Acetylcholine Provocation Testing for Coronary Vasomotor Disorders.

Vasospastic angina and microvascular angina are increasingly recognized as important causes of myocardial ischemia in patients with angina or myocardial infarction and non-obstructive coronary arteries (ANOCA/MINOCA). Unlike obstructive coronary artery disease, these conditions result from dynamic alterations in epicardial and/or microvascular tone that are not detected by standard coronary angiography. Intracoronary acetylcholine (ACh) provocation testing enables direct assessment of coronary vasomotor function by evaluating both endothelial integrity and smooth muscle reactivity. Moreover, A

Remarkable Response to Etoposide and Cisplatin in Aggressive-Variant Prostate Cancer With Low Prostate-Specific Antigen Levels: A Case Report.

Prostate-specific antigen (PSA) is a cornerstone of screening for prostate cancer (PC); however, serum PSA levels may remain deceptively low in certain high-grade, aggressive subtypes. Aggressive-variant prostate cancer (AVPC) is a clinical entity characterized by rapid progression, bulky lymphadenopathy, and visceral metastases despite low or non-correlative PSA levels, often requiring treatment approaches beyond conventional therapies. Here, we report the case of a 69-year-old Japanese man with multiple comorbidities, including diabetes, hyperlipidemia, hypertension, angina pectoris, and low

Implantable cardioverter-defibrillator utilization in coronary artery spasm: A case series of cardiac arrest survivors and a patient with typical angiography.

BACKGROUND: Coronary artery spasm (CAS) can trigger severe angina pectoris, acute coronary syndrome, or sudden cardiac arrest (SCA). Calcium channel blockers and nitrates are recommended treatments, but the role of implantable cardioverter-defibrillators (ICDs) remains controversial. This study explored ICD application in CAS via a case series of cardiac arrest survivors and a patient with typical angiography findings. CASE PRESENTATION: Four patients with recurrent angina (3 with prior cardiac arrest) were diagnosed with suspected or definite CAS based on predefined criteria and coronary ang

Case Report: Fabry disease mimicking coronary artery disease and hypertrophic cardiomyopathy-a 15-year diagnostic delay.

BACKGROUND: Fabry disease is a rare X-linked hereditary lysosomal storage disorder. Its cardiac manifestations often overlap with those of hypertrophic cardiomyopathy or coronary artery disease, leading to significant delays in diagnosis. CASE SUMMARY: A 60-year-old male patient presented with exertional angina pectoris, which had persisted for 15 years. Initially diagnosed with coronary artery disease, he underwent percutaneous coronary intervention. Despite successful revascularization, he subsequently developed progressive left ventricular hypertrophy, heart failure, bilateral hearing loss

Deep learning-based multi-contrast windowing for task-specific medical image visualization.

The presence of excess leucocytes in the semen has been associated with male infertility. According to the WHO, concentrations of more than 106 leucocytes ml-1 are considered as leucocytospermia, indicating genital tract infections. Up to now, no consensus has been achieved on how leucocytes should be quantified in semen. Using the peroxidase staining and monoclonal antibodies to CD15, CD45 and CD68, we found significant differences between the detection methods. Only 47.4% of the semen samples that were assessed as leucocytospermic by CD45 were identified as such by peroxidase staining. The c

Alloxan attenuates glucosamine-induced NF-κB activation and proinflammatory gene expression under normoglycemic conditions.

Glucosamine (GlcN), a critical substrate in the hexosamine biosynthetic pathway, is known to modulate inflammatory responses in macrophages depending on extracellular glucose concentration. In hyperglycemic conditions (25 mM glucose), GlcN suppresses the production of nitric oxide (NO) and decreases the expression of inducible nitric oxide synthase (iNOS). Conversely, under normoglycemic conditions (5 mM glucose), GlcN paradoxically enhances lipopolysaccharide (LPS)-induced iNOS expression, NO production, and the upregulation of additional proinflammatory mediators. In this study, we examined

Pooled screening for endogenous HHV-6 in subjects with coronary artery disease.

Endogenous human herpesvirus 6 (eHHV-6) is integrated into the genomes of approximately 1% of individuals and has been linked to angina pectoris which, like myocardial infarction (MI), generally denotes coronary artery disease. To investigate this association further, we screened 2976 genomic DNA samples from patients with MI for eHHV-6 using multiplex qPCR. We identified 17 eHHV-6-positive individuals (0.57%), including 6 with eHHV-6A, 10 with eHHV-6B, and 1 with the solo-DR form, an eHHV-6B variant characterized by the presence of a single direct repeat (DR) within the host genome. Recurrent polymorphic ventricular tachycardia without chest pain: an unusual presentation of focal coronary artery spasm.

Coronary artery spasm (CAS), or Prinzmetal angina, is a recognised cause of myocardial ischaemia in non-obstructed coronary arteries which typically presents with anginal chest pain. This case report describes an atypical presentation of CAS in a 68-year-old white British male with cardiovascular risk factors. The patient presented with recurrent palpitations and pre-syncope, with no chest pain. Ambulatory electrocardiography (ECG) monitoring revealed recurrent polymorphic ventricular tachycardia (PMVT). Coronary angiography identified moderate stenosis of the right coronary artery (RCA), with

Prevalence of Patent Foramen Ovale in Patients with Non-Obstructive Coronary Artery Disease (PROVA) Study.

(1) Background: Prevalence of patent foramen ovale (PFO) in the general population is estimated at around 24%. We hypothesized that right-to-left shunting (RLS) resulting from PFO might contribute to angina symptoms in patients with coronary artery spasm (CAS), potentially triggered by vasoactive metabolites. Therefore, the aim of this study was to investigate the prevalence of PFO-related RLS in patients with documented CAS. (2) Methods: This single-center prospective cohort study included patients with documented CAS undergoing transthoracic echocardiography (TTE), including a contrast bubbl

Association Between Vasomotor Response to Ergonovine Provocation Test and Non-Severe Coronary Artery Stenosis.

BACKGROUND: Coronary artery spasm (CAS) causes vasospastic angina in patients without significant coronary artery stenosis. However, there has been limited investigation regarding the CAS patterns within this population. This study aims to evaluate the incidence and patterns of CAS in patients with non-severe coronary artery stenosis using intracoronary ergonovine (ERG) provocation test. METHODS: A total of 884 patients experiencing angina with non-severe coronary artery stenosis who underwent intracoronary ERG provocation test between January 2018 and December 2022 were enrolled. Young-Onset Type 2 Diabetes and Its Association with Variant Angina in an Adolescent.

BACKGROUND Variable angina pectoris is a special type of unstable angina pectoris, the pathological basis of which is coronary artery spasm, resulting in rapid myocardial ischemia and hypoxia. This spasm may be related to autonomic nervous dysfunction, coronary endothelial cell dysfunction, inflammation, genetics, and other factors. Here, we focus on the association between autonomic dysfunction and variable angina in young-onset type 2 diabetes and differentiation of its variant, angina pectoris, from early acute myocardial infarction. CASE REPORT We present the case of an adolescent patient

Variant angina associated with a CGRP receptor antagonist: a case report.

BACKGROUND: Calcitonin gene-related peptide (CGRP) plays a pivotal role in migraine pathophysiology, and CGRP receptor antagonists are increasingly used for acute and preventive treatment. While considered to have minimal cardiovascular risk, real-world safety data remain limited, particularly in patients with cardiovascular comorbidities. We report a rare case of variant angina associated with atogepant, a novel oral CGRP receptor antagonist. CASE PRESENTATION: A 57-year-old woman presented with new-onset, paroxysmal chest pain over three days, described as a squeezing sensation with diaphor

Concurrent Extracerebral Vasoconstriction in Patients with Reversible Cerebral Vasoconstriction Syndrome: A Cross-Sectional Study.

Background: Reversible cerebral vasoconstriction syndrome (RCVS) is an uncommon and often underrecognized neurovascular disorder. We aimed to investigate the clinical presentations associated with extracerebral vasoconstriction in patients diagnosed with RCVS. Methods: In this cross-sectional study, we analyzed data from a single-center cohort of patients with RCVS in Korea. Extracerebral vasoconstriction in individuals diagnosed with RCVS was defined by the following criteria: (1) the presence of sudden and severe pain in extracerebral regions (primarily the chest or abdomen) coinciding with

Geographical distribution of ALDH2 rs671 polymorphism in Chinese angina pectoris patients.

BACKGROUND: Mitochondrial acetaldehyde dehydrogenase 2 (ALDH2) plays a critical role in the metabolism of ethanol and nitroglycerin. Mutations in ALDH2 reduce enzymatic activity, impairing nitroglycerin metabolism. ALDH2 genetic polymorphisms exhibit significant regional variations across China. This study investigates the distribution of ALDH2 allelic and genotypic frequencies in patients with angina pectoris across northern (Luoyang, Henan), eastern (Jiaxing, Zhejiang), and southern (Guilin, Guangxi) China, and explores the relationship between ALDH2 genotypes and selected clinical character

Cardioneuroablation for coronary artery vasospasm: a case report.

BACKGROUND: Coronary artery spasm (CAS) occurring at night may be caused by excessive vagal tone. In patients with CAS related to high vagal tone, cardioneuroablation (CNA) may offer a promising alternative for the radical treatment of medication-refractory CAS. CASE SUMMARY: We report a case of a 66-year-old male with frequent and uncontrolled coronary artery spasms, usually occurring during sleep at night. Despite treatment with multiple antispasmodic medications including calcium channel blockers and long-acting nitrates, the patient still experienced severe symptomatic chest pain with ST-

Unraveling the causal link between lipidomes and unstable angina pectoris: A Mendelian randomization analysis.

Unstable angina pectoris remains a significant global health burden. Understanding the causal relationships between lipidomes and unstable angina pectoris is crucial for preventive strategies. We conducted a Mendelian randomization (MR) analysis to explore these relationships. This study utilized lipidome data from 377,277 participants and included 14,281 unstable angina pectoris cases and 364,801 controls. We applied inverse-variance weighted (IVW) methods for MR analysis, considering 179 lipid species as exposures and unstable angina pectoris as the outcome. This MR analysis investigated the

Campfire Smoke as a Plausible Trigger for Vasospastic Angina: Insights From a Case Report.

BACKGROUND Vasospastic angina (Prinzmetal angina) has been linked to many risk factors, including tobacco smoking. Although numerous studies have reported associations between air pollutants and cardiovascular outcomes, the relationship with vasospastic angina remains poorly documented. We report the case of a 45-year-old man with normal coronary arteries who developed episodes of chest pain due to vasospastic angina following inhalation of campfire smoke. CASE REPORT A 45-year-old man with a history of eczema, dyslipidemia, Gramineae allergy, and past tobacco use participated in a 3-day campi
