Long-Term Outcomes of Patients With Suspected Angina Pectoris Following Clinical Decision-Making Guided by ATP-FFR Versus Nicorandil-FFR.

BACKGROUND: Fractional flow reserve (FFR)-guided revascularization improves long-term outcomes in patients with coronary artery disease. Although adenosine triphosphate (ATP) is the standard hyperemic agent, nicorandil (NIC) has emerged as a practical alternative due to its combined nitrate-like and ATP-sensitive potassium channel-activating properties. However, data on long-term vessel-related outcomes guided by NIC-induced FFR are limited. AIMS: To compare long-term clinical and vessel-based outcomes in patients with suspected angina pectoris whose treatment strategies were guided by ATP-in

Prognostic value of soluble ST2 in cardiac magnetic resonance defined myocardial infarction with non-obstructive coronary arteries.

BACKGROUND: Cardiac magnetic resonance (CMR) plays an essential role in identifying the underlying etiology of myocardial infarction with non-obstructive coronary arteries (MINOCA). However, risk stratification in patients with CMR-confirmed MINOCA remains insufficiently defined. Soluble suppression of tumorigenicity 2 (sST2), a biomarker related to myocardial fibrosis and adverse cardiac remodeling, may provide additional prognostic information in this population. This study aimed to evaluate the prognostic value of sST2 in patients with CMR-confirmed MINOCA. METHODS: This single-center retr

Effects of cardiac rehabilitation on the severity of angina, health-related quality of life, and exercise capacity among adults living with microvascular angina: a systematic review and meta-analysis.

AIMS: To evaluate the effect of exercise-based cardiac rehabilitation (CR) on the severity of angina, health-related quality of life (HRQoL), and exercise capacity in adults living with microvascular angina (MVA). METHODS AND RESULTS: Fourteen online databases were searched to identify randomized controlled trials (RCTs) comparing adults with MVA receiving CR to those receiving a control intervention involving no exercise. Meta-analyses using random-effects models were used to calculate mean differences or standardized mean differences (SMD). Of 15 873 reports identified, 5 studies (222 parti

The significance of ST-segment elevation in the perioperative period of conduction system pacemaker implantation.

BACKGROUND: Conduction system pacing (CSP) is becoming the standard of care in cardiac electrotherapy. To achieve the best physiological effect, the pacing lead should penetrate deeply into the myocardium of the interventricular septum (IVS). While the beneficial effect of CSP on left ventricular function has been proven, the safety profile and late complications, including IVS perforation, local myocardial damage, and ST-segment changes, continue to be assessed. AIMS: The aim of this study was to assess the prevalence, clinical significance, and potential mechanisms of ST-segment elevation (

Coronary Revascularization of Ischemic Territories Improves Myocardial Perfusion, Increases Submaximal Aerobic Capacity and Reduces Exercise-induced Angina in Patients With Chronic Coronary Syndrome.

Chronic coronary syndrome (CCS) is associated with reduced stress perfusion, a predictor of adverse cardiovascular outcome. To what extent myocardial perfusion is related to cardiorespiratory fitness is not completely known. The aim was to investigate if change in myocardial perfusion correlates to change in aerobic capacity after revascularization. Sixty-two patients with suspected or established CCS underwent cardiopulmonary exercise testing (CPET) and [13N]NH3 positron emission tomography (PET) before and after elective invasive coronary angiography. Thirty-six patients were revascularized

Diagnostic performance of exercise stress testing findings and coronary microvascular dysfunction in patients with angina with non-obstructive coronary artery disease.

BACKGROUND: Coronary microvascular dysfunction (CMD) is common among patients with angina with non-obstructive coronary artery disease (ANOCA) and leads to poorer clinical outcomes. Exercise stress testing (EST) was shown to have a high specificity for detecting CMD. However, the relationship between diagnosing CMD using different invasive physiological parameters and thresholds and the association between EST findings and the endotype of CMD remains unknown. METHODS: This multicentre, prospective cohort study enrolled 117 patients with ANOCA who underwent EST prior to invasive coronary angio

Stable angina in young women.

Ischaemic heart disease (IHD) is the primary cause of cardiovascular death worldwide. Worryingly, the prevalence of IHD is increasing in younger women, with a 3% increase between 1990 and 2019. Although global IHD death rates have decreased in younger women overall, IHD mortality in younger women is increasing in certain high-income countries. Angina is the primary presenting symptom of suspected IHD and coronary artery disease. The presence of angina in patients with stable coronary artery disease is associated with an increased risk of major cardiovascular events compared with those without

Angina after percutaneous coronary interventions.

Persistence or recurrence of angina after percutaneous coronary intervention (PCI) has gained recognition as an unfortunately common condition, which defeats the most frequent purpose of performing PCI in patients with stable coronary artery disease, the relief or reduction of anginal symptoms. Many aspects of this problem remain unknown and underexplored, with clinical practice guidelines providing limited information on guidance for its causative diagnosis, prevention and management. This review article aims to provide a contemporary perspective on the problem of post-PCI angina, leveraging

Hemodynamic Phenotyping in ME/CFS and ANOCA: Complementary Insights from Coronary Function Testing and Invasive Cardiopulmonary Exercise Testing.

This single-center cohort study sought to evaluate the association between Myalgic Encephalomyelitis/Chronic Fatigue Syndrome (ME/CFS) and Angina with Non-Obstructive Coronary Arteries (ANOCA), by determining the observed frequency of ME/CFS among patients with ANOCA diagnosed by coronary function testing (CFT). Additionally, we performed an exploratory analysis of the diagnostic utility of invasive cardiopulmonary exercise testing (iCPET) in ME/CFS patients with CFT-confirmed ANOCA, with the aim of identifying the predominant physiological mechanism responsible for exertional limitation in th

Stratified treatment of myocardial infarction with non-obstructive coronary arteries: the PROMISE trial.

BACKGROUND AND AIMS: Myocardial infarction with non-obstructive coronary arteries (MINOCA) is associated with a significant risk of mortality, rehospitalization, and angina burden. Despite its clinical impact, no randomized clinical trials have hitherto evaluated optimal management strategy for MINOCA. The PROMISE trial was designed to assess whether a stratified treatment improves clinical outcomes in patients with MINOCA as compared to standard care. METHODS: PROMISE is a multicentre randomized trial. Patients with MINOCA were randomized 1:1 to either a stratified treatment based on a compr

Role of routine surveillance stress testing in patients with or without imaging-guided or physiology-guided PCI.

OBJECTIVE: The optimal follow-up strategy for high-risk patients who underwent imaging-guided or physiology-guided percutaneous coronary intervention (PCI) remains uncertain. We investigated whether routine surveillance stress testing after PCI provides clinical benefit when the procedure is guided by intravascular ultrasonography (IVUS) or fractional flow reserve (FFR). METHODS: In the Pragmatic Trial Comparing Symptom-Oriented vs Routine Stress Testing in High-Risk Patients Undergoing PCI randomised trial, 1706 high-risk patients who underwent PCI were assigned to either routine functional

Allergic vasospastic angina: a systematic review.

BACKGROUND: Allergic vasospastic angina (also referred to as Kounis syndrome) is an under-recognised subtype of vasospastic angina in which coronary artery spasm is precipitated by an allergen. This study evaluates the clinical characteristics, mechanisms and management of this intriguing angina subtype. METHODS: A systematic review of reported cases of allergic vasospastic angina from 1992 to 2022 was performed using Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. Each case report was then analysed for patient characteristics, presenting signs/symptoms and both

Endotypes of angina with non-obstructive coronary arteries: a prospective multicentre study.

BACKGROUND AND AIMS: Angina with non-obstructive coronary arteries (ANOCA) is a prevalent myocardial ischaemic syndrome, and women are disproportionately affected. Mechanisms of ischaemia are challenging to diagnose and treatment is empirical. METHODS: Consecutive patients with angina (or equivalent symptoms), no angiographically severe stenosis and fractional flow reserve > 0.80 undergoing coronary functional testing were prospectively enrolled in nine centres in Europe and North America. Haemodynamic endotypes were assessed measuring coronary flow reserve and resistance using an intracorona

Long-term prognostic impact of complete revascularisation defined by CT-derived fractional flow reserve.

BACKGROUND: Complete revascularisation has been associated with improved short-term outcomes in patients with coronary artery disease, but whether these benefits persist long-term and can be defined non-invasively remains uncertain. We investigated the long-term prognostic impact of complete versus incomplete revascularisation determined by coronary CT angiography-derived fractional flow reserve (FFRCT). METHODS: In this prospective multicentre study, 900 patients with new-onset stable angina and at least one coronary stenosis of 30% or greater on coronary CT angiography were followed for a m

Simultaneous bright- and black-blood three-dimensional whole-heart magnetic resonance imaging for integrated coronary plaque detection and luminal stenosis assessment: A prospective comparison with coronary computed tomography angiography.

BACKGROUND: Coronary computed tomographic angiography (CCTA) is a first-line test for anatomical evaluation of the coronary arteries in stable chest pain. Despite technical advances, CCTA requires breath hold and exposes patients to ionizing radiation and contrast agents. Coronary cardiovascular magnetic resonance angiography (CCMRA) has been limited by long and unpredictable scan times, lower spatial resolution, and restricted plaque characterization. We developed a novel cardiovascular magnetic resonance sequence, Bright-blood and black-blood phase SensiTive (BOOST) sequence, which produces
