[Treatment and Prospects of Focal Segmental Glomerulosclerosis].

Focal segmental glomerulosclerosis (FSGS) is a refractory kidney disease that poses substantial clinical challenges. FSGS is primarily characterized by proteinuria and progressive loss of renal function. Its pathogenesis is complex and varied, involving immune-mediated injury, podocyte dysfunction, genetic factors, and changes in renal hemodynamics. According to pathologic features and etiology, FSGS can be classified into primary, hereditary, secondary, and undetermined-cause forms. Existing therapeutic strategies are mostly based on specific disease classifications and include the administra

Low-renin hypertension.

Low-renin hypertension (LRH) affects approximately 30 percent of patients with arterial hypertension and represents a spectrum of heterogeneous disorders characterized by low renin levels, increased sodium reabsorption, and expanded circulating volume. LRH includes monogenic and acquired secondary forms; however, the majority of the patients-particularly older individuals and those of African descent-present essential hypertension. Primary aldosteronism is the most frequent secondary cause, marked by excessive and autonomous aldosterone secretion. Other monogenic forms, such as Liddle syndrome

Malignant hypertension induces thrombotic microangiopathy and renal failure: A case report.

RATIONALE: Thrombotic microangiopathies (TMA) caused by malignant hypertension is an acute and critical disease among rare diseases. Although renal biopsy pathology is a golden indicator for diagnosing kidney disease, it cannot distinguish between primary and secondary TMA and requires a comprehensive diagnosis in conjunction with other laboratory tests and medical history. PATIENT CONCERNS: A 33-year-old young man was hospitalized due to unexplained kidney failure. DIAGNOSIS: A young man was admitted to the hospital with unexplained renal failure with severe heart failure and was diagnosed

Renal Denervation in Asia: 2025 Asia Renal Denervation Consortium (ARDeC) Consensus Statement Endorsed by the Hypertension Cardiovascular Outcome Prevention and Evidence in Asia (HOPE Asia) Network.

The second Asia Renal Denervation Consortium consensus conference shared information and developed updated recommendations for renal denervation (RDN). Current evidence confirms that RDN significantly reduces blood pressure across all metrics (office, home, and ambulatory) throughout 24 hours. Modern RDN approaches target the distal main renal artery and branches where nerves more closely approximate the vessel wall. Understanding renal artery anatomy is crucial; the main renal artery typically divides into anterior and posterior divisions as first-order branches, which further subdivide into

Primary vesicoureteral reflux presenting as hypertensive crisis and thrombotic microangiopathy.

A 10-year-old boy with a history of febrile urinary tract infections presented with a hypertensive crisis and thrombotic microangiopathy (TMA). Functional and genetic complement testing was normal, and TMA resolved with blood pressure control, suggesting a primary hypertensive aetiology. Renal biopsy confirmed chronic tubulointerstitial nephritis (CTIN), likely secondary to recurrent pyelonephritis and renal scarring after ruling out other potential causes. High-grade vesicoureteral reflux was confirmed by voiding cystourethrography. After 4 years, he has progressed to stage III chronic kidney

Benefits of aldosterone receptor antagonism in chronic kidney disease: the BARACK-D RCT.

BACKGROUND: Chronic kidney disease affects around 10% of the global population and is associated with significant risk of progression to end-stage renal disease and vascular events. Aldosterone receptor antagonists such as spironolactone have shown prognostic benefits in patients with heart failure, but effects on patients with chronic kidney disease are uncertain. OBJECTIVES: To determine the effect of low-dose spironolactone on mortality and cardiovascular outcomes in people with chronic kidney disease stage 3b. DESIGN: Prospective randomised open blinded end-point trial. SETTINGS: Three

Heart failure events in randomized controlled trials for adults receiving maintenance dialysis: a meta-epidemiologic study.

BACKGROUND AND HYPOTHESIS: Heart failure is characterized as cardiac dysfunction resulting in elevated cardiac filling pressures with symptoms and signs of congestion. Distinguishing heart failure from other causes of similar presentations in patients with kidney failure is challenging but necessary, and is needed in randomized controlled trials (RCTs) to accurately estimate treatment effects. The objective of this study was to review heart failure events, their diagnostic criteria, and adjudication in RCTs of patients with kidney failure treated with dialysis. We hypothesized that heart failu

Coronary Artery Calcification on Chest Computed Tomography as a Predictor of Cardiovascular Adverse Events in Patients With COVID-19 - A Multicenter Retrospective Study in Japan.

BACKGROUND: Coronary artery calcification (CAC) detected through chest computed tomography (CT) strongly predicts cardiovascular events in asymptomatic individuals undergoing primary prevention. Few studies with limited sample sizes have investigated the predictive value of CAC for cardiovascular complications in COVID-19. This study examined the impact of CAC on cardiovascular complications using a large-scale COVID-19 database. METHODS AND RESULTS: This multicenter retrospective cohort study used data from the Japan COVID-19 Task Force database. After exclusion based on missing information,

A nuanced scrutiny of the segmental renal artery spectral Doppler in "Pickering" the right diagnosis.

Approximately 5-10% of patients with hypertension have secondary hypertension. We describe a case of secondary hypertension from bilateral renal artery stenosis (RAS): "Pickering syndrome." This is a case of hypertension secondary to bilateral RAS which provides an opportunity to review secondary hypertension with a specific focus on RAS, in terms of when to consider work up, causes of secondary hypertension, diagnostic testing, and treatment.

Adverse pregnancy outcomes and lung function later in life: The CARDIA lung study.

BACKGROUND: Pregnancy events are linked to future maternal health, although relationships between adverse pregnancy outcomes (APOs) and maternal respiratory health are unknown. METHODS: This is a secondary analysis of CARDIA, a multicenter cohort study. We included all women who had delivered a live infant. APOs were defined as a history of ever having a small-for-gestational-age infant, preterm birth, gestational diabetes mellitus, or hypertensive disorder of pregnancy. Women were categorized as having no, one or >1 pregnancy with any APOs. Outcomes included year 30 lung function, annual cha

Imaging in Renovascular Hypertension: State of the Art.

Renovascular hypertension (RVH) results from activation of the renin-angiotensin system by renal arterial stenosis (RAS). RAS can result from either intrinsic narrowing or extrinsic compression. Intrinsic narrowing may be caused by atherosclerosis, fibromuscular dysplasia, vasculitis, dissection, thrombus, aneurysm, arteriovenous fistula, neurofibromatosis type 1, or midaortic syndrome. Extrinsic compression can be from retroperitoneal fibrosis, neoplasm, or subcapsular hematoma or fluid. Diagnostic evaluation for RVH is triggered by either a high index of suspicion in a patient with hypertens

Atherosclerotic renal artery stenosis in the post-CORAL Trial Era. A narrative review.

Atherosclerotic renal artery stenosis (ARAS) represents a common manifestation of systemic atherosclerosis and remains an underrecognized cause of secondary hypertension, chronic kidney disease, and cardiovascular morbidity. Although often clinically silent, progressive narrowing of the renal artery may result in renovascular hypertension, ischemic nephropathy, or cardiac destabilization syndromes such as recurrent pulmonary edema. The pathophysiology of ARAS extends beyond simple flow limitation, involving renin-angiotensin-aldosterone system activation, oxidative stress, microvascular rarefa

Malignancies and glomerulonephritis: when to suspect and when to screen?

Glomerular diseases may occur secondary to malignancies. Age-specific cancer screening is recommended for patients with glomerular diseases and may be extended based on the specific risk associated with the detected histopathologic pattern. Membranous nephropathy is the prototype of cancer-associated glomerulonephritis, with 10% of cases presenting with malignancy within a year from diagnosis. Among antigens that are expressed in patients with membranous nephropathy thrombospondin type 1 domain-containing 7A and neural epidermal growth factor-like-1 are often reported in patients with underlyi

Bilateral Idiopathic Sclerochoroidal Calcification: A Case Report.

Sclerochoroidal calcification (SCC) is a rare and often asymptomatic condition characterized by the deposition of calcium pyrophosphate in the sclera and choroid. It predominantly affects elderly individuals of Caucasian descent and is most often idiopathic, although it can be secondary to systemic disorders such as hyperparathyroidism or chronic renal disease. The lesions appear as irregular, white-yellow plaques located in the mid-periphery, typically along the vascular arcades. We present the case of an 80-year-old patient with bilateral idiopathic SCC, incidentally discovered during an oph

Longitudinal associations between iron status and patient-reported outcomes in incident dialysis patients: a DOMESTICO substudy.

BACKGROUND: More liberal use of iron therapy is favored in dialysis patients based on lower erythropoietin need and clinical outcomes. However, it remains unclear whether higher iron stores are associated with better patient-reported outcomes. We assessed the longitudinal associations of ferritin and transferrin saturation (TSAT) levels with patient-reported outcomes in incident dialysis patients. METHODS: This prospective cohort study included incident dialysis patients who had completed at least one patient-reported outcome questionnaire and had undergone a laboratory assessment (e.g., ferr

Perirenal Adipose Tissue and Hypertension: Observational and Genetic Analyses.

BACKGROUND: Perirenal adipose tissue (PRAT) consists of white and brown adipocytes with good vascularization and dense innervation, which could influence the blood pressure. We aim to investigate the association of PRAT thickness with risks of overall and specific forms of hypertension. METHODS: We measured PRAT thickness in the UK Biobank and CONPASS (Chongqing Primary Aldosteronism Study). We prospectively examined the correlation between PRAT thickness and incident hypertension in the UK Biobank. We cross-sectionally explored associations between PRAT thickness and common forms of hyperten

Complement activation in secondary thrombotic microangiopathies.

Secondary thrombotic microangiopathies (TMAs) represent a heterogeneous group of diseases associated with a high risk of kidney failure and death despite available therapeutic strategies. Strong evidence implicates complement activation in the pathogenesis of secondary TMA, and emerging data increasingly suggest that pharmacological blockade of the complement improves the outcomes in patients with secondary TMA. Certain forms of secondary TMA, including postpartum TMA, TMA with coexisting hypertensive emergency and de novo TMA after kidney transplantation exhibit a high prevalence of pathogeni
