The European bifurcation club randomized trial of stepwise provisional stenting vs drug-coated balloon therapy for nonleft main true coronary bifurcations: The EBC DCB trial.

BACKGROUND: Drug-eluting stents (DES) are associated with a 2% annual failure rate and have worse outcomes at coronary bifurcations. Recent randomized control data have shown the noninferiority of drug-coated balloons (DCB) vs stents at 1 year. Possible benefits of DCBs include the lack of a metal prosthesis, absence of stent malapposition, lack of obligate antiplatelet treatment, and maintenance of vessel geometry and function. HYPOTHESIS: The use of DCBs in the treatment of nonleft main bifurcation disease is noninferior to a DES strategy for Target Bifurcation Failure at 1 year. STRATEGY

Formulation-Specific Cardiovascular Outcomes with High-Dose Eicosapentaenoic Acid: A Systematic Review and Meta-analysis.

BACKGROUND/OBJECTIVE: Residual cardiovascular risk persists despite intensive statin therapy in patients with established atherosclerotic cardiovascular disease (CVD). Omega-3 fatty acids, particularly high-dose eicosapentaenoic acid (EPA), have been proposed as adjunctive therapy, yet trial results conflict, likely due to formulation differences. We conducted a formulation-focused meta-analysis to determine whether high-dose EPA-dominant supplementation reduces cardiovascular events and to quantify the impact of mixed EPA/docosahexaenoic acid (DHA) regimens on efficacy. METHODS: Following Pr

Determinants of Long-Term Mortality in Patients with Acute Coronary Syndromes Requiring CICU Admission: Diagnosis Versus Vulnerability in a Contemporary Real-World Cohort.

Background: Contemporary cardiac intensive care units (CICUs) increasingly care for elderly and multimorbid patients, yet the relative contribution of admission diagnosis versus underlying comorbidity burden to long-term prognosis remains unclear. We aimed to identify independent determinants of 5-year mortality following emergent CICU admission, with particular focus on acute coronary syndromes (ACS). Methods: We retrospectively analyzed 1299 consecutive adults admitted emergently to a tertiary CICU between 2017 and 2019. Baseline characteristics, comorbidities, and in-hospital management wer

Prognostic Value of Inflammatory Status in Patients with Acute Coronary Syndromes: A Single-Center Experience.

Background/Objectives: Acute coronary syndromes (ACS) encompass a spectrum of clinical entities from unstable angina to non-ST-segment elevation myocardial infarction (NSTEMI) and ST-segment elevation myocardial infarction (STEMI), all associated with significant morbidity and mortality. Inflammation plays a central role in the pathophysiology of ACS, contributing to atherosclerotic plaque destabilization, myocardial injury, and adverse clinical outcomes. Inflammatory biomarkers, together with N-terminal pro-B-type natriuretic peptide (NT-proBNP), are increasingly used for risk stratification,

A large-scale 12-lead electrocardiogram dataset for acute coronary syndrome prediction containing 19,955 ECGs.

Acute coronary syndrome(ACS) is a common cardiovascular disease and a severe type of coronary heart disease. Electrocardiograms(ECGs) are the initial and indispensable examination for patients suspected of ACS in emergency treatment. Particularly in patients with occlusion myocardial infarction(OMI), prompt medical intervention is imperative. Although ECG-based artificial intelligence(AI) models have achieved high diagnostic accuracy in conditions such as atrial fibrillation and heart failure, their application to ACS prediction has been limited by the scarcity of large, openly available ECG d

TREAT-Netv2: regional wall motion-informed video-tabular fusion for ACS treatment prediction.

PURPOSE: Acute coronary syndrome (ACS) is a major cause of cardiovascular mortality. While coronary angiography enables definitive diagnosis and intervention, its invasiveness and limited availability delay treatment, disproportionately affecting rural and remote communities. Development of noninvasive, predictive tools for early revascularization may improve triage and outcomes. METHODS: We propose TREAT-Netv2, a regional wall motion-informed video-tabular fusion network for ACS treatment prediction that integrates echocardiograms (echo) and electronic medical records. The model extracts reg

Diagnostic Value of D-Dimer in Patients Visiting the Emergency Department with Symptoms of Acute Coronary Syndrome: A Retrospective Observational Study.

BACKGROUND: D-dimer is a fibrin degradation product formed during fibrinolysis and reflects the activation of coagulation and fibrinolytic pathways. The role of D-dimer in acute coronary syndrome (ACS) remains uncertain. This study was designed to analyse differences in D-dimer levels in patients with different forms of ACS and the diagnostic efficiency and possible complementary role of the D-dimer biomarker. METHODS: This study applied a retrospective observational design and included adult patients presenting to the emergency department with chest pain. Differences in D-dimer levels across

Prevalence and predictors of dyspnea in acute coronary syndrome patients receiving ticagrelor: A prospective cohort study in Iran.

The occurrence of dyspnea in patients with acute coronary syndrome (ACS) has always been considered a challenging diagnostic and therapeutic clinical scenario. Ticagrelor, a platelet receptor inhibitor, has beneficial effects in the prevention of ischemic events and mortality, but can cause dyspnea in ACS patients. In this study, we intend to investigate the prevalence of dyspnea in patients with ACS who are under treatment with ticagrelor. This prospective observational cohort study included 200 patients diagnosed with ACS from March 2020 to March 2022 and referred to the Cardiology Departmen

Major Adverse Cardiovascular Events after liver transplantation: A call for a shared definition.

Cardiovascular complications are a leading cause of morbidity and mortality after liver transplantation. Major Adverse Cardiovascular Events (MACE) are increasingly used as composite endpoints in both clinical trials and observational studies; however, definitions are highly variable, limiting comparability. Liver transplant recipients present unique pathophysiological features and perioperative risks that are not adequately captured by standard cardiovascular frameworks. We conducted a systematic review of 49 studies reporting MACE after liver transplantation, including randomized trials, pro

Preemptive cardioprotection with a small molecule in rodents that suppresses genes predictive of heart failure.

Ischemic heart disease is a leading cause of death worldwide. While percutaneous coronary intervention (PCI) restores blood flow in acute coronary syndrome (ACS), reperfusion injury exacerbates myocardial damage, contributing to heart failure (HF). Preemptive administration of a cardioprotective agent could help counter the imminent proinflammatory insult of PCI and reperfusion. Administering BT2, a small-molecule MAPK kinase/extracellular signal-regulated kinase inhibitor, 24 hours before and during ischemia in rats before reperfusion reduced infarct size by ~70% and preserved cardiac functio

Prevalence of malnutrition in patients with acute coronary syndrome undergoing coronary angiography in Thailand: A retrospective observational study.

Malnutrition is an important prognostic factor in patients with acute coronary syndrome (ACS), but it remains under-recognized in routine practice, particularly in Thailand, where local data are limited, and no population-specific nutritional screening tool has been validated. The Prognostic Nutritional Index (PNI) and Nutritional Risk Index (NRI) have been associated with mortality and major adverse cardiovascular events (MACEs) in patients with ACS, but their clinical usefulness in Thai patients remains unclear. This study aimed to determine the prevalence of malnutrition among patients with

Sex disparities in young adults aged 20-40 with acute coronary syndrome.

BACKGROUND: Sex disparities in young adults aged 20-40 with acute coronary syndrome (ACS) are rarely known in China. We aimed to explore these sex-based differences, using a nationwide, population-based database. METHODS: Clinical data of patients with ACS (n = 89,772) registered in the Chinese Cardiovascular Association (CCA) Database-Chest Pain Centre were analyzed, including ST-elevation myocardial infarction (STEMI, 57.7%), non-ST-elevation myocardial infarction (NSTEMI, 20.7%), and unstable angina (UA, 21.5%) across males (94.0%) and females (6.0%). RESULTS: Young males show a significa

Systematic Druggable Genome-Wide Mendelian Randomization Identifies Genetically Supported Therapeutic Targets for Coronary Artery Disease.

OBJECTIVE: Coronary artery disease (CAD) remains a leading cause of mortality worldwide, with substantial unmet therapeutic needs. This study aimed to identify and prioritize genetically supported therapeutic targets for CAD using Mendelian randomization (MR). METHODS: We implemented a two-sample MR framework to infer the causal effects of blood druggable cis-expression quantitative trait loci (cis-eQTLs) on CAD. To consolidate MR findings, we applied Steiger filtering, Bayesian colocalization, and multiple sensitivity analyses. Mediation and phenomewide MR analyses were employed to investiga
