Risk of BPH-LUTS according to the PM₁₀ exposure: A longitudinal nationwide cohort-based study.

PURPOSE: Benign prostatic hyperplasia (BPH) is a major disease that causes lower urinary tract symptoms (LUTS) such as abdominal pressure, urgency, residual urine, weak urination, and delayed urination in elderly men. We attempted to identify risk factors for the development of BPH-LUTS, including particulate matter (PM). MATERIALS AND METHODS: We utilized customized research database of the National Health Insurance Corporation. A total of 2,200,546 subjects were included, consisting of BPH-LUTS (n=601,291) and non-BPH-LUTS (n=1,599,255). We classified the PM₁₀ exposure level based on median

Hypertriglyceridemic waist and subclinical atherosclerosis: a systematic review and meta-analysis.

BACKGROUND: Atherosclerosis is a chronic, progressive inflammatory disease with a long asymptomatic period. Disease progression can eventually lead to acute cardiovascular events, such as myocardial infarction, unstable angina, and sudden cardiac death. Hypertriglyceridemic waist (HTGW) has received much attention recently as a cheap and non-invasive test. The purpose of our study was to explore whether the HTGW phenotype could be used as an effective test for subclinical atherosclerosis. METHODS: We screened seven databases (PubMed, Cochrane Library, EMBASE, CNKI, WANFANG, CBM, and Web of Sc

Reduction in circulating endothelial progenitor cells caused by obstructive sleep apnea-related hypoxemia and its association with the severity of coronary artery disease.

BACKGROUND: Obstructive sleep apnea (OSA) is a risk factor for coronary artery disease (CAD), while endothelial progenitor cells (EPCs) are critical for vascular repair. This study investigated the associations among OSA-related hypoxemia, circulating EPC, and CAD severity. METHODS: This prospective study enrolled patients with unstable angina undergoing coronary angiography. All participants underwent overnight polysomnography to determine the apnea-hypopnea index (AHI) and the sleep apnea-specific hypoxic burden (SASHB). Circulating EPCs were quantified using flow cytometry. CAD severity wa

Ceramide-based machine learning models for the diagnosis of unstable angina: a prospective multicenter study.

BACKGROUND: Coronary artery disease is a critical public health issue. While traditional biomarkers like cardiac troponin show high sensitivity, their utility in unstable angina (UA) patients is limited. Recent research has identified novel biomarkers, including ceramides, which may correlate with atherosclerotic plaque stability and acute coronary syndrome (ACS) risk. This study aimed to develop a machine learning-based model to evaluate ceramide's diagnostic efficacy in patients with UA. METHODS: We conducted a prospective study from April 2021 to August 2022 at two medical centers, compari

Stanford Type B Aortic Dissection in an Adult Patient With Unrepaired Coarctation of the Aorta.

Coarctation of the aorta (CoA) is a congenital narrowing typically detected in childhood; survival into adulthood without repair is uncommon. Aortic dissection (AD) is an exceedingly rare complication in this specific context, particularly involving the descending aorta. We describe a case of a 46-year-old Caribbean-Black male with a medical history of chronic hypertension (HTN) who presented with unstable angina and hypertensive crisis. Emergent computed tomography angiography revealed critical proximal descending CoA with poststenotic dilatation and an acute Stanford type B AD. He was initia

Feasibility and Safety of the ETcath200 Robotic PCI System for Complex Coronary Lesions: A Prospective Single-Center Study.

BACKGROUND: International robotic-assisted PCI platforms such as CorPath 200 and CorPath GRX have demonstrated procedural safety and efficacy but are limited by the absence of real-time tactile feedback and cumbersome device exchange. The ETcath200, the first domestically developed robotic PCI system in China, introduces guidewire force-sensing to provide real-time visual resistance feedback that partially compensates for the lack of direct tactile sensation, along with streamlined device exchange and an intuitive joystick-roller interface. AIMS: This study aimed to evaluate the feasibility,

Prognostic Role of the PRECISE-DAPT Score in Acute Coronary Syndrome and Different Antithrombotic Treatment Strategies.

INTRODUCTION: The PRECISE-DAPT score is a useful tool for predicting the risk of bleeding after percutaneous coronary intervention (PCI) requiring dual antiplatelet therapy. We aimed to validate the PRECISE-DAPT score as a mid-term mortality predictor in acute coronary syndrome (ACS). METHODS: All patients with ACS hospitalized between October 2018 and October 2023 were analyzed. Mortality data were acquired in cooperation with the Institute of Health Information and Statistics of the Czech Republic. We used a standard PRECISE-DAPT threshold ≥ 25. A receiver operating characteristic (ROC) cur

Per-Protocol Analysis of Chlorthalidone Versus Hydrochlorothiazide for Cardiovascular Event Prevention-Diuretic Comparison Project.

BACKGROUND: The DCP (Diuretic Comparison Project), a pragmatic trial, evaluated whether chlorthalidone compared with hydrochlorothiazide would reduce the risk of nonfatal cardiovascular disease or noncancer-related death. The intent-to-treat analysis found no difference in such comparison (hazard ratio, 1.04 [95% CI, 0.94-1.16]). The objective of the current study is to estimate the per-protocol effect of chlorthalidone (12.5/25 mg daily) compared with hydrochlorothiazide (25/50 mg daily) in preventing major adverse cardiovascular events among older patients with hypertension. METHODS: The ef

[Development of a novel clinical prediction model for musculoskeletal chest pain in the emergency department].

INTRODUCTION: The diagnostic work-up of patients presenting to the emergency department with chest pain remains a major clinical and health-economic challenge worldwide. Although most cases are caused by benign, non-cardiopulmonary conditions, the primary goal of emergency care is the timeous, but safe exclusion of life-threatening disorders - mainly acute coronary syndrome and pulmonary embolism. Excessive diagnostic testing increases both healthcare costs and emergency department overcrowding. Preliminary observations suggest that previously unrecognized spinal asymmetry or scoliosis is more

The effect of inflammatory factors on unstable angina risk, from the gene level.

Inflammation and its genetic regulation play crucial roles in the development of unstable angina (UA), a key manifestation of coronary artery disease (CAD). However, the genetic determinants that influence cytokine production and interindividual inflammatory responses remain incompletely understood, particularly in the Chinese population. This study aimed to clarify the relationship between inflammatory cytokine gene polymorphisms, serum cytokine expression, and UA risk. A hospital-based case-control study was conducted in 160 patients with UA and 280 age- and sex-matched healthy controls. Par

Prognostic importance of exercise electrocardiography in patients with suspected microvascular disease.

BACKGROUND: Coronary microvascular dysfunction (CMD) is associated with a poor prognosis but is difficult to diagnose non-invasively. In a recent paper, ST-segment depression on exercise-ECG was found to have a very high positive predictive value (PPV) for CMD in a highly selected group of patients. OBJECTIVES: In the present study of chest pain patients, we used myocardial perfusion single photon emission CT (MPS) to rule out ischaemia due to epicardial coronary stenosis. In the remaining patients with suspected CMD, we studied the prognostic value of ex-ECG findings. METHODS: A single-cent
