ECG trained artificial intelligence for the detection of patients with inducible myocardial ischemia.

AIMS: Myocardial ischaemia is associated with adverse prognosis. Identifying high-risk individuals who require a stress test is challenging, and a practical screening tool to detect these patients, especially in asymptomatic individuals, is lacking. We aimed to develop an artificial intelligence (AI) model based on a resting 12-lead electrocardiogram to detect patients with inducible myocardial ischaemia. METHODS AND RESULTS: An AI model was developed using 12 074 resting 12-lead ECGs from 11 700 patients and tested on 1342 patients at two hospitals. Patients with inducible ischaemia were def

Influencing factors of patient delay for decision-making in older patients with acute coronary syndrome: a study based on the Andersen's Behavioral Model.

OBJECTIVE: This cross-sectional study aimed to identify factors influencing patient delay in older adults with acute coronary syndrome (ACS) using Andersen's Behavioral Model at a tertiary hospital in Zhejiang Province, China. METHODS: A total of 153 older adults diagnosed with ACS who survived to hospital admission were included from January 2023 to June 2023. Logistic regression analysis was used to determine factors independently associated with a longer patient delay following symptom onset. RESULTS: The median delay in seeking medical care was 100.00 min (55.00, 450.00 min). Female sex

Clinical profiles, management, and outcomes of acute coronary syndrome patients in hospitals without reperfusion therapy: a multicentre study in Addis Ababa, Ethiopia.

BACKGROUND: Acute coronary syndrome (ACS) is a leading cause of morbidity and mortality worldwide, disproportionately affecting low- and middle-income countries. Although ACS has been studied in Ethiopia, there is limited evidence on patients managed without reperfusion therapy, particularly across multiple tertiary public hospitals. This study aimed to assess the clinical profiles, in-hospital management, and outcomes of non-reperfused ACS patients in a multicentre Ethiopian setting. METHODS: We conducted a multicentre retrospective study of adult ACS patients admitted to three tertiary publi

Hair Manganese as a Marker of Cardiometabolic Status Rather than Coronary Artery Disease Severity-An Exploratory Pilot Study.

BACKGROUND: Manganese (Mn) is an essential trace element with antioxidant properties; however, excessive exposure may contribute to inflammation and vascular dysfunction. Hair analysis provides an indicator of long-term Mn exposure. This study evaluated the relationship between hair Mn levels, acute coronary syndrome (ACS), coronary artery disease (CAD) severity, and cardiovascular risk factors, with particular emphasis on metabolic status in a cardiometabolic population. METHODS: Hair Mn concentration was measured using inductively coupled plasma optical emission spectrometry (ICP-OES) in 80

Acquired Hemophilia A During Prasugrel Therapy After Recurrent Acute Coronary Syndrome.

BACKGROUND: Acquired hemophilia A (AHA), a rare autoimmune bleeding disorder caused by factor VIII inhibitors, may be overlooked in patients receiving antiplatelet therapies. CASE SUMMARY: A 60-year-old woman underwent percutaneous coronary intervention for non-ST-segment elevation myocardial infarction, receiving aspirin and prasugrel. After recurrent unstable angina, repeat percutaneous coronary intervention was performed, and dual antiplatelet therapy (DAPT) was resumed. One month later, she developed diffuse ecchymoses and progressive anemia. Investigations indicated AHA with isolated act

Aslanger's and de Winter's patterns associated with acute left coronary artery disease: A case report of two patients.

Current guidelines classify acute coronary syndrome (ACS) into unstable angina (UA), ST-segment elevation myocardial infarction (STEMI), and non-ST-segment elevation myocardial infarction (NSTEMI). However, this classification pattern may lead to inadequate recognition of acute coronary occlusion (ACO), particularly when electrocardiographic (ECG) findings are atypical. An increasing number of "STEMI-equivalent" ECG patterns are considered significant features of ACO. The Aslanger's pattern is regarded as indicative of right coronary artery (RCA)-related inferior wall myocardial infarction, wh

Beyond the STEMI paradigm: Prevalence, predictors, and angiographic signatures of Wellens, de Winter, and Aslanger patterns in acute coronary syndromes.

OBJECTIVE: Several high-risk electrocardiographic (ECG) phenotypes in acute coronary syndrome (ACS)-Wellens, de Winter, and Aslanger-signal critical ischemia or occlusion despite absent diagnostic ST-segment elevation myocardial infarction (STEMI) and would benefit from expedited reperfusion. Data on their joint estimates are scarce. The objectives were: (1) to quantify their prevalence; and (2) to compare their baseline characteristics and angiographic vessel-burden. METHODS: In this prospective single centre observational study, 1223 consecutive ACS patients undergoing invasive coronary ang

Relation between albuminuria levels and the incidence of cardiovascular events in adult population in Northern Mexico: A 5-year follow-up survival analysis.

BACKGROUND: Albuminuria has emerged as a key marker for cardiovascular (CV) and renal risk. Despite its clinical relevance, its role as a continuous and stratified predictor of ischemic CV events remains underexplored in Latin American populations, further evidence is needed to assess the prognostic value of urinary albumin-creatinine ratio (UACR) in this context. METHODS: A retrospective cohort of Mexican adults with UACR measured in 2019 and 5-year follow-up was conducted. UACR levels were classified per 2024 CKD Prognosis Consortium and KDIGO guidelines. Cox model regressions were used to

Prognostic Value of the Platelet-to-Lymphocyte Ratio in Patients With Acute Coronary Syndrome and Its Correlation With Angiographic Findings.

Introduction Acute coronary syndrome (ACS) is a major manifestation of coronary heart disease and is associated with significant morbidity and mortality. Inflammatory and thrombotic mechanisms play an important role in its pathogenesis. The platelet-to-lymphocyte ratio (PLR) has emerged as a simple and readily available inflammatory marker that may have prognostic value. The present study aimed to evaluate the association between PLR and major adverse cardiovascular events (MACE) in patients with ACS. Materials and methods This prospective observational study was conducted at a tertiary care c

Limitations of 24-hour MII-pH monitoring to diagnose functional heartburn: a diagnostic reassessment.

Functional heartburn (FH) is typically diagnosed by 24-hour multichannel intraluminal impedance-pH (MII-pH) monitoring; however, relying solely on 24-hour MII-pH may miss gastroesophageal reflux events due to day-to-day variability. In addition, since FH is a diagnosis of exclusion, other underlying conditions may be undiagnosed. This study aimed to assess the clinical course of FH and identify potential alternative diagnoses. Patients with heartburn refractory to proton pump inhibitors or potassium-competitive acid blockers who underwent endoscopy, high-resolution manometry, and MII-pH monito

Rethinking small vessel CTOs: collateral channels as a bridge to subsequent complex intervention-a case report.

BACKGROUND: The retrograde approach has been integrated into global practice for managing challenging chronic total occlusion (CTO). A fundamental limitation of this technique is the requisite presence of navigable collateral channels. Thus, the incomplete exposure of collateral pathways renders complete interventional revascularisation of multi-vessel CTOs a persistent challenge. CASE SUMMARY: A 53-year-old man was admitted for unstable angina. Coronary angiography demonstrated three-vessel CTOs, with a long right coronary artery (RCA) CTO, a proximal left anterior descending artery (LAD) CT

Prognostic Value of Regadenoson Stress Perfusion CMR.

Background/Objectives: Regadenoson is increasingly used as a vasodilator stress agent for perfusion cardiac magnetic resonance (CMR) imaging due to its favorable pharmacologic profile. However, its long-term prognostic value in patients with myocardial ischemia remains insufficiently established. Methods: We retrospectively analyzed all regadenoson stress-CMR studies performed at our institution between May 2017 and July 2020, including patients with follow-up longer than three months. Inducible ischemia and late gadolinium enhancement (LGE) were assessed using standardized criteria. The prima

Validation of the SMART-REACH model after stroke and the effect of colchicine by atherosclerotic cardiovascular disease risk category: a secondary analysis of the CONVINCE randomised clinical trial.

INTRODUCTION: The Colchicine for prevention of vascular inflammation in Non-CardioEmbolic stroke (CONVINCE) trial showed that recurrent events were significantly reduced among colchicine-adherent non-cardioembolic stroke patients in the on-treatment analysis. This study aimed to validate the SMART-REACH risk score in stroke patients, and to determine whether colchicine's efficacy varies by baseline atherosclerotic cardiovascular disease (ASCVD) risk. PATIENTS AND METHODS: Patients with non-severe non-cardioembolic ischaemic stroke/transient ischaemic attack (TIA) were randomised to colchicine

Surgical versus percutaneous revascularization in multivessel non-ST-elevation acute coronary syndrome: a meta-analysis of 77 989 patients.

Multivessel coronary artery disease (MVD) significantly complicates the management of patients with non-ST-elevation acute coronary syndrome, worsening their clinical and angiographic outcomes. However, the optimal revascularization strategy in this subset of patients is still controversial. Our systematic review and meta-analysis aimed to compare coronary artery bypass grafting (CABG) and percutaneous coronary intervention (PCI) in patients with MVD and non-ST-elevation acute coronary syndrome . We performed a comprehensive search of four electronic databases (PubMed, Scopus, WOS, and Cochran

Impact of cardiac rehabilitation on cardiovascular outcomes in patients with coronary artery disease following percutaneous coronary intervention: a meta-analysis.

BACKGROUND: As of 2025, cardiovascular disease (CVD) still remains a major threat to human life. Acute coronary syndrome (ACS) is still considered life-threatening and therefore prompt diagnosis and immediate interventions are mandatory. Nowadays, cardiac rehabilitation (CR) has shown to improve patients' outcomes following cardiac intervention. In this analysis, we aimed to show the impact of CR on cardiovascular outcomes in patients with coronary artery disease following percutaneous coronary intervention (PCI). METHODS: Studies which compared the cardiovascular outcomes in participants who

Life's essential 8 in relation to incident cardiovascular disease and mortality: the prospective study of Isfahan cohort study (2001-2013).

BACKGROUND: There is no evidence regarding the association between Life’s Essential 8 (LE8) and the risk of cardiovascular disease (CVD) and mortality among Iranians. Therefore, we aimed to investigate the relationship between LE8 and incident CVD and mortality in the population. METHODS: In total, 5324 participants free of CVD at baseline were included. Participants were categorized into 3 groups of high (80 ≤ LE8 < 100), moderate (70 ≤ LE8 < 80), and low (LE8 < 70) LE8 score. Multivariable-adjusted Cox models were used to evaluate the association of LE8 with CVD, CVD subtypes (myocardial inf
