Corneal injury associated with scrub typhus following mite exposure in a child in Taiwan: a case report.

BACKGROUND: Scrub typhus is a mite-borne zoonosis endemic to the Asia-Pacific region. Although ocular complications are occasionally reported during later stages of infection, direct mechanical ocular injury related to mite exposure has rarely been reported. Recognition of early ocular manifestations may facilitate timely diagnosis and treatment of this potentially serious infection. CASE PRESENTATION: We describe a rare case of a 9-year-old Taiwanese boy who developed acute ocular and systemic symptoms after exposure to mites during a visit to Alishan Forest, Taiwan. Live mites were identifi

Scrub Typhus Presenting with Isolated Xerostomia.

Scrub typhus is a mite-borne rickettsial infection that typically presents with fever, rash, and eschar; however, atypical manifestations may lead to delayed or missed diagnosis. The present report involves a case of a 65-year-old woman who presented with persistent xerostomia (dry mouth) as the sole ongoing symptom at the time of evaluation, without rash, eschar, or ongoing fever. She had a history of outdoor exposure in an endemic area, and laboratory testing revealed transient liver dysfunction and hyponatremia associated with polydipsia. Although empiric antimicrobial therapy was not initi

Clinical characteristics and an admission-time predictive model for severe scrub typhus and secondary HLH in adults.

BACKGROUND: Scrub typhus can rapidly progress to severe disease with multi-organ dysfunction. Secondary hemophagocytic lymphohistiocytosis (HLH) is an increasingly recognized hyperinflammatory complication associated with high mortality, but adult data and simple admission-time risk tools remain limited. METHODS: We conducted a single-center retrospective cohort study of hospitalized adults with scrub typhus at the First Affiliated Hospital of Guangzhou Medical University (May 2013-July 2025). Severe scrub typhus was defined by major organ involvement, shock, or in-hospital death. Variables a

Transformation and allelic exchange in Orientia tsutsugamushi.

UNLABELLED: Orientia tsutsugamushi is a mite-transmitted obligate intracellular bacterium that causes the potentially deadly zoonosis, scrub typhus. The absence of genetic tools for Orientia has limited studies of the microbe-host interactions that underlie scrub typhus. To address this gap, we developed a protocol for transforming and achieving allelic exchange in O. tsutsugamushi str. Ikeda. From evaluating multiple cell lines and antibiotics, we found that contact-inhibited EA.hy926 human endothelial-like cells best supported Orientia replication, and that chloramphenicol was an effective s

An unusual presentation of scrub typhus as tetany: A case report.

Scrub typhus is a mite-borne rickettsial infection caused by Orientia tsutsugamushi and is transmitted through the bite of larval trombiculid mites. It is an important cause of acute febrile illness in endemic regions. Although the disease typically presents with fever, rash, lymphadenopathy, and eschar, its clinical manifestations are highly variable and may include severe systemic complications. We report a case of scrub typhus presenting with symptomatic hypocalcaemia and hypophosphataemia probably the result of renal tubular electrolyte loss, highlighting a potentially under-recognized met

Geographic and Orientia infection status influence on the bacterial microbiome of free-living chiggers in North Carolina, USA.

Chiggers (larval Trombiculid mites) serve as vectors for Orientia species that cause scrub typhus, a potentially serious illness in humans with a broadening global distribution. To date, there is limited research on the chigger microbiome in the United States (US) compared to some other parts of the world. Investigating chigger bacterial communities is essential for understanding the potential role they play in pathogen transmission dynamics within these arthropods. This study investigated the bacterial communities of free-living chiggers collected from sites across the three ecoregions in Nor

Complete genomes of two Ikeda-genotype Orientia tsutsugamushi isolates from South Korea reveal within-lineage divergence and contrast with the Boryong reference strain.

Orientia tsutsugamushi strain Ikeda is a scrub typhus reference strain originally described in Japan, and Ikeda 56-kDa type-specific antigen sequence types have also been reported in South Korea. However, complete genome resources for South Korean Ikeda-genotype isolates remain limited. Here, we generated complete genomes for two archived clinical O. tsutsugamushi isolates from northern South Korea, CH219 and K4-135, using a PacBio HiFi and Illumina hybrid assembly approach and compared them with the Japanese reference strain Ikeda and the South Korean reference strain Boryong. Both genomes we

Non-linear effects and lag patterns of eco-environmental factors on scrub typhus: a spatiotemporal analysis in Yunnan, China, 2015-2022.

BACKGROUND: Yunnan Province is characterized by a high incidence of scrub typhus and complex ecological environments, where epidemic patterns are shaped by the dual influence of the plateau monsoon system and geomorphological evolution. However, macro-scale meteorological indicators often fail to accurately capture climatic features within such rugged terrains, leaving the multi-factor coupled spatiotemporal associations of scrub typhus epidemics elusive. METHODS: This study integrates Wavelet periodicity analysis and STL decomposition to identify the temporal rhythms and structural mutations

Clinical and Laboratory Features of Scrub Typhus among Adult Patients with Acute Febrile Illness in Tak and Nakhon Phanom Provinces, Thailand, 2017-2020.

Scrub typhus (ST), caused by Orientia tsutsugamushi, is an underdiagnosed cause of acute febrile illness in Southeast Asia. Acute febrile illness surveillance data from febrile patients presenting to health facilities in two Thailand border provinces (Tak [north] and Nakhon Phanom [northeast]) were analyzed to estimate ST prevalence and describe clinical and epidemiological characteristics and risk factors. Scrub typhus cases were classified as confirmed (polymerase chain reaction-positive), probable (acute-to-convalescent rise in ELISA IgM/IgG plus ≥2 clinical features), or suspect (positive

Improving admission clerking standards and physical health assessments in febrile patients: A prospective clinical audit at a tertiary care hospital.

INTRODUCTION: Clinical audit is an effective tool to improve patient care, which in turn has an impact on clinical outcome.Patients who present with fever usually are generally admitted based on competency-based criteria, where admission is determined on the severity of the illness assessed through clinical and laboratory findings. Bacterial infections were linked to high sepsis-related mortality. This highlights the importance of comprehensive and standardized admission clerking and physical assessment for patients presenting with fever. This audit aimed to evaluate the documentation of histo

Spatiotemporal prediction of scrub typhus incidence and environmental risk factors in Republic of Korea: a Bayesian hierarchical approach.

BACKGROUND: Scrub typhus is a vector-borne infectious disease that is a major public health concern in Republic of Korea. Environmental factors are associated with disease transmission. However, comprehensive predictive models incorporating multiple environmental determinants while accounting for spatial and temporal correlations remain limited. This study aimed to characterize the spatiotemporal patterns of scrub typhus incidence and develop a Bayesian spatiotemporal prediction model incorporating multiple environmental covariates across 229 administrative units in the Republic of Korea from

National surveillance of tick- and mite-borne diseases in Japan, 1999-2025: a rapid surveillance reporting framework using the jpinfect R package.

The Government of Japan has conducted weekly national surveillance of notifiable infectious diseases for over 25 years. As of December 2025, 87 notifiable diseases are under surveillance, although the list has changed over time as new infections have emerged and others have been reclassified. Although weekly data are openly available, their formatting and preparation have been labour-intensive. An open-source R package, jpinfect, has the potential to improve data accessibility and analytical workflows for these data sets. In this study, we used the jpinfect package to summarize national survei

Predictors of mortality in scrub typhus fever: A cross-sectional study from central India.

BACKGROUND OBJECTIVES: Scrub typhus, an acute febrile illness caused by Orientia tsutsugamushi , has re-emerged as a major public health challenge in India, especially in rural and resource-constrained areas. Due to its nonspecific presentation and limited diagnostic access, early diagnosis is often missed, leading to potentially fatal complications like acute respiratory distress syndrome (ARDS), renal failure, liver dysfunction, and encephalopathy. This cross-sectional study aimed to identify clinical and laboratory predictors of mortality in scrub typhus patients and emphasize the vital rol

High levels of antisense transcription from numerous genes in the obligate intracellular bacterium Orientia tsutsugamushi.

Orientia tsutsugamushi (Ot) is a cytoplasmic, gram-negative, obligate intracellular bacterium that causes the human disease scrub typhus. It has a highly repetitive genome, whereby approximately 50% is composed of multiple copies of a proliferated integrative and conjugative element, the Rickettsial amplified Genetic Element (RAGE). Previous RNA sequencing analysis revealed a high level of antisense transcription in Ot, particularly from RAGE-encoded genes, with 18% of all genes having a sense:antisense ratio <1 and 5% with a ratio <0.1. In our current study, we have confirmed the earlier RNA

Reemergence of Scrub Typhus Associated with Leptotrombidium akamushi Mites and Karp-Like Orientia tsutsugamushi Genotype Bacteria, Japan, 2024-2025.

Scrub typhus is a miteborne infectious disease caused by Orientia tsutsugamushi bacteria. In Japan, most cases occur in spring and autumn and are transmitted by Leptotrombidium pallidum and L. scutellare mites; summer scrub typhus, transmitted by L. akamushi mites, is rarely reported. In August 2024, a pregnant woman had scrub typhus develop after attending a fireworks event in Niigata Prefecture, Japan, resulting in fetal death. A Vietnam-related Karp-like O. tsutsugamushi genotype, distinct from the Kato type usually associated with L. akamushi mites, was detected from patient samples. Field

Molecular detection of Orientia tsutsugamushi infection in bats from the China-Myanmar border.

Orientia tsutsugamushi (Ot), the causative agent of tsutsugamushi disease (TD), has been detected in Muridae, Soricidae, Cricetidae, Canidae, Artiodactyla, and birds, as well as in its trombiculid mite vectors. However, to date, scarce reports have documented Ot infection in bats. TD is an ancient zoonotic disease transmitted through the bite of infected trombiculid mites. The global disease burden of TD, particularly in impoverished regions, warrants renewed attention and a reevaluation of public health strategies. In this study, we analyzed bat samples for Ot using nested PCR (nPCR) and real
