Distinguishing Leptospirosis from Other Causes of Acute Encephalitis Syndrome: Clinical Insights.

INTRODUCTION: Acute encephalitis syndrome (AES) is a serious public health problem in India, especially in Uttar Pradesh, where children and adolescents are particularly affected. Although viral diseases such as Japanese encephalitis (JE) are the main cause, many cases remain unexplained. AES could be caused by a zoonotic leptospirosis, but its role is uncertain. AIM: The aim is to assess the prevalence and clinical features of leptospirosis in children with AES in a tertiary care hospital and compare the etiology with other causes. MATERIALS AND METHODS: Children aged 1-14 years with AES or

Concurrent Scrub Typhus, Dengue, and Leptospirosis: A Rare Triple Co-Infection: A Case Report and Comprehensive Literature Review.

In endemic areas, acute undifferentiated febrile illness has a wide differential with overlapping features that can lead to misdiagnosis. Although triple co-infection with scrub typhus, dengue, and leptospirosis is extremely rare, high clinical suspicion and early detection are vital to avoid delayed treatment, multi-organ dysfunction, and mortality.

Development of One Health-based community surveillance system to study zoonotic diseases: a study protocol.

Animals are responsible for a substantial burden of infectious diseases and pose a serious threat to human health, particularly in peri-urban areas where human-animal-environment interfaces are intensified due to rapid urban expansion. This protocol aims to develop a One Health-based community surveillance system to assess the prevalence, transmission dynamics and risk factors associated with brucellosis, leptospirosis and scrub typhus in a peri-urban setting in Odisha, India. We will conduct a prospective mixed-methods cohort study over 3 years in two peri-urban blocks of Khordha district. Ab

Clinical and Laboratory Profile of Patients with Tropical Coinfections Admitted at a Tertiary Care Center in North India.

BACKGROUND: Tropical coinfections (CI) are the simultaneous occurrence of two or more vector-borne diseases in a single host. The prevalence of such illnesses is not uncommon among tropical and subtropical regions such as India; however, these CIs have not been systematically studied prospectively. Mixed infections can prove potentially detrimental if underdiagnosed or undertreated. We undertook this study to estimate the prevalence and compare the clinical profile, laboratory characteristics, and various outcomes among the patients with tropical CI who presented with acute undifferentiated fe

A tale of dual battle against leptospira and scrub typhus coinfection at a peripheral hospital in Kashmir.

Scrub typhus results from infection by Orientia tsutsugamushi, a bacterium transmitted to humans via larval mite (chigger) bites. On the other hand, Leptospira interrogans causes leptospirosis, a zoonotic bacterial infection typically contracted through exposure to contaminated water or soil. Scrub typhus and leptospirosis are important zoonotic infections increasingly recognized as public health concerns in tropical regions like India. Transmitted by mites and contact with contaminated water or soil respectively, they often cause nonspecific febrile illness, leading to frequent misdiagnosis a

Unique aspects of acute kidney injury associated with infections in the tropics: presentation, challenges, and opportunities.

"The tropics" currently account for around 40% of the world's population and face unique environmental challenges, with limited resources, a deficient health system, and a rudimentary sanitary infrastructure, in addition to a lack of adequate preventive measures and poor surveillance programs. Cases of acute kidney injury in the tropics are to a very large extent driven by infections such as leptospirosis, scrub typhus, dengue, malaria, Ebola virus, yellow fever, and HIV, with a pathogenesis driven by volume depletion, hypotension, and immune-mediated mechanisms. Many of these infections also

Clinical profile of patients presenting with acute febrile illness during monsoon season for 3 successive years - A cross sectional observational study.

BACKGROUND: Acute febrile illness (AFI) is the common presentation in tropical countries like India, particularly during the monsoon season. The diagnostic challenge in resource-limited settings often leads to presumptive management without identifying the specific etiology. Understanding the clinical spectrum and etiology of AFI is crucial for timely diagnosis and effective treatment. OBJECTIVES: To evaluate the clinical, biochemical, and radiological profiles of adult patients admitted with acute febrile illness during the monsoon months over three consecutive years and to identify the prev

Linking AKIN stages with tropical infection induced acute kidney injury among Indian patients.

Tropical acute febrile illness often causes AKI in India and outcome linkage with AKIN staging is still limited. Hence, we studied 150 adults with tropical infection related AKI staged by AKIN at 48 hours and followed till discharge or death. Leptospirosis was the main cause and higher AKIN stage showed more oliguria organ dysfunction longer stay higher dialysis need with mortality rising to 40.9% in stage III. This study adds a clear stage based risk gradient plus simple bedside red flags and basic labs for early triage and it supports timely aggressive support and dialysis referral to reduce

Climatic drivers of vector-borne diseases: A nationwide causal inference study in China.

OBJECTIVES: Vector-borne diseases (VBDs) impose a significant burden in China, with complex spatiotemporal patterns driven by heterogeneous and nonlinear climatic influences, complicating the identification of causal drivers. METHODS: To fill this gap, we conducted a nationwide study across 365 city-level regions in mainland China, covering nearly 0.8 million cases of eight types of VBDs, with a time span of 18 years. An empirical dynamic modeling framework was applied to reconstruct system dynamics directly from time-series data, and identify causal climatic drivers for eight VBDs via conver

Diagnostic challenges in re-emerging rickettsioses: why current tools fall short.

SUMMARYRickettsial diseases, encompassing scrub typhus, spotted fever group rickettsioses, and typhus group rickettsioses, represent a significant and escalating public health threat worldwide, particularly in the Asia-Pacific and sub-Saharan African regions. Despite their high morbidity and potential for fatal outcomes if left untreated, these infections remain notoriously underdiagnosed due to their nonspecific clinical presentation, which frequently overlaps with other acute undifferentiated febrile illnesses (AUFIs) such as dengue, malaria, and leptospirosis. This review evaluates the evol

Rodent and Ectoparasite-Borne Disease Surveillance Support During the Balikatan Exercise in 2024 and 2025.

Zoonotic diseases are a primary threat to military readiness during deployments in the Asia-Pacific region. To characterize these risks, we conducted a multi-site, multi-year 'One Health' biosurveillance study during the Balikatan (BK) 2024 and 2025 exercises in the Philippines, sampling rodents, ectoparasites, and the environment at four key training locations on Luzon Island. A comprehensive biosurveillance study at four BK training sites revealed distinct, evolving, and site-specific zoonotic disease risks that require tailored health protection strategies. The risk is most acute at Clark A

Deciphering the "Doxycycline Deficiency Syndrome": Defining the Characteristic Clinical, Laboratory and Imaging Findings in Patients Presenting with Leptospirosis, Q Fever and Rickettsial Diseases in Far North Queensland, Tropical Australia.

Background: The "doxycycline deficiency syndrome" is a colloquial term used in tropical Australia to describe the overlapping, non-specific findings seen in leptospirosis, Q fever, and the rickettsial diseases scrub typhus and Queensland tick typhus (QTT). A laboratory confirmed diagnosis is frequently delayed, however, early differentiation of these potentially life-threatening infections has important implications for clinical management. Methods: We examined consecutive laboratory-confirmed cases of leptospirosis, Q fever, and rickettsial diseases managed at a referral centre in tropical Au

Leptospirosis and its co-infections in India: prevalence, geographic distribution, and clinical implications.

BACKGROUND: Leptospirosis is a zoonotic disease prevalent in low and middle-income countries and tropical regions. It can be mimicked by endemic infectious diseases that present simultaneously, like dengue, malaria, melioidosis, or scrub typhus. However, there is a dearth of documented evidence about the frequency of coinfection, as well as related symptoms and mortality. The study aims to determine the frequency and geographical extent of coinfections in leptospirosis patients in India. METHODOLOGY: We systematically searched databases, including Scopus, PubMed, Embase, and Web of Science, f

Standardizing the approach to acute undifferentiated febrile illness: Development of an expert consensus-based algorithm for India.

BACKGROUND: Acute Undifferentiated Febrile Illness (AUFI) poses an immense challenge to diagnosis in India because it can be caused by various pathogens. The clinical presentation is often similar across causes, and the number of potential causative organisms continues to increase due to changes in climate and ecology. To counter these challenges, it was necessary that a holistic diagnostic algorithm based on a set of predefined criteria be formulated by the Indian Council of Medical Research (ICMR). METHODS: A Core Committee and Expert Committee were formed with members including infectious
