Thirty two patients presenting with acute transmural inferior wall myocardial infarction underwent cardiac catheterisation and angiography within 12 hours of the onset symptoms. Twelve lead electrocardiograms performed within one hour of catheterisation showed ST segment depression in the anterior precordial leads in addition to inferior wall changes in 17 patients and no ST segment changes in the anterior leads in 15. When the clinical, arteriographic, and ventriculographic variables were compared between the two groups no significant differences were noted with regard to age, sex, risk facto

Electrocardiographic ST-segment depression in the anterior precordial leads is a frequent observation during the initial hospital phase of acute transmural inferior myocardial infarction (MI), but is of uncertain significance. No available clinical studies have examined the prevalence of inferoseptal necrosis complicating inferior MI. Therefore, the clinical course, electrocardiographic features, radionuclide angiograms and cardiac enzyme changes in 57 patients with transmural inferior MI who did not have prior anterior or concomitant "true posterior" MI, associated anterior or posterolateral

We used two-dimensional echocardiography to assess asynergy in the right (RV) and left (LV) ventricles in 34 selected patients with electrocardiographic evidence of acute transmural inferior myocardial infarction (IMI) 40 +/- 3 (SE) h from the onset of pain. We measured the extent of asynergy, defined as akinesis and/or dyskinesis, for RV and LV in 4 short axis sections and computed asynergy for both ventricles. The incidence of asynergy was 100% in the inferior LV, 76% in the inferior ventricular septum and 76% in the inferior RV. We found a direct correlation between peak creatine phosphokin

Coronary arteriographic findings in patients with acute transmural inferior infarction were studied from 57 patients (51 men and 6 women). Their ages ranged from 28 to 72 years with a mean of 50 years. Twenty-six patients (Group A) had minimal (less than 0.1 mV) or no precordial ST-segment depression. Thirty-one patients (Group B) had precordial ST-segment depression of 0.1 mV or more. The two groups showed consistent differences in frequency of the left anterior descending artery (LAD) stenosis, multivessel disease, mean peak plasma creatinine phosphokinase (CPK, IU/L), and mean ejection frac

A number of studies have suggested that coronary spasm may play a role in the genesis of myocardial infarction. We carried out a provocation test with 0,4 mg intravenous methylergometrine in 131 patients undergoing routine coronary angiography at least 6 weeks after transmural myocardial infarction. The 124 men and 7 women of equal mean age of 49,6 years underwent the investigation on average 27 +/- 9 days after the date of onset of myocardial infarction. Seventy four patients had anterior infarcts (antero septal, anterior, apical), 55 had inferior infarcts (inferior, diaphragmatic and true po

The clinical experience with 37 patients with acute transmural inferior wall myocardial infarction who were assessed for evidence of right ventricular involvement is reported. On the basis of currently accepted hemodynamic criteria, 29 patients (78%) had evidence suggestive of right ventricular infarction. However, only 5 (20%) of 25 patients demonstrated right ventricular uptake of technetium pyrophosphate on scintigraphy. Two-dimensional echocardiography or isotope nuclear angiography, or both, were performed in 32 patients; 20 studies (62%) showed evidence of right ventricular wall motion d

To evaluate the incidence, time course and significance of reciprocal change, 25 consecutive patients admitted with their first acute transmural myocardial infarction were studied with serial electrocardiography and two-dimensional echocardiography. Reciprocal change was noted in all patients with inferior infarction (mean maximal ST segment depression 3.53 +/- 1.97 mm) and 70% of patients with anterior infarction (mean maximal ST depression 1.45 +/- 0.8 mm, p = 0.001). When initially present, reciprocal change had resolved within 24 hours in 59% of patients. The sum of reciprocal ST depressio

Right ventricular involvement has been shown to be common in the acute phase of infero-posterior myocardial infarction. The aim of this prospective study was to assess the diagnostic and prognostic value of the different criteria obtained by clinical and paraclinical methods of investigation. Forty patients (35 men, 5 women: mean age 57,1 years) admitted consecutively with this type of transmural infarct without any other cause of acute or chronic volumic or barometric overload of the RV were investigated. In addition to clinical data, the following paraclinical investigations were carried out

Fifty-six patients with acute transmural anterior wall myocardial infarction (MI) were investigated with a 24-electrode grid and 34 patients with an acute transmural inferior wall MI were investigated with standard ECG leads II, III, and aVF in order to study the length of time after the onset of pain during which the development of Q waves and reduction of R waves progress. These ECG changes continued for 18-26 h after onset of pain but the majority appeared during the first 12 h.

Left ventricular ejection fraction and regional wall motion were assessed by multigated equilibrium radionuclide ventriculography within 24 hours of onset of first acute transmural myocardial infarction (MI) in 32 patients. Abnormal left ventricular wall motion was noted in all 16 patients with anterior infarction and in 14 of 16 (87.5%) patients with inferior infarction. Regional wall motion abnormalities frequently included areas adjacent to and remote from those predicted by the ECG location of ST elevation and pathologic Q waves. Such remote wall motion abnormalities were associated with r

In a series of 75 patients with transmural acute myocardial infarction, a right to left ventricular filling pressure ratio equal to or greater than 0.65 was assumed to be indicative of associated right ventricular infarction. Eleven (24%) out of 45 patients with infero-posterior myocardial infarction had such hemodynamic evidence of right ventricular infarction (Group A). The remaining 34 patients with infero-posterior myocardial infarction (Group B) and the 30 patients with anterior myocardial infarction did not. To evaluate the incidence of left-ventricular thrombosis (LVT) in recent myocardial infarction (MI), M-mode and two-dimensional echocardiography (2DE) were performed in 200 patients enrolled in a rehabilitation program, three to 12 weeks after the acute episode (average 6 weeks). LVT was detected in 36 of 125 patients with anterior MI and in 4 of 75 patients with inferior MI. The presence of LVT correlated well with the electrocardiographic extension of myocardial necrosis; 90% of LVT was detected in patients with extensive MI. The same behaviour seems to be found in MI without evidence of ECG

To assess various factors associated with anterior S-T segment depression during acute inferior myocardial infarction, 47 consecutive patients with electrocardiographic evidence of a first transmural inferior infarction were studied prospectively with radionuclide ventriculography an average of 7.3 hours (range 2.9 to 15.3) after the onset of symptoms. Thirty-nine patients (Group I) had anterior S-T depression in the initial electrocardiogram and 8 (Group II) did not have such "reciprocal" changes. There was no difference between the two groups in left ventricular end-diastolic or end-diastoli

The purpose of this study was to define the sequential changes in left and right ventricular ejection fraction (LVEF, RVEF) and regional LV wall motion following first transmural acute myocardial infarction (AMI). Fifty-four patients with either anterior (n = 28) or inferior (n = 26) infarction underwent radionuclide ventriculography (RNV) within 48 hours of onset of chest pain (study 1), between days 3 and 6 (study 2), and again between days 7 and 25 (study 3). Twenty-six of the patients with anterior MI (93%) had initial LVEF less than 0.54, compared with 13 of 26 patients (50%) with inferio

Clinical and necropsy observations are described in 61 patients with a healed transmural myocardial infarction, 33 with and 28 without a clinical history of acute myocardial infarction. There were no significant differences between the 2 groups of patients in mean age, sex, or frequency of angina pectoris, chronic congestive heart failure, systemic hypertension, sudden coronary death, or fatal acute myocardial infarction. Compared with the patients with clinically recognized acute myocardial infarction, the patients with clinically unrecognized (silent) infarction had a significantly (p less t

To search for electrocardiographic clues of infarction of the right ventricle, we recorded the right-thoracic unipolar leads from V3R to V6R and the following abdominal leads: MD, ME and MI, in a group of 40 normals (control group) and in a group of 40 patients with their first acute transmural myocardial infarction (25 posterior-inferior, 15 anterior wall). We described the predominant ECG morphologies in the control waves (more than 0.04 sec. duration) could be registered in the right anterior thoracic wall in healthy subjects. Based on the findings from our control group and those reported

Radionuclide imaging was useful for noninvasively visualizing a location and size of infarction in two patients with acute myocardial infarction. In one patient, inferior subendocardial myocardial infarction was detected by 99mTc-PYP myocardial scintigraphy and then inferior transmural infarction was observed as more intense radioactivity with a larger size. In the other patient with acute extensive anterior myocardial infarction, akinesis of the left ventricular wall and left ventricular aneurysm were confirmed by first-pass radionuclide angiocardiography and multigated equilibrium cardiac po

The movements of the left ventricular wall in cases of myocardial infarction were studied by ultrasono-cardiotomography and M-mode echo-cardiography. Ultrasono-cardiotomographic and echocardiographic data were collected from 15 consecutive patients with transmural myocardial infarction ranging in age from 28 to 60 years (4 with antero-septal infarction, 5 with postero-inferior infarction, and 6 with infarction of other areas) and also from healthy control subjects. In myocardial infarction, a delay in timing and a decrement in amplitude of the excursion of the infarcted area were observed. As

To study the incidence of left-ventricular thrombosis after transmural myocardial infarction, we performed serial two-dimensional echocardiography in 70 consecutive patients. Thirty-five patients had inferior-wall infarction: none had a left-ventricular thrombus. The other 35 had anterior-wall infarction: 12 had left-ventricular thrombi. Thrombi were diagnosed an average of five days after the infarction (range, one to 11 days). All patients with left-ventricular thrombi had severe apical-wall-motion abnormalities (akinesis or dyskinesis). Twenty-six of the 35 patients with anterior infarction
