Clinical features in patients with recurrent myocardial infarction.

A retrospective investigation of 420 patients who had survived the acute phase of myocardial infarction revealed 63 reinfarctions (in 57 patients) within three months. Twenty-eight patients died without reinfarction during the same period, and 335 survived three months without reinfarction. The reinfarction patients were significantly more often women, had more frequently a history of previous myocardial infarction and hypertension, and their myocardial infarctions were more often non-transmural and localized to the anterior wall of the heart. The ECG of each patient that was registered neares

Regional cardiac dilatation after acute myocardial infarction: recognition by two-dimensional echocardiography.

To assess the early topographic changes after acute transmural myocardial infarction, we studied 28 patients during the first two weeks after infarction by serial two-dimensional echocardiography. Regional end-diastolic segment lengths and wall thicknesses for anterior and posterior left ventricular walls were calculated. Eight patients showed infarct expansion, with disproportionate dilatation and transmural thinning in the infarcted zone, that was significantly different (P less than 0.005) from changes in non-infarcted regions. This regional expansion led to an overall left ventricular dila

Studies on the relationship between ST-segment elevations and extent of infarction following coronary artery occlusion in dogs.

Epicardial ST-segment alterations were determined 15 min. following acute ligation of the left anterior descending coronary artery in dogs, and the extent of the left ventricular wall infarcted at each mapping site was determined 24 hours later. A close correlation was observed between the ST-segment elevation and the percent of left ventricular wall infarcted (r = .891; P less than .001) at the same mapping site. A significant portion of the left ventricular wall remained viable at sites with less than 8 mV elevation; however, the chance of transmural infarction became high at sites with elev

[Prodromal symptoms of acute myocardial infarction (author's transl)].

75 out of 100 patients who had survived myocardial infarction, had had prodromal symptoms. 62 patients reported specific prodromi such as first manifestations of angina pectoris or increase in duration and intensity of angina pectoris which had existed for some time previously, and in some cases had been accompanied by general complaints. A further 13 patients had only general symptoms such as onset of dyspnoea, palpitations, or a drop in performance during the 3 months prior to the infarct. The primary manifestation of symptoms consisted of exercise-induced angina pectoris at the beginning of

Regional left ventricular function assessed by contrast angiography in acute myocardial infarction.

The relationship of segmental left ventricular (LV) wall motion abnormalities to LV function 2-6 days after acute transmural myocardial infarction (MI) was investigated in 45 patients by quantitative contrast ventriculography. Patients were divided into four classes according to the MIRU criteria. Segmental wall motion was assessed by determining the percentage of systolic shortening (deltaS) along nine hemiaxes and the extent of akinetic or dyskinetic abnormally contracting segments (% ACS) expressed as a percentage of end-diastolic perimeter. When compared with that in 17 normal control-subj

Myocardial infarction with patent coronary arteries.

A 62-year-old patient suffered an acute anterior wall infarction as documented by clinical, electrocardiographic and enzymatic evidence. His ECG returned to a near normal pattern on the 5th day. The next day the patient died suddenly. Post mortem examination revealed a transmural anterior infarction with a mural thrombus, and a fresh thrombus in the left renal artery, but the coronary arteries were patient with only slight arteriosclerotic changes. We assume that a coronary thrombus might have been the cause of the infarction, with subsequent lysis. Such early lysis (6 days after the infarctio

The noninvasive diagnosis of right ventricular infarction.

We evaluated scintigraphy and echocardiography for the diagnosis of right ventricular (RV) infarction. Of 26 patients with acute transmural myocardial infarction (MI), six with inferior MI had abnormal radionuclide uptake localized to the RV free wall on infarct scintigraphy or segmental akinesis of the RV free wall on gated radioangiography or both. These six patients with RV involvement (group I) were compared with the remaining nine with inferior MI (group II) and 11 with anterior MI (group III). RV/LV area ratios determined radioangiographically were significantly greater in group I than g

Sequential radionuclide assessment of left and right ventricular performance after acute transmural myocardial infarction.

Ventricular performance was evaluated sequentially in 31 patients with uncomplicated acute transmural myocardial infarction (13 anterior and 18 inferior). Left ventricular ejection fraction, ejection rate, regional wall motion, and right ventricular ejection fraction were ascertained using first-pass radionuclide angiocardiography on four occasions during hospitalization. Inferior infarction resulted in a greater reduction in right ventricular ejection fraction than anterior infarction (mean +/- SEM; 48 +/- 2 versus 56 +/- 2%, P less than 0.01). In contrast, in anterior infarction there was gr

[Comparison of the changes in the QRS complex of the ECG with data from a pathologicoanatomic study in acute myocardial infarct].

A comparison of electrocardiographic and anatomic data was made in 74 patients with acute myocardial infarction with the purpose of investigating QRS-complex changes in macrofocal myocardial infarction, and establishing the correlation between ECG data and the volume of the infarcted myocardium. It has been shown that subendocardial and transmural necrosis of the anterior wall of the left ventricle was in all cases reflected on ECG by QS, while transmural necrosis of the posterior wall was associated in 86% of the cases with the presence of QR, Qr or qR on ECG. Thus, the estimation of myocardi

Acute myocardial infarction with normal and near normal coronary arteries. Documentation with coronary arteriography within 12 1/2 hours of the onset of symptoms in two cases (three episodes).

Three instances (in two patients) of acute myocardial infarction associated with arteriographically normal or near normal coronary arteries are reported. One patient with a lateral infarction had a normal coronary arteriogram and hypokinesia of the lateral wall. Another patient had two infarctions: (1) a transmural inferior-lateral infarction associated with occlusion of the most distal segment of the posterior descending branch of the right coronary artery, and (2) a transmural anterior-lateral-superior infarction associated with occlusion of the most distal segment of the left anterior desce

Coronary hemodynamics during reperfusion following acute coronary ligation in dogs.

The coronary hemodynamic effects of re-establishing blood flow to ischemic myocardium and the regional distribution of myocardial flow during reperfusion were studied in anesthetized open-chest dogs. A large portion of the left ventricular wall was rendered ischemic by occlusion of the left anterior descending coronary artery for 2 hours. During reperfusion of the LAD, coronary resistance in the reperfused vasculature increased progressively for the first 3 hours, while resistance in the intact LC vasculature was unchanged. Minimal resistances in the reperfused vascular bed, calculated from me

Acute myocardial infarction in a young female with familial hyperbetalipoproteinemia.

A young woman, 21 years of age, suffered an acute transmural inferior wall myocardial infarction. EKG changes and serum enzymes accompanied a typical history. Coronary angiography perfomed 2 months after the acute episode showed complete occlusion of the right coronary artery and some mild disease of the circumflex and left anterior descending branches. Family history of type II hyperlipoproteinemia was present and she was known to have cholesterol since she was 12 years of age.

Quantitative relationships between potassium-43 imaging and left ventricular cineangiography following myocardial infarction in man.

To evaluate the quantitative relationships between resting potassium-43 (43K) myocardial imaging and left ventricular segmental contraction abnormalities, 15 patients were studied by both radionuclide and contrast angiographic techniques at least two months following transmural myocardial infarction. The ECG location of infarction involved the anterior wall alone in six patients, inferior wall alone in three patients, both anterior and inferior walls in five patients, and in one patient ECG-anatomic correlation was obscured by newly developed left bundle branch block. 43K defects were noted in
