Intravascular Lithotripsy in Left Main Stem Percutaneous Coronary Intervention.

BACKGROUND: IVL is increasingly used for coronary calcium modification; however, prospective data for its use in LMS lesions are lacking. AIMS: To evaluate the feasibility, safety, and mechanical efficacy of intravascular lithotripsy (IVL) for calcium modification in calcified distal left main stem (LMS) percutaneous coronary intervention. METHODS: In this prospective, multicentre, open-label study across five United Kingdom centers (NCT04319666), 50 patients with obstructive distal LMS disease and IVUS-confirmed circumferential calcification (maximum calcium arc ≥ 270° in at least one segme

Hippo-YAP signaling: A novel pathway regulating platelet activation and thrombogenesis.

AIM: To investigate the functional significance of Hippo-YAP signaling in platelet activation and thrombosis. METHODS: Expression and phosphorylation of YAP were examined in platelets exposed to physiological agonists as well as mechanical cues (adhesion to matrices having varying stiffness and activation of mechanosensitive channel Piezo1). Involvements of LATS kinase and RhoA-ROCK-actomyosin axis in YAP signaling were evaluated. Effect of pharmacological inhibition of YAP on platelet activation parameters (αIIbβ3 activation, adhesion, α-granule secretion, extracellular vesicle release, and

Microplastics and Nanoplastics as a Novel Cardiovascular Risk Factor: A Narrative Review.

Microplastics and nanoplastics (MNPs) are pervasive environmental contaminants now detected throughout the human cardiovascular system, including atherosclerotic plaques, coronary and carotid arteries, cardiac tissue, peripheral blood, and vascular thrombi. Against the backdrop of cardiovascular disease (CVD) accounting for approximately 17-20 million deaths annually, MNPs have emerged as a candidate modifiable cardiovascular risk factor. In the landmark prospective cohort by Marfella and colleagues (N Engl J Med 2024), MNP-laden carotid plaques conferred a 4.53-fold increased risk of major ad

Left atrial mural infective endocarditis complicated by coronary embolism and left ventricular myocardial infarction in a cat.

A three-year-old female Devon Rex was referred for postpartum complications. On presentation, physical examination revealed depression, hypothermia, pale mucous membranes and a grade II/VI left apical systolic murmur. Laboratory tests showed severe non-regenerative anaemia, moderate leucocytosis and increased cardiac troponin I (4.16 ng/mL; upper hospital limit <0.2 ng/mL). Transthoracic echocardiography showed no remodelling or dysfunction of either cardiac chamber and no valvular abnormalities. However, a 6-mm echogenic structure with well-defined margins was identified in the left atrial (L

Intravascular imaging from research tool to guidelines: a 35-year long journey.

Initially developed to overcome the limitations of angiography in visualizing plaque morphology, intravascular ultrasound and optical coherence tomography have advanced dramatically in resolution, acquisition speed, and analytical automation. These techniques now provide detailed, real-time assessment of coronary artery structure, enabling precise stent sizing, calcium characterization, and optimization of percutaneous coronary interventions. Clinical trials and meta-analyses demonstrated that intravascular imaging-guided PCI significantly reduces restenosis, thrombosis, target lesion failure,

No-Stent Management of High Coronary Thrombus Burden in Young Patients with STEMI: A Four-Case Series.

Atherosclerotic plaque rupture is the most common mechanism underlying ST-segment elevation myocardial infarction (STEMI); however, young patients (≤50 years old) may present with a massive coronary thrombus in the absence of clearly demonstrated flow-limiting severe atherosclerotic stenosis. Routine direct stent implantation in these highly thrombotic lesions may increase the risk of distal embolization, impaired coronary blood flow, and long-term stent thrombosis. This report describes the angiographic outcomes of a retrospective case series of four young male patients managed without perman

Atypical de winter pattern preceding anterior STEMI: clinical judgement beyond rigid ECG criteria.

BACKGROUND: The de Winter ECG pattern should raise strong suspicion for an unstable thrombotic culprit lesion associated with high-risk anterior ischemia requiring urgent expert evaluation - classically involving the proximal left anterior descending artery (LAD) - that frequently does not fulfil conventional ST-segment elevation criteria and is therefore regarded as a STEMI equivalent. CASE PRESENTATION: We report the case of an 85-year-old woman with sudden-onset chest pain radiating to both shoulders. The prehospital 12-lead ECG showed atypical de Winter pattern, prompting direct transport

Advances in Drug-Eluting Angioplasty Balloon Coatings, Clinical Implications and Future Directions: A Mini Review.

Drug-eluting angioplasty balloons are a highly effective treatment for neointimal hyperplasia post-balloon angioplasty and in-stent restenosis. Current drug-eluting angioplasty balloons have restenosis rates approximating 20%, and both paclitaxel, the current drug coating of choice, and sirolimus, an alternative coating being evaluated in early clinical studies, delay re-endothelialisation, potentially predisposing to thrombosis. There remains a paucity of efficacious alternatives to these coatings. Research into alternative drug-eluting balloon coatings is the source of intense investigation

Bleeding complications in patients with out-of-hospital cardiac arrest treated with cangrelor and oral P2Y12 inhibitors.

AIMS: Cangrelor is used to bridge the gap of insufficient platelet inhibition in patients with out-of-hospital cardiac arrest (OHCA) undergoing percutaneous coronary intervention (PCI). METHODS AND RESULTS: In a retrospective chart review study, we investigated the incidence of bleeding and stent thrombosis in patients with OHCA undergoing PCI who received either cangrelor and transition to an oral P2Y12 inhibitor or an oral P2Y12 inhibitor alone. Subgroups consisted of patients treated with conventional cardiopulmonary resuscitation (CPR) and extracorporeal CPR. The primary endpoint was Blee

Drug-coated balloon strategy following intravascular lithotripsy to treat coronary de novo lesion: mid-term clinical outcomes of a preliminary experience.

OBJECTIVES: The management of patients with calcified de novo lesions remains a major clinical challenge even in the era of drug-eluting stents (DES). Drug-coated balloon (DCB) therapy has emerged as an alternative to DES to treat de novo lesions. Nevertheless, the management of calcified lesions using intravascular lithotripsy (IVL) combined with DCB to treat de novo lesions has not been investigated. The authors report the short-term results of our preliminary experience with this novel approach. METHODS: The authors conducted a multicenter retrospective study of 14 patients who underwent p

Coronary Stent Failure in Patients With Diabetes: A Nationwide Observational Study From SWEDEHEART.

OBJECTIVE: The association between diabetes and coronary stent failure is poorly established with second-generation drug-eluting stents (DES). We aimed to evaluate the risk of stent failure in patients with diabetes compared with those without diabetes after implantation of second-generation DES. RESEARCH DESIGN AND METHODS: All patients in Sweden who received second-generation DES between 2010 and 2020 were included and categorized into three groups: type 1 diabetes, type 2 diabetes, and without diabetes (reference group). The primary end point was stent failure, defined as in-stent restenos

Non-ST-segment elevation acute coronary syndrome: understanding the full spectrum to guide management.

Acute coronary syndromes are caused by obstructive coronary thrombosis complicating myocardial ischaemic disease and are dichotomously classified according to their electrocardiographic presentation as ST-segment elevation myocardial infraction or non-ST-segment elevation acute coronary syndrome, either without or with elevated myocardial necrosis markers. Initial diagnosis and risk stratification are reliably guided by symptoms, electrocardiographic changes, and troponin elevation and require exclusion of alternative conditions, such as Type 2 myocardial infarction and myocardial injury due t

Selective Use of Thromboaspiration in STEMI: CMR Evidence Against Routine Practice.

Thromboaspiration (TA) has been proposed as an adjunct to primary percutaneous coronary intervention (PCI) in ST-elevation myocardial infarction (STEMI) to reduce thrombus burden. However, its effect on microvascular perfusion remains uncertain, and concerns have been raised about its potential to aggravate microvascular injury. This study aimed to evaluate the impact of TA on microvascular obstruction (MVO) using cardiac magnetic resonance imaging in a large cohort of STEMI patients. We prospectively enrolled 460 STEMI patients treated with primary PCI within 12 hours of symptom onset. TA was

Dual antiplatelet therapy escalation and de-escalation.

Dual antiplatelet therapy (DAPT) remains a key element of secondary prevention after percutaneous coronary intervention. However, its optimal duration and intensity continue to pose challenges due to the trade-off between ischaemic protection and bleeding risk. Over the past decade, evidence has progressively emerged for strategies of DAPT modulation, with particular emphasis on de-escalation approaches. Randomized trials have consistently shown that reducing antiplatelet intensity, whether by lowering the dose, discontinuing one drug, or switching to a less potent agent, can mitigate bleeding

Precision Management of Coronary Stent Thrombosis: A Contemporary Guide.

PURPOSE OF REVIEW: Stent thrombosis (ST) is an uncommon but serious complication of percutaneous coronary intervention (PCI), associated with high morbidity, mortality, and a substantial risk of recurrence. Although its incidence has declined with contemporary drug-eluting stents (DES), outcomes remain poor. This review aims to summarize contemporary evidence on ST, including mechanism, risk factors, and management strategies to improve PCI outcomes. RECENT FINDINGS: The risk of ST is multifactorial, influenced by patient comorbidities, lesion characteristics, procedural technique, and device-

Microplastics: A Modifiable Cardiac Risk Factor.

Globally, cardiovascular disease is the most prevalent cause of death, and there are other risk factors involved that are not captured through traditional means. We present a review of the literature on microplastic exposure and discuss the clinical implications of microplastics for cardiovascular disease. Experimental studies show that micro- and nanoplastics induce oxidative stress, mitochondrial dysfunction, endothelial dysfunction, inflammation, thrombosis, dyslipidemia, and direct cardiotoxicity. These microplastics and nanoplastics have been identified in coronary plaque. Emerging clinic

Pre-appraisal commentary on the PARTNER 3 seven-year results: The wrong button at the top.

The 7-year outcomes of the Placement of Aortic Transcatheter Valves 3 (PARTNER 3) trial comparing transcatheter aortic valve implantation (TAVI) with surgical aortic valve replacement (SAVR) in low-risk patients indicate that the narrowing difference between the two strategies reflects study design convergence rather than true clinical equivalence. Although the composite endpoint of death, stroke, and rehospitalization appeared comparable, important asymmetries persisted. Notably, approximately one quarter of patients in the SAVR group underwent concomitant coronary artery bypass grafting, whe
