Phosphatidylserine-binding receptor, CD300f, on macrophages mediates host invasion of pathogenic and non-pathogenic rickettsiae.

Some arthropod-borne obligate intracellular rickettsiae are among the most virulent human pathogens. Rickettsia species modulate immune (e.g., macrophages; MΦ) and non-immune cell (e.g., endothelial cells) responses to create a habitable environment for host colonization. In particular, MΦ play a crucial role in either terminating an infection at an early stage or succumbing to bacterial replication and colonization. However, our understanding on how Rickettsia species invade host cells, including MΦ, remain poorly defined. In this study, we describe a mechanism of host invasion by Rickettsia

Distinguishing Multisystem Inflammatory Syndrome in Children From Typhus Using Artificial Intelligence: MIS-C Versus Endemic Typhus (AI-MET).

BACKGROUND: The pandemic emergent disease multisystem inflammatory syndrome in children (MIS-C) following coronavirus disease-19 infection can mimic endemic typhus. We aimed to use artificial intelligence (AI) to develop a clinical decision support system that accurately distinguishes MIS-C versus endemic typhus (MET). METHODS: Demographic, clinical, and laboratory features rapidly available following presentation were extracted for 133 patients with MIS-C and 87 patients hospitalized due to typhus. An attention module assigned importance to inputs used to create the 2-phase AI-MET. Phase 1 u

SFG and TG seropositivity in Humans suspected of TBD in Yucatan, Mexico.

Since 1996, the incidence of rickettsiosis has been increasing in Yucatán, Mexico, but recent prevalence data are lacking. This study aimed to determine exposure to the Spotted Fever Group (SFG) and Typhus Group (TG) in human serum samples suspected of tick-borne diseases (TBD) between 2015 and 2022. A total of 620 samples were analysed using indirect immunofluorescence assay (IFA) to detect IgG antibodies against SFG (Rickettsia rickettsii) and TG (Rickettsia typhi), considering a titer of ≥64 as positive. Results showed that 103 samples (17%) were positive for R. rickettsii and 145 (24%) for

A One Health Perspective on the Resurgence of Flea-Borne Typhus in Texas in the 21st Century: Part 1: The Bacteria, the Cat Flea, Urbanization, and Climate Change.

Flea-borne typhus (FBT), due to Rickettsia typhi and R. felis, is an infection typically causing fever, headache, rash, hepatitis, and thrombocytopenia. About one quarter of patients suffer pulmonary, neurologic, hematologic, renal, hepatic, cardiac, ocular or other complications. In the 21st century, the incidence of FBT has increased in both Texas and California compared to the 1990s. In this paper, county-level epidemiological data for the number of cases of FBT occurring in Texas for two decades, 1990-1999 and 2010-2019, were compared with respect to county of residence, urbanization, and

Rickettsia Felis Case Cluster in a Military Family.

Cases of flea-borne spotted fever (FBSF) have been on a steady increase in the southern United States and produce the highest numbers of reported vector-borne disease cases in Texas. Historically, cases were largely murine typhus caused by Rickettsia typhi and characterized by exposure to rodent fleas. However, more recently, spotted fever rickettsiosis caused by R. felis and characterized by exposure to the cat flea, Ctenocephalides felis, may be the dominant disease. We describe the management of a child with a complex clinical picture, including sickle cell β-thalassemia, bullous pemphigoid

Epidemiological and clinical predictors of murine typhus in hospitalized patients in central israel: association with urban construction.

Murine typhus (MT), caused by Rickettsia typhi, is a zoonotic disease with a nonspecific clinical presentation, making diagnosis challenging. During 2022-2023, we observed in our hospital a rise in MT cases, prompting an investigation into clinical predictors and epidemiological factors, including potential links to a light rail system constructed in the hospital's vicinity. We conducted a retrospective case-control study of hospitalized febrile patients tested for rickettsial disease at the Wolfson Medical Center in central Israel (2022-2023). Cases were defined by positive R. typhi PCR or se

Estimating scrub typhus and murine typhus incidence among adolescents and adults in Yangon, Myanmar.

OBJECTIVES: Rickettsioses are frequent causes of treatable febrile illness in Southeast Asia, including Myanmar. Accurate estimates of the incidence of rickettsioses are needed to inform investments in disease prevention and control. We sought to estimate the incidence of rickettsioses among adults and adolescents by combining sentinel hospital surveillance with a healthcare utilisation survey in Yangon, Myanmar. METHODS: We conducted a household-based healthcare utilisation survey in the Yangon Region from 12 March through 5 April 2018. Multipliers derived from this survey were then applied

TlyC, a conserved hemolysin in Rickettsia, contributes to spotted fever pathogenesis in mice.

UNLABELLED: Rickettsia circulates between mammalian hosts and hematophagous arthropod vectors by exploiting their intracellular environment. With advances in rickettsial genetic tools, recent studies have identified novel molecular mechanisms involved in Rickettsia-host-vector interactions. However, a significant knowledge gap exists in understanding how Rickettsia modulates its virulence functions to survive in two drastically different environments (mammalian hosts vs arthropod vectors). Bacterial hemolysins play a crucial role in neutralizing innate immune functions through pore-forming act

Organ Donor Transmission of Rickettsia typhi to Kidney Transplant Recipients, Texas, USA, 2024.

Murine typhus, a fleaborne disease caused by the bacterium Rickettsia typhi, is found throughout temperate and tropical regions of the world. Transmission of R. typhi to humans involves several species of fleas, and most infections result from direct inoculation of R. typhi-infected flea feces into abrasions in the skin. We describe the transmission of R. typhi from an organ donor in Texas, USA, to 2 kidney transplant recipients. The donor and 1 recipient died from the infection. The occurrence of R. typhi transmission via transplantation is a harbinger for the reemergence of murine typhus in

Murine Typhus in Heart and Kidney Transplant Recipients: First Reported Cases Diagnosed by Microbial Cell-Free DNA Sequencing.

Murine typhus, caused by Rickettsia typhi, is a flea-borne illness that can present as an undifferentiated febrile syndrome and is often underrecognized in immunocompromised individuals. We report the first known cases in heart and kidney transplant recipients, both diagnosed using microbial cell-free DNA (mcfDNA) sequencing. The heart transplant patient presented with encephalopathy and hyponatremia, while the kidney transplant patient had fever and gastrointestinal symptoms. In both cases, mcfDNA sequencing enabled early detection of R. typhi prior to IgG seroconversion, allowing timely init

Bioinformatics analysis of Rickettsia typhi autoimmune associations and screening of Streptomyces-derived inhibitors.

BACKGROUND: Rickettsia typhi is the causative agent of epidemic murine typhus and Rocky Mountain spotted fever. The infection can affect multiple vital organs, including the heart, lungs, kidneys, and brain. Doxycycline is the recommended treatment but inflammation, mal-response, and drug resistance may arise. No natural product inhibitors have been reported against this bacterium. AIM: The objective of this study was to establish a potential connection between autoimmune disorders triggered by R. typhi, identify therapeutic targets within its core proteome, and explore novel natural product

Complete genomes of Rickettsia typhi reveal a clonal population.

Murine typhus, caused by infection with Rickettsia typhi, is a neglected disease contributing to infectious disease burden in south- and southeast Asia. Despite its importance, we have minimal knowledge of the genomics of R. typhi, with only four complete genomes being sequenced prior to this work. We sequenced a further 25 genomes including historical strains collected before 1976 from both human and rat hosts, and recent genomes isolated from patients at a single hospital in Laos. Whole genome SNP analysis reveals extremely low levels of genetic diversity across the 29 genomes, with overall
