BACKGROUND: Myocardial necrosis following acute myocardial infarction may result in three major mechanical complications: papillary muscle rupture, free-wall rupture, and ventricular septal rupture (VSR), all of which are associated with devastating hemodynamic consequences and may compromise short-term survival. Although the incidence of these complications has markedly declined with the widespread adoption of early percutaneous coronary interventions, the surgical management of VSR remains highly demanding and requires substantial expertise. Moreover, the optimal timing of surgery remains co

BackgroundPost-cardiotomy cardiogenic shock (PCCS) remains one of the most lethal complications of cardiac surgery, with high in-hospital mortality despite major improvements in operative techniques, and intensive care management. Temporary mechanical circulatory support (tMCS) is widely applied as a rescue therapy for refractory PCCS. However, outcomes remain suboptimal, also due to delayed initiation after the onset of severe hemodynamic collapse and irreversible compromised end-organ states. This review summarizes current concepts of PCCS management and evaluates the emerging role of prophy

Mechanical complications of acute myocardial infarction (AMI) - including left ventricular aneurysm (LVA), ventricular septal defect (VSD), free wall rupture, and papillary muscle rupture - are rare in the current reperfusion era but carry high mortality. Silent myocardial infarction (MI), particularly in diabetic patients, often goes unrecognized until these complications manifest. The concurrent occurrence of multiple mechanical complications following a silent AMI is exceedingly rare and poses significant diagnostic and therapeutic challenges. We report a 70-year-old woman with diabetes mel

Acute ischemic mitral regurgitation is a rare but potentially catastrophic complication following acute myocardial infarction (AMI), characterized by severe clinical presentation and high mortality. Meanwhile, advancements in primary percutaneous coronary intervention (PCI) have reduced the incidence of acute mitral regurgitation (AMR). The surgical approach remains the standard treatment but is associated with high rates of complications and in-hospital mortality, particularly in patients with cardiogenic shock or mechanical complications, such as papillary muscle rupture. Mitral transcathete

Mechanical complications following acute myocardial infarction (MI) represent some of the most challenging conditions in contemporary cardiology, often leading to rapid clinical deterioration and high mortality despite advances in reperfusion therapy. These complications span a spectrum of presentations, from early-phase structural disruptions such as ventricular septal rupture, papillary muscle rupture with acute mitral regurgitation (MR), and left ventricular free wall rupture (LVFWR), to later-stage manifestations, including true ventricular aneurysms and pseudoaneurysms. While surgical int

INTRODUCTION: With the implementation of early reperfusion therapy, the number of complications in patients with acute coronary syndrome (ACS) has diminished significantly. However, ACS patients are still routinely admitted to units with high-level monitoring such as the coronary or intensive care unit (CCU/ICU). The cost of these admissions is high and there is often a shortage of beds. The aim of this study was to analyze the complications in contemporary emergency department (ED) patients with ACS and to map patient management.
METHODS: This observational study was a secondary analysis of

Acute mitral regurgitation (MR) is not a rare finding following acute myocardial infarction (AMI). It may develop due to papillary muscle rupture (primary MR) or due to rapid remodeling of the infarcted areas leading to geometric changes and leaflets tethering (secondary or functional MR). The clinical presentation can be catastrophic, with pulmonary edema and refractory cardiogenic shock. Acute MR is a potentially life-threatening complication and is linked to worse clinical outcomes. Until recently, medical treatment or mitral valve surgery were the only established treatment options for the

Cardiogenic shock (CS) is defined as end-organ hypoperfusion as the consequence of primary myocardial dysfunction. Among the diagnostic criteria are a systolic blood pressure < 90 mmHg, acute renal failure (oligoanuria), ischemic hepatitis, cyanosis and cold, clammy skin. Accepted hemodynamic cutoffs are a cardiac index < 2,2 (l/min)/m2 and a pulmonary capillary wedge pressure > 15 mmHg. It should be acknowledged, that a normal blood pressure does not rule out CS; there is a nonhypotensive variant of CS demonstrating all the signs mentioned above (including elevated lactate levels) while the b

Acute myocardial infarction (AMI) can result in ischemic, mechanical, arrhythmic, embolic, or inflammatory complications. The development of mechanical complications following AMI is associated with a significantly reduced short-term and long-term survival. Since the introduction of primary percutaneous coronary intervention as the principal reperfusion strategy following acute ST-elevation myocardial infarction, the incidence of mechanical complications, including rupture of the left ventricular free wall, papillary muscle, and ventricular septum, has reduced significantly to less than 1%. Most woody species undergo morphological, physiological, and biochemical changes during ontogenetic development from juvenile to adult phases, especially in terms of clonal potential, growth vigor, and adventitious rooting capacity. This study focused on generating information to support the vegetative propagation of adult individuals of Khaya spp., aiming to optimize the mechanisms involved in the induction of adventitious rooting and its association with indole-3-butyric acid (IBA). The experiment evaluated the rejuvenation/reinvigoration of plant tissues from three Khaya species by comparin

Ischemia-induced mitral insufficiency (IMI) can occur when a papillary muscle ruptures in the acute phase of myocardial infarction (MI) or, more commonly, when ischemic heart disease reaches the chronic stage, with or without infarction. In the latter case it can be distinguished from organic mitral regurgitation because the structure of the valve and the subvalvular apparatus are not affected. Many factors contribute to the complex mechanism of IMI: incomplete closure of the valve is mainly a result of changes in the geometry of the left ventricle, the mitral annulus, papillary muscles and to

Reviewed are the current surgical methods for the treatment of mechanical complications after an acute myocardial infarction, specially the subacute ventricular rupture, the most frequent complication, and in which we have the greatest experience in the world. Mechanical complications constitute the second cause of death after myocardial infarction, following pump failure. The most frequent mechanical complication is ventricular rupture, which is the cause of death in 26% of the cases of acute myocardial infarction. The setting of solid diagnostic tools, with the association of echocardiograph

Coronary artery bypass grafting is a straightforward solution to the problem of myocardial ischemia secondary to coronary artery disease. More recently, the complexities of complications following myocardial infarction have been elucidated with respect to various aspects. Disastrous sequelae, such as rupture of the ventricular septum, the left ventricular wall and the papillary muscle, impose many intricacies on the inevitable surgical treatment; this form of cardiac surgery is still a tour de force. Life-threatening ventricular tachycardias due to transmural myocardial infarction and/or left
