Gender, diversity, and inclusion in phenotypic and genetic characterization of acute coronary syndromes: rationale and design of the prospective multicentre GEDI-ACS registry.

AIMS: Despite an overall decline in cardiovascular mortality in recent years and advances in diagnosis and treatment, acute coronary syndromes (ACS) remain a leading cause of morbidity and mortality among women worldwide. Sex-specific risk factors and mechanisms remain under-recognized and complicate early diagnosis and management. METHODS AND RESULTS: The GEDI-ACS registry (PNRR-MCNT2-2023-12377431; NCT06441942) is a prospective, multicentre, non-randomized clinical study aiming to identify the phenotypic and genetic profiles of women with ACS. The study is enrolling 100 consecutive women pr

Efficacy and safety of dual and triple antiplatelet therapies in East Asian patients with acute coronary syndrome: a systematic review and network meta-analysis of randomized controlled trials.

This systematic review and network meta-analysis (NMA) evaluated the efficacy and safety of dual antiplatelet therapy (DAPT) and triple antiplatelet therapy (TAPT) in East Asian patients with acute coronary syndrome. Twenty-six randomized controlled trials comparing DAPT (P2Y12 inhibitor with aspirin) or TAPT (cilostazol with clopidogrel-based DAPT) were included from four databases. Primary outcomes were major adverse cardiac events (MACEs) and major bleeding events. Frequentist NMA was performed using RRs. The reference comparator was clopidogrel-based DAPT. TAPT significantly reduced MACEs

The role of systemic inflammatory response index in predicting myocardial infarction in patients with unstable angina.

BACKGROUND: Unstable angina (UA) is a high-risk presentation of acute coronary syndrome (ACS) that can rapidly progress to myocardial infarction (MI) if not identified and managed promptly. Inflammation plays a key role in plaque instability and thrombotic events, making inflammatory biomarkers useful tools for early risk assessment. The Systemic Inflammatory Response Index (SIRI), derived from peripheral blood cell counts, has emerged as a novel marker of systemic inflammation, but its prognostic utility in UA remains underexplored. OBJECTIVE: The purpose of this study was to investigate the

Cocoa flavanol supplementation and prevention of cardiovascular disease: a novel analysis of the COSMOS randomized trial using "win ratio".

Cocoa flavanols may reduce cardiovascular disease (CVD) risk, yet large randomized trials remain inconclusive. The COcoa Supplement and Multivitamin Outcomes Study (COSMOS) suggested a modest, nonsignificant benefit using Cox models, which do not account for event severity in composite outcomes. To address this, we applied generalized pairwise comparison (GPC), or "win ratio" (WR), to assess cocoa flavanols versus placebo on hierarchical CVD outcomes among healthy older US adults. This secondary analysis of COSMOS, a randomized, placebo-controlled, 2 × 2 factorial trial of cocoa extract and mu

Frailty and Recurrent Cardiovascular Events in Patients With Obstructive Sleep Apnoea: The SAVE Study.

BACKGROUND: Frailty is a common syndrome in patients with cardiovascular disease (CVD). Whether frailty modifies the risk of recurrent cardiovascular events in patients with established CVD and obstructive sleep apnoea (OSA) is uncertain. We aimed to determine associations of frailty and the risk of recurrent cardiovascular events in adults with moderate-severe OSA and established CVD. METHODS: Post hoc analyses of the international Sleep Apnea Cardiovascular Endpoints (SAVE) trial where participants from 89 clinical centres in seven countries with moderate-to-severe OSA and established CVD w

Cardiotoxicity risk in metastatic castration-resistant prostate cancer: enzalutamide versus abiraterone.

BACKGROUND: Chemotherapy-naïve patients with metastatic castration-resistant prostate cancer (mCRPC) treated with abiraterone (ABI) have a higher cardiovascular (CV) event-related hospitalization risk than those treated with enzalutamide (ENZA). This study aimed to assess CV event risk in chemotherapy-naïve patients with mCRPC treated with ENZA or ABI and to provide outcome data based on CV disease (CVD) history using the US Medicare database. METHODS: Chemotherapy-naïve patients with mCRPC (≥ 65 years) who initiated ENZA or ABI (September 2014 − May 2017) were included. The primary endpoint w

Sleep timing irregularity in midlife: association with incident major adverse cardiac events and cardiovascular disease mortality over a 10-year follow-up.

BACKGROUND: Sleep timing reflects daily routines and lifestyle patterns, which influence cardiovascular health through circadian mechanisms that regulate cardiovascular processes. Wearable devices enable sensor-based assessment of sleep timing, offering insights into daily behavior. This cohort study examined how the regularity of wearable device–determined sleep timing (bedtime, wake-up time, and sleep midpoint) predicts incident major adverse cardiac event (MACE) and cardiovascular disease (CVD) mortality over a 10-year follow-up in midlife. METHODS: The study included 3,231 participants (39

Intravascular ultrasound-guided tip detection-antegrade dissection re-entry as a bailout technique for coronary artery dissection: a case report.

BACKGROUND: Coronary artery dissection during percutaneous coronary intervention (PCI) may lead to haemodynamic collapse. Intravascular ultrasound (IVUS)-guided tip detection-antegrade dissection re-entry (TD-ADR) may serve as a bailout strategy. We describe a unique case in which IVUS-guided TD-ADR successfully achieved true lumen re-entry after severe coronary artery dissection. CASE SUMMARY: A 64-year-old woman who had received long-term immunosuppressive therapy for rheumatoid arthritis was admitted for unstable angina. Subsequent evaluation revealed severe multivessel disease, and the pa

Development and evaluation of an artificial intelligence-based electrocardiogram prediction model for emergency chest pain patients.

BACKGROUND/OBJECTIVES: Rapid triage and etiological differentiation are critical for patients with acute chest pain in the emergency department. The 12-lead electrocardiogram (ECG), as a non-invasive, readily available, and cost-effective diagnostic modality, provides immediate information and serves as the first-line tool for clinical evaluation. However, ECG interpretation remains highly dependent on clinician expertise and is subject to inter-observer variability. Artificial intelligence (AI)-based analytical methods can deliver automated, consistent, and real-time assessment, thereby poten

Clinical Benefits of Invasive Strategy in Stable Angina Patients with Low Systolic Blood Pressure: A Post Hoc Analysis of the ISCHEMIA Trial.

Background: The ISCHEMIA trial demonstrated no overall prognostic benefit of an initial invasive strategy over optimal medical therapy (OMT) in patients with chronic coronary syndrome (CCS) and moderate-to-severe ischemia. However, managing patients with stable angina and low systolic blood pressure (SBP) remains challenging due to limited tolerance to vasodilatory anti-anginal drugs and the uncertain role of revascularization in improving long-term outcomes for this subgroup. Objectives: This study aimed to estimate the treatment effect of an initial invasive strategy (INV) compared with cons
