Intravascular imaging guidance in complex PCI: a meta-analysis of randomized trials across complex lesion subsets.

BACKGROUND: Recent randomized trials have reported conflicting results regarding the clinical benefit of intravascular imaging (IVI) in complex percutaneous coronary intervention (PCI). The aim of this research was to evaluate whether IVI guidance reduces target vessel failure (TVF) in complex PCI. METHODS: PubMed, Scopus, and the Cochrane Library were searched through June 2026 for randomized trials comparing IVI-guided with angiography-guided PCI in trial-defined complex PCI or extractable complex-lesion cohorts. Risk ratios (RRs) were pooled using random-effects models. Prespecified analys

Early Ulnar Artery Occlusion and Recanalization After Coronary Angiography via Distal Ulnar Access: A Doppler-Followed Case Series.

This single-center retrospective case series assessed early ulnar artery patency by Doppler ultrasonography (DUS) after coronary angiography via distal ulnar access (DUA), described flow evolution in patients with follow-up, and reported access-related complications. Thirty-eight consecutive patients with a day-3 DUS after coronary angiography via DUA between September 2023 and March 2024 were included. All procedures were performed by 1 operator using a short 6F sheath and 5000 IU intra-arterial heparin. Flow was categorized as thrombosis/no flow, monophasic, or triphasic; the primary endpoin

One Month Versus Three Months of Aspirin After Percutaneous Coronary Intervention: A Reconstructed Time-to-Event Analysis.

P2Y12-inhibitor monotherapy after abbreviated dual antiplatelet therapy (DAPT) reduces bleeding after percutaneous coronary intervention, but whether aspirin can be stopped after one month rather than three remains uncertain. We reconstructed time-to-event data from published Kaplan-Meier or cumulative-incidence curves of 11 randomised trials (37,443 patients) using the Guyot algorithm, grouped trials by aspirin-withdrawal timing, and analysed within-trial monotherapy-versus-continued-DAPT effects using stratified Cox regression and random-effects meta-analysis across 0 to 30, 30 to 90, and 90

Low-dose prasugrel versus clopidogrel for dual antiplatelet therapy after percutaneous coronary intervention: insights from the STOPDAPT-2 total cohort.

Dual antiplatelet therapy (DAPT) with low-dose prasugrel (3.75 mg daily) is widely implemented after percutaneous coronary intervention (PCI) in Japan. However, the efficacy compared with clopidogrel-based DAPT has not been fully examined in large-scale research. STOPDAPT-2 Total Cohort enrolled 5,997 patients who underwent PCI without major in-hospital complications. Among them, 2,547 patients received prasugrel-based DAPT and 3,402 patients received clopidogrel-based DAPT at discharge within 30 days after index PCI. After propensity score matching, 2,163 patients in each group were compared

Perioperative Myocardial Injury and Infarction: Thrombotic Mechanisms, Diagnostic Challenges, and Antithrombotic Management.

BackgroundPerioperative myocardial injury (PMI) and perioperative myocardial infarction (PMIn) are common, often clinically silent complications after noncardiac surgery and are associated with substantial short- and long-term mortality. Surgical stress promotes a prothrombotic milieu through platelet activation, coagulation cascade amplification, impaired fibrinolysis, endothelial dysfunction, and plaque destabilization.PurposeThis narrative review synthesizes current evidence on the thrombotic mechanisms, diagnostic challenges, and antithrombotic management of PMI/PMIn, with emphasis on risk

Inadvertent Coronary Sinus Cannulation During Central Venous Catheter Placement in a Liver Transplant Recipient with Persistent Left Superior Vena Cava: A Case Report.

Persistent left superior vena cava (PLSVC) is the most common thoracic venous anomaly and is usually asymptomatic. Central venous catheterization through a PLSVC can be challenging, and inadvertent advancement of catheters into the coronary sinus may result in arrhythmias, perforation, thrombosis, or inaccurate hemodynamic measurements. We report a case of coronary sinus cannulation by a left internal jugular dialysis catheter in a liver transplant recipient with previously unrecognized PLSVC. Initial chest radiography suggested catheter termination within a duplicated left superior vena cava;

Coronary Vasculitis and Myocarditis in Acute Systemic Lupus Erythematosus Flares With Macrophage Activating Syndrome.

Cardiovascular disease is a major cause of morbidity and mortality in systemic lupus erythematosus (SLE). Although accelerated atherosclerosis is well recognized, SLE-related coronary vasculitis and microvascular injury remain underdiagnosed. Cardiac magnetic resonance (CMR) offers unique tissue characterization that may reveal atypical patterns of myocardial injury. We describe 2 patients with active SLE who presented with acute cardiac symptoms and elevated cardiac biomarkers. CMR demonstrated both classic inflammatory findings of lupus myopericarditis (subepicardial late gadolinium enhancem

Pathway-Guided and Patient-Centered Therapeutic Strategies in Atherosclerotic Cardiovascular Disease.

Atherosclerotic cardiovascular disease (ASCVD), encompassing coronary artery disease (CAD), cerebrovascular disease (ischemic stroke, IS; and transient ischemic attack, TIA), and peripheral artery disease (PAD) of atherosclerotic origin, remains a leading cause of morbidity and mortality worldwide despite major advances in prevention and treatment. Although atherosclerosis arises from the convergence of endothelial injury, lipid accumulation, inflammation, and thrombosis, the relative contribution of these pathogenic processes varies markedly among individuals, due to the effects of genetics,

Right Coronary Artery Thrombosis Causing ST-Elevation Myocardial Infarction After a Coronary Artery Disease Reporting and Data System Score of 0, With Disseminated Intravascular Coagulation During Idiopathic Acute Pancreatitis: A Case Report.

Acute pancreatitis may cause electrocardiographic abnormalities and troponin elevation that mimic acute myocardial infarction. True coronary thrombosis is less common. The development of disseminated intravascular coagulation shortly after percutaneous coronary intervention creates a difficult balance between bleeding control and prevention of early stent thrombosis. A 68-year-old man with no previous pancreatitis presented with acute interstitial pancreatitis. Evaluation did not identify biliary disease, alcohol use, hypertriglyceridemia, hypercalcemia, medication exposure, autoimmune pancrea

Immune-inflammatory profiles, coronary artery disease, and peripheral artery disease in people with HIV.

BACKGROUND AND AIMS: People with HIV (PWH) have high risk of coronary (CAD) and peripheral (PAD) artery disease independent of traditional risk factors. Single immune-inflammatory biomarkers provide limited insight into disease risk and phenotypes, whereas integrative profiling may reveal novel mechanisms. Here we investigated associations between immune-inflammatory profiles, CAD, and PAD in PWH. METHODS: We included PWH from the Copenhagen Comorbidity in HIV Infection (COCOMO) Study with undetectable HIV RNA and measurements of 25 immune-inflammatory biomarkers. Participants underwent coron

Anterior ST-Segment Elevation Myocardial Infarction With Coronary Thrombosis in a Young Man with Newly Diagnosed HIV Infection and Pneumocystis jirovecii Pneumonia.

Human immunodeficiency virus (HIV) infection is associated with an increased cardiovascular risk through chronic inflammation, immune dysregulation, endothelial dysfunction, and prothrombotic mechanisms. However, the mechanisms underlying acute coronary events in young patients with newly diagnosed advanced HIV infection remain incompletely understood. We report the case of a 36-year-old man with newly diagnosed HIV infection and severe immunosuppression who was initially evaluated for suspected systemic vasculitis and subsequently diagnosed with Pneumocystis jirovecii pneumonia. Microplastics and Nanoplastics as a Novel Cardiovascular Risk Factor: A Narrative Review.

Microplastics and nanoplastics (MNPs) are pervasive environmental contaminants now detected throughout the human cardiovascular system, including atherosclerotic plaques, coronary and carotid arteries, cardiac tissue, peripheral blood, and vascular thrombi. Against the backdrop of cardiovascular disease (CVD) accounting for approximately 17-20 million deaths annually, MNPs have emerged as a candidate modifiable cardiovascular risk factor. In the landmark prospective cohort by Marfella and colleagues (N Engl J Med 2024), MNP-laden carotid plaques conferred a 4.53-fold increased risk of major ad

Left atrial mural infective endocarditis complicated by coronary embolism and left ventricular myocardial infarction in a cat.

A three-year-old female Devon Rex was referred for postpartum complications. On presentation, physical examination revealed depression, hypothermia, pale mucous membranes and a grade II/VI left apical systolic murmur. Laboratory tests showed severe non-regenerative anaemia, moderate leucocytosis and increased cardiac troponin I (4.16 ng/mL; upper hospital limit <0.2 ng/mL). Transthoracic echocardiography showed no remodelling or dysfunction of either cardiac chamber and no valvular abnormalities. However, a 6-mm echogenic structure with well-defined margins was identified in the left atrial (L

Intravascular imaging from research tool to guidelines: a 35-year long journey.

Initially developed to overcome the limitations of angiography in visualizing plaque morphology, intravascular ultrasound and optical coherence tomography have advanced dramatically in resolution, acquisition speed, and analytical automation. These techniques now provide detailed, real-time assessment of coronary artery structure, enabling precise stent sizing, calcium characterization, and optimization of percutaneous coronary interventions. Clinical trials and meta-analyses demonstrated that intravascular imaging-guided PCI significantly reduces restenosis, thrombosis, target lesion failure,

No-Stent Management of High Coronary Thrombus Burden in Young Patients with STEMI: A Four-Case Series.

Atherosclerotic plaque rupture is the most common mechanism underlying ST-segment elevation myocardial infarction (STEMI); however, young patients (≤50 years old) may present with a massive coronary thrombus in the absence of clearly demonstrated flow-limiting severe atherosclerotic stenosis. Routine direct stent implantation in these highly thrombotic lesions may increase the risk of distal embolization, impaired coronary blood flow, and long-term stent thrombosis. This report describes the angiographic outcomes of a retrospective case series of four young male patients managed without perman
