Drug-Coated Balloons Beyond In-Stent Restenosis.

Drug-coated balloons (DCBs) are rapidly gaining prominence, owing to the associated expanding clinical applications and therapeutic potential in interventional cardiology. Moreover, a growing body of evidence from randomized trials, observational studies, and meta-analyses supports the efficacy of DCBs across a broad spectrum of coronary lesions, positioning these devices as a viable, stent-free alternative to conventional strategies. DCBs aim to lower the risks inherent to stent implantation, such as stent thrombosis and in-stent restenosis (ISR), and to enable abbreviated dual antiplatelet t

Acute embolic myocardial infarction following surgical resection of a left atrial myxoma.

A woman in her early 40s presented with an acute anterior ST-segment elevation myocardial infarction and new-onset atrial fibrillation (AF), 4 weeks after surgical resection of an incidental, asymptomatic left atrial myxoma. Preoperative CT coronary angiography was normal, and there was no history of AF.Emergency coronary angiography revealed complete occlusion of the left anterior descending artery. Percutaneous coronary intervention was unsuccessful at restoring coronary flow. Transthoracic echocardiography identified two large left atrial thrombi, and intravenous heparin was commenced. Surg

Optimal Timing of Complete Revascularization in Patients With ST-Elevation Myocardial Infarction and Multivessel Disease: A Systematic Review and Meta-Analysis.

Complete revascularization (CR) in patients with ST-segment elevation myocardial infarction (STEMI) and multivessel disease (MVD) is recommended, but the timing of revascularization, either immediate or staged remains a topic of debate. A systematic search of MEDLINE, Scopus, and Cochrane databases was performed to identify randomized controlled trials (RCTs) that evaluated patients with STEMI and MVD and compared outcomes between immediate CR versus staged CR. The primary outcome was major adverse cardiovascular events. Eleven RCTs were included in this analysis with 4,472 patients assessed a

Pulmonary hemorrhagic lesions and coronary artery aneurysm accompanied by thrombosis.

This document presents a case report of a 74-year-old male patient admitted with three days of persistent chest pain accompanied by hemoptysis. Diagnostic imaging, including chest CT and coronary CT angiography, revealed pulmonary hemorrhagic lesions appearing as ground-glass opacities, predominantly located in the right lung, as well as a coronary artery aneurysm with surrounding thrombus in the circumflex artery. The case underscores the potential interplay between coronary vascular anomalies and pulmonary complications, emphasizing the value of advanced imaging techniques in diagnosis.

Presumed Mechanisms Underlying Lipoprotein(a)-caused Atherosclerosis.

Lipoprotein(a) (Lp(a)) is increasingly recognised as an independent and causal risk factor for atherosclerotic cardiovascular disease. Although the underlying mechanisms remain incompletely defined, evidence supports a multifactorial role for Lp(a) in atherogenesis. Lp(a) contributes to endothelial dysfunction, promotes vascular inflammation and enhances lipid retention and oxidation within the arterial wall. These changes drive foam cell formation and smooth muscle cell activation, hallmarks of early plaque development. In addition, Lp(a) exerts prothrombotic effects through structural homolo

Case report: aortic root thrombus presenting with syncope and embolic events.

BACKGROUND: Aortic root thrombus is a rare but potentially life-threatening condition, often associated with systemic embolization or obstruction of coronary arteries (myocardial infarction). The most common reason is a complication of left ventricular assist device (LVAD) implantation, but it may occur in the absence of LVAD, for instance in structural valve disease or prothrombotic disorders (antiphospholipid syndrome, haemolytic diseases, and myeloproliferative disease, making diagnosis challenging. CASE SUMMARY: We report the case of a 47-year-old man with a history of deep venous thrombo

Revolutionizing Modern Percutaneous Coronary Intervention: Drug-Coated Balloons for Diabetics With Small Vessel Disease. A Narrative Review.

Diabetic patients with small-vessel coronary artery disease represent a challenging population in interventional cardiology. Conventional drug-eluting stents (DES) show limited efficacy due to enhanced neointimal proliferation, microvascular dysfunction, and impaired endothelial healing. This narrative review synthesizes contemporary evidence on drug-coated balloon (DCB) angioplasty efficacy, safety, and procedural strategies in diabetic small vessel disease (SVD). A comprehensive literature search of PubMed, Scopus, and Web of Science was conducted through October 2025, focusing on randomized

Incomplete Kawasaki Disease With Rapid Coronary Aneurysm Progression.

BACKGROUND: Giant coronary artery aneurysms occur in 0.16% to 1% of children with Kawasaki disease; in incomplete disease, the incidence is unclear, but coronary lesion risk is similar or higher. CASE SUMMARY: We report a case of incomplete Kawasaki disease in a 2-year-old boy that, despite early treatment, progressed to severe coronary involvement. Initial echocardiography was normal, but repeat imaging revealed diffuse giant coronary artery aneurysms (left anterior descending artery, right coronary artery, left circumflex artery). He required multiple anti-inflammatory therapies (intravenou

Drug coated balloon vs. drug eluting stent in calcified de novo coronary lesions: Long term outcomes from the SPARTAN registry.

AIM: Coronary artery calcification presents major challenges during PCI, including device deliverability, suboptimal lesion preparation, impaired stent expansion, increased restenosis, stent thrombosis and mortality. Drug-coated balloons (DCB) offer a stent-less alternative, but evidence in de novo calcified lesions is limited. This study compared long-term clinical outcomes between DCB and drug-eluting stent (DES) strategies in such lesions. METHODS: We retrospectively identified all patients undergoing PCI for operator reported angiographically calcified de novo lesions between January 2015

Thromboprophylaxis in congenital and acquired heart disease across the lifespan. A French consensus position statement: Part 2 - from adolescence to adulthood.

Patients with congenital and acquired heart disease are exposed to a lifelong thromboembolic risk that increases progressively from adolescence to adulthood because of residual lesions, arrhythmias, venous congestion, repeated interventions and acquired co-morbidities. Management is further complicated by concurrent bleeding risk. To provide practical expert consensus recommendations on thromboprophylaxis in congenital and acquired heart disease from adolescence to adulthood. The Roles of Lipoprotein(a) in Atherosclerosis - Minding the Knowledge Gaps.

PURPOSE OF REVIEW: Although the association of lipoprotein(a) (Lp(a)) with coronary heart disease was reported over 60 years ago, and subsequent studies have identified a causal and independent role for Lp(a) in disease, many fundamental unanswered questions persist surrounding the biology of this enigmatic lipoprotein. There remain critical questions surrounding the structure and metabolism of Lp(a) and the unique biochemical properties of Lp(a) that drive its pathogenic properties in the vasculature. These questions are critical to address as we rapidly approach the availability of drugs tha

Compound heterozygosity with methylenetetrahydrofolate reductase (MTHFR) C677T and A1298C mutations likely causing recurrent thrombotic events in a middle-aged man: a case report.

Methylenetetrahydrofolate reductase (MTHFR) gene mutations, particularly homozygous mutations, have been associated with a higher incidence of venous thrombosis, coronary heart disease, and obstetric complications. We report the case of a 41-year-old man who presented with multiple vascular thrombotic events over a period of 4-5 years, including deep vein thrombosis with pulmonary thromboembolism, cerebral venous thrombosis and posterior circulation stroke. The patient was found to have elevated serum homocysteine levels and subsequently underwent genetic testing for MTHFR mutations after othe

Baseline use of aspirin or statins with oral estrogen and progestogens to prevent incident arterial or venous thrombotic events: a secondary analysis of the Women's Health Initiative trial.

OBJECTIVE: To evaluate whether effects of oral hormone therapy (HT) on risks of venous and arterial vascular events differ by baseline statin or aspirin use. METHODS: We performed time-to-event analysis using data from the Women's Health Initiative menopausal HT randomized trials to assess risk of thrombotic events. Women were randomized to oral conjugated equine estrogens (CEEs) alone or placebo among women with prior hysterectomy (n = 10,739), and CEE with medroxyprogesterone acetate (MPA) or placebo among women with an intact uterus (n = 16,608), stratified by baseline personal use of stat

Impact of Heparin and Enoxaparin Anticoagulant Prophylaxis on Improving Acute Mortality in Burn Patients.

Burn patients have a higher chance of developing thromboembolic complications, leading to worsened mortality rates, so prophylactic anticoagulation is important. Anticoagulants such as enoxaparin, a low-molecular-weight heparin, and unfractionated heparin (UFH) have been frequently used as chemical prophylactic treatments of thromboembolism. Enoxaparin has been shown to have lower mortality and higher efficacy in surgical patients and coronary artery disease patients. The aim of the study is to assess mortality and compare the safety of enoxaparin and heparin in acute burn patients. A retrospe

Multidomain Biomarkers as Predictors of Cardiovascular Risk in Acute Coronary Syndrome: A Prospective Evaluation.

Acute coronary syndrome (ACS), driven by inflammation and thrombosis, remains a leading cause of morbidity globally. While traditional risk scores are useful, the prognostic value of combining inflammatory and autoimmune biomarkers remains understudied. This study aimed to evaluate the predictive role of high-sensitivity C-reactive protein (hs-CRP), platelet factor 4 (PF4), D-dimer, and antiphospholipid antibodies (anticardiolipin and anti-β2-glycoprotein I) for the development of major adverse cardiovascular events (MACE) in patients with ACS. We conducted a prospective cohort study at a tert
