clinical studies of calcifediol supplementation in anephric patients and in chronic kidney disease provide evidence for significant extra-renal vitamin D production. The clinical findings are supported by recent laboratory-based studies th

Secondary causes and renal outcomes in apparently resistant hypertension: a retrospective cohort study.

OBJECTIVE: In this study, we aimed to determine the prevalence and spectrum of secondary hypertension among patients with apparently resistant arterial hypertension (aRH) and to compare the rate of decline in estimated glomerular filtration rate (eGFR) between those with primary aldosteronism (PA) and true resistant arterial hypertension (RAH). METHODS: This retrospective cohort study involved 790 patients with aRH referred to a hypertension excellence centre. All patients fulfilled pharmacological criteria for aRH and underwent a screening protocol to evaluate secondary hypertension. We comp

[Diagnostic yield of hypertension evaluation in a nephrology day hospital].

INTRODUCTION: Secondary hypertension (HTN) accounts for 5–10% of HTN. The actual diagnostic yield of a comprehensive etiological work-up in nephrology-specialized centers remains poorly documented. MATERIALS AND METHODS: We conducted a single-center retrospective study including all patients evaluated in the day hospital program of the ESH-accredited Hypertension and Nephrology Unit at Rouen University Hospital for suspected secondary HTN, between 2018 and 2019. The standardized protocol combined clinical assessment, laboratory testing, imaging, and multidisciplinary discussion. RESULTS: One

Comprehensive evaluation of secondary causes of hypertension: Integrated screening for obstructive sleep apnea and primary aldosteronism.

INTRODUCTION: Obstructive sleep apnea (OSA) and primary aldosteronism (PA) are common and treatable conditions that are often overlooked in the evaluation of hypertension. We assessed the prevalence and relative contribution of identifiable causes of hypertension using a comprehensive diagnostic approach. METHODS AND MATERIALS: In this cross-sectional study, consecutive adults (aged 18-80 years) with hypertension from the endocrinology OPD were screened for secondary causes. All participants underwent standardized blood pressure assessment and systematic evaluation for major causes of seconda

Addressing evidence gaps in haemodialysis care: a critical review of clinical trials, technological innovations, and implementation priorities.

Kidney failure treated with maintenance haemodialysis (HD) carries high risks of premature mortality, hospitalisation and impaired quality of life, yet important evidence gaps persist across routine HD care. The Dialysis Research Innovation Network, part of the UK Kidney Research Consortium, convened the inaugural UK Haemodialysis Research Nexus in Birmingham on 11th May 2026. There were fifty-six delegates: 22 doctors, 13 nurses, six dietitians, one renal scientist, seven people in other roles (including one patient), and seven whose role was not recorded. The meeting reviewed recent trials,

Glucocorticoid dose at discharge and the risk of infectious complications and rehospitalization in renal diseases.

BACKGROUND: Glucocorticoids have diverse adverse effects, particularly infections that may be severe or opportunistic. Therefore, glucocorticoids are often initiated during hospitalization; however, the optimal glucocorticoid dose at discharge remains uncertain. This study examined whether the discharge dose of glucocorticoids influences infection onset and rehospitalization in patients with renal disease. METHODS: This was a multicenter retrospective cohort study conducted at eight hospitals from 2013 to 2023. Adults initiated on oral prednisolone ≥ 30 mg/day during hospitalization for renal

Factors Affecting Risk of Cardiovascular Disease (CVD) Events in a Global CVD Prevention Cohort of People With HIV.

BACKGROUND: Persons with HIV face increased risks of major adverse cardiovascular events (MACE), partially mitigated by statin therapy. METHODS: We characterized factors associated with MACE and MACE subcomponents among PWH enrolled in REPRIEVE, globally. Our primary outcome measure was time-to-first primary MACE. Secondary outcome measures included time-to-first (1) hard MACE (cardiovascular disease [CVD] death, myocardial infarction [MI], or stroke), (2) MI, or (3) stroke. For each outcome, Cox proportional hazards models were used to estimate the hazard of baseline risk factors. RESULTS:

Phenotypic comorbidity network analysis of nasopharyngeal carcinoma: a multicenter study based on hospital discharge records.

BACKGROUND: Nasopharyngeal carcinoma (NPC) patients frequently have multiple comorbidities that affect treatment efficacy and survival. Prior studies focused only on individual common comorbidities, lacking a comprehensive analysis of the full spectrum of chronic diseases in NPC patients from endemic areas. This study adopted phenotypic comorbidity networks (PCNs) based on large-scale hospital discharge data to systematically characterize the overall comorbidity patterns of endemic NPC patients. METHODS: This retrospective cohort study was conducted using data from NPC patients identified in

Liver-Metabolic Phenotypes and Renal Vulnerability in Community-Acquired Sepsis: Insights from the SepsisFAT Cohort.

Background: Metabolic-dysfunction-associated steatotic liver disease (MASLD) is associated with adverse outcomes in sepsis, but risk stratification within MASLD remains insufficiently defined. We investigated whether an admission liver-metabolic phenotype framework combining cardiometabolic burden with liver injury/fibroinflammatory risk markers identifies clinically relevant organ-support vulnerability in community-acquired sepsis. Methods: This secondary analysis of the prospective SepsisFAT cohort (378 adults with community-acquired sepsis) classified patients into four phenotypes by cardio

Trends in Age-Related Health Outcomes in People with HIV in Spain, 2006-2023.

BACKGROUND: With antiretroviral therapy (ART), people with HIV (PWH) live longer and face age-related health challenges. The long-term evolution of non-AIDS comorbidities in this population remains unclear. We assessed temporal trends in incidence and age at onset of major age-related outcomes in PWH. METHODS: We conducted a longitudinal study using CoRIS, a nationwide cohort of ART-naive PWH in Spain. Participants were grouped into 3 calendar periods (2006-2011, 2012-2017, and 2018-2023). The primary outcome was serious non-AIDS events (SNAEs), including major cardiovascular events, non-AIDS
